Ten patients with normal liver and kidney functions were admitted to a urology department for prostatectomy because of benign prostatic hypertrophy. They received a twice daily administration of 2 tablets of sulfametrole/trimethoprim (Lidaprim) for 4 days; the last dose of 2 tablets was given 4 hours before surgery. Having been enucleated by the suprapubic route, the prostatic glands were examined both histologically and chemico-analytically. The concentrations of the chemotherapeutically active substances were determined in the plasma and in the prostatic tissues. The prostatic tissue concentration of sulfametrole was 24,0 +/- 8,6 mg/kg and that of trimethoprim 7,1 +/- 2,2 mg/kg.

Leiomysarcomas, rare malignant tumors of the veins, are located predominantly in the inferior vena cava. We report our experience with a leiomyosarcoma in the superior vena cava of a 44-year-old white man. The lesion had been locally excised and then recurred 2 1/2 years later. The recurrent tumor was widely excised and the superior vena cava was repaired with a saphenous vein patch graft. The area of the recurrence was treated with cobalt 60 therapy. Four years later, 6 1/2 years after initial treatment, the patient is alive and well without evidence of recurrence.

Since the development of automatic surgical stapling instruments in the Soviet Union in the 1950s and their description in the American literature in 1961, American surgeons have used them for a variety of pulmonary resections. However, there is no account of their use in this country for pneumonectomy by "en masse" stapling of the hilar structures. We have used this whenever it was applicable in 41 consecutive pneumonectomies since 1967. There were 2 postoperative deaths, 1 due to pulmonary embolism and the other to myocardial infarction. There were no complications attributable to the operative technique used. The only empyema developed nine months postoperatively and was not accompanied by bronchial fistula. Cancer-free survival was 43% at two and one-half years and 33% at five years. The technique of en masse stapling of hilar vessels renders pneumonectomy easier, safer, and faster than the technique of individual ligation.
