[Protracted diarrhea. Etiopathogenia and management (author's transl)].

For the purpose of twelve cases on protracted diarrhea, the etiopathogenia from this illness is reviewed and the tolerance and inconvenients of treatment with intravenous feeding is assessed, being the authors addicted of employing antibiotics, cholestyramine, diets free of disaccharides and fundamental diets; in such a manner that parenteral nutrition for its inconvenients and complications should only be used in case of failing the rest of therapeutic methods.

Steroids treatment of granulomatous hepatitis complicating Coxiella burnetii acute infection.

Granulomatous hepatitis associated with Coxiella burnetii acute infection has an adverse clinical course in some patients. Surprisingly, it does not respond to antibiotic but to steroids treatment. A hypersensitivity mechanism has been implicated. A case of granulomatous hepatitis complicating C. burnetii acute infection is reported, which was refractory to antibiotics but, as in four other cases previously reported, showed a complete response to steroids. This case was found to support findings that moderate doses of steroids can be useful in patients with granulomatous hepatitis complicating C. burnetii infection and showing no response to antibiotic treatment.

Cytomegalovirus infection in simultaneous pancreas-kidney transplantation.

INTRODUCTION: In this open-label multicenter study, 205 simultaneous pancreas-kidney (SPK) transplant recipients between 1998 and 2000 were randomly assigned to tacrolimus or cyclosporine-microemulsion (ME). All patients received concomitant rATG induction therapy, mycophenolate mofetil and short-term corticosteroids. We report the 3-year data related to the occurrence, severity and effect of cytomegalovirus (CMV) infections. The type of CMV prophylaxis and treatment was at the discretion of the investigator. RESULTS: The overall incidence of CMV infection was 34% with no difference in incidence between the tacrolimus and cyclosporine-ME treatment arms. Statistically significant fewer CMV infections occurred among patients who received ganciclovir (22%) than those who did not receive prophylaxis (42%; P = .0075) or were treated with acyclovir (43%; P = .0066). The CMV infection rate according to donor recipient CMV serological status was: D-/R- group 11%, which was lower than the D-/R+ group at 40% (P = .0035), the D+/R+ group at 37% (P = .0024), or the D+/R- group at 52% (P = .00001). Among the last three groups, the infection rate was lower in patients on ganciclovir than those with no prophylaxis or on acyclovir (22% vs 64%; P = .00001). The incidence of acute rejection episodes was higher among patients without ganciclovir prophylaxis. No difference was observed in actuarial patient, kidney, or pancreas survival rates between patients with versus without infection. CONCLUSIONS: Ganciclovir prophylaxis effectively prevented CMV infection in SPK transplant recipients, especially in higher risk groups. An effect of CMV prophylaxis on the incidence of rejection is possible.

Early development of NADPH diaphorase-expressing neurons in the brain of the urodele amphibian Pleurodeles waltl.

The distribution of nitrergic cells was studied in the brain of the urodele amphibian Pleurodeles waltl during embryonic and larval stages by means of NADPH-diaphorase histochemistry. The first positive neurons were observed at embryonic stage 30 in the ventrolateral area of the caudal rhombencephalon. Subsequently (stage 33b), weakly reactive cells appeared in the isthmic tegmentum, the mesencephalic tegmentum, the hypothalamus, and the nucleus of the solitary tract. At initial larval stages (34-38), two new groups of NADPH-diaphorase-positive cells appeared in the caudal telencephalon (the amygdaloid region) and in the middle reticular nucleus. During the beginning of the active larval life (stages 39-42), reactive cells were found in the granule cell layer of the olfactory bulb and in the telencephalic pallium. As in the adult, the nitrergic cells in the central nervous system are widely distributed during early development, pointing to important roles of nitric oxide through ontogenesis. The sequence of appearance of nitrergic cells suggests an early involvement in reticulospinal control most likely related to locomotor behavior.

Standardization and decay data of 237Np.

This paper reports contributions from participants in the EUROMET project (No. 416) which was entitled "237Np research into problems relating to purification, characterization and standardization". Primary standardizations were made by the defined low solid angle, coincidence, 4pi alpha, 2pi alpha and liquid scintillation counting methods. Secondary standardizations were made with calibrated gamma-ray spectrometers. Absolute X-ray, gamma-ray and alpha-particle emission probabilities were also determined. The results for the successful conclusion of both primary and secondary standardization are presented together with the values for alpha-particle and gamma-ray emission probabilities determined in this exercise. Several significant inconsistencies remain with the gamma-ray emission probabilities, and these are highlighted.

Skeletal maturity and socio-economic status in Portuguese children and youths: the Madeira growth study.

BACKGROUND: Skeletal maturity is used to evaluate biological maturity status. Information about the association between socio-economic status (SES) and skeletal maturity is limited in Portugal. AIMS: The aim of this study is to document the skeletal maturity of youths in Madeira and to evaluate variation in maturity associated with SES. SUBJECTS AND METHODS: The study involved 507 subjects (256 boys and 251 girls) from the Madeira Growth Study, a mixed-longitudinal study of five cohorts (8, 10, 12, 14 and 16 years of age) followed at yearly intervals over 3 years (1996-1998). A total of 1493 observations were made. Skeletal age was estimated from radiographs of the hand and wrist using the Tanner-Whitehouse 2 method (TW2). Social class rankings were based on method. Five social rankings were subsequently grouped into three SES categories: high, average and low. RESULTS: Median for the radius, ulna and short finger bones (RUS scores) in the total sample of boys and girls increased curvilinearly across age whereas median for the 7 (without pisiform) carpal bones (Carpal scores) increased almost linearly. The 20-bone maturity scores demonstrated distinctive trends by gender: the medians for boys increased almost linearly while the medians for girls increased curvilinearly. SES differences were minimal. Only among children aged 10-11 years were high SES boys and girls advanced in skeletal maturity. Madeira adolescents were advanced in skeletal maturity compared with Belgian reference values. CONCLUSION: The data suggests population variation in TW2 estimates of skeletal maturation. Skeletal maturity was not related to SES in youths from Madeira.

Progress in understanding humoral rejection in kidney transplantation: implications for patient management.

After more than 40 years of clinical renal transplantation, the contribution of humoral immunity to the pathogenesis of allograft rejection is progressively being clarified. With the advent of a new generation of immunosuppressive agents, the production and consequences of anti-donor alloantibodies can now be controlled. In the upcoming years, immunosuppressive regimens that will specifically control both T- and B-cell responses may further improve long-term allograft survival, if the immunosuppressive efficacy of such regimens is not hampered by an increase in infectious, neoplastic or cardio-vascular complications.

[Acarbose-induced acute hepatitis. Report of two events in the same patient].

A 57-year-old woman with non-insulin-dependent diabetes mellitus and inadequate glycemic control was prescribed acarbose (100 mg 3 times daily). Two months later she presented with acute hepatitis (ALT 2,300 IU/l). Other causes of liver damage were excluded. Three months after acarbose had been discontinued, all results of laboratory tests returned to normal values. Three years later the patient was given acarbose again. Acarbose (100 mg three times daily) had been added to glibenclamide (15 mg daily) 2 weeks before she presented with acute hepatitis (ALT 2,778 IU/l). Acarbose was stopped and the results of liver tests returned to normal within 2 months. Of the eight cases of acarbose-associated hepatotoxicity previously reported, five (as well as the two presented herein) were Spanish. The latency period, from the start of drug therapy to the onset of liver injury, was relatively long (> 2 months). We suggest that acarbose be included in the list of drugs which may induce acute hepatitis.

[Familial vesicoureteral reflux (author's transl)].

Sixteen members of three families, eight of which had vesicoureteral reflux, are studied. Authors report a high incidence in females (7 cases) and atrophic chronic pyelonephritis (75% of the serie). Only one girl presented disminution of renal function. The types of inheritance was dominant autosomal in the A family and autosomal recessive in the B and C families. Finally the norms of investigation of the familial vesicoureteral reflux are appointed.

[Value of the C reactive protein, urinary lactic dehydrogenase and renal capacity of concentration in the topographic diagnosis of the urinary infection in infancy (author's transl)].

In 39 children with urinary infection renal capacity of concentration, serum C-reactive protein and presence of urinary lactic dehydrogenase are studied pointing to the establishment of the topography of the damage. C-reactive protein levels higher than 20 micrograms/ml are 100 per 100 reliable in the diagnosis of pyelonephritis. Moreover, value of this test is confirmed as a guide of therapeutic efficacity. Difficulty of concentrating urine above 800 mOsm/l is 70 per 100 reliable and is a useful method for demonstrating parenchymal damage. The urinary lactic dehydrogenase was superior to 20 units/l in 64 per 100 of the cases, but other studies are necessary for interpretation of these data. These findings are similar to those obtained in the study of the isoenzymes.

[Psycho-oncology: psychological disorders during autologous bone marrow transplantation].

Use of BMT (bone marrow transplatation) has rapidly grown in the last few years. It extends to a variety of neoplastic illnesses and hematological malignancies. This procedure includes implicit appearance of many and important stressors, both physical and psychological, due to the illness and to chemotherapy treatments, among which the appearance and severe colateral effects during the stay in hospital has a special relevance. The need to consider the influency of predictive variables like, prior experiences, optimistic vs. pesimistic expectancies and the strategies of coping used in the adaptation through the treatment, are a central point in the study of the fluctuations of the different psychological responses and their interrelation with the physiology symtomatology which are present during the different phases of the process of BMT.
