[Tuberculoma of the heart. Contribution of echography. Apropos of a case].

An 18-year old woman was admitted to hospital with signs of right heart failure and atrial tachycardia. Ultrasounds showed a tumoral mass in the tricuspid valve. Post-mortem examination revealed that the mass was a tuberculoma obstructing the tricuspid orifice. The exceptional character of this case is emphasized: lesions of the heart have been found in less than 0.3 p. 100 of tuberculous patients autopsied. The different anatomical aspects of tuberculosis of the heart are mentioned, and it is suggested that echocardiography should be performed systematically in all patients with severe, disseminated tuberculosis. Tuberculoma of the heart should figure on the list of intraparietal and intraluminal masses visualized at echocardiography.

[Hepatic amebiasis at Niamey Hospital (Nigeria)].

The authors report the results of a prospective study on hepatic amebiasis realized in national hospital of Niamey (Republic of Niger). Hepatic amebiasis is uncommon in Niger as in other hot and dry countries of Africa whereas it is common in hot and damp areas of the continent. These results point out the hypothesis of a hepatotropic strain of Entamoeba histolytica uncommon in Sahelia area whereas the amebiasis infestation is frequent.

[HB virus infection and delta surinfection in Sahelian Africa].

78 hospitalized patients were selected when presenting with at least one of these signs: hepatomegaly, jaundice, ascites, oesophageal varices, abdominal venous pattern, splenomegaly. All had radioimmunoassays for hepatitis B surface antigen (HBsAg) and antidelta antibody (78/78). Acute or chronic hepatic disease was diagnosed in 56 patients: 7 acute viral hepatitis, 13 chronic hepatitis, 23 non alcoholic hepatic cirrhosis, and 13 hepatocellular carcinoma. Twenty-two patients with other diagnoses served as controls. Serum antidelta was present in each group: acute viral hepatitis (2/7), chronic hepatitis (2/13), non alcoholic hepatic cirrhosis (9/23), hepatocellular carcinoma (3/13), controls (2/22). Every patient with acute or chronic hepatic disease and positive serum anti-delta was positive for serum HBsAg. Amony controls, 2 patients with positive serum antidelta were negative for serum HBsAg but positive for antiHBs. Delta superinfection is present in the sahelian region; Patients with acute viral hepatitis, chronic hepatitis, non alcoholic hepatic cirrhosis, and hepatocellular carcinoma are electively infected. Patients with acute or chronic hepatitis and positive serum antidelta have hepatitis B virus evolutive infection (positive serum HBsAg).

Urinary tract lesions due to Schistosoma haematobium infection assessed by ultrasonography in a community based study in Niger.

The comparative prevalences of urinary tract lesions in 2 villages where urinary schistosomiasis was endemic and in a control village free from infection were assessed by ultrasonography. Of the 822 residents greater than 4 years of age in the first village (Sébéri) where the prevalence of infection was 57%, 279 were examined by ultrasonography. The prevalence of bladder lesions was 71% in those 5-14 years of age, 57% among adult men, and 24% among women in this endemic village, compared to 10%, 16%, and 6%, respectively, in the control village. Renal lesions were infrequent among adults and there was no significant difference in the rates between Sébéri and the control village. Among children, moderate hydronephrosis was absent in the control village, but was observed in 19% of the boys 5-14 years of age and 2% of the girls 5-14 years of age in Sébéri. Sixty schoolchildren of the second endemic village with urinary egg counts greater than or equal to 100 eggs/10 ml of urine were examined by ultrasonography. The overall prevalence and the severity of bladder lesions in the 2 endemic villages were significantly related to the urinary egg count.

[Intestinal parasitic diseases of school children in the Republic of Niger].

Coprologic surveys realized in schoolchildren in a few western areas of the Republic of Niger provided the following results. Concerning protozoa, the prevalence of amebiasis-infestation is strong in all the studies areas, giardiasis is more frequent in dry areas. As for the helminths, Hymenolepsis nana is met everywhere, the prevalence of ancylostomiasis decreases gradually from the south to the north and the endemic area of Schistosoma mansoni is limited to the extreme south of the country. The other helminthiasis are rare.

[Chronic hepatitis, liver cirrhosis, primary liver cancer and virus HB infection. Epidemiologic study in a sahelian hospital milieu. Apropos of 185 cases].

From December 1982 to June 1985, 185 patients with signs of chronic liver disease are investigated in National Hospital (Niamey, Republic of Niger). A first group ("Hepatopathy") of 131 patients (75 males, 56 females) is made of 3 chronic liver diseases: 35 chronic hepatitis (CH), 58 hepatic cirrhosis (HC), 38 hepatocellular carcinoma (HCC). A second group ("Controls") of 54 patients (29 males, 25 females) is made of miscellaneous diseases other than CH, HC, HCC. In the first group ("Hepatopathy") serum hepatitis B virus (HBV) surface antigen (HBsAg-radioimmunoassay) is present in 64.9% (85/131). In the second group ("Controls") serum HBsAg is present in 3.7% (2/54) (Chi-2 test P less than 10(-9)). Serum HBsAg prevalence is more than 50% in each group of CH, HC and HCC. A maximum value is observed in males with HCC (75.7%). These results demonstrate the importance of evolutive HBV infection in the course of CH, HC and HCC in the sahelian region.

[New cases of hydatidosis in Niger].

New cases of hydatidosis in the Republic of Niger. Five cases of hydatidosis diagnosed in the Republic of Niger are reported. All localisations are extra-hepatic: spleen: 2, lungs: 1, mediastinum: 1, parotid gland: 1. West african hydatidosis is uncommon and remarkable for the predominance of extra-hepatic localizations.

[Urinary bilharziasis and sideropenic anemia. Study of their relation in 2 villages in Niger].

In order to assess Schistosoma haematobium as an aetiologic factor in sideropenic anaemias, we compared a village with high prevalence of urinary schistosomiasis and a village without urinary schistosomiasis as a control. In the infested village, anaemias (haemoglobin value lower than 12 g/dl) especially affects children between 5 to 14 years old (71%:45/68). Boys being the most infestated group with S. haematobium (prevalence:100%; mean egg output: 496 eggs/10 ml of urine) are also the most anaemic group (82%:23/28). Comparing the two villages shows that urinary schistosomiasis increases the risk of acquiring anaemia by 30% among children aged 5 to 14. Furthermore, the parasitic disease reduces of 1 g/dl the mean haemoglobin level of adult males. On the other hand, S. haematobium is of minor importance as a cause of anaemia among women. The others aetiologies remain preponderant.

[2 cases of hydatidosis in the Republic of Niger].

Two cases of hydatidosis have been observed at the hospital of Niamey, one involving the tibia and the other one the liver and the spleen. For unknown epidemiological factors, this helminthiasis is rare in man in western Africa.

[Early syphilitic hepatitis. Two cases].

Two patients are presented as cases of secondary syphilitic hepatitis. Clinical and biological signs coincided with usual description of this affection. Syphilitic serological tests were all very positive. These two patients were homosexual men. In one case, we have found treponema in hepatic biopsy, underelectron microscope only, which confirmed the diagnosis. Evolution of both patients was improved by anti-syphilitic penicillin the rapy. Thus, this is an important diagnosis, which should not missed, because of the good prognosis of this affection with specific treatment, and because of the increasing frequency of syphilis for 20 years.

[Disseminated candidiasis. 23 cases collected in an intensive care unit].

Twenty-three cases of disseminated candidiasis (DC) were observed between 1965 and 1981. In 23 cases the DC was a complication of a preexisting disease process (16 abdominal surgery, 18 bacterial septicemia, 16 acute renal failure, 4 carcinoma) requiring polytherapy (23 antibiotherapy, 5 parenteral nutrition, 4 corticotherapy). The most frequent foci were ocular (6), peritoneal (5), renal (4), and pulmonary (3). Diagnosis was confirmed by blood culture (20), urine culture (12), culture of the catheter tip (7), isolation of candida from a localised focus and by anti candida antibodies in some patients. Nine patients were not treated and died; 14 patients were given antifungal agents: 6 died, three despite cure of DC. The recognition of favouring circumstances, early diagnosis and systematic treatment of all foci of candidiasis allow effective prophylaxis against DC; the last case in our series was in 1978.

[Richettsial pericarditis and pleurisy].

The authors report two cases of rickettsial disease due to R. Conori with mainly pericarditis in one case, sero-fibrinous pleurisy in the other. They then recall a few general data concerning this rickettsial disease and the very restricted place that it occupies in the etiology of pericarditis and, even more so, in the case of pleurisy. The conditons of diagnosis, which are mainly serological, are discussed, in particular with regard to pericarditis and tuberculous pleurisy.
