[Appendicular peritonitis: comparative study of antibiotic therapy and a topical bactericidal agent].

Fifty four patients operated on for appendicular peritonitis where randomly divided. In the first group intravenous Dalacin C and Spectacilline were given during five days postoperatively; in the second, Taurolin, a bactericid, was administered by intraperitoneal route. There was a statistical difference in either treatment, neither on intraperitoneal nor parietal septic complications. The use and way of prophylactic antibiotic administration are discussed.

Administration, Topical↗

[EEA stapler in colon surgery].

The EEA stapler, for accomplishing stapled anastomoses in the gastrointestinal tract seems of a great interest. It opens new possibilities in case of carcinoma of the middle part of the rectum. It also makes the re-anastomose after a Hartmann procedure much easier. The results of 63 anastomoses on the colon or the rectum are reported.

Adenocarcinoma↗

[Defense in favor of the Hartmann operation].

Hartmann's operation has been too frequently underrated. The new reconstructive potential offered by the EEA stapler is steadily widening the indications for Hartmann's operation. In the treatment of complicated acute diverticulitis in particular. Hartmann's operation significantly lowers postoperative mortality and morbidity. The results in 130 cases of acute diverticulitis undergoing emergency surgery are discussed.

Adult↗

[Recto-vesical fistula (author's transl)].

23 cases of recto-vesical fistula are described; their diagnostic and therapeutic approach are discussed. The entero-vesical fistula result mainly from diverticular disease but may be secondary to tumor or Crohn's disease of the large bowel too. The treatment needs allways surgery: bowel resection and bladder extraperitonisation. Results and literature are compared.

Adult↗

[Cimetidine prevention of stress ulcers].

A study of 2015 cases from an intensive care unit shows that prophylactic administration of cimetidine and antacid produces a statistically significant decrease in stress ulcer, as compared with antacid alone or sulpirid and antacids.

Antacids↗

[Echographic diagnosis of cholelithiasis].

Ultrasonography of the gallbladder is a simple diagnostic tool in a clinical situation suggesting the presence of gallstones. The radiologic-surgical survey of our one hundred most recent cases reveals no false-positives and only very few false-negatives. These results are discussed.

[Transvaginal drainage in rectum amputations].

In cases of abdomino-perineal excision of the rectum, a monobloc excision of the posterior vaginal wall and the rectum is performed. The flaps thus formed are rotated and sutured to the side-walls of the cavity. The perineal fat is approximated and the skin sutured. The "new vagina" is packed. The vaginal epithelium covers in less than three weeks the posterior raw area reconstituting a normal vagina. The packing is irrigated daily with an antiseptic. Postoperative cares are more easier.

Drainage↗

[Stercoraceous perforations of the colon. 2 unusual presentations].

Two cases of stercoraceous perforation of the colon are described in a psychiatric patient and a hemodialysis patient. The unusual symptomatology was acute respiratory failure with hypovolemic shock in one case and acute rectal bleeding in the other. The etiology, clinical manifestations, and treatment are discussed.

Colon, Sigmoid↗

[Starch peritonitis].

Starch peritonitis is a rare postoperative complication of which 3 cases are here reported. The pathogenesis, diagnostic procedures, symptoms, treatment and prevention of starch granulomatous peritonitis are discussed.

Female↗

[Volvulus of the colon].

Twenty-two cases of volvulus of the colon are reported. The conservative and surgical methods of treatment are discussed. It should be emphasized that a preliminary X-ray study of the colon with Gastrografin is necessary to identify the nature and site of obstruction and any associated lesions.

Aged↗

[Pleural empyema and Tauroflex. Study of a new bactericidal agent].

A pilot study of 8 cases of lung empyema treated with taurolin, a new bactericidal agent, is reported. Rapid sterilization of exudates and favorable clinical course are discussed. Thanks to the absence of toxicity and its bacteriological efficacy, taurolin is a useful therapeutic agent in pleural empyema. Its use in the prophylaxis of complications in thoracic surgery is suggested.

Anti-Infective Agents, Local↗

[Results of a fast colic preparation].

Experience with whole-gut irrigation as a method of bowel preparation in 100 patients undergoing surgery is described. The use of this technique, the physiological records and the results are very satisfactory. Bowel movements start on the 3.2 postoperative day; stools are passed on the 5.7 day. Despite the absence of antibiotic prophylaxy, there were only 3.7% anastomotic leakage and 3.4% wound septic complications.
