Technology acquisition: trends in imaging.

A great deal has happened in the imaging area this year. Our imaging special report examines changes in the field: from hospitals' new negotiating tactics to the impact of the new Medicare capital payment regulation. In an in-depth report, we examine new trends developing in hospitals' and hospital groups' contracting for imaging equipment purchasing. Then we take a look at how hospitals' technology acquisition strategies--and vendors' research and development plans--are being remolded by the new capital regulation. Finally, we revisit the capital fold-in regulation itself.

Questions about geographic reclassification spur debate.

The recent uproar over problems with Medicare's hospital geographic reclassification process has produced few long-term solutions so far. Experts say the large number of hospitals being reclassified indicates that the current methods are inadequate. They hope the current debate eventually will lead to a broader solution. The recent problems "certainly seem to have strengthened the resolve to see if there is an answer everyone can agree on," says one observer.

Outpatient bundling regulation would be costly for hospitals.

A new federal outpatient bundling regulation could plunge hospitals into a costly billing and administrative nightmare. Among its provisions, hospitals would be required to bill Medicare for diagnostic procedures or tests provided to outpatients by outside suppliers--even if provided outside hospitals' outpatient departments.

Health care heats up as potential presidential campaign issue.

Health care professionals and election analysts agree that health care will be a bigger issue in the 1992 election than it was in 1988. Among the reasons: Some prominent members of Congress are active in health care issues, media coverage of health insurance problems is growing, and several major health care reform bills have been introduced in Congress.

Formation and properties of smooth muscle myosin 20-kDa light chain-skeletal muscle myosin hybrids and photocrosslinking from the maleimidylbenzophenone-labeled light chain to the heavy chain.

Experimental conditions which permit the exchange of smooth muscle 20-kDa light chain into skeletal muscle myosin are described. The hybridization does not result in the regulation of actin-activated ATPase activity of the hybrid myosin by smooth light chain phosphorylation. Further, the KCl dependence of the Mg-ATPase activity of the hybrid was similar to that of skeletal muscle myosin. The dephosphorylation of the smooth light chain in the hybrid did not induce a conformational change in the hybrid from the 6 S to the 10 S state, thereby indicating that the conformational transition is dependent also on the nature of the heavy chain subunit. Exchange of the smooth light chain premodified at its Cys-108 by photolabile 4-(N-maleimido)benzophenone and photolysis resulted in crosslinking to the heavy chain subunit. Immunopeptide mapping using a monoclonal antibody against residues 1-23 at the N-terminus of the skeletal muscle myosin heavy chain identified the location of the photocrosslinking site to be beyond 92 kDa away from the N-terminus.

Employers, insurers collaborate on quality-measurement study.

A dozen employers and their managed care plan partners are participating in a new study to test whether patients' perceptions of their health status can help to measure quality of care in managed care settings. The study, which the participants say is the largest of its kind, illustrates the growing efforts by employers throughout the United States to demonstrate the quality of health care offered to their employees.

HCFA considers gradual approach to outpatient payment reform.

Government officials may advocate incremental reform of Medicare's outpatient payment system over more drastic measures, sources say. The Health Care Financing Administration has not yet made a decision on its recommendations for reform, which are due to Congress in September. Sources at HCFA say a full-scale reform plan may not be feasible now. Meanwhile, the Prospective Payment Assessment Commission, the congressional advisory panel responsible for reviewing HCFA's recommendation, has until March 1992 to turn in its report. Thus, October 1992 would be the earliest date for implementation. But some experts say even that's optimistic.

Capital fold-in makes payment forecasting difficult for hospitals.

The public review period for the Health Care Financing Administration's proposed regulation to fold capital payments to hospitals into the prospective payment system isn't over yet. But analysts are already saying HCFA's plan would hamper hospitals' ability to predict Medicare capital revenues. "The ability to make a long-range judgment about the regulation is difficult because we don't know the long-range picture. Even what they've [government officials] laid out for the first year is only an estimate," notes Paul Rettig, executive vice president of the American Hospital Association.
