Presumptive adrenergic neurons containing phenylethanolamine N-methyltransferase immunoreactivity in the medulla oblongata of neonatal swine.

Given the importance of the swine (Sus scrofa) as an animal model for human development, physiology and disease, neurons containing the epinephrine-synthesizing enzyme, phenylethanolamine N-methyltransferase (PNMT), were mapped in the medulla oblongata of neonatal swine as a first step in identifying their roles in central autonomic control. Neurons were labeled immunocytochemically by using an antiserum to PNMT raised in rabbits against trypsin-treated enzyme purified from the bovine adrenal gland. The general regional organization of neurons expressing PNMT (-like) immunoreactivity (ir) in the neonatal swine was similar to data obtained in other species and, in some aspects, more closely resembled the pattern observed in the primate brain. Immunolabeled cells appeared to be more abundant and caudally more extensive than observed in other adult animals. PNMT-immunoreactive (ir) neuronal somata, however, were largely confined to the reticular formation in the ventrolateral quadrant and the nucleus tractus solitarii (NTS) and more restricted in distribution than those expressing tyrosine hydroxylase (TH) and dopamine beta-hydroxylase (D beta H)-ir on serial transverse sections. A close correspondence was observed between the distributions of TH- and PNMT-ir neurons and processes throughout the C1 and C2 areas. However, in the C1 and C3 regions TH-ir neurons outnumbered those containing D beta H and PNMT-ir. In contrast, cell groups enriched in PNMT-ir neurons and processes were characterized by relatively weak D beta H-ir. In the ventrolateral medulla (VLM), PNMT-ir cell bodies were concentrated rostrally and extended from the caudal pole of the facial nucleus to a level posterior to the calamus scriptorius. The rostral VLM was characterized by an admixture of bipolar and multipolar primarily medium-diameter immunostained neurons. A prominent cell column (condensation) organized ventromedially to the nucleus ambiguus pars compactus (NAc). A loosely organized cluster bordered the lateral aspect of the special visceral efferent column; another smaller aggregate was located in the ventromedial reticular formation adjacent to the inferior olive. At middle medullary levels, PNMT-ir neurons formed two distinct subgroups (dorsal and ventral) interrupted by a band of precerebellar relay neurons that extended between the medial and lateral limbs of the lateral reticular nucleus of Walberg. At obex, the dorsal cell group formed a diagonal array and assumed a position dorsal and dorsolateral to the medial limb of LRN. This group was distinguished by bipolar neurons with axes of orientation directed perpendicularly to the majority of neurons in the rostal VLM or those lying near the caudal ventromedullary surface.(ABSTRACT TRUNCATED AT 400 WORDS)

An evaluation of IUD insertion by a non-clinical delivery system.

To evaluate the safety, effectiveness, side-effects, and continuation rate associated with non-clinical IUD insertion compared to IUD insertion in routine clinical facilities, a cohort comparative study was done on two groups of IUD users self-selected for either the non-clinical or clinical delivery system in Yogyakarta Special Area Province. During the months of January to March 1985, 454 non-clinical and 625 clinical acceptors were recruited into this study. However, only 414 non-clinical and 605 clinical acceptors fulfilled the inclusion criteria and were included in the analysis. Bleeding and pain were the most common side-effects reported by acceptors. The frequency of bleeding was higher in the non-clinical group than in the clinical group. However, the difference was a not statistically significant relative risk (RR) of 1.09 (0.8-1.4) to 1.2 (0.9-1.5). Severe menstrual blood loss mostly occurred in the first three months after insertion and then declined in the rest of the year in both groups. The frequency of pain was not clearly different between the two groups during the whole year, with RR 0.8 (0.5-1.3) to 1.3 (0.8-2.0). The actual number of pregnancies was very small and the difference was not statistically significant (p = 0.375). The 12-month expulsion rates were 7.22 per 100 IUD users for the non-clinical group and 7.00 for the clinical group. The difference was not statistically significant (p = 0.660). The cumulative probability of women who were still using IUDs at 12 months was 89.7% and 89.1% for the non-clinical and clinical groups, respectively. The non-clinical delivery system seemed to be responsive to an unmet demand for family planning among women with lower educational background compared with those in the clinical setting service. While the characteristics of a population obviously influence the contraceptive behavior, this study shows that the non-clinical delivery system family planning method provides a similarly efficacious and safe service to the usual clinical system.

Effects of isoflurane on transport across the blood-brain barrier.

The blood-brain barrier (BBB) plays an important role in regulating and restricting transfer of molecules into the interstitial space of the brain. Anesthetic agents may affect the permeability of the BBB. In this investigation, the effect of isoflurane on the transport of small hydrophilic molecules across the BBB was studied in rats by measuring the blood-brain transfer coefficient (Ki) and the regional cerebral blood flow (rCBF) and by calculating the capillary permeability-surface area (PS) product. In the control group, after a femoral artery and vein were catheterized under isoflurane anesthesia, rats were allowed to remain awake for 2 h before measuring Ki (n = 6) or rCBF (n = 11). In the two experimental groups, rats were anesthetized with 1% and 2% isoflurane respectively, and their lungs were mechanically ventilated through a tracheal tube. Ki was measured (1% n = 6, 2% n = 7) using 14C-alpha-aminoisobutyric acid, and rCBF was measured (2% n = 7) using 14C-iodoantipyrine. Two percent isoflurane did not affect rCBF in 9 of 13 brain regions. Blood flow was less in the lateral and posterior cortex and greater in the medulla and pons when compared with the control group. Ki was less in 11 of 13 brain regions in both the 1% and 2% isoflurane groups than in the control group. Quantitative determination of morphometric indices of the total and perfused capillary network of the newborn pig brain.

The purpose of this study was to determine if the newborn pig brain had a reserve of unperfused capillaries during normoxia. To accomplish this, a method was developed to determine the volume fraction, surface area, and number of both total and perfused capillaries in the newborn pig brain. Newborn pigs of either sex, 2-7 d old, were used. FITC-dextran, molecular weight 147,000, was used as a plasma marker to visualize the perfused capillaries. Alkaline phosphatase staining was used to stain the total capillary bed of the brain. Our results showed that FITC-dextran stayed within the vascular compartment, as it was not seen in areas that were not subsequently visualized with alkaline phosphatase staining. Eighty-four to 86% of the alkaline phosphatase-stained capillaries could be visualized with perfusion markers (india ink or FITC-dextran) in different brain regions. Similar results were obtained in two animals using a basement membrane stain, silver methenamine. The total volume fraction of capillaries (mm3/mm3) was cortex 0.055 +/- 0.012, cerebellum 0.062 +/- 0.011, and medulla 0.039 +/- 0.012. Capillary surface area (mm2/mm3) of different brain regions averaged cortex 23.2 +/- 1.8, cerebellum 24.8 +/- 2.5, and medulla 15.8 +/- 2.9. The total number of capillaries (per mm2) was cortex 375 +/- 37, cerebellum 329 +/- 37, and medulla 216 +/- 32. The time course of filling of the capillaries indicated that approximately 50% were perfused at 6 s, which increased to over 80% at 12 s and remained unchanged thereafter.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of fentanyl on alpha-aminoisobutyric acid transfer across the blood-brain barrier.

The effect of fentanyl on the transfer of small hydrophilic molecules across the blood-brain barrier was studied in rats by measuring the blood-brain transfer coefficient (Ki) and the regional cerebral blood flow (rCBF) and by calculating the capillary permeability-surface area product. In the control group (n = 14), after a femoral artery and vein were catheterized under isoflurane anesthesia, the rats were allowed to remain awake for 1.5 h before measuring Ki (n = 8) using intravenous 14C-alpha-aminoisobutyric acid or rCBF (n = 6) using 14C-iodoantipyrine. In the fentanyl groups, rats were injected with 25 micrograms/kg (low (n = 6) or 100 micrograms/kg (high dose) of fentanyl (n = 14), followed by a continuous infusion at a rate of 50 or 200 micrograms.kg-1.h-1, respectively. Their lungs were mechanically ventilated. The Ki (low dose, n = 6; high dose, n = 8) and rCBF (high dose, n = 6) were measured 1 h after fentanyl infusion. The Ki was lower in 9 of 13 brain regions in the low-dose fentanyl group and in 7 of 13 brain regions in the high-dose fentanyl group than in the control animals. The average value of Ki in all the brain regions was 8.6 +/- 4.6 microL.g-1.min-1 in the control group, 5.2 +/- 2.9 microL.g-1.min-1 in the low-dose fentanyl group, and 5.7 +/- 2.9 microL.g-1.min-1 in the high-dose fentanyl group. High-dose fentanyl did not significantly affect rCBF in any brain region studied. The value of the regional permeability-surface area product was similar to the corresponding regional Ki in the groups studied. In conclusion, fentanyl decreased the transfer of small hydrophilic molecules across the blood-brain barrier, as demonstrated by a decreased Ki and permeability-surface area product, without significant changes in rCBF.

The role of maternal thyroid hormones in maintaining early pregnancy in threatened abortion.

To evaluate a possible role of thyroid hormones in maintaining early pregnancy, serum levels of thyroid hormones, TSH and thyroxine-binding globulin (TBG) in 32 patients with a clinical diagnosis of threatened abortion were compared between two groups of patients with favorable and unfavorable pregnancy outcome. Serum levels of T4, T3, free T4 and free T3 levels determined at the onset of clinical signs of threatened abortion were found to be significantly lower in patients (N = 11) who subsequently aborted compared to patients (N = 21) who did not. Serum TSH levels did not differ between the two groups. Serum TBG levels in the patients who subsequently aborted were lower compared to patients with favorable pregnancy outcome. Furthermore, serum levels of T4 and T3 at the onset of threatened abortion in patients who subsequently did not abort were significantly higher compared to levels before pregnancy, whereas little increase in serum T4 and T3 levels relative to the pregnant levels was observed at the onset of clinical signs in the patients who subsequently aborted. These data imply a possible role of thyroid hormones in maintaining early pregnancy, and suggest that maternal serum level of thyroid hormone may be one of the endocrine factors responsible for the outcome of threatened abortion.

Leukemic infiltrate versus anterior uveitis.

Anterior segment leukemic infiltrate (ASLI) is rare; it is found in 0.5% to 2.5% of all leukemic relapses. Its clinical manifestations, at times, are similar to those of anterior uveitis (AU). We report the cases of three patients, all of whom had ASLI as their primary manifestation of relapse. We observed some major clinical differences between ASLI and AU that may help differentiate these two conditions without using anterior chamber paracentesis and/or iris biopsy. The development of resistance to leukemic therapy is the major factor in treatment failure.

Strain difference in phenylethanolamine N-methyltransferase activity and immunoreactivity of medulla oblongata of Sprague-Dawley and Long-Evans hooded rats.

Previously, we reported that retinal phenylethanolamine N-methyltransferase (PNMT) activity of Sprague-Dawley (SD) rats was significantly higher than that of Long-Evans (LE) hooded rats. However, there were no noticeable differences observed in PNMT-immunostaining patterns between the retinae of LE and SD rats. In order to examine this discrepancy, we extended this study to areas of the medulla oblongata harboring PNMT-containing cell bodies. In the present report, we demonstrate that the enzyme activity as well as the immunoreactivity of PNMT in medulla oblongata of SD rats were significantly higher than that observed in the LE strain.

A comparative clinical trial of the TCu 380A, Lippes Loop D and Multiload Cu 375 IUDs in Indonesia.

The Copper T 380A (TCu 380A), Lippes Loop D (LLD) and the Multiload Cu 375 (MLCu 375) IUDs were evaluated for safety and efficacy in a multicenter randomized clinical trial in Indonesia. A total of 2992 women were enrolled into the study and data for 2845 women were analyzed (147 cases did not meet protocol criteria). Although study IUDs were randomly assigned, LLD users in this study were older and of higher parity than TCu 380A and MLCu 375 users. The 24-month gross cumulative life-table pregnancy rates for TCu 380A, LLD and MLCu 375 users were 1.2, 2.2 and 2.7, respectively. The 24-month expulsion rates for TCu 380A, LLD and MLCu 375 users were 6.7, 7.5 and 5.3, respectively. Overall, 24-month continuation rates were 85.5%, 85.0% and 85.4% for the respective device groups. Differences in both gross and age- and parity-adjusted life-tables rates for the major outcome variables were not statistically significant at 24 months postinsertion. The study suggests that the TCu 380A, LLD and MLCu 375 IUDs seem to be safe and effective contraceptive options for Indonesian women.

Effects of MK-801 on cerebral regional oxygen consumption in focal cerebral ischemia in rats.

This investigation tested in rats whether MK-801, an N-methyl-D-aspartate receptor antagonist, would improve the balance of oxygen supply and consumption in the focal ischemic area of the brain induced by occlusion of the middle cerebral artery. Fifteen minutes after middle cerebral artery occlusion, 5 mg/kg MK-801 was administered intravenously to the MK-801 group (n = 12), and normal saline was given to the control group (n = 12). One hour after the occlusion in each group, regional cerebral blood flow was determined in six rats using [14C]iodoantipyrine, and regional arterial and venous oxygen saturations were determined using a microspectrophotometric technique in the other six rats. In both groups of animals, the cerebral blood flow of the ischemic cortex was significantly lower than that of the contralateral cortex (36 +/- 16 [SD] and 67 +/- 14 ml/min/100 g for the control group; 33 +/- 10 and 58 +/- 11 ml/min/100 g for the MK-801 group, respectively). Oxygen extraction was significantly higher in the ischemic cortex (8.8 +/- 2.1 ml O2/100 ml blood) than in the contralateral cortex (5.6 +/- 0.3) for the control group. However, for the MK-801 group, there was no significant difference between the ischemic cortex (6.1 +/- 1.0) and the contralateral cortex (5.7 +/- 1.1). Oxygen extraction in the ischemic cortex of the MK-801 group was significantly lower than that of the control group. Calculated ischemic regional oxygen consumption was similar to the nonischemic values in the control group, whereas the ischemic value was reduced to 61% of the value of the contralateral cortex in the MK-801 group.(ABSTRACT TRUNCATED AT 250 WORDS)

Alpha-thalassaemia: prevalence and pattern in northern Pakistan.

The level of Hb Bart's is directly related to the inheritance of alpha-thalassaemia gene. Hb electrophoresis for Hb Bart's in the cord blood is a very simple method of finding out the prevalence of alpha-thalassaemia gene in a given population. A study was, therefore, carried out to find out the prevalence of alpha-thalassaemia gene in the population of northern Pakistan by estimating the concentration of Hb Bart's in 500 cord blood samples during the period 1986-87 at AFIP Rawalpindi. Hb Bart's was detected in 12 neonates, thus indicating a rate of 2.4% of general population as carrier of alpha-thalassaemia gene. Two distinct groups were recognized. The first group was composed of 9 (75%) neonates, in which Hb Bart's levels varied between 2.0% to 3.5%, while the second group of 3 neonates (25%) showed a level of 5.8% to 6.3%. The former group was considered to be carrying alpha-thalassaemia-2 gene and the latter group as carrier of alpha-thalassaemia-1 gene. Neither Hb H disease nor Hb Bart's hydrops foetalis syndrome was detected in this series.

Strain differences between albino and pigmented rats in monoamine-synthesizing enzyme activities of brain, retina and adrenal gland.

The present study compared the activities of some of the monoamine synthesizing enzymes in several brain regions, the retina as well as adrenal gland of albino Sprague-Dawley (SD) and Long-Evans hooded (LE) rats. Brainstem, hypothalamic and retinal tyrosine hydroxylase (TH) activity were significantly higher in LE than in SD. In addition to higher enzyme activity, a larger number of TH-immunoreactive perikarya as well as a higher concentration of TH-immunoreactive processes were observed in the retina of LE rats. There was no strain difference in TH activity of caudate nucleus (CN) and substantia nigra (SN). In contrast to brain regions and retina, adrenal TH activity was markedly higher in SD than in LE animals. Aromatic L-amino acid decarboxylase (AADC) activity of both the brainstem and adrenal gland in the LE strain was lower than in SD animals. No differences in the AADC activity of hypothalamus, SN and CN were found between LE and SD strains. Phenylethanolamine N-methyltransferase (PNMT) activity of the hypothalamus, retina and adrenal gland of LE strains was significantly lower than in SD rats. In spite of the difference in the enzyme activity, there were no marked morphological changes observed in PNMT-immunostaining patterns between the retina of LE and SD rats. Tryptophan hydroxylase activity of both the brainstem and hypothalamus did not exhibit strain differences.(ABSTRACT TRUNCATED AT 250 WORDS)

Anatomical substrates of cholinergic-autonomic regulation in the rat.

UNLABELLED: Acetylcholine (ACh) plays a major role in central autonomic regulation, including the control of arterial blood pressure (AP). Previously unknown neuroanatomic substrates of cholinergic-autonomic control were mapped in this study. Cholinergic perikarya and bouton-like varicosities were localized by an immunocytochemical method employing a monoclonal antiserum against choline acetyltransferase (ChAT), the enzyme synthesizing ACh. In the forebrain, bouton-like varicosities and/or perikarya were detected in the septum, bed nucleus of the stria terminalis, amygdala (in particular, autonomic projection areas AP1 and AP2 bordering the central subnucleus), hypothalamus (rostrolateral/innominata transitional area, perifornical, dorsal, incertal, caudolateral, posterior [PHN], subparafascicular, supramammillary and mammillary nuclei). Few or no punctate varicosities were labeled in the paraventricular (PVN) or supraoptic (SON) hypothalamic nuclei. In the mid- and hindbrain, immunoreactive cells and processes were present in the nucleus of Edinger-Westphal, periaqueductal gray, parabrachial complex (PBC), a periceruleal zone avoiding the locus ceruleus (LC), pontine micturition field, pontomedullary raphe, paramedian reticular formation and periventricular gray, A5 area, lateral tegmental field, nucleus tractus solitarii (NTS), nucleus commissuralis, nucleus reticularis rostroventrolateralis (RVL), and the ventral medullary surface (VMS). In the PBC, immunoreactive varicosities identified areas previously unexplored for cholinergic autonomic responsivity (superior, internal, dorsal, and central divisions of the lateral subnucleus, nucleus of Koelliker-Fuse and the medial subnucleus). In the NTS, previously undescribed ChAT-immunolabeled cells and processes were concentrated at intermediate and subpostremal levels and distributed viscerotopically in areas receiving primary cardiopulmonary afferents. In the nucleus RVL, cholinergic perikarya were in proximity to the VMS and medial to adrenergic cell bodies of the C1 area. Punctate varicosities of unknown origin and dendrites extending ventrally from the nucleus ambiguus overlapped the C1 area and immediate surround of RVL. IN CONCLUSION: 1) Cholinergic perikarya and putative terminal fields, overlap structures that are rich in cholinoreceptors and express autonomic, neuroendocrine, or behavioral responsivity to central cholinergic stimulation (PHN, NTS, RVL). The role of ACh in most immunolabeled areas, however, has yet to be determined. Overall, these data support the concept that cholinergic agents act at multiple sites in the CNS and with topographic specificity.(ABSTRACT TRUNCATED AT 400 WORDS)

Changes in the circulating levels of hemoglobin, hematocrit, serum iron and total iron binding capacity after four methods of surgical tubal sterilization.

One-hundred women requesting voluntary surgical sterilization were included and enrolled into four groups (each n = 25). Four sterilization modalities were used namely: Falope ring, electrocoagulation, clip and Pomeroy ligation. The circulating levels of hemoglobin, hematocrit, serum iron and total iron binding capacity were studied before sterilization and at 3, 6 and 12 months thereafter. No significant changes were observed at 3 and 6 months post-sterilization. At 12 months a significant increase in the hemoglobin and serum iron was observed with a significant decrease in total iron binding capacity. Falope ring, clip and Pomeroy ligation were of a higher rank in this context versus electrocoagulation.

A modification of a popular North American model surgery technique is described, with a brief comparison with the key-spacer planning system of Lockwood. This technique enables bimaxillary movements to be planned with greater accuracy than with the Lockwood system.
