[Primary vesico-uretero-renal reflux in adults: therapeutic outcome. Apropos of 68 cases].

The authors compare two series of adult patients presenting with primary vesico-uretero-renal reflux. The first series was treated before 1974, and the second, over the past ten years. The salient feature of this comparison is the reduction in the number of renal failures. More systematic investigation of the reflux highlights a greater number of cases which will evolve without further impairment of renal function. This point has a considerable bearing on operative indications.

[Empyema caused by Staphylococcus aureus in children: the experience of the Albert Royer Children's Hospital at Fann University Hospital, Dakar].

We report a retrospective study of 58 cases of empyema caused by Staphylococcus aureus at the Albert Royer Children's Hospital at the Fann University Hospital, between January 1st 1992 and December 31st 1995. Staphylococcus aureus is the principal bacterium responsible for pleural effusions in children (54%), way ahead of Streptococcus pneumoniae (19%). Most of the children affected (86%) are less than 30 months old. Theses infections are often serious due to the resistance of the bacterium to the usual antibiotics, the lack of solid research data and mechanical complications associated with the effusion. Treatment is based on the use of an appropriate bactericidal antibiotic treatment and pleural drainage.

[Vesicovaginal fistulas: etiopathogenic and therapeutic aspects in Senegal].

Obstetrical vesico-vaginal fistulas are still very common in developing countries, because of to the difficulties of medical facilities. From a series of 123 cases recorded during 6 years, we analyse their epidemiological, anatomo-clinical, and therapeutic aspects. A simplified classification is purposed.

[Uretero-vesical injuries during pelvic surgery in women].

Injuries to the ureter and the bladder are common complications in female pelvic surgery. Unfortunately, they remain often unrecognized per operatively. Intravenous pyelography and renal ultrasonography are very efficient for post operative diagnosis. The indications of nephrectomy have became rare because of the efficacity of the procedures of plastic and reconstructive urological surgery.

[Neonatal bacterial infections in the tropical zone].

One thousand and ten miscellaneous samples collected from infected newborns, were tested at the bacteriology laboratory of A. Royer Children's Hospital from 1983 to 1991. These samples included 471 blood cultures, 114 pus of various origins, 410 cerebrospinal fluids and 15 urines. One bacteria or bacterial soluble antigens were detected in 294 samples (29.2%). Positivity percentage was 29.2% for septicemia, 68.4% for suppurations, 17.8% for meningitidis and 33.3% for urinary tract infections. Altogether, we isolated 156 enterobacteria (53%), 14 Gram negative bacilli (4.7%) and 124 cocci (42.1%) among them 19 streptococci (A, B, C) and 25 pneumococci. Three major species were identified: S. aureus (25.8%), Klebsiella spp (19.7%) and Escherichia coli (14.6%). The most efficient antibiotics against all strains were AKN, CTX, CRO and GEN.
