[Pelvio-ureteral tumor 5 years after uretero-cutaneostomy].

A 70-year-old woman visited our hospital with the chief complaint of intermittent gross hematuria from right ureterocutaneostoma. She had received total cystectomy and bilateral ureterocutaneostomy because of bladder tumor (transitional cell carcinoma, grade 3, stage T2) 5 years earlier. Urine cytology showed transitional cell carcinoma (TCC). The right retrograde pyelogram showed shadow defects of right pelvis and upper ureter, computed tomographic scan showed fatty density mass of right pelvis. Preoperative diagnosis was right pelvic and ureteral tumors. Right nephroureterectomy was performed on May 26, 1986. Histology revealed TCC, grade 3 on the pelvis and upper ureter. Cases of pelvio-ureteral tumors of ureterocutaneostomy were collected from the literature and reviewed.

[Clinical studies of varicocele. 2: Radiographic examination and measurement of spermatic vein pressure in varicocele patients].

There are two types of internal spermatic veins (ISV) by radiographic examinations. In type 1, ISV is dilated with some collateral venous branches. On the other hand, ISV of type 2 is narrowed with increased collateral venous branches or abnormalities such as the circumaortic renal are seen. In these groups, pressure of renal vein and ISV were measured. As a result, renal vein pressure was about the same in type 1 as in the controls at 9.5 +/- 2.3 cmH2O but was higher in type 2 at 17.0 +/- 2.8 cmH2O. As for ISV, it was also about the same in type 2 as in the controls at 13.9 +/- 6.8 cmH2O but was markedly higher in type 2 at 33.3 +/- 3.9 cmH2O. When selective renal venography was performed, a contrast medium back-flow was seen in 9 out of 11 type 1 varicocele patients, but it was not seen in any of the cases with the nutcracker phenomenon. These results suggested that type 1 may be caused by the structural and functional disorders in the valve between the renal vein and ISV. In contrast, pathogenesis of type 2 varicocele may be due to an abnormality of the ISV in the embryo.

[Transfer of cefoperazone into exudate from postoperative wound].

The levels of cefoperazone (CPZ) in the exudate from the postoperative would were measured in 10 cases for which CPZ was employed for prophylaxis of postoperative infections. The mean total volume of the exudate from postoperative wounds in these patients was 96.3 +/- 77.81 ml on Day 1, 14.6 +/- 17.1 ml on Day 2, 17.2 +/- 14.3 ml on Day 3, 32.0 +/- 17.0 ml on Day 4 and 34.0 ml (n = 1) on Day 5 of the postoperative period, and their CPZ level was, respectively 31.3 +/- 16.3, 31.2 +/- 19.4, 15.0 +/- 7.04, 13.4 +/- 6.78, and 2.45 micrograms/ml (n = 1), respectively with the level exceeding 30 micrograms/ml on Day 2 of the postoperative period and gradually decreasing thereafter. Recovery of CPZ during this period was 2941.1 +/- 2775.8 on Day 1, 375.3 +/- 445.9 on Day 2, 261.9 +/- 477.5 on Day 3, 371.4 +/- 10.5 on Day 4 and 83.3 micrograms (n = 1) on Day 5 of the postoperative period. In other words, of the total daily dose, 0.074, 0.009, 0.007, 0.009 and 0.002 appeared in the wound on Day 1, 2, 3, 4 and 5 of the postoperative period, respectively. Judging from the daily changes in the level of CPZ in the exudate and from the clinical results that all cases except one responded to this drug, it is evident that CPZ is highly useful for prophylaxis of postoperative infections, particularly for prophylaxis of infections of the postoperative wounds.

Purification and characterization of alpha-N-acetylgalactosaminidase from skipjack liver.

By means of a simple procedure involving two gel filtrations and an ion-exchange chromatography, alpha-N-acetylgalactosaminidase was purified to an electrophoretically homogeneous form from skipjack liver, in which the enzyme is the dominant glycosidase. The final alpha-N-acetylgalactosaminidase preparation contained practically no other glycosidase activities except alpha-galactosidase activity, which amounted to 0.8% of the alpha-N-acetylgalactosaminidase activity and may be an intrinsic activity of the enzyme. The molecular weight of the enzyme was estimated to be 80,000 at pH 4.2 and 40,000 at pH 7.2 by molecular sieve chromatography, and to be 35,000 by SDS-polyacrylamide gel electrophoresis. The enzyme was most active at pH 4 and was inactive above pH 7. These results suggest that skipjack alpha-N-acetylgalactosaminidase exists as an active dimer at acidic pH and as inactive monomer at neutral or alkaline pH. The enzyme efficiently liberated the N-acetylgalactosamine unit from ovine submaxillary glycoprotein which had been desialylated by neuraminidase. The Km value and maximum velocity were 4.28 mM and 409 mumol/min X mg for p-nitrophenyl alpha-N-acetylgalactosaminide, and 0.0543 mM and 1.19 mumol/min X mg for ovine submaxillary asialoglycoprotein.

[Studies of enzymuria and proteinuria: Report 1: N-acetyl-beta-glucosaminidase in urological diseases].

N-Acetyl-beta-glucosaminidase (NAG) exists in renal tubules and seminal tract. We measured the NAG level in patients with urinary tract and seminal tract diseases and evaluated its clinical significance. The urinary NAG level was high in renal tubular function disorders such as glomerulonephritis, diabetic nephropathy and hydronephrosis. In these patients, NAG-isoenzyme studies indicated that the A-type isoenzyme decreased while the B-type isoenzyme increased. On the other hand, in the seminal tract disease patients, the urinary NAG level was high in benign prostatic hypertrophy with a catheter indwelling and in prostatitis. Urinary and seminal NAG-isoenzymes were measured in normal subjects. The isoenzyme patterns for urinary NAG and seminal NAG differed. The urinary NAG isoenzymes were measured in acute prostatitis patients, we found that the A-type isoenzyme decreased and the B type increased. In conclusion, when the A/B ratio of the NAG-isoenzyme is close to 24.9/75.0 in the urine, the presence of urological diseases in which seminal plasma could be mixed in, especially prostatic diseases, should be suspected.

[Comparison of urinary and seminal N-acetyl-beta-glucosaminidase isoenzymes].

N-acetyl-beta-glucosaminidase (NAG) and its isoenzymes were measured in the urine and seminal plasma of healthy volunteers. Urinary NAG level was 2.62 +/- 1.30 U/L (mean +/- standard deviation), 1.99 +/- 0.77 U/g creatinine and seminal NAG level was 2370 +/- 1007 U/l. Urinary NAG level was 2.59 +/- 1.44 U/l, 1.93 +/- 0.80 U/g creatinine in males and 2.67 +/- 1.15 U/l, 2.08 +/- 0.78 U/g creatinine in females, and there was no significant sex difference. NAG isoenzymes in the urine and seminal plasma were divided into two major peaks, A and B. The A:B ratio was 78.0 +/- 6.5: 21.9 +/- 6.5 in the urine and 24.7 +/- 3.2: 75.2 +/- 3.2 in the seminal plasma, and was significantly different. Urinary NAG isoenzyme was 80.3 +/- 6.7: 19.6 +/- 6.7 in males and 74.1 +/- 4.3: 25.9 +/- 4.3 in females, and there was no significant difference between the sexes. These results indicated that urinary and seminal NAG can be differentiated by measuring the isoenzymes. Furthermore, the comparison of seminal NAG isoenzymes before and after vasectomy indicated that seminal NAG may be affected not only by the sperm but also by the prostatic fluid.

[A case of giant carcinoma of the prostate].

A 74-year-old male complaining of lower abdominal mass was admitted to our hospital on August 1, 1985. He had also suffered from dysuria and bilateral lower limb edema for a year. At the time of admission, two fist-size hard masses were palpable on each suprainguinal abdomen. Remarkable venous dilatation on the surface of the whole abdomen was recognized. An enlarged stone-like hard prostate was palpable by rectal examination. Laboratory findings revealed moderately increased blood sedimentation rate and a high value of prostatic acid phosphatase. Urethrocystogram showed remarkable compression of both lateral walls of the bladder medially and the floor of the bladder was highly elevated. Computed tomography revealed almost the whole pelvic cavity occupied by the tumor which had invaded the retroperitoneal space upward as far as the level of the hilus of the kidney. Histological diagnosis was well-differentiated adenocarcinoma of prostate. Bilateral orchiectomy was performed on August 29, 1985 and he was treated with estramustine phosphate (Estracyt) 560 mg a day. Two months later, there was remarkable regression of the tumor and urethrocystogram was almost normal. Related reports are also reviewed.

[Enoxacin concentration in human prostatic tissue].

The present study was under taken to evaluate the penetration of enoxacin into prostatic tissue. Ten patients with benign prostatic hypertrophy entered the study. Enoxacin was administered orally at a dose of 200 mg, three times one day before and twice on the day of operation. Blood and urine samples were taken 30 min before operation and prostatic tissue samples were taken during operation. The mean concentration of enoxacin was 4.87 +/- 1.39 micrograms/ml in serum and 5.81 +/- 2.95 micrograms/g in the prostatic tissue. The mean ratio of the prostatic concentration/serum concentration was 1.21 +/- 0.63 (range 0.82-3.00).

Augmentation of ethanol-induced enhancement of dimethylnitrosamine and diethylnitrosamine metabolism by lowered carbohydrate intake.

Hepatic metabolism in vitro of dimethylnitrosamine (DMN), diethylnitrosamine (DEN), 3,4-benzopyrene (BP) and 7,12-dimethylbenz[a]anthracene (DMBA) was assayed using the liver of rats maintained for 3 weeks on liquid diets containing three levels of carbohydrate (CHO) with or without supplementation of ethanol (5 g/100 ml). Ethanol consumption increased the metabolism of DMN and DEN to different degrees depending on the diet consumed simultaneously. Lowered CHO intake, which by itself increased the metabolism, dose-dependently augmented the increase due to ethanol: the lower the CHO intake, the more remarkable was the enhancement caused by ethanol. Ethanol increased microsomal cytochrome P-450 content only when combined with low-CHO diets. It was confirmed by a Salmonella mutagenicity test that the enhancement of DMN and DEN metabolism was accompanied by increased capacity of the liver to activate these nitrosamines to mutagens. On the other hand, neither lowered CHO intake, nor ethanol consumption, nor their combination affected the metabolism of BP and DMBA. Ethanol combined with a diet deficient in CHO even suppressed the metabolism of BP.

[Studies on the cellular immune response in patients with urinary bladder carcinoma. XI: Evaluation of specific ADCC activity].

We previously reported that the activity of antibody dependent cell-mediated cytotoxicity (ADCC) of lymphocytes from the patients with urinary bladder carcinoma was lower than that of normal subjects. We performed the specific ADCC assay using SRBC as the target cells, coated with tumor extract from the urinary bladder carcinoma. At the same time, the antiserum was collected from the immunized rabbit by the same tumor extract. The addition of the antiserum from the immunized rabbit gave the highest value for the patient sub-group that produced a precipitin band (Ouchterlony gel diffusion assay). Therefore it is suggested that the reaction of lymphocytes to tumor associated antigens is maintained to a considerable extent, although ADCC activity of the patient group was decreased. However, the antibody producing phenomenon toward the tumor associated antigen might be restrained.
