[Low birth weight: rate and risk factors in the Guédianwaye district (suburb of Dakar, Senegal)].

The authors report the results of a six months survey about the low weight of birth (L.W.B.) in the district of Guédiawaye, suburb of Dakar. The purpose is to determine the rate of L.W.B. in two main maternity hospitals of that locality and the associated risk factors. It is a survey with a control group. This survey results in a 10.7% rate, according to the data of another retrospective survey in the same maternity hospitals in 1992 (10.03%). The risk factors are mainly: celibacy, primiparity, an associated pathology with the pregnancy, the physical strain of mothers correlated to the income and to the previous L.W.B.

[Place on information in health services use by mothers in Guediawaye].

The authors report the results of a survey in a suburban healthcare structure. The purpose was to evaluate the knowledge of mothers about the healthcare delivered through a childhood and maternal healthcare program in Senegal. 244 women were selected at random. The consultant mothers came more for their children than for themselves (79% of consultations were for their children, 49% for themselves). 60% to 80% of women are unaware of basic notions currently teached through the "Information-Education-Communication (I.E.C)" program. All that requires to elaborate with these women healthcare strategies, which are in adequacy with their needs in these peripheral healthcare structures, and which are supported by an adequate and regularly evaluated I.E.C.

[Functional capacity of fibrinogen and erythrocyte aggregation in the diabetic].

The aim of this work is to study the possible influence of a qualitative modification of fibrinogen on its capacity of erythrocyte aggregation. Fibrinogens were isolated and then purified from blood sample of 15 insulin-dependent diabetics and of 7 healthy subjects. The erythrocyte aggregative effect of the purified fibrinogens was measured for various concentrations (0 g/1-5 g/l). The erythrocyte aggregation was determined by the ECHO CELL whose functioning principle is based upon ultrasonic interferometry. Results allow one to distinguish three kinds of fibrinogen: the first kind inducing a similar erythrocyte aggregation as that obtained by the fibrinogens from healthy subjects, the second kind inducing an erythrocyte hyperaggregation, the third kind inducing an erythrocyte hypoaggregation. Thus the possibility of a qualitative abnormality of fibrinogen, affecting its erythrocyte aggregative capacity in diabetic patients, could be suggested. This qualitative abnormality of fibrinogen might be due to a modification of its molecular structure because of an abnormal elevated glycation during diabetes.

[Delayed emergency repair of traumatic ruptures of the posterior urethra].

The management of posterior urethral injuries is still a matter of controversies. Some authors recommend a primary or delayed emergency repair procedure, while others prefer late repair. The analysis of our series of fifteen cases showed that when patients are treated by delayed emergency operation, the results are usually good. However, when repair is performed later, results are less satisfactory, and the rate of impotence is higher. We conclude that delayed emergency repair operation is the procedure of choice in the management of ruptured posterior urethra.

[Medico-social problems of young adolescents in Guediawaye (suburban area of Dakar-Senegal)].

A survey in the district of Guediawaye, towards teenagers from 12 to 16 years old, recruited according to a stratified risky method of sampling in 4 different areas has been carried out in order to evaluate their main medico-social problems. It ended up to the following findings: the majority live in hard socio-economic conditions stamping by promiscuity, a weak family income and a low level of education of the parents. 39.2% have been placed under the guardianship of a relative other than the parents. 98.8% do not find spare time structures in their environment. The usage of tobacco and drug is respectively 14% and 1%; 79.4% of students have hardly access to the school stationery, while the teenagers in job apprenticeship have constraints linked to long hours of work (95%). The morbid affections are too varied, dominated by stomach aches (37.8%) far before traumatics (6%); the most frequent therapeutic is resort traditional or empiric treatment. The authors advocate the setting up of a medico-social center making interfere all persons, implied resources in the undertake of the teenagers.

[Purulent pleurisy in the child].

We studied 122 cases of empyema in children. The mean age was 3 years. We found malnutrition in 33% of the patients. 60% of them had large pleural effusion and 56% associated pneumonia. Staphylococcus aureus (51%) and Streptococcus pneumoniae (18%) were the most frequently isolated bacterias in pleural fluid. The mortality rate was 6.5%. 53% of the children had closed chest tube drainage. There were minor abnormalities in pulmonary function tests. The best treatment of empyema in children is closed chest tube drainage and an antimicrobial therapy with antistaphylococcus antibiotic.

[Bacteriology of neonatal septicemia in Dakar].

Between February 1983 and February 1991, 471 blood cultures from newborns with neonatal infection, aged from 0 to 30 days, admitted to the Pediatric Hospital Albert Royer, Dakar, were performed. In 141 (30.3 percent) cases, one bacterium was isolated. Three major causative organisms were identified: Klebsiella spp (28.6 percent), E. coli (19.5 percent) and S. aureus (17.4 percent). Altogether, we isolated 99 (69.2 percent) enterobacteria, 7 (4.8 percent) other Gram negative bacilli and 37 (25.8 percent) Gram positive cocci among which 25 S. aureus, 10 streptococci (7 group B, 2 group D, 1 group A and 2 pneumococci). The most efficient antibiotics were amikacin and third generation cephalosporins.

["In-dartos" orchidopexy in the treatment of cryptorchism].

Undescended testis is a common congenital abnormality. The main complications are infertility and venue of malignant testicular tumor. If early orchiopexy can prevent infertility, it remains a high risk of venue of a malignant tumor. So, the best technique of orchiopexy is that which allows an easy clinical follow-up of the undescended testis after surgical treatment, and doesn't involv the hematoseminal barrier. From the analysis of fourty cases of scrotal pouch orchiopexy and a review of the literature, it appears that this procedure is efficient and gives more than 90% of good results.

[Posterior urethral valves in Senegalese children. Fourteen cases].

Fourteen pediatric cases of posterior urethral valves in patients aged 6 months to 14 years (with four infants and ten older children) are reported. The main symptoms were vesical, including dysuria, acute urinary retention and dribbling. One patient presented with diarrhea, vomiting and dribbling. Urethral valves were looked for as part of the evaluation for urinary lithiasis in one patient, and in another urinary lithiasis developed following the diagnosis of urethral valves. In half the cases, onset of symptoms occurred within one year of birth. Urethrocystography allowed to evidence the valves and to evaluate repercussions on the bladder including hyperplasia (7 cases) and diverticula (2 cases). The intravenous urogram disclosed bilateral ureterohydronephrosis in eight cases. Pathogens recovered from the urine included Klebsiella (3 patients), Pseudomonas (4 patients), Proteus (one patient), and E. coli (one patient). Most patients were treated by catheter lamination under antimicrobial therapy. Two patients died from renal failure. Results were considered satisfactory in nine cases with follow-ups ranging from 1 month to 4 years. Three patients were lost to follow-up. Early diagnosis is essential and management should rely mainly on endoscopic resection which reduces the length of the hospital stay and the risk of infection.

[Neonatal bacterial infections: a retrospective study over 8 years].

In a retrospective study, one thousand and ten miscellaneous samples coming from infected newborns and tested at the bacteriology laboratory of A. Royer Children's Hospital from 1983 to 1991 were reviewed. These samples included 471 blood cultures, 114 pus of various origins, 410 cerebrospinal fluids and 15 urines. One bacterium or bacterial soluble antigens was detected in 294 samples (29.2%). Positivity percentage was 29.2% for septicemias, 68% for suppurations, 17.8% for meningitidis and 33.3% for urinary tract infections. There major species were identified: S. aureus (25.8%), Klebsiella spp (19.7%) and E. Coli (14.6%). Altogether, we isolated 156 enterobacteria (53%), 14 Gram negative bacilli (4.7%) and 124 cocci (42.1%) among which 19 streptococci (A, B, C) and 25 pneumococci. The most efficient antibiotics against all strains were AKN, CTX, CRO and GEN.

[Malignant Leydig cell tumor. A case observed at the Dakar University Hospital].

The authors report the first malignancy composed of interstitial testicular cells in a Senegalese man. Interesting features of this case include occurrence in a 58-year-old adult, short duration of the disease, development of metastases before discovery of the primary tumor, severity of pain, and absence of feminization.

[Hydranencephaly: report of a case].

The authors report the first Senegalese observation of hydranencephaly in a three-week-old baby to underline the unusualness of this deformity and the efficiency of transfrontal echography in its diagnosis.

[Congenital adrenal hyperplasia apropos of 7 cases at the University Hospital Center of Dakar].

The authors depict the main clinical tables relating to endocrine and cytogenetic study on 7 confirmed cases and stress the difficulties they faced taking care of these children. These were dominated by: the unavailability of sufficient suitable medicine; and the absence of complementary examinations indispensable for biologically checking the children treated and for their therapeutic readjustment.

[Esophago-tracheal fistulas: report of a case].

The authors report the first case in Senegal of an esophagotracheal fistula isolated in a suckling child 1,1/2 months old, and stress: the clinical profile and the rarity of this deformity; the difficulties of providing for such patients in view of the close medico-surgical collaboration required.

[Congenital unilateral absence of a pulmonary artery].

Congenital absence of a pulmonary artery is a rare entity often followed by complications. Up to now only two patients have done well after revascularization of the affected lung. We have had the opportunity to operate upon the two youngest patients suffering from an absence of a right or left pulmonary artery. In both cases revascularization of the lung was achieved with the use of a PTFE graft. In the second case, a non previously described technique has been applied successfully. Both children have been doing well up to now. This experience has led us to believe that revascularization of the affected lung should be done even in very young patients and that pulmonary hypertension may regress.
