[Surgical treatment for intraductal proliferative lesions].

Intraductal proliferative lesions of the breast including benign, borderline and malignant diseases continue to be one of the most difficult diagnostic and therapeutic problems to the pathologist and surgeon. The optimal management should be done according to the different characteristics of the diseases. However, it is difficult to obtain the precise information of the individual disease before surgery. Selected ductolobular segmentectomy of the breast was performed in 73 patients who had been preoperatively diagnosed as having intraductal papilloma due to the clinical examinations. The specimen obtained from 25 patients were subjected to 3-dimensional (3-D) reconstruction analysis of their duct architectures. As the result, intraductal papilloma was divided into central papilloma and peripheral one. It revealed that none of the nine patients originating in the central duct had carcinoma and 6 of the 16 (37.5%) patients with peripheral papilloma coexisted with carcinoma in the terminal ductal-lobular units (TDLUs). This procedure is consisted of the total excision of affected ductolobular system. The favorable results is obtained in the cosmesis and breast feeding, and there is no recurrent case for the follow up period from 1 to 13 years.

[Endoscopic needle bladder neck suspension for female stress incontinence].

Thirteen operations of endoscopic bladder neck suspension (Raz method) were performed for 12 patients with stress incontinence between October, 1986 and August, 1988. In all patients, except for one with small bladder capacity, the operations were successful. Disappearance of incontinence was maintained in 10 patients 5-27 months (mean 15.3 months) after the operation, and the success rate of this operation was 77% (10/13) at this follow up point. The operative technique was easy except for the adjustment of pulling up strength of the thread for suspension. We believe that the endoscopic needle bladder neck suspension will be performed more widely because this operation can be done safely in a short time without major complications and can produce very good results.

[A case of retroperitoneal schwannoma].

A 39-year-old man, who had been pointed out to have a lower abdominal mass, was admitted to our hospital. Physical examination revealed a child-head-sized mass with smooth surface in the middle part of the lower abdomen. Computerized tomographic scan showed the non-cystic mass located in the retroperitoneal space from the level of L5 to S5. An open biopsy was performed and pathological diagnosis was benign schwannoma (Antoni A and B type). Radical operation could not be performed because of his refusal. In addition to this case, 40 cases of retroperitoneal schwannoma reported in Japan are reviewed and discussed.

[Infected solitary renal cyst extending to subphrenic abscess--a case study and review of the literature].

A 54-year-old female, who had been mediated for simple type schizophrenia for the past eleven years, was hospitalized with left lumbar discomfort or pain. After admission, the following abnormalities were apparent serologically; leucocytosis of 15700/mm, erythrocyte sedimentation rate of 147 mm/h, hyper alpha 2-globulinemia (675 ng/dl of haptoglobulin). Urine cultures were negative. Left renal simple cyst with infection was evaluated by intravenous urography, ultrasonogram, computed tomographic (CT) scan., and renal angiogram. In addition, left subphrenic abscess behind the spleen, which had to be penetrated through retroperitoneum, could be diagnosed. So, in our case, cystic wall resection (unroofing) and/or conservatively percutaneous cystotomy for pus drainage were all not indicated instead of radical operation for abscess removal completely. CT scan is mostly available to make differential diagnosis whether the infectious renal cyst influence to the neighbouring organs beyond the renal capsule, or not. Here, we discussed the pathogenesis on the infection of solitary renal cyst, and some problems reported in the literature.

[A case of renal leiomyoma with extensive cystic degeneration].

An extremely rare case of renal leiomyoma presenting as a cystic mass, involving the upper pole of the right kidney in a 59-year-old woman, complaining of right flank pain is reported. Some discussion regarding pathogenesis of leiomyoma with cystic change, diagnostic feature, malignant potential of leiomyoma and others was done. Since 1948, 19 cases including our own case of renal leiomyoma have been reported in the Japanese literature.

[Clinical application of immunoenzyme assay for prostatic acid phosphatase].

We compared the values obtained by double-antibody radioimmunoassay (RIA) and immunoenzyme assay (IEA) for measuring the prostatic acid phosphatase (PAP) in human serum. Mean PAP value of IEA in normal persons was 0.582 +/- 0.228 ng/ml (M+2SD, 1.038 ng/ml). Correction of two methods in benign prostatic hypertrophy (BPH) and prostate cancer patients was excellent [r = 0.9504, Y (IEA) = -0.4378 + 0.6529X (RIA)]. False positive cases were two by IEA and one by RIA in BPH patients.

[Clinicopathological study on IgA nephritis].

Clinicopathological study on IgA nephritis was carried out in order to clarify the factors to promote the progression of the disease. Total 118 cases of IgA nephritis confirmed with renal histology of the biopsied tissue were followed up in our kidney center at least two years following the biopsy. As a result, histological changes tended to be more severe in elder group at the time of biopsy and longer history of the disease. Renal function tended to be decreased in the cases with more proteinuria. Nephrotic syndrome seems to be one of the processes in which the disease is exacerbated. In addition, 21 cases with rather progressive courses following the biopsy revealed poor control of blood pressure, frequent upper respiratory infection, and more physical activities on their lives, which appear to be the risk factors. Clinical symptoms such as hematuria and proteinuria and histological changes were significantly more severe in the cases with IgA depositions spreading into the glomerular basement membrane, as compared to the ordinary cases with IgA depositions, limited in the mesangium. These results suggested that IgA depositions in glomerular basement membrane might be also one of the poor prognostic factors.

[Long-term follow-up study of transcatheter arterial chemotherapy in patients with advanced breast cancer].

Intra-arterial infusion chemotherapy (I.A) was applied to 32 cases with advanced breast cancer. The subclavian artery was intubated and one shot of 30 mg of adriamycin was given every 3 days, with a total dose of over 150 mg. Then mastectomy was performed after this chemotherapy. Long-term follow-up study revealed that the response rates of the primary tumor to I.A chemotherapy provided the prospects for their prognosis. The 3-and 5-year survival of good responders (% tumor regression greater than or equal to 70%) was significantly better than those of poor responders (% tumor regression less than 70%). Then, four patients with advanced breast cancer were treated with combination of I.A infusion chemotherapy of Adriamycin and photoradiation by argon laser. The mean interval time for 50% tumor regression was shorter than in the cases treated with Adriamycin alone. In vitro, the effects of the photoradiation for MCF-7 breast cancer cell line were observed only at a high concentration of Adriamycin.

[Squamous cell carcinoma co-existing vesical leukoplakia--case study and its clinicopathological analysis of the pathogenesis].

A 66-year-old female complained of fever and lumbar pain was diagnosed as the squamous cell carcinoma associated with vesical leukoplakia cystoscopically. Histopathological figures showed various forms of squamous metaplasia, involving squamous cell carcinoma nests within it. Now, the relationship between vesical leukoplakia and squamous cell carcinoma is not obscure. Then the treatment of the leukoplakia has been considered conservatively (fulguration, acidification of the urine and anti-microbials). Now, we can classify the lesions of squamous metaplasias into two types in the urinary bladder. The possibility of the malignant change in the leukoplakia lesions could be suggestive of the histological characteristics; irregularly down growth and disarrangement of the basal cells and/or dispersed basement membrane in rete pegs in the metaplasia.

Renal cell carcinoma with solitary contralateral adrenal metastasis.

We report on 2 cases of renal cell carcinoma with solitary contralateral adrenal metastasis which was demonstrated six months and one year, respectively, after radical nephrectomy. Pulmonary metastases developed six months after adrenalectomy in 1 patient; the other patient is alive without any evidence of disease twenty-two months after adrenalectomy.

[The influence of percutaneous nephrolithotomy on renal function evaluated by 99m-Tc-DTPA renoscintigram].

The influence of percutaneous nephrolithotomy (PNL) on renal function was evaluated by 99m-Tc-DTPA renoscintigraphy. The renoscintigram was recorded and regions of interest of both kidneys (R) and background (BKG) were settled. Renal uptake ratio was calculated by the following formula. Renal uptake ratio = (formula; see text). The renal uptake ratio of 50 patients were compared before and up to 6 months after PNL. Most cases showed an increase or no change in post-PNL renal uptake ratio in operated kidney. Five cases (10%) showed decreased renal uptake ratio of post-PNL. No risk factors for the renal damage caused by PNL such as age, sex, stone size, number of nephrostomy and course of PNL were detected when compared with increased or no change groups. In cases of complete staghorn calculi in the decreased renal uptake ratio group, compensatory hypertrophy of contralateral kidney PNL course might occur, and the function of the operated kidney might not have recovered. More during cases and longer follow-up is necessary for the recovery of renal function.

[A case report of non-functioning adrenal carcinoma diagnosed by percutaneous needle biopsy and which responded to preoperative chemotherapy].

A 51-year-old female complained of right upper abdominal pain. Non-functioning adrenal carcinoma was suspected from the laboratory data of elevated lactate dehydrogenase level and total amount of urinary excretion of 17-ketosteroid. To deny the possibility of other diseases, such as retroperitoneal sarcoma, a percutaneous needle biopsy of the tumor was carried out. As the pathological diagnosis was adrenal carcinoma, pre-operative chemotherapy consisting of cis-dichlorodiammineplatinum (CDDP), adriamycin, 5-fluorouracil, cyclophosphamide and o,p'-DDD was carried out twice. The tumor size decreased after chemotherapy, and we could operate on the patient safely and relatively easily. Almost all of the tumor was replaced by a necrotic mass. However, cancer cells still remained in the peripheral lesion of the tumor, and the pathological diagnosis was the same as that of biopsy specimen. Although chemotherapy of adrenal carcinoma has not been established yet, we believe from the experience of this case that this disease should be treated by chemotherapy including CDDP.

[Modulation of anticancer effects of immunochemotherapeutic agents in various nutritional environments].

It is well known that some kinds of immunological functions, s.c. NK activity, IFN or IL-2 production of spleen cells are decreased in mice with protein calorie malnutrition (PCM) compared with those of well-nourished mice. In this study, nutritional conditions of cancer bearers were demonstrated to have serious influence on the effects of immunochemotherapeutic agents as shown herewith. (1) Intra-tumoral 5-FU concentration was lower in PCM mice or starved mice than in mice fed a normal diet. (2) Transplanted mammary tumor, MM-48, was paradoxically grown in PCM mice after 7 days i.p. injection of OK-432 or Lentinan, but the tumor burden was relatively decreased after the same treatment in N-group mice. (4) The life prolongation effect of rIL-2 and OK-432 against ascitic hepatic carcinoma, MH-134, was recognized in C 3H/He mice. On the other hand, there was no observable effect by sequential i.p. injection of PCM mice sera together with rIL-2 and OK-432. (5) Clinically, a randomized study of advanced or recurrent gastric cancer patients treated with MMC and FT(MF) with or without Lentinan was performed. Excellent end-point results were obtained only in Lentinan-administered patients with normal protein levels, but no such effects were noted in patients with low protein levels (below 5.9 g/dl). These findings suggest that the nutritional environment of the cancer bearing host has an important role in the effect of some kinds of BRMs.
