Inhibition of human platelet function by parsalmide.

2-Propargyloxy-5-amino-N-butyl-benzamide (parsalmide), an antiinflammatory drug, inhibits human platelet function in vitro and ex vivo. The degree of inhibition of ADP- and collagen-induced platelet aggregation "in vitro" is dose-dependent and parsalmide shows a more marked effect than equal concentrations of acetylsalicylic acid (ASA) and indometacin. Moreover, malondialdehyde levels in human PRP after addition of thrombin were inhibited at the same degree by parsalmide, ASA and indometacin. In ex vivo experiments, the administration of 400 mg p.o. of parsalmide in arthropathic patients shows a significant inhibitory effect on ADP-induced platelet aggregation at 3 and 24 h after treatment, while a drop of malondialdehyde levels was observed only at 3 h after treatment. No significant inhibitory effect was observed in collagen-induced platelet aggregation.

Subaortic stenosis caused by two discrete membranes.

A case of subaortic stenosis caused by two discrete membranes is presented. Following demonstration of the two distinct sites of obstruction by intraventricular pressure gradients and angiocardiogram, the two membranes were excised through a transaortic approach. The importance of being aware of the occurrence of this rare anomaly is stressed, and its surgical implications are discussed.

Diagnosis and treatment of fungal infections in patients with hematologic malignancies.

A diagnosis of deep-seated mycosis was made in 54 patients with hematologic malignancies, severe neutropenia and fever, based on a set of clinical and laboratory criteria. Standardized antifungal treatment was started in 31 patients who seven days after onset of fever had not responded to antibiotics; the fungal infection was cured in 13, all of whom had a simultaneous remission of neutropenia, whereas the other 18 who did not respond to antifungal treatment, all had a falling or static neutrophil count. None of the 23 patients who were given no or inadequate antifungal treatment survived regardless of the neutrophil count and/or phase of the hematologic disease. We discuss the suitability of utilizing empirical criteria for a diagnosis of disseminated fungal infection as a basis for starting antifungal therapy in this type of patient.

Scanning electron microscopy of podocytes of the renal glomerulus of rats with nephrotoxic serum nephritis.

Nephrotoxic serum nephritis was produced in rats by intraperitoneal injections of rabbit nephrotoxic serum. Heterologous and autologous phases of nephritis were demonstrated by immunofluorescence and ultrastructural studies. Scanning electron microscopy showed changes in the surface of the podocytes, swelling of the arm-like cytoplasmic processes, and disappearance of the pedicles. The podocyte alterations are discussed.

Thrombosis prevention with ticlopidine after femoropopliteal thromboendarterectomy.

Fifty patients submitted to thromboendarterectomy in the femoropopliteal district for obstructive arterial disease of the lower limbs, documented angiographically, were randomly allocated to treatment with ticlopidine (500 mg/day) (T) or a placebo (P) in double-blind conditions. The treatment was started as soon as possible after the surgical intervention and was continued for six months or until a clinically evident reocclusion occurred. There were 46 patients available for assessment, 23 in each treatment group both of which were comparable in preoperative characteristics and type of surgery. Doppler ultrasonography at the end of treatment showed three cases of reocclusion of the operated segment in the T group versus six occlusions and seven significant stenoses in the P group (p = 0.003). Clinically, the active treatment significantly reduced the incidence of both acute ischemia during the treatment (four cases, all in the P group) and residual claudication at the end of treatment: in the T group five patients presented persistent or recurrent symptoms versus 16 in the P group (p = 0.001). Hemostatic function tests showed a marked inhibition of platelet activation in the T group. The treatment was well tolerated.

Essential thrombocythemia in pregnancy: report of four cases.

We report four pregnancies in four patients with essential thrombocythemia (ET). In two cases pregnancy resulted in spontaneous abortion at 6 and 11 weeks of gestation, respectively. The other two had a full-term, normal delivery; one of them received no therapy during the pregnancy, whereas the other was treated with acetylsalicylic acid from 11 weeks for signs of threatened abortion. No thrombotic or hemorrhagic complications were observed in any of the patients during pregnancy or the post-partum period. We believe that pregnancy in patients with ET represents a risk. Close monitoring is therefore indispensable and antiaggregant therapy might be of benefit.

Rosai-Dorfman syndrome with extranodal localizations and response to glucocorticoids: a case report.

A case of sinus histiocytosis with massive lymphadenopathy (Rosai-Dorfman syndrome) in a 42-year-old man is described. The disease involved the respiratory tract and skeletal apparatus, led to considerable general impairment and was unusually responsive to glucocorticoids. Molecular and immunophenotype analysis seem to confirm the reactive nature of a disorder that shows profound T-cell immunodeficiency.
