Somatostatin in the treatment of severe gastrointestinal bleeding from peptic origin. A multicentric controlled trial.

Results of a multicentric controlled trial on the efficacy of somatostatin in the treatment of severe gastrointestinal bleeding from peptic origin is reported. The trial was carried out on 90 patients presenting with severe bleeding from hemorrhagic gastric disease in 20 cases, from gastric ulcer in 31 and from duodenal ulcer in 39. Diagnosis was ascertained by endoscopy. Fifty-three patients were treated with somatostatin; in 95% of cases the bleeding was controlled in an average time of 7h 37'. Thirty-seven patients were treated with H2 receptors antagonists and the bleeding was controlled in 76.3% of cases in an average time of 21h. Cases treated with somatostatin required a lower amount of blood transfusion (1 Unit/patient, against 2.7 Units). The better efficacy of somatostatin, at least on a temporary basis, in the control of severe upper gastrointestinal bleeding is underlined while keeping in mind that prevention of recurrence is still an open question.

[3,3'-bi-1,3-thiazolidine]-4,4'-dione system. I. Synthesis, conformational analysis and pharmacological activities of 2,2'-diaryl derivatives.

The two configurational isomers, d1, and meso, of some 2,2'-diaryl-[3,3'-bi-1,3-thiazolidine]-4,4'-dione derivatives have been obtained and characterized. The 1H-N.M.R. spectra of both stereoisomers in DMSO-d6 solution are temperature-dependent and their dynamic behaviour has been related to the hindered rotation of the N--N bond. Evidence for an orthogonal disposition of the two heterocyclic rings, in the ground state, is presented. These new bicyclic compounds possess interesting antiinflammatory, analgesic and antipyretic activities as well as low acute toxicity and ulcerogenicity. The antiinflammatory effect, which in some compounds is higher than that of indomethacin and phenylbutazone, appears to be a function of the relative disposition of the substituents at position 2 and 2': generally the meso isomers are more active than the d1 ones. The lipophilicity of the tested compounds was determined by reversed-phase thin-layer chromatography.

[Indications and technic for autotransfusion in surgery].

The authors, after considering the advantages of autotransfusion, report their experience about 14 election patients operated on, and emphasize the usefulness of the usefulness of the method, designed to remove the iatrogenous pathology involved in the transfusions of homologous blood.

Synthesis and pharmacological activity of some 1-alkylamino-3-(2'-methoxy-4'-diethylcarbamoyl-phenoxy)propan-2-ols.

The synthesis and pharmacological properties of some 1-alkylamino-3-(2'-methoxy-4'-diethylcarbamoylphenoxy)propan-2-ols, related to beta-adrenergic drugs, are described. The above compounds were found to possess beta 1 and beta 2-adrenolytic activity associated with local anesthetic and antiarrhythmic activities, which are clearly inferior to those of propranolol.

[Agenesis of the femoral vein in the framework of vascular dysplasias of the lower limbs. Clinical case].

Venous dilatations of the lower limbs, especially if associated with others abnormalities, may reflect pathogenetic mechanisms far more complex and diverse than plain insufficiency of the sephena, all the way to congenital vascular anomalies of all possible types and still dubious classification. In such conditions a prerequisite of prime importance is the complete exploration of the whole arterial and venous vasculature of the limb, first of all in order to avoid rash therapeutic decisions, and then to help clarify the diagnosis and select the most appropriate treatment from among the numerous technics available.

[Role of post-embolectomy anticoagulant therapy in the prevention of recurrences].

The authors review two homogenous groups of patients subjected to surgical embolectomy, one group receiving standard anticoagulant therapy before and after surgery, and the other not receiving that treatment regularly or at all. The results obtained in the two groups indicate that anticoagulant therapy plays a role of no mean order in the prevention of postoperative relapses.

[Role of the vasa vasorum in the process of remote vascularization of the Wesolowski Weavenit arterial prosthesis (experimental research)].

Experimental research was carried out for the purpose of studying the behaviour of the vasa-vasorum in a group of animals in which, by means of a Weavenit prosthesis, the section of aorta lying between the branches of the renal arteries and the trifurcation of the terminal oartic section had been substituted. From the results of the research the Authors conclude by asserting that above all the technique they used allowed excellent visualisation of the vasa-vasorum, as shown both by the histological preparation coloured with haematoxylin-eosin and by the preparations carried out under the stereomicroscope. Furthermore, in the microscopic observations there is the indisputable presence of vessels of varying calibre which, from the neo-adventitia through the meshes of the prosthesis, are carried to the level of the neointima, confirming that the vasa-vasorum constitute the most important reactive phenomenon for purposes of vascularisation of the prosthesis.
