Interstitial irradiation of brain tumors: a review.

As an adjuvant to surgery, radiation therapy has consistently proven to be the most successful form of treatment for primary and secondary malignant brain tumors and possibly for inoperable benign tumors. Because the risk of radiation necrosis of normal brain limits the amount of radiation that can be given by external beam therapy at conventional dose rates, interstitial radiation of brain tumors is a logical alternative treatment approach. We discuss the radiobiological advantages of low dose rate irradiation and intratumoral placement of sources that make interstitial irradiation an attractive treatment for brain tumors and review the history of clinical brachytherapy for intracranial neoplasia.

Oral contraceptives and postmolar trophoblastic disease.

One hundred patients, managed for molar pregnancy at the New England Trophoblastic Disease Center, were selected at random to determine if the contraceptive method following molar evacuation influenced the incidence of postmolar trophoblastic disease. Following molar evacuation, 58 (58%) patients used oral contraceptives and 42 (42%) patients used barrier methods (foam, condom, and/or diaphragm). Postmolar trophoblastic disease developed in 11 (18.9%) patients using oral contraceptives and in 6 (14.3%) patients using barrier methods (P greater than .10). The mean human chorionic gonadotropin (hCG) regression time after molar evacuation was 7.0 weeks in patients using oral contraceptives and 7.2 weeks in patients using barrier methods. The 2 groups of patients were comparable in age, gravidity, molar histology, pretreatment hCG titers, and exposure to prophylactic chemotherapy. Oral contraceptives do not appear to increase the risk of postmolar trophoblastic tumors and therefore may be safely prescribed after molar evacuation during the entire interval of gonadotropin monitoring.

Investigation of relationship between lecithin:cholesterol acyltransferase activity and coronary atheroma, diagnosed by selective coronary angiography.

140 consecutive patients undergoing selective coronary angiography in the investigation of chest pain had fasting blood specimens collected. 24 specimens were unsuitable due to hemolysis or to incomplete coronary angiography studies. In the 116 remaining patients the serum was analyzed for total cholesterol, free cholesterol, and the initial rate of cholesterol esterification. LCAT activity was determined both as the percentage esterification per minute, and as nmols/ml/min. All the coronary angiograms were examined by a single cardiologist who was unaware of the results of the serum tests. Fixed predetermined criteria were used, resulting in all patients being classified as one of four groups. 36 patients had no detectable atheroma, 23 mild, 21 moderate, and 36 severe. Statistical analysis revealed a significant relationship between the presence and severity of coronary atheroma with the patient's age, and an equally strong relationship was found with the patient's total serum cholesterol. No statistically valid correlation could be found between LCAT activity (as determined by esterification in the patient's native serum) and atheroma. Free cholesterol levels also did not correlate with the presence of antheroma. An interesting finding was the presence of severe antheroma in a small number of patients with low normal level of serum cholesterol and high LCAT activity.

Copper coproporphyrin excretion in familial coproporphyria.

Analysis of stool specimens from a patient with familial coproporphyria by high-performance liquid chromatography revealed that 112 microgram per gram of dry feces (14% of the total porphyrin present) was copper coproporphyrin. Examination of stool specimens from other patients with this disease confirmed the presence of significant amounts of both copper coproporphyrin and coproporphyrin. Further investigation showed that the copper coproporphyrin was probably formed by a nonenzymic incorporation of copper by the coproporphyrin in either the bile or feces.

Fixed-bed charcoal hemoperfusion. Treatment of drug overdose.

Fixed-bed activated charcoal cartridges were used for hemoperfusion in the treatment of 54 patients with overdose of one or more drugs, including barbiturate, glutethimide, ethchloryvnol, meprobamate, methyprylon, methaqualone, salicylate, and diazepam. The most dramatic improvement was noticed in patients with phenobarbitol intoxication; they were admitted in stage 3-4 coma and were either awake or arousable by verbal communication at the end of 1 1/2 to 3 1/2 hours of hemoperfusion. Other intoxications improved slowly and required longer duration of treatment. The clearance rates of the drugs with hemoperfusion were greater than those usually achieved with hemodialysis. The data demonstrate the efficacy and usefulness of charcoal hemoperfusion for the management of drug overdose.

Primary open-angle glaucoma and sensitivity to corticosteroids in vitro.

Corticosteroid inhibition of mitogen induced lymphocyte transformation was studied in patients with definite or suspected primary open-angle plaucoma (POAG). Patients without glaucoma served as normal controls. From a dose response curve with prednisolone-21-PO4 or prednisolone the value of 50% inhibition (I50) was determined for each patient. In Series I it was necessary to disqualify 70% of the data whereas in Series II less than 5% were excluded. In Series I the median I50 (M X 10(-8) prednisolone-21-PO4) was 12 for 11 controls and 8 for 18 total POAG'S (p less than .01). In Series II the opposite result was obtained. The median I50 was 8 for 49 controls and 12 for 79 total POAG'S (p less than .01). In series II similar results were obtained with prednisolone. There was no difference between definite and suspected glaucoma patients. The cause of the discrepancy between the present two series and among the other published studies is not clear. The source of normal controls, from either an eye clinic or volunteer groups, may be a factor.

Physicians' response to abnormal results of routine urinalysis.

To determine the clinical usefulness of routine urinalysis, the records of 400 patients were examined for results of the first urinalysis following admission to hospital, and the attending physician's response to abnormal findings was evaluated. Results were abnormal for 116 patients (29.0%); there were 22 (5.5% of total urinalyses) abnormalities of chemical constituents (protein, glucose or bilirubin was present) only, 56 (14.0%) of sediment only and 38 (9.5%) of both chemical constituents and sediment. The attending physician did not respond to abnormal results in 50.9% of the 116 instances.
