Fiberendoscopy of the gastrointestinal tract in children. A series of 100 examinations.

This article reports the results of our first 100 examinations of the upper gastrointestinal tract and the colon in children and infants, using standard fiberscopes and also pediatric models. The procedure could be performed under sedation in 80% of the cases and general anesthesia was required mainly with standard fiberscopes. Compared to roentgenographic findings, fiberendoscopy proved to be more reliable in superficial mucosal lesions and gastrointestinal bleeding. Colonoscopy though difficult, is valuable in diagnosing rectal bleeding lesions and as a follow-up technique in inflammatory bowel diseases.

Fiberendoscopic monitorised dilatation of oesophageal strictures in children.

Oesophageal dilatations were performed 55 times in 10 children aged 2 months to 8 years presenting with acquired strictures of several origins. Classical bougienage with rubber material and newer technique with the Eder-Puestow's metallic olives system were used successively. The conjunction of the latter with systematic use of fiberendoscopy enhances considerably the safety of the procedure and permits better functional results.

[Radiation monitoring in the area of metallic dental replacement during cobalt 60 gamma radiation and in electron radiation of oral tumors].

In the radiotherapy of malignant tumors of the oral cavity, the exact distribution of radiation dosage in the neighborhood of metallic dental replacement has been uncertain so far. Many authors therefore asked that crowns and bridges be removed before radiotherapy. By means of lithium fluoride thermo-luminescence dosimetry, punctate measurements of doses are possible for the first time. In view of the dose values obtained, removal of metallic dental replacement before the beginning of radiotherapy as a prophylactic measure to prevent radioosteomyelitis appears not to be necessary.

Discharge of newly synthesized proteins in pure juice collected from the human pancreas. Indication of more than one pool of intracellular digestive enzymes.

The pancreatic secretion of 6 normal human volunteers was collected by endoscopic cannulation of the main pancreatic duct. The appearance of newly synthesized proteins was monitored at 1-min intervals after labeling with [75Se]methionine. The minimum transit time of these proteins from their site of synthesis to the acinar lumen was 36 +/- 8 min. Stimulation of protein secretion by a rapid intravenous injection of caerulein (40 ng per kg), 1 hr after [75Se]methionine administration, greatly decreased (by 73% on an average) the specific radioactivity of the discharged proteins. These data support the concept of a functional heterogeneity of proteins secreted by the human pancreas.

[Ulcer of the colon and Mallory-Weiss syndrome (author's transl)].

A 71 year old man presents with a high and low hemorrhage with no signs of associated shock. Endoscopy shows a laceration of the cardia (Mallory-Weiss syndrome). The low hemorrhage is attributed to a left colic lesion that appears to be an ischemic colitis. Colonoscopy in addition to baryum enema is useful in the differential diagnosis of colon lesions.

Pancreatic secretion of total protein and of three hydrolases collected in healthy subjects via duodenoscopic cannulation. Effects of secretin, pancreozymin, and caerulein.

Pancreatic secretion of total protein and of three types of hydrolases (amylase, lipase, and thymotrypsinogen(s) was determined in human subjects free of gastrointestinal disease. Pure pancreatic juice was collected by canulation of Wirsung's duct under duodenoscopy. Rapid intravenous injection of pancreozymin (1 Crick-Harper-Raper U per kg) stimulated secretion of total protein and hydrolases within 1 min. This stimulation was of short duration and had a half-life of 70 to 100 sec. Rapid intravenous injection of caerulein (20) ng per kg) induced a similar response. Parallel variations in total protein, amylase, lipase, and chymotrypsinogen(s) were observed in the three experiments where complete enzyme analyses were performed.
