[Selective arteriography of the supra-aortic trunks by right humeral approach with a 4F catheter].

Three hundred and sixty seven patients were studied for cerebrovascular disease with intra-arterial digital subtraction. Examination were performed using a right transbrachial approach with a 4F catheter. Selective catheterization of the carotid arteries was possible in 98% for the left and 99% for the right. Image quality was consistently good or excellent. There was 2 neurologic complications and 1 local complication (hematoma) requiring surgery few weeks later. Selective carotid and vertebral catheterizations by the brachial route is safe, simple, accurate and rapid (15 mn) and can be employed in outpatients.

[Computerized tomography in the diagnosis of craniomandibular disorders: radiological-surgical comparison].

Among 500 C.T. scan of temporomandibular joint (T.M.J.), examined since 1982 by bilateral direct sagittal method (Department of Radiology, Pr. A. TREHEUX, C.H.U. Nancy-Brabois), the authors have retained 14 cases of patients with symptoms related to dysfunction of the T.M.J. cured by surgery (Department of Maxillo Facial Surgery, Pr. STRICKER, C.H.U. Nancy). These cases were chosen among hundred patients annually examined by C.T. scan, for various diseases (dysfunctions of the T.M.J., traumatisms, infections, inflammatory diseases...). These correlations between radiology and surgery about 26 T.M.J. (2 patients underwent surgery only on one side) were: an accuracy with surgical findings for 19 cases (76%); in 6 cases (23%), a meniscus anteriorly displaced, non detected by C.T. scan was found by surgery; 2 cases of meniscus perforations (one in the frontal plane, the other sagittal) were surgical findings; in 1 case, a displacement was under-valued by C.T. scan; in 3 cases, arthrosic changes (1 case of REITER syndroma) were characterized by C.T. scan. The authors emphasize the value and the limits of evaluation of the internal derangements of the T.M.J. with direct sagittal C.T.

Cyst formation in gastric heterotopic pancreas: report of two cases.

Cyst formation is an uncommon pathologic finding in heterotopic pancreas. It is sometimes reported in a duodenal location but very rarely in a gastric location and leads to very large lesions of unusual endoscopic and radiographic appearance. Two cases of gastric heterotopic pancreas are described in which cystic zones in the gastric wall were found at preoperative ultrasound or computed tomography.

Spinal involvement in alveolar echinococcosis: assessment of two cases.

Two cases of spinal involvement in alveolar echinococcosis are reported. Conventional radiographs showed bone lysis and spondylitis; computed tomographic scans showed detail of local spread to the ribs, other vertebrae, and soft tissues. Differential diagnosis, which includes other infections and hydatidosis, is difficult, and the diagnosis may be suggested by evidence of a primary hepatic focus, geographic propensity to the infection, and laboratory findings.

[Extension to the thoraco-abdominal wall of actinomycoses. Value of computed tomography].

Four patients with deep visceral actinomycosis infections presented evidence of extension to the thoracic or abdominal wall. Investigation of the chronic parietal suppuration by CT scan imaging allowed definition of the nature and extent of parietal infiltration, and notably to demonstrate its anatomical continuity with the often unrecognized deep focus: chronic lung disease, appendicular or colon mass, chronic genital infection in patients with intra-uterine device.

[Tuberculous trochanteritis: value of ultrasonic diagnosis and x-ray computed tomography. Apropos of 3 cases].

Trochanteritis is a rare osteoarticular site of tuberculosis which initiates within the serous bursa of the gluteus maximus and extends only secondarily to the greater trochanter. Its course is a slow one. Diagnosis is usually delayed but should be made earlier by the use of ultrasound and the CT scan. These provide details of the bone remodelling detected on standard radiography, and show the presence of a variable fluid collection of a topography that attaches it to the serous bursa of gluteus maximus, and provides a guide to diagnostic needle puncture for biopsy. Overall data allow improved adaptation of medico-surgical treatment.

[Tumefactive biliary sludge. Ultrasonic aspects].

Twenty two patients presenting a typical sonographic pattern of tumefactive biliary sludge are studied. In all cases was observed a polypoid mobile, rounded mass, without acoustic shadow. The lesion stayed in the gallbladder in 15 cases and in distended main bile duct in 3 others. There was a double localisation in 4 cases. Seven verified cases proved to be constituted of very thick biliary sludge.

[X-ray computed tomography and synovial chondrometaplasia].

Scanography allows diagnosis of radiotransparent intra-articular foreign bodies and establishment of their synovial origin, sometimes even in the absence of joint opacification. It provides an overall assessment of the joint that can be improved by arthrography. Clinical and standard radiologic pictures of primary synovial chondrometaplasia (synovial osteochondromatosis) and intra-articular foreign bodies can cause confusion if the latter are modified by secondary chondrometaplasia. Scanographic analysis of islets of chondrometaplasia suggests their primary nature in the presence of plaques of calcification of a small size, of low density and diffused throughout the image while peripheral, lamellar, dense calcifications are suggestive of secondary lesions.

[Adrenal metastases of bronchial cancer: value of x-ray computed tomography].

Up until now, adrenal metastases from bronchial cancer have always been an autopsy diagnosis with a frequency as high as 45%. These metastases can now be diagnosed by means of computed tomography of the adrenal glands and most of the recent studies report an incidence of 10 to 15% of adrenal metastases independently of the histological type of bronchial cancer. The authors stress the need to perform this examination in the context of pre-treatment staging and for the follow-up of bronchial cancer. The outcome of these adrenal metastases is still poorly understood, but prospective studies could provide more information concerning their prognostic significance. In cases of operable bronchial carcinoma with an isolated adrenal metastasis, it is important to be able to detect this lesion which may be able to be excised prior to the thoracic surgery.

[Echographic and x-ray computed tomographic aspects of pneumatosis cystoides of the colon in the adult. Apropos of a case].

A case of pneumatosis cystoides Coli located in the sigmoid in an adult is reported, with emphasis on US and CT aspects. CT patterns of pneumatosis cystoides Coli are highly significative; particularly attenuation values measured in the endoluminal polypoid images are characteristics and represent an interesting contribution to the diagnosis of this disease.

[Radiologic imaging of annulo-ectasia disease of the aorta. Experience with 26 cases].

The authors describe the radiologic appearance of annulo-aortic ectasia in 26 patients who had plain chest radiography and aorto-seriography. Two also underwent C.T. Aortography is the most performing method to appreciate the extent of aortic ectasia, the perturbations of the aortic root and to diagnose acute or chronic dissections. Aortography remains a most valuable complement of echocardiography for preoperative staging.
