Histological changes, rhinoscopical findings and nickel concentration in plasma and urine in retired nickel workers.

Histological examination of nasal biopsy specimens, rhinoscopical evaluation and nickel analysis in plasma and urine were performed on 59 retired nickel workers. The histological characteristics were graded according to a numerical classification table. Workers with short employment and short retirement time had a significantly lower mean histological scores than those with long employment and long retirement. Thirteen persons (22%) showed epithelial dysplasia, possibly representing precancerous lesions. Also included in the material were four cases of nasal carcinoma. Presence of hyperplastic/polypoid nasal mucosa was inversely related to the duration of retirement, possibly reflecting gradual regression of the mucosal swelling after termination of the irritation. The nickel concentration in plasma and urine was significantly higher among former roasting/smelting workers than in former electrolysis and non-process workers. This is probably a consequence of the slow release of heavy soluble nickel compounds that have accumulated in the organism at the former place of work.

Sinonasal cancer and exposure to softwood.

In order to study the connection between occupation and sinonasal cancer we have reviewed the files and histological sections of 70 patients (24 females and 46 males) treated at the Department of Oto-rhino-laryngology, National Hospital of Norway. Detailed information concerning previous occupations was obtained by telephone interviews according to a standardized questionnaire. This pilot study revealed a strong association between wood dust exposure and sinonasal cancer. Of 12 wood dust exposed men, 11 had been exposed exclusively to softwood. While squamous cell carcinoma was the predominant type of cancer in joiners, carpenters and loggers, non-Hodgkin lymphomas appeared to be associated with employment in saw- and planingmill firms. A nationwide case-control study is under preparation for further substantiation of the health hazards connected with exposure to softwood and other possible occupational factors related to sinonasal cancer.

The nasal mucosa in softwood exposed furniture workers.

Recent reports suggest that softwood exposed woodworkers may have an increased incidence of sinonasal carcinoma. The present study was undertaken in order to evaluate the histological changes, especially the presence of possible precancerous lesions, in the nasal mucosa of furniture workers exclusively exposed to softwood. Histological examination of nasal biopsies from 44 furniture workers and 37 controls revealed a higher degree of metaplastic changes in the former group. In addition we observed four cases (9%) of dysplasia among softwood exposed workers. Nasal epithelial dysplasia is morphologically similar to dysplasia in other organs where the precancerous state of this lesion has been proved. Acceptance of nasal dysplasia as a precancerous lesion means that histological examination of biopsies is an appropriate tool in identifying occupational groups with an increased incidence of sinonasal carcinoma.

Intracytoplasmic lumina with and without cilia in both normal and pathologically altered nasal mucosa.

Two different types of intracytoplasmic lumina have been observed electron microscopically in pseudostratified and metaplastic human nasal mucosa: one type contains microvilli and the other contains both microvilli and cilia. The latter type was seen only in pseudostratified columnar epithelium, while the former was also observed in different stages of metaplasia. From these observations we conclude that the type of intracytoplasmic lumina is determined by the differential potential of the cells in which they occur. Serial sectioning demonstrated that these structures sometimes communicate with the extracellular space. The distinctly different ultrastructure of microvilli lining the intracytoplasmic lumina and the foldlike extensions of the cell membrane favors an intracellular genesis rather than a formation by invagination of the cell membrane. It is suggested that intracytoplasmic lumina occur as a consequence of dysregulation in the process of epithelial maturation.

The invasive front of carcinomas. The most important area for tumour prognosis?

Various molecular events of importance in tumour spread, like the gain and loss of adhesion molecules, secretion of proteolytic enzymes, increased cell proliferation, and the initiation of angiogenesis occur at the tumour-host interface (invasive front). We have hypothesised that molecular or morphological characteristics at the invasive front area of various carcinomas may reflect tumour prognosis better than other parts of the tumour. Consequently, we recently developed a simple malignancy grading system restricted to the deep invasive front area of head and neck squamous cell carcinomas. This grading system proved to have additional prognostic value over the established prognostic factors. All similar studies performed so far have confirmed the high prognostic significance of the invasive front grading in squamous cell carcinomas at different locations. In this review paper we describe the system and the hypothesis on which it has been developed. The reproducibility of the grading is acceptable for further extended studies. Interestingly, observations of similar invasive front alterations in different adenocarcinomas suggest that the invasive tumour front may underlie the biological aggressiveness of carcinomas of glandular origin, as well.
