[A case report of emphysematous pyelonephritis].

A 51-year-old male patient with diabetes mellitus consulted his home doctor because of high fever and right flank pain. Urinalysis showed marked pyuria. Treatment with antibiotics was not completely effective, and he was referred to our hospital for further examination and treatment. CT scan showed an abnormal gas shadow in right renal parenchyma. He was diagnosed with emphysematous pyelonephritis and right subcapsular nephrectomy was done after the control of diabetes mellitus. We reviewed 57 cases of emphysematous pyelonephritis including our case in the Japanese literature, and we discussed about its etiology, symptomatology, choice of treatment and prognosis.

[Tuberculin skin test among medical college students].

The incidence of active tuberculosis in Japan has markedly decreased in the past few decades. It is also reported that the prevalence of tuberculosis infection in young population has been decreasing in accordance with the decline in the overall occurrence of tuberculosis. The above mentioned facts indicate that the younger population has a greater risk of developing tuberculosis once exposed to tuberculosis infection. Actually, the epidemics of tuberculosis among the adolescent population have often been reported in recent years. We performed a tuberculin skin test on our medical college students, who might be exposed to tuberculosis infection during their student clinical internship, in order to obtain the information about their present status of reaction to the tuberculin skin test. The data obtained from students between 18 and 25 years old were analyzed using Student's t test and Kruskal-Wallis's method. The mean size of erythema in the group of 23, 24 and 25 years old were significantly greater than that in the group of 20, 21 and 22 years olds. Similarly the latter was significantly greater than that in the group of 18 and 19 years olds. Furthermore, it was proven by the Kruskal-Wallis's method that the older a group was, the greater the chance of a large erythema. One explanation for this fact might be a decrease in the prevalence of tuberculosis infection in the younger generation as described above. Another explanation seems to be related to the revision of the Tuberculosis Prevention Law in 1974.(ABSTRACT TRUNCATED AT 250 WORDS)

[Evaluation of the results of extracorporeal shock-wave lithotripsy (ESWL) for solitary upper urinary tract stone].

At Osaka City University Hospital, we performed extracorporeal shock-wave lithotripsy (ESWL) for 1277 patients in a total of 1788 sessions using Dornier kidney lithotripter Model HM III from July, 1985 to the end of December, 1988. Among the patients with a solitary stone, 911 cases were available for the follow-up study and we have compared the results among these cases in respect to the stone location and size. The results obtained were as follows: the ratio of kidney stone to ureter stone was found to be 2.2:1 in male, and 3.8:1 in female patients. This indicates that ureter stones are more frequently found in male than in female patients. In addition, we compared the size of kidney stones found in the patients including 415 male and 243 female patients. Stones larger than 20 mm were more frequently found in female than in male patients. Moreover, a stone composition study of these patients showed that the contents of both phosphate calcium and magnesium ammonium phosphate were higher in female than in male patients. ESWL performed against stones at pelvis and calyces exhibited the best results. The results obtained with the stones less than 20 mm in diameter were especially favorable with a success rate of 100% for the stones less than 10 mm and 83% for the stones 10-20 mm, in diameter, while the results with the stones larger than 20 mm in diameter were rather poor with a success rate of 31%. ESWL performed against ureter stones showed poor results with a success rate of 63% for the stones smaller than 20 mm in diameter.

[Clinical study of estramustine phosphate disodium (Estracyt) on prostatic cancer--results of long-term therapy for 38 patients with prostatic cancer].

Estramustine phosphate disodium (Estracyt) was used in the treatment of 38 patients with prostatic carcinoma for at least 1 year. Of these patients 37 patients were treated with Estracyt as primary treatment and 1 patient had been treated with another antiandrogenic therapy before the Estracyt treatment. Estracyt was given orally in a dose of 560 mg/day in divided oral doses. The clinical evaluation was done for the change of PAP, the relapse rate, the survival rate and the side effect. Among 22 cases which had shown abnormally high PAP values before the treatment started, the values decreased or normalized in 21 cases (95.5%) in the first year of administration of Estracyt. In 6 cases, however, the values increased again in the second year or later. Relapse was observed in 10 (26.3%) out of 38 cases. Relapse rate was 2.6%, 51.7%, and 51.7%, at the first, third, and fifth year, respectively. Survival rate was 97.4% at the first year, 88.5% at the third year, and 68.8% at the fifth year for the follow-up study. Side effects were observed in 14 (36.8%) out of 38 cases. The main side effect was gynecomastia. Gastro-intestinal disturbance and edema were also observed. However, there were only 2 cases (5.2%) in which administration of Estracyt had to be discontinued.

[A randomized controlled study of PSK combined immuno-chemotherapy for adenocarcinoma of the lung. The Advanced Lung Cancer Immuno-chemotherapy Study Group].

In a randomized controlled study of chemotherapy (CDDP 100 mg/m2 day 1, VDS 3 mg/m2 day 1, 8, 15) vs. immuno-chemotherapy combined with PSK 3 g/day for adenocarcinoma of the lung (stage III, IV, p. s. 0, 1, 2), response rate for 169 cases with completed extramural review was 14.2%. As for the response rates for 138 complete cases, the chemotherapy group showed 17.9%, and the immuno-chemotherapy group was 16.9%. MST were 330 days and 331 days, respectively. In stage III cases, the response rates were 11.1% in the chemotherapy group and 37.5% in the immuno-chemotherapy group (p = 0.046). MST were 457 days (65.3 weeks) and 576 days (82.3 weeks), respectively. In terms of survival curve, it was suggested that the immuno-chemotherapy group was superior to the chemotherapy group (logrank test p = 0.075), but in stage IV cases, there was nothing outstanding in the immunochemotherapy group.

Functional differences of human basophils with different densities.

It has been reported that blood basophils comprise two populations of different densities. In this study, we compared the responses of two populations of basophils (band-1 and 2 basophils) to several secretagogues and anti-allergic agents. Twelve patients with bronchial asthma, 3 patients with Japanese cedar pollinosis and 6 healthy subjects were included in this study. Band-2 basophils collected from the interface between Percoll of density 1.078 and 1.068 g/ml released histamine better in response to anti-IgE than band-1 basophils taken from the interface between plasma and Percoll 1.068 (band 1 vs. band 2, 13.2 +/- 2.7 vs. 21.4 +/- 3.6% at 1:10(5), p less than 0.01; 47.1 +/- 5.0 vs. 57.7 +/- 6.1% at 1:10(4), p less than 0.01). Band-2 basophils also responded better to formyl-methionyl-leucyl-phenylalanine than band-1 basophils (34.4 +/- 4.9 vs. 43.2 +/- 5.6% at 10(-6) M, p less than 0.01; 35.6 +/- 5.0 vs. 45.2 +/- 5.7% at 10(-5) M, p less than 0.01). In contrast, band-1 basophils were much more sensitive to the calcium ionophore A23187 stimulation than band-2 basophils (13.0 +/- 3.7 vs. 6.3 +/- 1.8% at 0.05 microgram/ml, p less than 0.05; 52.5 +/- 4.3 vs. 33.8 +/- 3.9% at 0.1 microgram/ml, p less than 0.01; 71.3 +/- 3.3 vs. 51.5 +/- 4.2% at 0.2 microgram/ml, p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

[Treatment with a new preparation using peplomycin emulsion in hydroxypropylcellulosum. Analysis of immediate response in sixty-two patients with bladder cancer].

To 62 vesical tumor patients, 30 ml of a HPC-PEP mixed solution was infused into the urinary bladder. The HPC-PEP solution was prepared by adding 15 ml of 2% hydroxypropylcellulosum (HPC) to 15 ml of peplomycin (PEP)/physiological saline solution in which 90 mg PEP had been dissolved. Accordingly, a total of 30 ml of the mixed solution was used. On day 7 after treatment, a clinical evaluation was made through endoscopic observation and urine cytodiagnosis. The results of the evaluation were expressed by assigning appropriate grades for the improvements made. As for the overall clinical effects, 10 patients demonstrated excellent improvement (CR) but 26 patients exhibited no improvement (NC). The overall clinical efficacy for all cases was 58%. When the evaluation was made according to severity (T1, T2, or T3), the results were as follows: of 43 T1 patients, 9 patients were judged CR, and 23 patients were PR; and of 15 T2 patients, 1 patient was CR and 3 patients were PR. However, all T3 patients were NC. The clinical efficacy for the T1 and T2 groups was 74.4% and 26.4%, respectively. A significant difference was observed between the T1 group and the T2 and T3 groups (p less than 0.05). Evaluation made according to the degree of histopathological differentiation (G1, G2 and G3) demonstrated the following results: of 23 G1 cases, 6 cases were CR, 9 cases were PR; of 32 G2 cases, 4 cases were CR and 16 cases were PR; and of 7 G3 cases, only 1 patient was found to be PR.(ABSTRACT TRUNCATED AT 250 WORDS)

[Transurethral uretero-lithotripsy of ureteral stones in Osaka City University].

During the 39 months since the introduction of transurethral lithotripsy (TUL) for the treatment of ureteral stones at our hospital in August 1985, TUL was performed a total of 200 times in 178 patients with ureteral stones. Among them, 111 patients had left ureteral stones and 65 had right ureteral stones, while 2 patients had ureteral stones in both sides. The stones were divided into upper ureteral stone (84 patients) and lower ureteral stone (94 patients) at above and below the iliac brim. 89% of the stones were less than 2 cm in diameter. Most of the patients were given lumbar anesthesia, and a guide wire was inserted into the ureter. The ureter was dilated with a ureteral bougie, and a 13F or 14F Storz ureteroscope was inserted. The stones were disintegrated by an ultrasound lithotripto and removed using forceps and a basket catheter. After the TUL procedure, a double J catheter was indwelled and removed within 5 days. The results were evaluated by DIP which was done 2 to 3 months after TUL. The success rate included residual stones less than 4 mm in diameter, as they could be spontaneously discharged. As a result, the success rate for upper ureteral stones was 53%, and it was higher for smaller stones. On the other hand, the success rate for lower ureteral stones was 85% and significantly higher. The main reasons for failure were the upper migration of the stones (60%) and inability to insert the ureteroscope up to the stone due to ureterostenosis and ureteral perforation (39%).(ABSTRACT TRUNCATED AT 250 WORDS)

[N-acetyl-beta-glucosaminidase in tissues of various human urogenital organs].

The urinary level of N-acetyl-beta-D-glucosaminidase (NAG) was reported to be a useful marker for the assessment of renal function. This enzyme was also detected at a high activity in seminal fluid. In our preliminary report, seminal NAG activity was 1,000 times higher than urinary NAG level in healthy volunteers. Moreover, the fraction ratio A to B isozymes was changed in seminal fluid independently from that in urine. In order to study the source of seminal NAG, NAG activity and the isozyme fraction were determined in the tissues of the genital organs (prostate, testis, epididymis, and seminal vesicle) as well as the urinary organ (kidney). Various levels of NAG activities were obtained, the mean tissue concentration (Unit/g protein) being 26.7 +/- 2.4, 4.3 +/- 30.5, and 8.75 +/- 0.72 unit/g, respectively, in the testis, epididymis and seminal vesicle. In particular the mean concentration of 97.7 +/- 23.3 was significantly high in the prostate. In regard to the isozyme fraction of NAG, B fraction (%) accounted for 15.1 +/- 5.5, 33.6 +/- 8.2, 37.7 +/- 0.1, and 70.6 +/- 4.5 percent, respectively, in the testis, epididymis, seminal vesicle and prostate. It was to note that the A to B fraction ratio in the prostate corresponded to that in the seminal fluid. These results suggest that seminal NAG is mainly from the prostate.

[Evaluation of transrectal longitudinal ultrasonography of chronic non-bacterial prostatitis].

Transrectal longitudinal ultrasonography was performed in 34 patients with chronic non-bacterial prostatitis. Prostatic stones and cystic lesions were associated with chronic non-bacterial prostatitis in 67.6% and 11.8%, respectively. The annual relapse rates of clinical symptoms were related to the presence of prostatic stones and cystic lesions. Transrectal ultrasonography plays a very important role in predicting the clinical courses of chronic non-bacterial prostatitis.

[A new clinical trial in intravesical chemotherapy with instillation of peplomycin preparation as an emulsion in hydroxypropylcellulosum--preliminary study of patients with bladder tumor].

A new peplomycin (PEP) preparation was employed as an emulsion form in hydroxypropylcellulosum (HPC), which is familiar to have a liniment effect with a strong affinity to mucosa of the organ, in the management of 6 patients with bladder tumors. This emuled PEP in HPC (HPC-PEP) was instillated and thereafter, maintained for at least 2 hours at the frequency of once a week. All 6 patients had histopathologically confirmed T1 or T2 transitional cell carcinoma on biopsied specimen. The histological grading of the tumors were divided to 2 in G1, 3 in G2, and 1 in G3 of transitional cell carcinoma. The instillated agent was prepared from the mixture of 15 ml of 6% HPC and 90 mg PEP in 15 ml saline prior to use. A response to this treatment was evaluated primarily by cystoscopic findings and urine cytology 7 days after the instillation. In the present study, the beneficial effects of this treatment were briefly segregated into 4 categories as follows: excellent, good, fair and no responses were referred respectively to as complete eradication of visible tumor with negative urine cytology, a complete disappearance of the primary tumor with a persistent positive cytology, regression of the tumor size and no change of the primary tumor with sustained positive cytology. According to this estimation, 6 patients were categorized to 1 in excellent, 2 in good, 2 in fair and 1 in no responses to this new therapeutic preparation. Furthermore, urinary and serum levels of PEP were determined daily for 7 days after the instillation.(ABSTRACT TRUNCATED AT 250 WORDS)

[Studies of enzymuria and proteinuria: Report 2; Relationship between glomerular filtration rate and urinary enzymuria and proteinuria].

Urinary splitting enzymes and proteins including N-acetyl-b-D-glucosanimidase (NAG), beta 2 microglobulin (beta 2MG), and alpha 1 microglobulin: (alpha 1MG), which are established to be useful in the evaluation of renal dysfunction, especially renal tubular impairment, were measured to determine the extent of renal tubular impairment in relation to the degree of glomerular dysfunction in patients with renal deterioration. In healthy volunteers, urinary NAG, alpha 1MG and beta 2MG levels were 3.5 +/- 1.4 (mean +/- SD) U/g creatinine, 2.5 +/- 2.0 mg/l and 88 +/- 75 micrograms/l, respectively. While serum beta 2MG level (SBMG; microgram/ml) was between 2.0 and 2.9, among the patients with renal dysfunction, NAG, alpha 1MG and BMG levels showed 8.6 +/- 5.5, 9.8 +/- 5.8 and 785 +/- 1,264, respectively, and further elevated to 10.1 +/- 5.0, 16.5 +/- 0.7 and 525 +/- 440, respectively with a SBMG level over 3.0. Thus glomerular function was deteriorated, urinary alpha 1MG level elevated to significantly higher with a parallel to renal dysfunction. In patients with more severe renal dysfunction, the corresponding urinary NAG and alpha 1MG levels became significantly higher. However, urinary beta 2MG level was significantly higher in patients with moderate renal dysfunction compared to healthy volunteers only when SBMG level was more than 2.0. Therefore, it was more valuable to measure urinary alpha 1MG with NAG or beta 2MG at SBMG level of less than 1.9 and to determine urinary NAG and alpha 1MG at greater than 2.0. These results indicated that the measurement of the levels of urinary splitting enzymes and proteins is valuable in the evaluation of proximal tubular impairment, even when the degree of glomerular impairment is minimal.(ABSTRACT TRUNCATED AT 250 WORDS)

[CT findings of bronchiolo-alveolar carcinoma with special reference to the lucencies within abnormal shadows].

Computed tomography (CT) was performed in the patients with bronchiolo-alveolar carcinoma. CT shows not only nodular shadows and pneumonia-like shadows that have been reported on plain radiographs, but also the lucencies of various sizes within those abnormal shadows. These findings seem to be characteristic of bronchiolo-alveolar carcinoma on CT. In addition, serial CT often demonstrates prominent changes of the lucencies, even when little change was observed in the outlines of the abnormal shadows. In regard to the progression of this disease, more attention should be paid to the lucencies as well as the outlines of abnormal shadows.

[Study on urinary splitting enzymes and proteins. III. Effect of non-ionic contrast medium on renal function].

Renal toxicity of non-ionic contrast medium (iohexol) for drip infused pyelography (DIP) was studied in a randomized trial of nine patients with normal renal function. Urine samples were collected before and immediately after DIP, and analyzed for albumin, an index of glomerular permeability; gamma-glutamyl transpeptidase (gamma-GTP), a brush-border enzyme; N-acetyl-beta-glucosaminidase (NAG), a lysosomal enzyme; alpha 1 microglobulin (alpha 1MG) and beta 2 microglobulin (beta 2MG), an index to tubular proteinuria; and creatinine. The urinary excretion of enzymes and proteins was compared with urinary creatinine. Urinary excretion of gamma-GTP and NAG increased significantly (P less than 0.001, 0.02) after DIP. Urinary alpha 1 MG and beta 2-MG did not change significantly. The change of urinary albumin was mild. Our data suggest that non-ionic, low osmolal radiocontrast medium ioheol shows a lower renal tubular toxicity, and the brush-border enzyme gamma-GTP and lysosomal enzyme NAG are considered as a good index for renal tubular damage.

[A case of testicular tumor in an elderly man].

Testicular tumor is seen exclusively in men from 30 to 50 years old. In elderly men, the incidence of testicular tumors is rarer than that of other malignant tumors. Herein we report a case of an elderly patient with a testicular tumor comprised solely of seminoma. A 60-year-old man was seen at the urologic clinic of Osaka Kita Municipal Citizen's Hospital because of painless swelling of the left scrotal contents. At the time of clinical visit, physical examinations and scrotal sonography showed a fist-sized mass with solid echo pattern in the left scrotum. He was hospitalized for the exploration of the suspected testicular tumor. The testicular mass was removed surgically through the left inguinal incision. Histopathological examinations of the resected tumor revealed pure seminoma. Chest film, computerized axial tomography, retroperitoneal ultrasound study and specific tumor markers were all negative which indicated stage 1 testicular tumor confined to the scrotal region according to Dixon and Moore's staging. He received adjuvant irradiation at the level of mediastinal lymph node after the operation. Postoperatively, he recovered well without any evidence of recurrence. In this report, the prognosis of non-bulky pure seminoma in elderly men are briefly reviewed in association with a conventional therapy to this disease in elderly patients described in the literature.

[Clinical study on prostatic cancer patients].

Clinical observations on prostatic cancer were studied in 27 patients who had been managed in our department between April, 1980 and December, 1986. The mean age at the time of initial clinical visit was 70.6 years old with a range of 55 to 88 years old. Of all 27 patients, 15 men (55.6%) were senior citizens over 70 years old and indeed 23 men (85.2%) were over 60 years old. According to the general rules for clinical and pathological studies on prostatic cancer, there were 10 patients with stage A, 2 patients with stage B, and 15 patients with stage D disease. However, none of our patients had stage C foci of prostatic cancer. Histopathologically, biopsied or surgically resected specimen all showed adenocarcinoma. More frequently the incidence of poorly differentiated adenocarcinoma was found in the specimen from the patients with advanced clinical disease. Anti-androgen therapy with castration or a combined hormonal manipulation initially was done in 25 patients. Simple hormonal treatment using chlormadinone acetate (CMA) was given in 13 patients. Of 25 patients who received hormone treatment, 22 underwent castration whereas, 12 of 13 having undergone single hormonal therapy were castrated. Combined chemohormonal therapy using UFT and CMA or additionally given estramustine phosphate disodium (Estracyt) was subjected only to stage D disease of prostatic cancer. Of 15 patients surgically treated, 11 received transurethral resection of the prostate on the basis of initial diagnosis of benign prostate hypertrophy.(ABSTRACT TRUNCATED AT 250 WORDS)
