[Nodular peritoneal mesothelioma: apropos of an occasional report].

The complicated post-operative course of a patient who underwent an inguinal herniotomy, has allowed the diagnosis of peritoneal mesothelioma during laparotomy. This highlight the alternatives of an aggressive procedure, such as laparotomy, or a less invasive one, such as laparoscopy. The diagnostic criterion, should be when the clinical course suggests a peritoneal neoplasia; our more invasive technique allowed to perform an operation on the patient's internal organs which should be performed after laparoscopy anyway.

Visualizing dopamine and serotonin transporters in the human brain with the potent cocaine analogue [125I]RTI-55: in vitro binding and autoradiographic characterization.

The cocaine analogue RTI-55 was evaluated as a probe for in vitro labeling and localization of dopamine and serotonin transporters after death in the human brain. Kinetic, saturation, and competition binding experiments indicated complex interactions of the radioligand with the identification of multiple recognition sites. In membrane binding assays, the association of [125I]RTI-55 at 25 degrees C to putamen membranes was monophasic. In contrast, dissociation of [125I]RTI-55 occurred in two phases with t1/2 values of 9.4 and 36.5 min, respectively. Saturation analysis of [125I]RTI-55 binding demonstrated two binding sites in the human putamen with KD values of 0.10 +/- 0.02 and 1.81 +/- 0.46 nM. The binding of [125I]RTI-55 was displaced by a wide range of cocaine analogues and monoamine uptake inhibitors. The rank order of potency demonstrated in competition assays with human putamen membranes indicates that the radioligand labels cocaine recognition sites on the dopamine transporter (mazindol > GBR 12909 > GBR 12935 > paroxetine > nisoxetine > desipramine > or = fluoxetine > citalopram). In the human occipital cortex, [125I]RTI-55 recognized multiple binding sites with KD values of 0.02 +/- 0.01 and 4.18 +/- 0.46 nM. The rank order of potency for inhibition of [125I]RTI-55 binding to cerebral cortex membranes (paroxetine > citalopram > GBR 12909 > or = mazindol > or = nisoxetine > benztropine) suggests that [125I]RTI-55 labels the serotonin transporter in the human occipital cortex. Autoradiographic mapping of [125I]RTI-55 revealed very high densities of cocaine recognition sites over areas known to be rich in dopaminergic innervation, including the caudate, putamen, and nucleus accumbens.(ABSTRACT TRUNCATED AT 250 WORDS)

[Complications in a patient with a jejuno-peritoneal shunt by the Le Veen technique].

The case of a patient, who, 3 months after application of a Le Veen shunt for a refractory ascites, underwent the pleural effusion of ascitic fluid is reported. The adoption of a scintigraphic diagnostic technique allowed the demonstration of a direct ascitic fluid passage from the peritoneal to the pleural cavity and the subsequent successful pleurodesis, performed using an application of local tetracycline.

[The use of magnetic tracers in hemodynamics. A proposed model study].

Velocity of blood flow is critical in specific clinical situations. Available non invasive blood velocimeters gives only qualitative informations; quantitative estimates can be obtained invasively. We investigated the possibility of estimating the velocity of fluids using magnetic tracer. A model for the laminar blood flow inside a vessel was developed. A biomagnetometer was used to measure the fluid tracer velocity. The experimental setup was designed to mimic blood flow. A PVC tube simulated the vessel. The tube was connected to a bottle of normal saline. The tracer injection took place 25 cm distal to the saline bottle. The system was calibrated to a constant flow velocity of about 1 cm/s. A paramagnetic fluid (Magnevist, Schering, Berlin) was adopted as a tracer. In each session an amount of 2 cm3 was injected into the tube within 5 seconds. The recording equipment consisted of a second order gradiometer (baseline 7 cm, pick-up coil diameter 1.5 cm) coupled to a rfSQUID magnetometer. The recording bandwidth was dc-3 Hz, the sampling rate 32 Hz with 12 bit digitalization. The biomagnetic system was positioned immediately above the tube 55 cm away from the injection site. The total recording time for each session was 240 seconds. The injection of the tracer took place 60 seconds after the beginning of the data acquisition. Five measuring sessions in the earth magnetic field were performed at first and showed a low signal-to-noise ratio.(ABSTRACT TRUNCATED AT 250 WORDS)

Phase II study of cisplatin and 120-hour continuous infusion of 5-fluorouracil in patients with advanced pancreatic adenocarcinoma.

BACKGROUND: Advanced pancreatic carcinoma (APC) is a rapidly fatal disease and an active chemotherapy with palliative effects and impact on patient survival is needed. 5 fluorouracil (5-FU) combined with cisplatin (CDDP) has a recognized synergistic activity, but its activity in APC has never been well established. METHODS: Forty eligible patients (pts) with measurable APC were treated in a phase II trial with 5-FU 1000 mg/m2/day from day 1 to day 5 by continuous intravenous infusion and CDDP 100 mg/m2 on day 2. Eighty percent of the pts (36/40) had metastatic disease, 32.5% (13/40) were previously treated and 65% (26/40) had performance states of 2 or 3. RESULTS: Of 38 evaluable pts, one had a complete response and 9 achieved partial responses; the overall response rate (RR) was 26.5% (95% CI: 12% to 40%). The median duration of responses was 10 months (range 4-18). The RR in non-pretreated pts was 32% A palliative effect was seen in 45% of pts (17/38). The median survival was 7 months and 12 pts (29%) were alive at 1 year. Leukopenia was the most important toxicity; 11 pts (27%) had a grade 4 leukopenia and 3 had neutropenic fever. CONCLUSIONS: The combination of CDDP and 5-FU in continuous infusion seems an active and well tolerated treatment in APC and will be compared to standard therapy in a multicentric randomized trial.

Malignant melanoma: primary presentation in bone marrow and lymph node.

We describe an unusual presentation of malignant melanoma with simultaneous lymph node and bone marrow metastasis. In addition the disease was associated with immune-mediated thrombocytopenia. A brief remission was obtained with combination chemotherapy.

Measurement of segmental transit through the gut in man. A novel approach by the biomagnetic method.

The techniques commonly used to evaluate the transit of contents through the gut feature some limitations for being either inaccurate, invasive, inconvenient, or potentially dangerous for the subjects. Aim of this study was to establish a safe, noninvasive and accurate technique for the measurement of segmental oroanal transit time. We localized an orally ingested magnetic marker by means of a biomagnetic instrumentation that allows us to identify in a three-dimensional pattern the position of a biomagnetic source inside the body. The biomagnetic localizations were compared with the anatomical data obtained by magnetic resonance imaging investigations. The study was performed in 12 healthy subjects, and scans were taken every hour up to the arrival of the marker into the cecum; thereafter, scans were taken every 4 hr up to the elimination of the marker. In 99% of the isofield maps obtained from each field scan, the marker was localized within the bowel walls. The mean oroanal transit time was 56 +/- 5 hr, the mouth-to-cecum transit time was 13 +/- 1.7 hr, and the total colonic transit time was 43.5 +/- 5 hr (mean +/- SEM). Segmental colon transit did not show major differences among the regions considered, although most of the time was spent in the right colon. In fact, a good correlation was found between transit time through the right colon and oroanal and total colonic transit (r = 0.77, P < 0.02, r = 0.79, P < 0.02 respectively). In conclusion, this method might be a safe alternative to the techniques presently used in the clinical setting for the measurement of intestinal transit.

Halogenated isoniazid derivatives as possible antitubercular and antineoplastic agents. Note 1.

Halogenated arylhydrazones, 2-aryl-4-thiazolidinones and their 1,1-dioxides containing isoniazid (INI) moieties were prepared and explored for antimicrobial (against bacteria, Candida and mycobacteria) and antitumor activities. None of the tested compounds showed any marked antimicrobial effect. Furthermore, among the compounds submitted to NCI in vitro primary antitumor screening, those bearing 3-fluoro or -chlorophenyl substituents were found to possess selective inhibitory effects on the growth of non small cell lung cancer, whereas those with para-phenyl substituents displayed selective effects, sometimes statistically significant, against leukemias.

3,3'-Di[1,3-thiazolidine-4-one]system. IV. Synthesis and pharmacological properties of 3,3'(1,2-ethanediyl)bis [2-aryl-1,3-thiazolidine-4-one 1,1-dioxide] derivatives.

The 1,1' disulfones obtained by oxidation of the corresponding dl and meso 3,3'(1,2-ethanediyl)bis [2-aryl-4-thiazolidinone] compounds previously investigated, were evaluated as anti-histamine, anti-inflammatory, analgesic and anti-pyretic agents. All 2,2' fluorophenyl compounds were found to be significantly active in inhibiting carrageenin-induced edema, whereas only para-substituted derivatives were active on the histamine-induced bronchospasm in the guinea-pig. They also showed analgesic effects that reached and sometimes exceeded those of indomethacin and phenylbutazone used as reference drugs.

Piperazin-2,5-diones N,N'-substituted. III. Synthesis and pharmacological activities of some 1-aryl-4-arylidene (or heteroarylidene)aminopiperazin-2,5-dione derivatives.

The synthesis, antimicrobial and antitumor activities of new N,N'-substituted-2,5-dioxopiperazine derivatives are reported. The biological investigation showed that only the compounds containing the 5-nitrofurylidene moiety were active against some bacteria. All tested compounds showed no significant antitumor activity against lymphocytic murine leukemia P 388.

[Surgical treatment of obesity].

Morbid obesity is a disease of modern society. Surgery is indicated when there is no endocrinopathy, medical treatment failed and Body Mass index (W/H2) is more than 40 (III degree obesity of Garrow). Many different methods were suggested in connection with the type of obesity, the associated diseases and the psychical state of the patient. Jejuno-ileal by-pass intend to produce a generic malabsorption syndrome. Personal experience is however good, possibly due to the blind loop jejunostomy that we always prepare as a conclusion of the operation and which gives an excellent support for the early post-operative time.

[3,3'-bi-1,3-thiazolidine]-4,4'-dione system. III. Evaluation of the antimicrobial and antitumor properties of 2,2'-disubstituted derivatives and their 1,1'-disulfones.

A series of dl and meso 2,2'-disubstituted-[3,3'-bis-1,3-thiazolidine]-4,4'-diones and their 1,1'-disulfone derivatives have been tested for their antimicrobial activity against gram-positive, gram-negative and Candida strains and for antitumor activity against leukemic P.388 tumor system in mice. None of the tested bi-thiazolidinones showed any significant antitumor properties; only few 1,1'-disulfones exhibited some antibacterial activity.

[Intentional normovolemic hemodilution (preliminary results)].

The authors, after illustrating the advantages of hemodilution from the rheological, hemodynamic and district microcirculatory standpoint, illustrate their experience about 11 patients treated with such method. They point out the advantages this method can bring in both reducing the iatrogenous pathology involved in the hemodilution and reducing the difficulties faced by blood-banks and the costs burdening on the medical services.
