[Treatment of advanced cancer of the urinary tract with interleukin-2 (IL-2)].

Eight patients with advanced cancer of the urinary tract were treated with recombinant interleukin-2(IL-2) at our department between December, 1985 and September, 1987. Patients were given 5 x 10(5)-4 x 10(6) units of IL-2 by intravenous drip infusion daily. One of five patients who received IL-2 for over 28 days showed 62% regression in size of hepatic metastasis of bladder cancer on computed tomography and another showed marked improvement of Performance Status. The upward tendency of natural killer and lymphokine-activated killer activity of peripheral blood was observed during treatment. Fever, mental disturbance, hypotension and eosinophilia and others were recognized during administration of IL-2, and cardiac failure and disturbance of renal function were also recognized as severe side effect.

[A case of renal pseudotumor].

A case of renal pseudotumor in a 47-year old female is reported. She was admitted with the complaint of right flank pain. Intravenous pyelography showed filling defect in right renal pelvis and spreading deformity of calices with compression of infundibuli. Ultrasonography and computed tomography demonstrated round renal mass. Since possibility of hypovascular tumor was not completely ruled out by right selective renal angiography, subsequently performed, then right nephrectomy was done. Bisected specimen revealed mal position of renal lobe (lobar dysmorphism). Microscopic appearance showed normal renal parenchyma. The literature is also reviewed.

[Anti-cancer effects of BRMs associated with nutrition in cancer patients].

Nutritional deficiencies are believed to be instrumental in producing reduced immune responses in a variety of animal species. Malnutrition may result in an increase or a decrease in immune functions, depending upon its degree, and also the timing and severity of the nutritional protein deprivation. Our experimental data suggest that there is a significant impairment of cytotoxic activity against K-562 and of the ability of spleen cells to produce interferon in protein-deprived mice in comparison with control mice. Paradoxically accelerated tumor growth after administration of OK-432 or Lentinan was also noted in protein-deficient tumor-bearing mice. In addition, a clinical randomized study of advanced or recurrent gastric cancer patients treated with MMC and FT(MF) with or without lentinan was performed. We recognized excellent end-point results only in the lentinan-treated patients with normal protein levels, while no effect of this agent was seen in patients with low serum protein levels (below 5.9/dl). Aggressive postoperative chemotherapy for cases with distant lymph node metastasis was performed under active nutritional support without any depression of metabolic and immunological states, resulting in a good 5-year survival rate (36.9%).

Anatomy of the prostate from fetus to adult--origin of benign prostatic hyperplasia.

The anatomy of the prostate was studied in specimens from humans ranging from fetal age to 79 years. At all ages, the prostate can be divided into two zones, inner and outer. The distribution and relative size of the prostatic lobes is similar in the fetus and in the adult. In the fetus, the inner zone can be subdivided into lateral, subcervical, and anterior lobes and mucosal glands. The anterior and subcervical lobes usually disappear in prepuberty but may reappear in advanced age. The outer zone can be subdivided into the middle and posterior lobes. Benign prostatic hyperplasia arises from the lateral, subcervical, and anterior lobes of the inner zone and the middle lobe of the outer zone. It is suggested that previous investigators failed to describe a demarcation line or territory between the lateral and posterior lobes.

Inverted papilloma of the ureter with malignant transformation: a case report and review of the literature. The importance of the recognition of the inverted papillary tumor of the ureter.

Inverted papilloma of the ureter is a rare tumor generally considered to be a benign lesion. We present a case of ureteral inverted papilloma with malignant transformation and review the literature. Ten cases of ureteral inverted papillary tumors in Japan and 13 cases of inverted papillary tumors associated with malignancy in the urinary tract in the world literature have been analyzed to define their features. Inverted papillary tumors of the ureter have distinct clinicopathological features, but most are discovered retrospectively after nephroureterectomy. From the standpoint of clinicopathological features, inverted papillary tumor of the ureter should be distinguished from its exophytic counterpart with respect to the treatment. If the tumor can be diagnosed macro- and microscopically during surgery as inverted papilloma or malignant inverted papillary tumor without invasion, partial ureterectomy including the lesion may be recommended. Furthermore, we propose the hypothesis that inverted papillary tumor could be a neoplasm of basal cell origin in the transitional epithelium, termed 'basal cell urothelioma'.

[A case of large diverticular stone of the anterior urethra].

A case of large diverticular stone of anterior urethra is reported. A 70-year-old man was admitted for dysuria, miction pain and hard mass beneath the scrotal skin. Retrograde urethrogram revealed a large stone in anterior urethral diverticulum and extravasation of contrast medium through the fistula from diverticulum. Open resection of diverticulum was performed and the stone was removed. Three months after the operation, the urethrogram showed no abnormal findings. The stone was 40 x 19 x 17 mm in size, weighed 16 g and was mainly composed of calcium phosphate.

[Influences of percutaneous nephrolithotomy on renal function, an observation using the NAG-index and beta 2-microglobulin as indices].

To examine the influence of percutaneous nephrolithotomy (PNL) on renal function, we measured the activity of urinary N-acetyl-beta-D-glucosaminidase (NAG) and beta 2-microglobulin (beta 2MG) in patients with nephrolithiasis, before and 3, 5, 7, 14 days and 1 month after PNL. With PNL, renal function was recovered within one month, and PNL was thought to be a more protective method than pyelolithotomy. Recovery of renal function was delayed in the cases in which PNL had been attempted over three times. In the patients with preoperative high-grade hydronephrosis, the excretion of NAG and beta 2MG in the urine was delayed. NAG-index was considered to be a more sensitive parameter than beta 2MG to evaluate renal function. The NAG-index and beta 2MG values of bladder urine were in good correlation with those of nephrostomy urine 5 days or more postoperatively. Measurement of the urinary NAG-index 3, 7, 14, 30 days after PNL operation was considered valuable for determining changes in renal function.

[Antitumor effect of carboplatin and iproplatin on human urinary bladder and prostatic cancers grown in nude mice].

Antitumor effects of carboplatin and iproplatin, a second-generation cisplatin analog were studied using cisplatin sensitive human urinary bladder (NM-B-1) and prostatic cancers (PRO-1) grown in nude mice. Both NM-B-1 and PRO-1 were sensitive to carboplatin and iproplatin. The tumor-regression effect of carboplatin at the fourfold dose of cisplatin was comparable to that of cisplatin. Iproplatin at the eight to sixteen times dose of cisplatin showed a comparable tumor regression to cisplatin. The range of effective dose was wider in carboplatin than cisplatin and that of iproplatin was not so wide as cisplatin.

[Experimental studies on the enhanced effects of chemoendocrine therapy in breast cancer].

The clinical response rate of chemotherapy or endocrine therapy for advanced or recurrent breast cancer has been described as being about 30%, regardless of steroid hormone status. However, the response rate for endocrine therapy is about double in patients with positive estrogen receptor. In order to obtain a higher response rate than that for single therapy, experimental attempts were made to combine cytotoxic agents, antiestrogenic agents, ablative therapy and immunotherapy from the viewpoints of tumor cell kinetics, natural killer activity and interferon activity of spleen cells. Tamoxifen and orchiectomy were shown by flow cytometry to produce a G1 block, increasing the G0, G1 phase and decreasing the S phase, in MCF-7 cells in vitro and in SC 115 cells, in vivo respectively, for a long period. On the other hand, 5-FU showed most effective cytotoxicity in the S phase of both types of tumor cells in vitro and in vivo, although the depressed S phase recovered within 24 hours. Therefore, any combined chemo-endocrine therapy should be devised so that the endocrine therapy maintains a G1 block for a long period and performed immediately following chemotherapy during the decreased S phase of the tumor cells. N K activity of spleen cells were enhanced, and interferon production in spleen cells was not changed by ovariectomy in C3H/He mice compared with sham-operated mice. It is suggested that endocrine therapy may affect the immunopotentiality of cancer patients.

[Metastatic behavior of footpad-inoculated tumors in mice and the impact of OK-432 per os administration].

In the surgical treatment of cancer patients, it is very important to investigate the immunological potential of regional lymph nodes. The purpose of this study was to clarify the defence mechanisms of regional lymph nodes against cancer cell dissemination. The materials used for the study were C3H/He mice and a syngeneic breast cancer cell line (MM-48). When MM-48 tumor, 5 X 10(6) cells per mouse, was inoculated into the footpad, the footpad gradually increased in size, and half of the mice died within 20 days. The frequencies of metastasis for ipsilateral inguinal lymph nodes, axillary lymph nodes and mesenteric lymph nodes were as follows. On day 3 after tumor inoculation, no metastasis was found in any node. On day 6, all mice had inguinal lymph node metastasis, and half of the mice had metastases to axillary lymph nodes and mesenteric lymph nodes. On day 10, all mice had metastasis to all nodes. When 100KE/kg of OK-432 was administered orally every day, the titer of natural killer activity of mesenteric lymph nodes was remarkably increased after 7 days (from 12.3% to 62.9%). When mice were administered OK-432 simultaneously with tumor inoculation and on each day thereafter, the frequency of metastasis mesenteric lymph nodes was markedly decreased after day 7. It is thus suggested that regional lymph nodes may operate effectively against tumor cell attachment to the nodes.

[A clinical study of urological examinations in uterus malignant tumors before treatment].

Sixty six cases of uterus malignant tumors examined urologically before gynecologic treatment between 1980 and 1984 are reviewed retrospectively. The uterus lesion excluding 1 malignant mixed mesodermal tumor was cervical carcinoma in 55 cases and corpus carcinoma in 10 cases. Pathologically, 46 cases were squamous cell carcinoma, 10 were adenocarcinoma, 3 were squamous adenocarcinoma and 6 were unknown. Clinical staging according to classification of FIGO, was stage 0 in 3 cases, stage I in 23 cases, stage II in 28 cases, stage III in 3 cases, stage IV in 7 cases, and unknown in 2 cases. Only 4 cases (6.1%) had urological symptoms with no special relationship to the uterus lesion. Abnormal findings of urine tests were seen in 15 cases (22.7%). The incidence of hematuria and pyuria, which were seen in 12 and 10 cases, respectively, was high. Cystoscopic abnormality was revealed in 32 (51.6%) out of 62 cases. Protrusion of vesical wall was found in 11 cases (34.3%) and it was an important finding which showed a significant incidence. Edema was seen in 11 cases (34.3%). This was a sign of progression, and the observation on spread of lesion was important. Redness was seen in 3 cases (4.8%). It was considered to be the result of circulatory disturbance. At the same time observations on vascular change, edematous change etc. should be done in detail. Bleeding was seen in 1 case (1.6%), microscopic bleeding was seen more frequently than macroscopic bleeding. Abnormalities of ureteral orifice were significant only after other examinations were reviewed in addition to these signs. IVU was performed in 58 cases.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of retroperitoneal fibrosis].

We report a case of retroperitoneal fibrosis. A 75-year-old man complained of edema of bilateral lower limbs and lumbago. Blood urea nitrogen and serum creatinine were increased. Renal function was improved after he had bilateral percutaneous nephrostomies. Antegrade pyelography showed bilateral hydronephrosis, left ureteral obstruction and medial deviation with narrowing of the right ureter. CT revealed a soft tissue density surrounding the aorta, inferior vena cava and bilateral ureters in the retroperitoneal space. Inferior venocavagraphy displayed stenosis. Bilateral ureterolysis combined with omental sleeve plasty was performed. Post-operatively, the clinical course has continued to be good.
