[Prognosis and surgical indications in acute pancreatitis. Contribution of abdominal x-ray computed tomography].

The authors have retrospectively studied 17 patients with acute pancreatitis to see if computed tomography (CT) findings were correlated with the clinical type of acute pancreatitis. This study permit to say that a normal CT scan is possible with an edematous pancreatitis. There is no necrotizing pancreatitis with normal CT scan. But, in 11 CT scan with severe appearance, 6 patients had an edematous pancreatitis of good prognosis. Several explanations are given, but the important thing is the lack of correlation that make CT scan of a bad prognosis value. Any way, CT scan is still the best examination for diagnosis of acute pancreatitis.

[Role of echography in the diagnosis and monitoring of hepatic alveolar echinococcosis].

By means of a review of a series of 30 cases of hepatic alveolar echinococcosis, the authors propose a classification of the various sonographic patterns; they point out the heterogeneous form which associate necrosis and calcification inside an hyperechoic mass and is the most frequent. The interest of ultrasonography for detection of lesions, its limits for diagnosis of extension, the contribution of percutaneous biopsy are discussed, as its place in the survey of the affection.

[Radiology in the diagnosis of complicated forms of annular pancreas in adults].

Radiologic findings are described in 4 cases of annular pancreas in adults. The value of double contrast exploration of the duodenum is emphasized, this showing the characteristic image of "curtain-loop" stenosis of the right D2 border with associated marked dilatation of the suprastenotic duodenal zone. Ultrasound imaging shows features suggestive of the liquid distended genu superius. The onset of acute pancreatitis on an annular pancreas leads to images of pseudo-cystic necrosis and/or calcifications suggestive of the diagnosis because of their localization on ultrasound or C.T. examination.

[Evaluation of the role of angiography in the diagnosis and surveillance of hepatic alveolar echinococcosis].

The authors recall the main pathologic and physiopathologic data necessary to understand the angiographic patterns of hepatic alveolar echinococcosis, by means of a rereading of a personal series of 37 cases. They show that this affection has a diagnostic angiographic shape which associate four signs: avascular aspect of the lesions, regular invasion of vessels, presence of a collateral circulation, absence of neovascularisation. Digestive angiography stays a good diagnostic method for the parasitosis, when epidemiological context, ultrasound or CT scan gave no explicit conclusion. It keeps an interest in the preoperative evaluation and in the supervision of portal vein.

[X-ray computed tomography in the study of anterior instability of the shoulder. Study of a series of 23 cases].

The place of standard CT scan imaging more particularly when combined with double contrast arthrography, is defined in the preoperative screening of patients with anterior instability of the shoulder. The joint scan provides complete data on the anatomic elements of the shoulder lesion, particularly with reference to the glenoidal labrum and the anterior capsular structure. It is also of major diagnostic value in painful subluxations.

[Generalized adenomyomatosis of the gallbladder: ultrasonographic appearance in one case (author's transl)].

Ultrasonographic images were correlated with radiological and pathological findings in a case of generalized adenomyomatosis of the gallbladder. Emphasis is placed on the importance of the methods employed for filling the biliary tracts with opaque medium, to confirm a suspected diagnosis suggested by the heterogeneous irregular thickening, with a fringed internal limit, of the gallbladder wall.

[Riedel's lobe. Echotomographic aspects].

The acquired or congenital nature of Riedel's lobe, a rare anomaly of hepatic lobulation, is a subject open to controversy. Normally asymptomatic, it presents as a palpable mass in the right hypochondrium leading to possible confusion with an hepatic or a renal tumor. Correct diagnosis can be established by the characteristic findings an ultrasonography. The typical anatomical and clinical features of this hepatic morphological variant are discussed in relation to ultrasound recordings obtained from three patients with a Riedel's lobe.

Cardiac catheterization: dosimetry on patients.

Our study was made on 49 patients and we used capsules of lithium fluoride. The average fluoroscopy time was 32 min. and the average cinema film length was 76.5 metres. The relationship between the entry dose, the fluoroscopy time and the length of cinema film allowed us to calculate a fluoroscopy equivalent time (F.E.T) for each patient which was of 51 min. (32 min. + 76/4) as radiation dose of one minute fluoroscopy equal radiation dose of 4 metres of cinema film. The average heart dose was 0.70 X 10(-3) X FET (GY) or 70 mr/min. There was an absence of relation of the exit dosage to the diameter of the patient and a proportionality of the heart dose to the exit dose. Furthermore the maximum eye dose recorded was 200 mrads and the maximum gonadal dose was 350 mrads.

Cerebral computed tomography in premature infants, with an attempt at staging developmental features.

Cerebral computed tomography of 45 infants has been carried out for a better assessment of the normal appearance of the different anatomical structures. The skull vault is often asymmetrical, the dura mater is very dense, and the ventricular system is small in contrast with the large subarachnoid spaces. The brain parenchyma contains zones of low attenuation, mainly frontal. These may well be a normal developmental feature.

Oral cholecystography: is the fatty meal always necessary?

In our study we have collected 1000 consecutive cases of oral cholecystographic opacification of the gall-bladder and common bile duct; occasionally a complimentary intra-venous cholangiography had also been carried out. In 360 cases there was disease of the gall-bladder or a faint opacification of the common bile duct was abnormal in only three cases, which is a rate of 0,47%. Therefore the fatty meal after oral cholecystography should not be done in each case but only if there is a presumptive diagnosis of pancreatic disease, if there is pancreatic calcification, or if surgery has been previously performed on the biliary tract or duodenum.

[The value of arteriography in the diagnosis and treatment of post-traumatic secondary impotence (author's transl)].

Angiographic investigations in 9 patients with partial or total post-traumatic secondary impotence present for several years, confirmed the presence of traumatic vascular lesions involving, in nearly all cases, the artery of the penis or its branches (cavernous and bulbar arteries). Selective angiography of the pudendal arteries was able to guide treatment in 5 of these patients. Epigastrocavernous anastomoses, according to the microsurgical procedure developed by Michal, were performed with excellent results in 3 cases.

Alcohol dependence among patients admitted to psychiatric emergency services.

We assessed the prevalence of alcohol dependence among patients examined in the psychiatric emergency service of a general hospital. We compared socio-demographic data and psychiatric status of patients with and without alcohol dependence. One-hundred and four consecutive patients received by the psychiatric emergency service of Bichat-Claude Bernard Hospital (Paris, France) were assessed. Diagnosis of alcohol dependence, acute alcohol intoxication, and antisocial personality was determined according to DSM-IV criteria. Other psychiatric disorders were identified using a structured psychiatric interview, the Mini International Neuropsychiatric Interview (MINI). Prevalency rate of alcohol dependence was 37.5% among patients examined by the psychiatric emergency service. Alcohol-dependent patients were more often men than women and more often unemployed than non-alcohol-dependent psychiatric emergencies. They presented more dysthymia, acute alcohol intoxication, and antisocial personality than non-alcoholic patients followed by the psychiatric emergency service. Attempted suicide was as frequent in alcohol-dependent patients (23%) as in other patients (29%). Alcohol-dependent patients consumed alcohol more often when alone, and their alcohol consumption began more frequently in the morning. Patients seen in a psychiatric emergency service must be identified as a population at risk for alcohol dependence (37.5%). Alcohol-dependent patients are more often men and have a higher rate of unemployment. They present significantly more often dysthymia and acute alcohol intoxication associated to alcohol dependence.

Alveolar echinococcosis of the liver: MR findings.

Nineteen cases of proven hepatic alveolar echinococcosis were examined by magnetic resonance (MR) and the results were compared with CT. Fibrous and parasitic tissue showed low signal both on T1- and, generally, on T2-weighted images. In a few cases a high signal on T2-weighted images may be observed, due either to central necrotic zones or to small peripheral cysts. Central necrosis was more easily identified by MR than by CT. However, MR seemed to be less effective than CT in allowing us to reach a positive diagnosis, due to its inability to show microcalcifications. In addition, MR may not reveal small lesions. In most cases T1-weighted images revealed more clearly than CT did the margins of the lesions and the hepatic extension, especially to hepatic veins, vena cava, and perihepatic spaces.

MR patterns of dialysis arthropathy.

Ten patients (average age 51 years) on long-term hemodialysis (average duration 13.5 years) were examined by magnetic resonance (MR) (all cases) and CT (five cases) for cystic radiolucencies of the wrist, shoulders, and hips. MR and CT revealed more lesions of smaller size than plain films and both showed a constant communication with the joint space. Synovial hypertrophy was generally absent or very mild even in the case of large osseous erosions. The MR analysis of the content of the lesions in the wrist was quite variable: low signal on T1- and T2-weighted images (12 of 24), low signal on T1- and high signal on T2-weighted images (10 of 24), and high signal on T1- and T2-weighted images (2 of 24). The patterns of transplanted (four cases) or ungrafted (six cases) patients were indistinguishable. These results suggest an articular origin of the lesions, but different from synovial processes such as rheumatoid arthritis, and confirm their probable multifactorial pathogenesis.

[Study of the small bowel and mesentery in Crohn's disease: comparison of ultrasonics, x-ray computed tomography and small bowel transit in 18 patients].

The advantages of tomodensitometry and ultrasonography were compared with those of small bowel series in Crohn's disease of the small bowel in 18 patients. These two examinations give results which are inferior to those of small bowel series, for the positive diagnosis and the evaluation of the intraluminal extension of the disease, since their sensitivity was only 60 p. cent in identifying the pathological loop. On the contrary, their advantage lies in the evaluation of the intraluminal extension of the disease, which was correctly demonstrated with ultrasonography in 22 p. cent of cases, and tomodensitometry in 45 p. cent. This confirms the supplementary value of these two examinations in the study of the mesenteric involvement of the disease, screening of abscesses, and makes desirable a prospective evaluation of the influence of mesenteric abnormalities on the prognosis and surgical indications.

[Fractures caused by bony insufficiency of the sacrum. Clinical, radiographic, scintigraphic and x-ray computed tomographic symptomatology].

Fracture of sacrum due to bone deficiency is not a rare lesion and must be considered as a possible differential diagnosis of unexplained pelvic region pain. The various clinical semeiology elements are discussed in relation of 20 cases, together with circumstances of onset, enabling definition of a population at risk: elderly women with a bone metabolic disorder and/or a congenital or acquired architectural anomaly. Radiographic, scintigraphic and CT scan images are sufficiently characteristic for these non-invasive exploratory procedures to provide an exact diagnosis and the possibility of early treatment.
