Ultrasonographic assessment of the regression of bladder and renal lesions due to Schistosoma haematobium after treatment with praziquantel.

The effect of praziquantel on urinary tract lesions due to Schistosoma haematobium was assessed by ultrasound in an endemic village in Niger. Ten months after treatment with praziquantel, bladder and renal lesions among the 149 patients were assessed. The parasitological cure rate was 87%. Ultrasonography proved to be an excellent tool to assess pathological changes. It is reliable, quickly performed, well accepted by the community and permits the definition of a "regression-rate" of the pathological lesions. Ten months after treatment, the "regression-rate" of bladder lesions was 68% and of renal lesions was 73%. The presence of bladder lesions had a negative influence on the regression of renal lesions. Nearly all renal lesions, without bladder lesions, regressed within the ten months period of observation. This study permitted the identification of a group of persons whose bladder lesions did not regress and who were possibly more vulnerable to the development of bladder cancer, thus requiring long-term follow-up.

[Postpartum cardiomyopathy in the Sudanese-Sahelian area. Clinical and epidemiologic studies of 66 cases].

The authors report 66 cases of peri- and postpartum cardiomyopathy. The patients' age ranged from 16 to 42 years (mean +/- SD 30 +/- 7 years). All were black women native of the western part of the Republic of Niger, Sahelian in the north, Sudanese in the south. At first examination, all had signs of congestive cardiac failure. In 67 p. 100 of the cases these signs appeared during the first six postpartum weeks. Clinical, radioscopic and echocardiographic features were always those of dilated cardiomyopathy, even when arterial pressure was high. Hypertension was present at first examination in 50.8 p. 100 of the cases, but it remained stable under treatment in 13 p. 100. These data suggest an acute postpartum hypertension. The following risk factors of the disease were identified: rural living, absence of school attendance, low family income, multiparity, identical pathology after a previous pregnancy, postpartum "quarantine" period, ritual ablutions with very hot water, large amounts of sodium in the diet, hypertension, breast-feeding and postpartum oestrogen secretion decrease. Seasonal variations were noted, with doubling of new cases during the hot and humid season. The hypothesis of a latent gravidic myocarditis is discussed: the accumulation of risk factors during the postpartum period might trigger off the clinical disease. Forty seven patients were followed up for a mean period of 15 months. Seven died, 21 (31.8 p. 100) were in complete remission and 19 in partial remission. In case of relapse, complete remission was less frequent and appeared more slowly under treatment. Persistent cardiomegaly under treatment was of poor prognosis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Echographic study of the morbidity due to urinary bilharziasis in a hyperendemic village in Niger].

Three hundred and seven patients, 130 children and 207 adults, from a hyperendemic village, underwent an ultrasonography of bladder and kidneys. Prevalence of bladder lesions is 79.3% in children and 61.9% in adults, prevalence of hydronephrosis is 36.1% in children and 9.7% in adults. These study confirms the importance of urologic lesions from schistosomiasis in the irrigation schemes of the Niger river. There is no relationship between frequency and importance of lesions and urinary egg count.

[Intestinal parasitic infections in children in Niamey (Niger)].

Stools of 1,368 randomly selected children living in Niamey (Niger) were examined using two different parasitologic methods. One or more pathogenic parasites were recovered in 678 children (49.6%). Parasitic infections developed early and increased significantly after the age of two, which is the usual age for weaning. The most common parasitic infections were giardiasis (31.7%) and amebiasis (12.8%). The only commonly recovered helminth was Hymenolepis nana (10.8%). This predominance of intestinal protozoa is typical of Sahel pathology.

[Toxoplasmosis in pregnant women in Niamey (Niger)].

An investigation of antitoxoplasma antibodies was performed with ELISA among 218 pregnant women in Niamey (Niger). The overall prevalence is of 15.1%. Low prevalence rates are found like in most of the studies conducted in Sahelian Africa.

[Postpartum myocardiopathy. Clinical and echographic evaluations of the response to treatment. 30 cases observed in the Sudan-sahelian area].

Thirty cases of postpartum cardiomyopathy observed in a savannah-sahelian region of Africa (Niamey, Republic of Niger) are reported. The dilated cardiomyopathy was diagnosed by comparing clinical signs with electrocardiographic, radioscopic and echocardiographic findings. A simplified therapeutic regimen was used in all cases: rest during 2 months, sodium restriction, diuretic and digoxin therapy (1 tablet of each drug every other day). Patients were followed up for a mean period of 13 months. The course of the disease was evaluated at regular intervals by echocardiography which provided accurate information on left ventricular function. At the time of diagnosis all patients had congestive heart failure. Complete remission was defined as the absence of all signs and a normal social life. Twelve patients answered this definition with a mean follow-up of 13 months. Nine patients (mean follow-up 24 months) were in incomplete remission; a large left ventricular aneurysm had developed in 2 of them. In 6 other patients the follow-up was too short (1 to 8 months) for a prognosis to be formulated. In postpartum cardiomyopathy 3 factors are indicative of a poor prognosis: 1) absence of free interval between delivery and first signs of heart failure; 2) history of cardiomyopathy after previous deliveries; 3) poor response to treatment with persistent cardiomegaly. On the other hand, the severity of the initial cardiomegaly and impairment of ventricular contractility, and the presence of permanent hypertension or pericardial effusion have no significant influence on the response to treatment. Irreversible contraception is justified in great multiparous women. In primiparous and oligoparous women a temporary contraception is necessary, a new pregnancy being conceivable only after complete remission and under close supervision.

Mycetoma in the Republic of Niger: clinical features and epidemiology.

Mycetoma is a common disease in the Republic of Niger. In two hospitals 133 cases were observed. The major site of lesions was the foot. Actinomycetomata were seen more often than eumycetomata. Streptomyces somaliensis is prevalent in the north desert zone while Actinomadura pelletieri is common in the southern part of the country. Madurella mycetomatis, the usual etiologic agent of eumycetoma, is seen in both regions. The species incidence and distribution in Niger differs from those of the west and east African endemic areas.

[Morbidity due to bilharziasis caused by S. haematobium. Relationship between the bladder lesions observed by ultrasonography and the cystoscopic and anatomo-pathologic lesions].

Twenty-seven patients with proved urinary schistosomiasis and echographic bladder lesions were selected for cystoscopic examination and biopsy. All patients had specific lesions at cystoscopic investigation. Histologic examination confirmed diagnosis 26/27 cases. Ultrasonography appears as a very efficient method for detection of schistosomiasis bladder pseudo-tumors.

[Toxoplasmosis in Niger. A serological analysis of 400 subjects].

A toxoplasmosis serological study using an indirect immunofluorescence method was made in 400 subjects in the Republic of Niger because the epidemiologic findings of this disease were very poor. Serum antitoxoplasmic antibodies were present in 18.2%. In sahelian areas prevalence of such toxoplasmic antibodies is low in the majority of studies. Analysis of seropositivity and age pointed out a late seroconversion.

[Pseudomycetomatous subcutaneous phaeohyphomycosis].

The authors report the first Nigerian case of cutaneous and sub-cutaneous phaeohyphomycosis. It was characterized by its original macroscopy: the lesion was pseudo-tumoral and provided with fistulae. So it looked like a mycetoma. The diagnosis was finally made by histology: it showed the typically pigmented mycelium. Cultures have not been made. Etiological agents of phaeohyphomycosis being opportunistic, this disease occurs often among immunosuppressed patients. The knowledge of unusual clinical forms may allow to suspect the diagnosis and to cultivate the exudate: this provides the only specific mycologic diagnosis.

Delta superinfection in patients with chronic hepatitis, liver cirrhosis and hepatocellular carcinoma in a Sahelian area. Study of 112 cases versus 46 controls.

From October 1982 to June 1985 158 hospitalized patients in the National Hospital of Niamey, Republic of Niger, were selected whenever one of the following signs was found: hepatomegaly, jaundice, ascites, oesophageal varices, abdominal venous pattern, or splenomegaly. Investigations included hepatic echography (158/158), needle liver biopsy (68/158), radioimmunoassays for serum hepatitis B surface antigen (HBsAg; 158/158), anti-HBs (152/158), anti-HBc (129/158) and anti-delta antibody (anti-HD; 158/158). 112 patients with liver diseases comprised 28 with chronic hepatitis, 55 with non-alcoholic hepatic cirrhosis, and 29 with hepatocellular carcinoma (HCC). 46 patients with other diagnoses were used as controls. 71/112 liver disease patients were positive for HBsAg in serum compared with 1/46 controls (P less than 10(-9)). Prevalences of delta superinfection in patients with serum HBsAg (+) and anti-HD (+) were 45/112 (40.2%) in liver disease patients versus 1/46 (2.2%) in controls (P less than 10(-9)). Delta superinfection was very frequent in chronic hepatitis (8/28), non-alcoholic cirrhosis (24/55) and HCC (14/29). In chronic hepatitis, delta superinfection was more frequent in the chronic active form than in the chronic persistent type (not significant). Cirrhosis patients with delta superinfection were younger (10 years in males, 11 years in females) than those without (P less than 0.05).

Premenstrual syndrome in Sahelian Africa. A comparative study of 400 literate and illiterate women in Niger.

From March to November 1984, the authors examined 400 African women 14 to 30 years old. They were classed as urban or rural, literate or illiterate, wage-earning or not. Signs of premenstrual syndrome (PMS) were analysed and the intensity of symptoms evaluated. Only 5% of the women were asymptomatic. 31% described socially debilitating severe symptoms. The most common signs were painful breasts, lower abdominal pains and nervousness. Oedema of the legs was rare. PMS was more frequent in young, literate, urban women; illiterate rural women were more often asymptomatic and, when they were symptomatic, signs were less intense than in urban women. These differences are related to pregnancies and breast-feeding. Urban illiterate women were protected from PMS by the cycle of pregnancy-breast feeding-weaning-new pregnancy. In this group menstruation was rare because of the length of gravidic amenorrhea.
