Peripheral neovascularization complicating rhegmatogenous retinal detachments of long duration.

Nine eyes in nine patients with rhegmatogenous retinal detachments of 1-18 years, duration showed clinical evidence of significant retinal neovascularization in the equatorial region. The retinal new vessels showed a sea-fan configuration. Their extent in the periphery of the fundus ranged from 10 degrees to 180 degrees. All retinal detachments were associated with round atrophic retinal holes in the equatorial region. Retinal reattachment was achieved in all eyes with scleral buckling. The retinal new vessels totally regressed within 15 days to 3 months after retinal reattachment. It is believed that the peripheral retinal new vessels were secondary to retinal hypoxia resulting from decreased retinal blood flow in the detached retina.

Evidence for serotonin (5HT) binding sites on murine lymphocytes.

The binding of 3H-labeled serotonin (or 5-hydroxytryptamine: 5HT) to mouse lymphocytes was investigated. It was shown to be highly specific, time-dependent, saturable and partly reversible. Saturation analysis demonstrated a Kd of 198 nM and B max of 3.53 nM. We studied receptor specificity by using different types of serotonin antagonists, and numerous other substances. Serotonin was found to be the most effective drug among those tested in inhibiting the binding of 3H-5HT, having an IC50 of 194 nM. The fact that 5HTP, a 5HT precursor, had no inhibitory capacity indicated the high specificity of these 5HT binding sites. Dopamine was somewhat able to competitively inhibit 5HT fixation (IC50 = 27,000 nM), whereas norepinephrine and histamine had no effect. Lastly, we investigated the cellular specificity of this binding, and observed that nonmacrophage peritoneal cells extensively bound serotonin under the same conditions as spleen cells. This is the first direct demonstration of 5-hydroxytryptamine receptors on mouse lymphocytes. The presence of these binding sites can contribute to the understanding of the suppressive effect of 5HT on mouse immunoreactivity.

Treatment of retinal detachment after penetrating injury: heavy cryotreatment of the fibrous ingrowth as an adjunct to vitreoretinal microsurgery.

Heavy cryotreatment of the fibrous ingrowth remnants at the inner surface of the scleral wound was performed, as an adjunct to vitreo retinal microsurgery, in a series of 32 eyes affected with retinal detachment after penetrating scleral wound. Surgical success, with a follow-up of 6-42 months after gas disappearance, was achieved in 25 eyes (71%). However, permanent retinal reattachment was achieved with a single operation in only 64% of the eyes successfully operated on (16/25). Thirty-six per cent (9/25 eyes) of the retinal detachments that were eventually successfully operated on showed clinical evidence of gradual contraction of the vitreous base in the postoperative course, and required 2 or more operations. Experimental studies on standardized models are required to determine whether destruction of the fibrous ingrowth remnants with heavy cryo is of any value in the management of retinal detachment after penetrating scleral wound.

Microsurgery for retinal detachment repair.

Microsurgery for RD repair has brought about significant improvement in the management of easy RD as well as difficult and desperate cases. At present, prevention of severe PVR remains the only major problem in the management of rhegmatogenous RD.

Prognosis of cystoid macular edema after retinal detachment repair.

Twenty eyes of 20 patients who had undergone successful retinal detachment repair by means of microsurgery had cystoid macular edema (CME) disclosed by fluorescein angiography 10-12 weeks postoperatively. Clinical evaluation war repeated at 6-month intervals for a period of 12-30 months. Sixteen eyes were followed for at least 18 months. CME spontaneously cleared or dramatically decreased in 12 of the 16 eyes (75%), which were followed for at least 18 months. CME had spontaneously cleared in 50% of the eyes by the end of 1 year after surgery. For those eyes with transient CME, the mean visual acuity improved from 0.3 at 3 months postoperatively to 0.6 at the end of the follow-up period. The final visual acuity was 0.4 or better in 83.3% of the eyes with transient CME. None of the eyes with persistent CME developed premacular fibrosis or cystoid macular degeneration during the follow-up period. No prognostic factor for angiographic CME after repair of retinal detachment could be determined in the present series of patients.

[Plasma exchange and acyclovir in Behçet's disease].

It has been postulated that Behçet's disease may be a viral induced vasculitis. In the present study plasma exchanges were applied in an attempt to remove circulating immune complexes and restore cellular immunity. High doses of acyclovir were administered in association with plasma exchanges since it has been shown that herpes simplex virus type 1 might be involved in Behçet's vasculitis. Seven patients with severe ocular manifestations of Behçet's disease were selected for the study. Plasma exchanges were carried out at the rate of 3 per week during 3 weeks and then once every two weeks during 3 months followed by 1 per month during 3 months. 45 mg/day/kg acyclovir were administered intravenously for 21 days. At the end of the follow-up period comparative analysis of panretinal fluorescein angiographies before and after treatment showed no change of the fundus lesions in 3 patients, improvement in only 2 patients and significant worsening in 2 patients. The rather negative results of the present trial lead to assume that HSV-1 is not the virus involved, if any, in Behçet's disease. On the other hand, in the present study, the efficiency of plasma exchanges was transient.

[Retinal detachment caused by a tear at the ora serrata].

The results of a retrospective analysis of forty eight retinal detachments with dialysis observed in 43 patients are consistent with the data of the literature. 90.7% of the patients were under 40 years of age, and 40.2% were less than 20 years old. Thirty-three eyes (68.7%) had a single dialysis, 15 eyes (31.3%) had two separate dialysis, 30 of the 64 dialysis (46.9%) were located in the inferotemporal quadrant, 18 (28.1%) in the superotemporal quadrant, 8 (12.5%) in the superonasal quadrant, and 7 (11%) in the inferonasal quadrant. Fifty dialysis (78.1%) were less than 90 degrees in size, 9 (14%) were 90 degrees in size and 5 (7.8%) extended on 100 degrees to 150 degrees. Seventeen retinal detachments (37%) were associated with subretinal gliosis. Four eyes (8.3%) showed an avulsion of the vitreous base. One single eye (2%) had a posterior vitreous detachment. None of the retinal detachments was associated with clinical evidence of preretinal proliferative retinopathy. Fourty six retinal detachments were operated on. Surgical success was achieved in all eyes. Postoperative visual acuity of 20/40 or better was achieved in 37.8% of the eyes, 54% of the eyes whose maculae were preoperatively attached, 58% of the eyes whose maculae were preoperatively detached without clinical evidence of cystic degeneration and none of the eyes whose maculae showed preoperative evidence of cystic degeneration.
