Late systolic murmur of rheumatic mitral insufficiency.

Of 184 patients with acute rheumatic fever and associated mitral insufficiency encountered during a 15 year period, 34 manifested a mid-late systolic murmur or a nonejection click, or both, during the course of follow-up. The mid-late systolic murmur later disappeared in four patients whose condition is now considered normal. In one of the four, systolic prolapse of the mitral valve was demonstrated on an angiocardiogram obtained when the systolic murmur was present. Since disappearance of the murmur there has been no evidence of systolic prolapse on meticulous echocardiographic study of the mitral valve. In another child with angiographically demonstrated systolic prolapse of the mitral valve the systolic murmur has also disappeared, but systolic prolapse is still evident on echocardiographic study. None of the 34 patients with a mid-late systolic murmur manifested the T wave abnormalities commonly associated with the familial variety of mitral valve prolapse.

Low-dosage cytarabine therapy for herpes zoster with pneumonia.

Disseminated herpes zoster infection is frequently fatal. Cytarabine (cytosine arabinoside), a drug of variable efficacy, has offered some hope in the treatment of this condition. In this case of disseminated herpes zoster infection with pneumonia, there was remarkable recovery after low-dosage cytarabine infusion. The findings suggest that doses of the drug not exceeding 30 mg/sq m of body surface per 24 hours (30 mg/sq m/24 hr) may well be virucidal but not cytohematotoxic.

[Quantitative and virus specific serum IgM detection in newborns (author's transl)].

Serum IgM concentration was determined in umbilical cord blood samples from 1000 newborns. 34 cases presenting elevated IgM values (greater than or equal to 30 mg%) were tested for virus specific IgM antibodies. We could demonstrate such virus specific IgM antibodies against Cytomegalovirus (4), against Rubella, Influenza A and Influenza B (2 each) and against Coxsackie virus (1). The quantitation of IgM in blood samples of child bearing women showed no elevated values compared to those of non-pregnant women of similar age groups.

Age at onset in bipolar-related major affective illness: clinical and genetic implications.

Age-of-onset data were collected on 142 bipolar probands (61 males, 81 females) and 110 siblings. After correcting for demographic factors the age at illness onset did not distinguish bipolar from unipolar siblings or siblings from probands. This indicates that in a subset of pedigree data, bipolar and unipolar affective disorders may be different phenotypic manifestations of the same underlying disorder. A total of 43% of affected individuals gave evidence of illness before age 30 and 11% were ill as adolescents. Also 2% were affected prior to age 15. The data indicate that affective illness is not uncommon in adolescence and may occur as a childhood disorder. Estimation of the age-of-onset probability distribution was obtained by incorporating demographic factors (survivorship data) into the observed ages at onset of affected individuals. A square-root normal curve fits the age-of-onset probability density function. The implications for morbid risk prediction and genetic analysis of affective disorders were discussed.
