Renal abscess: emerging concepts of diagnosis and treatment.

A retrospective review of 12 cases of renal abscesses disclosed that gram-negative bacteria were the most common infecting organisms. There was a high incidence of underlying renal disease. Excretory urography was the most reliable investigative procedure, although renal arteriograms were necessary frequently. In carefully selected patients conservative non-operative management can be successfully but an operation was required for the majority of our patients.

Urethral strictures secondary to pelvic injury in children.

Surgical repair of a membranous urethral stricture is difficult because of the location and potential risks of incontinence, impotence and infertility. The treatment of 2 such strictures by the Badenoch pull-through urethroplasty is presented. The technique is described and its apparent advantages over other methods of repair are discussed.

A comparative study of salivation in affective disorders.

In two 4-week studies comparing the salivation of neurotic and endogenous patients, they were found to have reduced salivation in comparison to control group, but no statistically significant differences were noted between the neurotic and endogenous patients or when the two groups were compared with normal subjects. There was a statistically significant correlation between parotid salivation and sleep disturbance factor scores of the HAM-D in neurotic patients indicating greater sleep disturbance to be associated with higher salivation rates.

Diethylstilbestrol di-trimethylacetate (DSTMA). A long-acting estrogen.

A series of estradiol and diethylstilbestrol (DES) esters was prepared as part of a study of the structure-activity relations in estrogens. Among the esters tested, DES di-trimethylacetate (DSTMA) exhibited the most favorable combination of a low minimum-effective dose and prolongation of effect in all experimental groups (mice, rats, dogs). Toxicity of DSTMA was low. A clinical trial in four stage III and stage IV prostatic cancer patients showed that dosage levels of DSTMA as high as 75 mg per day for short periods were well tolerated and no toxic symptoms attributable to the estrogen itself were observed.

Rectal obstruction secondary to carcinoma of the prostate.

The records of 13 patients with symptomatic rectal obstruction secondary to prostatic carcinoma have been critically reviewed to provide criteria for identifying this atypical presentation. The site of obstruction varied from the anal verge to 17 cm by sigmoidoscopy. Significant clinical findings were ureteral obstruction on excretory urography in 12 of the patients, elevated acid phosphatase in eight, intact mucosa over the obstructing mass in seven and osteoblastic bone metastasis in six. Primary diagnosis was generally established by comparing histopathologically the prostate and the obstructing lesion. Colostomy was necessary in nine cases. In three patients colostomy was obviated in partially obstructing lesions by intravenous diethylstilbestrol diphosphate therapy. One patient had an inappropriate low anterior resection. Rectal obstruction is most commonly due to primary adenocarcinoma. Occasionally the obstruction is secondary to prostatic carcinoma masquerading as an intrinsic rectal cancer. Identification of the true nature of the illness is essential for proper treatment.

The role of ureteral stenting in the management of surgical injury of the ureter.

From 1965 through 1976, 35 patients with surgical injury of the ureter were treated by operations requiring ureteral anastomoses. Stenting devices were used in 31 patients with only 2 failures. Stents were omitted in 4 patients with 2 failures. We have been satisfied with and prefer stenting ureteral anastomoses in the management of surgical ureteral injury, particularly when there is delayed recognition of the injury or previous irradiation. Previous objections to stenting are unjustified.

[Thin-layer chromatography of active compounds from ointments and suppositories followed by direct quantitative analysis by remission (author's transl)].

The paper describes a method for simultaneous thin-layer chromatographic separation of hydrocortisone, hydrocortisone acetate or hydrocortisone caproate alongside dibucaine hydrochloride, hexachlorophene and clemizole undecylate as well as clemizole hexachlorophenate in ointments and suppositories. Development of thin-layer chromatograms is carried out on silica gel 60 F-254 pre-coated plates. All four active ingredients can be separated on one silica gel plate using one solvent system and determined directly by the remission method using a densitometer. Hydrocortisone and its two esters are measured at 248 nm, dibucaine hydrochloride at 325 nm, hexachlorophene at 300 nm, and clemizole undecylate as well as clemizole hexachlorophenate at 275 nm. Evaluation of thin-layer chromatograms takes place on-line from a linear calibration curve using an IBM 1800 computer. The described method is very suitable for analyses of these active ingredients in drug forms, such as ointments or suppositories and is reproducible with coefficients of variation of 1.29-3.56%.
