Combination of a viability test and a quantification method for Schistosoma haematobium eggs (filtration--trypan blue-staining-technique).

The efficiency of the polycarbamate membrane filtration technique for detecting Schistosoma haematobium eggs in urine has been increased by using a pore size of 14 micrometers diameter and the suction of a water jet pump. The technique permits filtration of several 1000 ml of urine in one test. Eggs concentrations of 1 egg in more than 1000 ml of urine were detected. Viability was assess after filtration by staining with trypan blue. Preliminary data of 118 Schistosoma haematobium-positive urine samples of 32 patients suggest that the filtration trypan blue-staining-technique is of high validity in light infection with scanty egg-output.

High granulocyte yield with a simple method of filtration leukapheresis.

A simple continuous filtration leukapheresis has been devised whereby two Leuko-Pak filters can be charged and eluted within 5 h with approximately 121 of blood in a closed system of tubes. Blood flows by means of gravity without the use of pumps. Without premedication with prednisolone the average yield from 10 donors was 3.66 x 10(10) granulocytes per leukapheresis and after premedication with 150 mg prednisolone orally it was 6.44 x 10(10) granulocytes per leukapheresis. The in vitro function of filter granulocytes was not reduced even after premedication with prednisolone. In comparison with leukapheresis by blood cell separators this modified method of filtration leukapheresis offers the following advantages: 1) the granulocyte yield per litre of whole blood processes is higher, 2) technical assistants in a blood-bank can carry out the method on several donors simultaneously, 3) the method is more economical than leukapheresis by blood cell separators.

[Acute drug-induced agranulocytosis: comparison between openward treatment and antimicrobial decontamination in a plastic isolation bed system (retrospective study of 30 cases) (author's transl)].

Thirty patients with severe drug-induced agranulocytosis were admitted to the Ulm University Hospital between January, 1968 and October, 1976. All of 13 already infected patients treated with antimicrobial decontamination in a plastic isolation bed system survived. But nine of 17 patients treated in the open ward died. The cases were not randomised and there was a difference in age between the two groups, but nevertheless the results suggest that antimicrobial decontamination and isolation is superior to treatment in an open ward.

Comparative trial on the therapeutic effectiveness of the new anthelmintic drug: ciclobendazole.

In a double-blind study in Cameroon the vermicidal effect of Ciclobendazole, a new Benzimidazole derivative, was evaluated and compared to Mebendazole. Ciclobendazole and Mebendazole were equally effective in the treatment of Ascaris and Hookworm infestations. An increase in the dosis of Ciclobendazole from 600 mg to 1200 mg did not lead to an improvement in the effectiveness. When treating trichuriasis significantly better results were achieved with Mebendazole (p = 0.01). Both drugs were tolerated equally well. Side effects, such as vomiting and diarrhoea, only occurred in a small percentage of cases.

Cutaneous leishmaniasis in Eastern Saudi Arabia. Epidemiological and clinical features in a nonimmune population living in an endemic area.

Cutaneous leishmaniasis is endemic in Saudi-Arabia. Nonimmune Europeans and their families living temporarily in this area were affected by the disease in great number, resembling an epidemic outbreak. Clinical features and treatment of the disease, problems of re-infection and immunity, as well as epidemiological factors are discussed. Using biochemical methods the parasite was identified as Leishmania tropical major.

Comparison of different methods for the detection of intestinal protozoa and helminths in stool.

A total of 1428 stool specimens were examined to evaluate the usefulness of different methods of detecting intestinal protozoa and helminths. The following methods were compared: a combination of routine laboratory methods (Normal saline, Lugol's solution and Iron-haematoxylin), the MIF technique (Mertheolate-iodine-formaldehyde) and the MF-Heidenhain method for protozoa. The Telemann method and MF-Telemann method for worm eggs. The MIFC technique for both protozoa and worm eggs. According to the criteria: diagnostic yield, accuracy, simplicity, time consumption and cost, it is concluded that the MIFC method is superior to the other methods used in this study.

The clinical efficacy of granulocyte transfusions: studies on the oral cavity.

Patients with severe granulocytopenia of different origin received granulocyte transfusions. The migration and phagocytic ability of the transfused granulocytes in the oral cavity ("orogranulocytes") served as a parameter for measuring the viability of the granulocytes in vivo. There was no difference between granulocytes collected by continuous flow centrifugation and those collected by filtration leukapheresis. Under the influence of the transfusion treatment mucous membrane ulcers (MMU) subsided and healed. Where ulcers were present the number of orogranulocytes (OG) increased after granulocyte transfusion. In addition there was a significant correlation between the number of granulocytes in the peripheral blood and the number of orogranulocytes. As a control, orogranulocytes were studied in a group of healthy persons with intact teeth, toothless persons with normal blood counts and one patient with an acute agranulocytosis.

[Maridi haemorrhgic fever: a new viral disease (author's transl)].

A new viral disease (Maridi haemorrhagic fever) occurred in the South Sudan in 1976. It was obviously identical with an epidemic which occurred at the same time in Zaire. The virus is morpologically closely similar to the Marburg virus. During the Maridi epemic 124 of 238 patients died (52%). Characteristic symptoms were fever and headache (100%), diarrhoea (83%), retrosternal pain (82%), vomiting (68%), haemorrhages (62%), morbilliform or vesicular rash (52%). At post-mortem there were changes in liver, kidney, myocardium and lungs, similar to those in the Marburg virus disease, as were those observed in bone marrow and peripheral blood. Despite these analagous findings, the clinical course and results of immunofluorescence indicate that it is a new disease. The epidemic ended after suitable isolation measures had been taken. There was no specific treatment but in some cases convalescent plasma and interferon were tried. The disease is transmitted among humans by direct contact or by contact with blood or excreta of patients. No animal reservoir has been found. It is possible for this disease to be imported also into countries with a modorate climate.

Silicone lymphadenopathy and synovitis. Complications of silicone elastomer finger joint prostheses.

We report two complications of silicone elastomer finger joint prostheses. In one patient, the prostheses broke, with silicone particles present in synovium ("detritic synovitis"). In another patient, silicone particles were found in an axillary lymph node five years after insertion of prostheses in the ipsilateral hand (prostheses were intact at the time). Microscopically, silicone particles in synovium and lymph node were identical to particles abraded from a new prosthesis.

[Connatal malaria (author's transl)].

A case of connatal malaria due to Plasmodium vivax in a newborn of turkish origin is presented. Most likely the infection was acquired by materno - fetal transfusion during labor. Pathophysiological aspects and mode of transmission of this rare disease are discussed.

High dose granulocyte substitution after bone marrow transplantation in a case of aplastic anaemia in childhood.

A bone marrow transplantation (BMT) was successfully carried out in a seven-year-old boy with aplastic anaemia. An HLA and MLC-identical brother served as donor. During the first 16 days after BMT between 0.55 and 7.11X10(10) granulocytes were transfused 14 times (an average of 4.03X10(10) per transfusion). The maximum increment in the peripheral blood after transfusion was over 4000 granulocytes per microliter. The course of local infections was taken as parameter for determining the clinical effectiveness of the granulocyte transfusion. Under the substitution therapy the necrotic ulcer of a fingertip and the mucous membrane ulcers in the oral cavity subsided. The presence of transfused granulocytes in the oral cavity ("orogranulocytes") could be established. There was a statistically significant relationship between the number of granulocytes in the peripheral blood and the number of orogranulocytes. It could be demonstrated that in children a temporary normalisation of the number of granulocytes in the peripheral blood can be attained by means of granulocyte substitution and that the transfused granulocytes are clinically effective.

[Immunoblastic lymphadenopathy with a pseudoleukemic blood picture].

The case of an immunoblastic lymphadenopathy with a leukaemoid reaction has been observed. All the clinical, serological and histological features described by previous authors were present. In addition, there were more than 1000000/mul nucleated peripheral cells, consisting of mature and a few immature granulocytes, a small number of normoblasts and many lymphocytes, "immunoblast-like" lymphoid cells and plasma cells. Polyclonality of the latter was demonstrated by serological methods. Spontaneous remission occurred and was apparently enhanced by the application of corticosteroids.
