Oxidative capability of hepatic tissue in late sepsis.

It has been previously demonstrated that mitochondria isolated from the livers of septic rats display an oxidative phosphorylation capability similar to that of mitochondria from normal hepatocytes. The present study was performed to determine whether this is also true at the level of the whole tissue or was perhaps the result of mitochondrial isolation. To study this, sepsis in rats was produced by cecal ligation and puncture (CLP). 16-19 hr after CLP (late sepsis) rats were sacrificed and the livers removed and placed in ice-cold Krebs Ringer's phosphate buffer. Slices less than 1-mm thick were prepared and placed in the well of a temperature controlled O2 monitor. O2 consumption by liver slices was monitored in buffer equilibrated with 100% O2 (high O2) or room air (low O2). Slices from both septic and sham-operated rats exhibited high initial rates of respiration in high O2 environment (34.3 +/- 3.4 and 31.5 +/- 2.5 microliters/min/gm, respectively). Rates of oxygen consumption were significantly lower after 20 min in high O2 and initially in low O2. Under each environmental condition there was no significant difference in O2 consumption between liver slices from sham-operated and septic rats. The decline in O2 consumption in high O2 was not reversed by resuspending in high O2 buffer but was further inhibited by KCN. These results support the view that depressed O2 consumption in late sepsis is secondary to decreased O2 delivery rather than a primary deficit in oxidative capability of the hepatocytes.

[Meckel's diverticulum (author's transl)].

Complications (inflammation, bleeding, perforation) occurred in 20 of 38 patients with Meckel's diverticulum. Complications were disproportionately common in children up to ten years of age. Because of the high mortality rate in the presence of complications (20%) it is recommended to search actively for Meckel's diverticulum during every upper-abdomen laparotomy, as long as the underlying disease and the operative procedure allow it.

[Somatostatin in the treatment of acute experimental pancreatitis (author's transl)].

An acute pancreatitis was induced in 20 Göttinger mini pigs by implantation of a T-tube and application of contrast medium. This process was demonstrated by laboratory and microscopic findings. Continuous infusion (n=11) of 2500 mcg somatostatin (GHIF) over 10 h showed a significant reduction of amylase and blood glucose level corresponding to the microscopic findings. The group of animals treated with somatostatin (GHIF) showed a ratio of 1:5 between necrotic and unaffected parenchymal tissues. By contrast, the histologic findings of the untreated group showed a ratio of 1:1. The investigations of blood glucose, gastrin and glucagon levels showed a rebound effect after ending the treatment with somatostatin.

[Kaposi's sarcoma (author's transl)].

Pathological epidemiological and clinical aspects of Kaposi's Sarcoma are demonstrated. The prevalence of this disease in African countries and the appearance similar to the Burkitt-Lymphoma indicate a probability of viral origin. The histo-pathologic differential diagnosis also includes other vascular tumours. The disease shows a characteristic variable mixture of spindle cells and vascular components. In case of multifocal appearance surgery should be omitted, its aim can be only to ensure the diagnosis. A combined cytostatic and/or radiation therapy is recommended.

[The Caroli syndrome].

Caroli's disease is characterized by congenital pseudocystic dilatation of the biliary tree. Inflammation and stone formation lead to clinical manifestation in the second decade. ERC and computed tomography are main diagnostic procedures. In cases, where the alterations are limited partial hepatectomy has been recommended. If dilatations are diffuse, the aim of surgical treatment is the performance of a choledochojejunostomy to realize drainage and prevent recurrent ascending cholangitis.

[Relaparotomy or endoscopic surgery in complications after bile duct surgery].

The number of cases in bile duct surgery has continuously increased since second world war. Correcting operation is necessary in 5 to 7% after cholecystectomy. Since 1973 more and more the endoscopic treatment takes place of surgery. Typical complications can be divided into two groups belonging to their manifestation. Early complications are due to an insufficient intraoperative diagnosis or injuries of bile tract. Late complications comprehend successive development of duct stenosis and cholangitis. The therapeutic possibilities of endoscopic papillotomy are directed only to the papilla and near-by sections of the choledochus, while surgical treatment is due to the more difficult performance of iatrogenic lesions. The indications to each method are shown and discussed by 138 surgical and 714 endoscopic treatments.

[Acute pancreatitis in animal experiments - a new method].

Acute pancreatitis was induced in 30 minipigs (Göttingen) by implantation of a T-drain into the Ductus Wirsungianus. The distal part of the T-drain, which led into the duodenum was stenosed to 1/3 of the original diameter. The inflammation was caused by stenosis of the Ductus Wirsungianus additionally by application of contrast medium. The course of the inflammation was demonstrated by biochemical, roentgenological and histological examinations.

[Echinococcus cysticus (author's transl)].

Echinococcus cysticus is a parasite; the human organism is invaded by cysticercus of the dog tape worm. In liver 60% of all viable embryos are retained, 30% in the lung, the last 10% spread via the arterial system into all other organs. Diagnosis can be established by serology, sonography, scintigraphy as well as invasive and non-invasive radiological examinations. Case reports of 20 patients with Echinococcus cysticus are given; the disease was localized 16 times in the liver, 3 times in the lung and once in the spleen. Drugs or x-ray treatment have no sufficient therapeutical effect. Trivial trauma can cause rupture of the cysts; therefore surgery is indicated.

[Experimentally induced pancreatitis (author's transl)].

Pancreatitis was produced in 29 mini-pigs by partial occlusion of the pancreatic duct; occlusion was performed by implanting a T-drain into the duct. Inflammatory changes were induced additionally by applying radiopaque material in excess. Acute and chronic pancreatitic lesions could be diagnosed by X-ray examination, evaluation of blood chemistry and by histology.
