[Clinical evaluation of surgical treatment for benign prostatic hypertrophy].

Prostatectomy was performed on 300 patients at our Hospital for the period of 5 years from January 1978 to December 1982. Among those cases, clinical evaluation was made on 290 patients whose medical records were available. Ages of the subjects ranged from 53 to 92 years with an average age at 71.6. Retropubic prostatectomy (RPP) was performed on 81 cases, and suprapubic prostatectomy (SPP) and transurethral resection of prostate (TUR) on 39 and 170 cases, respectively. Surgery was made on 206 cases under epidural anesthesia, 82 cases under spinal anesthesia, and 2 cases under general anesthesia. Preoperative laboratory tests showed abnormalities in 62% of the total. The most frequent preoperative complications were circulatory abnormality and urinary tract infection. Average surgical time was 78.6 min. for RPP, 69.8 min. for SPP, and 76.9 min. for TUR. Average blood volume transfused during operation was 446.3 ml for RPP, 430.8 ml for SPP, and 80.7 ml for TUR. Average period of catheter retention after surgery was 9.2 days for RPP, 10.1 days for SPP, and 4.9 days for TUR. Average postoperative admission period was 18.6 days for RPP, 20.6 days for SPP, and 14.1 days for TUR. Average weight of the isolated adenoma was 41.8 g for RPP, 30.2 g for SPP, 11.5 g for TUR. Postoperative complications such as epididymitis, traumatic infection, and hepatic dysfunction were often found. Latent prostatic carcinoma found in the isolated adenoma was found in 11 cases (3.8%). The presence of anomalies in preoperative laboratory findings did not influence the frequency of postoperative complications.

[A case of signet ring cell carcinoma of the urinary bladder].

A 60-year-old female complained of gross hematuria and urinary frequency on November 27th, 1982. Cystoscopic examination revealed papillary invasive tumor around the bladder neck and a transurethral biopsy showed signet ring cell carcinoma. Since there was no adenocarcinoma in any other organs, we diagnosed it as primary signet ring cell carcinoma of the urinary bladder. Total cystectomy with ileal conduit and post-operative irradiation were performed, but she died on May 13th, 1983. We summarize 16 cases of primary signet ring cell carcinoma of the urinary bladder including this case and discuss this rare condition.

[A case of renal cell carcinoma difficult to diagnose preoperatively because of hypovascular findings].

A 35-year-old man visited our out-patient clinic with the chief complaint of macroscopic hematuria. After admission to the hospital, examinations by CT scan, renal angiography, etc. revealed a hypovascular space-occupying lesion in the right kidney, but no definite diagnosis could be made; blood findings such as accentuated alpha 2-globulin and raised LDH suggested the possibility of malignancy; and right nephrectomy was performed. In view of the preoperative X-ray findings together with postoperative pathological findings, papillary renal cell carcinoma was suspected intensively. Some discussion was made on the differentiation of lesions of hypovascular to avascular tumors and the reliability of image diagnosis based on CT scan, angiography, and other methods.

[A case of prostatic malignant lymphoma].

A 65-year-old man was admitted for dysuria. He had been irradiated 60Co for malignant lymphoma of tonsils 2 years earlier. The findings of palpation of prostate, retrograde urethrogram and urethroscopy strongly suggested benign prostatic hypertrophy. Retropubic prostatectomy was performed and 18 g of "adenoma" was resected. By histological observation, the "adenoma" proved to be malignant lymphoma. This tumor belonged to follicular lymphoma, medium-sized cell type of LSG non-Hodgkin's lymphoma classification. After the operation, he left our hospital for a personal reason and received systemic chemotherapy at another hospital.

[Correlation between the growth of transplantable tumors under protein deprivation and intratumoral amino acid concentration].

Nutritional restriction in the diet has a deleterious influence on tissue and transplantable tumor growths. Correlation between the growth of 81 B or MM 48 mammary tumors and intratumoral amino acid concentration was studied in syngeneic C3H/He mice fed a diet with a low (D) or normal (N) protein content. D-mice revealed a relatively longer lag phase than N-mice, but showed almost the same growth curve. Some kinds of amino acid, alanine, valin, glycine, and so on were recognized in D-mice tumors at higher levels more than in N-mice. In protein malnutrition, urea, the final amino acid degradation product, was not detected in the transplanted tumor or muscle of D-mice. The high tissue levels of alanine, glycine and glutamine are undoubtedly related to their participation in many metabolic reactions, so that they are readily and extensively synthesized. We speculate that D-mice would maintain an adequate intratumoral amino acid level through interorgan relationships in order for the transplanted tumor to grow.

[Male adenomatoid tumor: report of two cases and a review of 100 cases in Japan].

Two cases of adenomatoid tumor are reported. The patients visited our hospital because of a painless mass in the scrotum. Preoperative diagnoses were epididymal tuberculosis in the first case and epididymal tumor in the second case. Removal of tumors were performed and histological diagnoses were adenomatoid tumor of the epididymis in both cases. One hundred cases of male adenomatoid tumor which have been reported in Japan are discussed and reviewed.

[A clinical study on tumor of the renal pelvis and the ureter].

The 39 cases of renal pelvic and ureteral tumors treated in our hospital from 1974 to 1983, were reviewed retrospectively. The 39 cases included 15 renal pelvic tumors, 21 ureter tumors and 3 ureteropelvic tumors. The incidence of these tumors was 0.15% among all new outpatients. Sex distribution was 25 males to 14 females, and mean age was 65.2 years old. Hematuria was the most frequent initial symptom and chief complaint. Of the patients, 28.6% had come to our hospital within one month after their initial symptoms appeared. "Filling defect", an important finding of IVP and RP, was revealed most frequently. The positive rate of urine cytology was 33.4%. Histological examination revealed 14 transitional cell carcinoma and 1 papilloma of 15 renal pelvic tumors and 19 transitional cell carcinoma, 1 polyp and 1 metastatic adenocarcinoma of 21 ureter tumors. Operative therapy was performed on 36 patients; 26 by nephroureterectomy with partial cystectomy. Vesical recurrence rate was 18.9%. The 5-year survival rate was 37.4% on renal pelvic tumor and 40.7% on ureter tumor. The factors, high grade, high stage, and ureteropelvic double tumor, which reflect the specificity of the malignant potential, also affected prognosis. In conclusion, post-operative adjuvant therapy was thought to be valuable in the near future.

[Clinical study of ureteral polyp: three cases report and review].

Three cases of ureteral polyp are reported. The first case was an adult fibrous polyp. The second was the first report as a single, lower ureteral polyp in a child in Japan. The third was a granulomatous polyp accompanied by ureteral stones. About 130 cases of ureteral polyp have been reported in Japan. Although it has been classified into 4 to 7 histological types, the concept of ureteral polyp itself is confusing as its origin remains controversial.

[Severe cyclophosphamide hemorrhagic cystitis controlled with transurethral electrocoagulation: a case report].

A case of massive cyclophosphamide hemorrhagic cystitis is presented. A 47-year-old woman with Evans syndrome was treated with a total dose of 56.6 g of cyclophosphamide for 2 years. Six months after the cessation of administration, massive hemorrhagic cystitis suddenly occurred following a common cold. Embolization of bilateral internal iliac arteries was ineffective. Therefore we performed transurethral electrocoagulation broadly for bleeding mucosa. The day after operation, bleeding from the bladder remarkably decreased and further transfusions were hardly required.

[A case of Fournier's gangrene requiring extensive debridement including bilateral orchiectomy and amputation of the penis].

With the evolution of antibiotic agents, necrotizing fasciitis of the male genitalia first described by Fournier has become rare. However, its mortality rate is still high. We present a severe case of this disease. An 80-year-old man who had poor controlled diabetes mellitus was admitted to our hospital with painful swelling of penoscrotal region. Chemotherapy using broad-spectrum antibiotics and drainage of penis, scrotum and lower abdominal wall was performed combined with insulin therapy. Despite the drainage and aggressive chemotherapy with insulin therapy, the gangrene enlarged rapidly and the patient's general condition was getting very poor. Then, we performed extensive debridement including bilateral orchiectomy and amputation of the penis. After the operation, the patient became afebrile and his general condition was improved.

[The ectopic ureter: a report of 7 cases].

Seven cases, 6 females and a male, of extravesical ureteral ectopia have been reviewed. In a girl, the ectopic ureter drained the upper pole moiety of the duplicated system. There were 6 patients with single ectopic ureters, including a case with absence of the contralateral kidney. Careful inspection of the vaginal wall and endoscopic examination of the urethra revealed the ectopic ureteral orifice in most cases. The use of renal scan was helpful in the diagnosis of a poorly functioning kidney which could not be demonstrated on intravenous urography. When associated with a poorly functioning dysplastic kidney, it was managed by nephroureterectomy. Ureteral reimplantation was performed in 2 cases, but no improvement of continence was noted in the case of the solitary kidney.

[Clinical examination of torsion of the spermatic cord--review of 177 cases reported in the recent Japanese literature including our present 7 cases].

Between January 1974 and December 1984, 16 patients underwent surgical exploration at our Hospital. Seven of these patients proved to have torsion of the spermatic cord and nine were strongly suspected to have this lesion in spite of lack of torsion during operation and the remaining 3 patients suffered from epididymitis. The 7 patients with torsion ranged in age from 14 to 22 years old. The left side was involved in 4 patients, and the right side in 3 patients. Six patients had internal rotation and 1 had external rotation. In all of them, intravaginal torsion was noticed at various degrees of twist (270 degrees-540 degrees). Orchidopexy was performed in 5 cases in which the operation could be done within 15 hours after onset of the symptom but in 2 cases in which 50, and 56 hours had elapsed orchidectomy was unavoidable. The 177 cases reported in the Japanese literature since Kumon's report including our present 7 cases were reviewed statistically in detail. Case reports of this lesion, especially in newborns, is increasing every year. The age distribution showed two peaks, one at the time of puberty (10-19 years old) and the other shortly after birth. There was a predominance for the left side, internal rotation and twisted degrees of 180 degrees to 360 degrees in this series. In the cases in which over 24 hours has elapsed before surgery, orchidectomy was performed frequently.

[Significance of active nutritional support for maintenance of immunopotentiation on the surgical treatment of esophageal cancer].

The correlation between nutrition and immunological potentiation on patients with cancer of the esophagus was investigated in preoperative and postoperative periods. Preoperative immunological status: These patients were already suffering from malnutrition, because of the impossibility of peros intake. Natural killer (NK) cell activity and interleukin-II inducing activity of peripheral mononuclear cells in the above patients were clearly lower than such activities of cells in the patients with breast cancer whose nutritional condition was as normal as that of healthy people. These two immunological parameters in the majority of the patients were improved by active total parenteral and enteral nutrition. However, there were a few patients whose nutritional status could not be improved. Postoperative immunological status: The movement of peripheral lymphocyte subpopulation was successively checked with several kinds of monoclonal antibodies such as OKT-3 (anti-T cells), Leu-7 (anti-NK cells), OKIa-1 (anti-B cells), OKT-4 (anti-inducer and/or suppressor T cells) and OKT-8 (suppressor and/or cytotoxic T cells). In nutritionally controlled patients, the population of OKT-3, Leu-7 and OKIa-1 showed a pattern similar to the preoperative value, but in undernourished patients, that population was disturbed soon after the operation. Patterns of OKT-4 and OKT-8 showed a converse correlation. Nearly two weeks after operation, all these five kinds of population had changed. Moreover, nitrogen balance recovered from negative to positive. The time of is thought to be the turning point of surgical metabolism and the immunological status.

[A case of seminoma with epididymal swelling].

A 36-year-old man noticed a discomfort in the right scrotum since January, 1981, and came to our hospital in October of the same year. Palpation detected a hard and painless tumor in the right epididymal tail and exploratory excision revealed an induration measuring 3 X 2 X 1 cm. The rapid diagnosis of tumors by frozen section was made and we diagnosed it as right epididymal seminoma. Right high inguinal orchiectomy was performed and histopathologically, tumor cells which mainly occupied the testis were observed. This case was diagnosed as seminoma which showed epididymal swelling as the clinical symptom.

Assessment of P300 latencies for individual EEG responses employing an adaptive correlating filter.

Correlations between latencies of P300 components in evoked potentials and reaction times were investigated in two normal subjects. The subjects were required to perform choice reaction tasks, responding to particular tones among three tones with different frequencies. EEGs were recorded at the Pz region monopolarly. Latencies of P300 for individual EEG responses were measured employing an adaptive correlating filter. Cross-correlation coefficients between the second templates employed to identify P300s and individual responses for the subjects ranged from 0.623 to 0.997, with the mean value being 0.931. The correlation coefficient between latencies of P300 and reaction times for the subjects was 0.659. Thus, the adaptive correlating filter was proved to be useful in psychophysiological studies.

[Chemotherapy of metastatic renal cell carcinoma with cis-diamminedichloroplatinum and other drugs].

Seven patients were entered in this study between November 1978 and February 1983. All patients had histologically proven renal cell carcinoma with widespread metastases. Patients ranged in age from 27 to 69 years, with an average of 54 years. All those studied had measurable radiological lesions of malignant disease. One patient was treated with only CDDP and the other 6 patients were treated with CDDP and other anti-cancer drugs (vinblastine, ifosfamide, bleomycin and/or adriamycin). Total doses of CDDP ranged from 20 to 250 mg, with an average of 99 mg. Four cases showed no change and 3 patients had progressive disease. Symptomatic improvement was obtained in only 2 patients. Management of metastatic renal cell carcinoma is discussed briefly.

[Carcinoma of urachus: report of 3 cases and review of literature].

Three cases of urachal carcinoma are presented. Chief complaints were passage of mucous urine in a 58-year-old man (case 1), hematuria and frequency in a 63-year-old woman (case 2) and hematuria in a 45-year-old man (case 3). Urine cytology were negative for all cases and serum CEA level was elevated in case 1. Mass in the area of the bladder dome were revealed by cystoscopic examination in all cases. CT scanning, TUR biopsy and cystogram were valuable diagnostic procedure. En bloc segmental resection were performed on all cases, and case 1 and case 2 have been well without disease for 36 and 40 months, respectively. Case 3 died 65 months after operation with disseminated carcinoma. Histologically mucin-producing adenocarcinoma were found in all cases. Statistic examination and discussion are made of 237 cases of urachal carcinoma reported in Japan.
