Acral lentiginous melanoma. A clinicopathologic entity.

Acral lentiginous melanoma (ALM) is the fourth clinicopathologic variant of malignant melanoma. It occurs on volar surfaces of hands and feet, subungual sites, and fingers or toes. It is characterized by slow lentiginous radial growth and central plaque-like thickening, heavily pigmented tumor cells, markedly thickened papillary dermis, and diffuse reticular infiltration. Lesions are unusually large and, in most cases, thick and ulcerated. There were 180 patients with acral melanoma (AM), which includes 67 in whom the specific features of ALM could be documented. One hundren sixty had primary lesions on ;the foot, and 20 occurred on the hand. There were 104 men and 76 women. There were 41 black patients and 139 whites. Five-year survivals following all modalities of therapy in 122 patients with Stage I acral melanoma is 63% for planter/palmar lesions, 58% for subungual lesions, and 27% for skin of digits. For the subgroup of Stage I patients with ALM treated by surgery and regional chemotherapy by perfusion, the five-year survival for all sites is 72% and 56% at 10 and 15 years, respectively. Survival in ALM is essentially the same as for all AM lesions.

Bacteremia in children following upper gastrointestinal endoscopy or colonoscopy.

Fifty children undergoing upper gastrointestinal endoscopy and 25 undergoing colonoscopy were studied prospectively with aerobic and anaerobic blood cultures for the development of bacteremia. Twenty-six of the endoscopies and all the colonoscopies were done under general anesthesia. Cultures were obtained before, at 5 min, and at 30 min after the procedure. Only a single positive blood culture was obtained in an upper endoscopy patient. All cultures from the colonoscopy patients were negative. Biopsy or polypectomy were not important variables in the development of bacteremia. It is concluded that the risk of bacteremia in children following upper endoscopy or colonoscopy is minimal and should not be considered a contraindication to their performance.

Mixed invasive infection with Alternaria species and Curvularia species.

Alternaria and Curvularia species were isolated from the nasal septum of a neutropenic patient and tissue invasion was documented histologically. The patient recovered without the use of systemic antifungal agents following surgical excision of the nasal septum and spontaneous resolution of neutropenia.

Effects of lithium carbonate and haloperidol on cognition in aggressive hospitalized school-age children.

A comparison of the effects of lithium, haloperidol, and placebo on cognition is reported for a sample of hospitalized school-age children with a behavioral profile of aggressiveness and explosiveness. In this double-blind study, patients were randomly assigned to one of three treatment conditions. The cognitive battery was administered at the end of a 2-week placebo baseline period and again after 4 weeks of treatment. It included a simple reaction time (RT) task with preparatory intervals of 1, 4, and 8 seconds, the Porteus Mazes, and the Matching Familiar Figures Test. Drug effects on cognition, when found, were mild. Slower and more variable RTs were found on the RT task in the haloperidol group (mean dose, 3.1 mg/day), particularly at the 4- and 8-second preparatory intervals in comparison to placebo. This appeared to reflect decreased ability to hold a preparatory set. No other effects of haloperidol on cognitive performance were found. Lithium carbonate (mean dose, 1150 mg/day) caused a deterioration in qualitative performance on the Porteus Maze Test when compared with haloperidol but had no effect on test quotient scores or on the other cognitive measures. Results are discussed in terms of dose effects and the influences of task demands. This is part of a study critically assessing the effects of lithium and haloperidol on behavioral symptoms and other parameters.

Growth of the fetal pigtailed macaque (Macaca nemestrina) I. Cephalofacial dimensions.

The prenatal growth of the macaque cephalofacial region is described using cephalofacial measurements taken on 89 fetal Macaca nemestrina whose known gestational ages range from 60 to 184 days. During the prenatal period, the macaque neurocranium flattens and elongates in an anteroposterior direction; the height of the upper face increases at a faster rate than facial breadth, and the ear dimensions grow at a faster rate than any other cephalofacial dimension. Formulae based on the independent variables of ear height and bizygomatic diameter are recommended for estimating fetal age in this species.

Pharmacotherapy for autistic children: a summary of research.

An overview of research involving pharmacotherapy in infantile autism is presented. Methodological issues relating to experimental design, rating instruments and ecological factors are considered. Classification of infantile autism and diagnostic problems are discussed. Research investigations conducted in order to define diagnostic subgroups in the etiologically heterogeneous population of autistic children are described. An attempt is made to relate biochemical findings to clinical drug response. Recent findings are presented indicating that a potent neuroleptic is able to yield simultaneously significant decrease of behavioural symptoms and improved learning under both clinical and laboratory conditions. The drug can be effective in conservative doses and administered over a period of 14 weeks without untoward effects. New research plans are introduced where attentional learning will be assessed in an operant conditioning paradigm using automated procedures.

In vitro assessment of antineoplastic therapy. New indication for thoracotomy?

Selection and determination of the efficacy of antineoplastic agents has been dependent upon the trial and error method of observing measurable disease. Such methods subject the patient not only to loss of precious time but to needless toxicity if the drug is ineffective. The clonogenic assay, an in vitro assessment of tumor cell sensitivity to antineoplastic agents, has the potential for individualizing therapy. In this assay, tumor cells exposed to various drugs are cloned in soft agar. In the 16 primary and 24 metastatic pulmonary tumors tested with this technique, a growth rate of 80% was achieved. Fifty-five percent of the primary tumors and 60% of the metastatic lesions responded in vitro to one or more of the test drugs. There were twelve possible correlations between in vitro and in vivo results. In four of 12 assays, in vivo sensitivity was predicted and three of four patients demonstrated a clinical response. No drug that was inactive in vitro had activity in vivo. Prior knowledge of in vitro sensitivity may dictate a more aggressive surgical approach to pulmonary metastatic disease, whereas in vitro resistance would call for more conservative treatment. Just as with estrogen receptor status in breast cancer, data derived from the clonogenic assay may ultimately be of such import that thoracotomy would be warranted solely for the purpose of obtaining tissue for the assay.

Sensory preference and overselective responding in autistic children.

Five autistic and five normal children were allowed to register a sensory modality preference by bar pressing to select either a visual (slides) or an auditory (music) stimulus. The children were then taught a discrimination between the presence or absence of a compound auditory-visual stimulus (white noise and light). Testing for stimulus overselectivity revealed that the autistic children attended to only one aspect of the compound stimulus. In all cases this was the sensory modality that was selected during the preference test. Significant correlations were obtained between Gesell developmental scores and degree of overselectivity. Normal children registered an equal preference for music and slides and displayed no overselectivity.
