Prosthetic materials and wound healing. Critical evaluation of six different materials.

Six different materials (three mesh and three cloth) commonly used for the repair of abdominal wall hernias were evaluated in 180 rats. Macroscopic, bacteriologic, mechanical and histologic criteria were used to investigate tolerance and resistance of the foreign material. Findings demonstrate the superiority of mesh over cloth materials.

Hepatic resection for isolated tumor-like nodules in the liver.

We have reported two cases of focal nodular hyperplasia of the liver in adults. One case is the first in which the regenerative process that occurred nine months after localized hepatic necrosis was implicated in the development of focal nodular hyperplasia. The other case concerned an idiopathic isolated liver nodule that occurred in a woman who never had taken oral contraceptives. These tumor-like lesions were removed by a right and a partial hepatectomy respectively. The two patients remained asymptomatic during the first 13 postoperative months with no additional therapy.

[Contribution to the study of gallstones. Morphological, radiological and cristallographic analysis (author's transl)].

The structure of gallstones has become of recent interest with the medical treatment of cholesterol stones. It seemed to us interesting to determine by morphological, radiological and above all cristallographic criteria the constituants of gallstones in order to draw up a practical classification which reflects better the biological reality. The present study confirms the existence of cholesterol and calcium carbonate in various types of stone, but reveals in addition the presence of cholesterol and bile salts associated with new mineral constituants identified for the first time (aluminium chlorite and silicate). The notion of pure stones, generally attributed to cholesterol and pigmented stones was only verified in the case of mineral stones.

[Serum alpha 1-fetoprotein and extrahepatic digestive cancers. Apropos of 4 further cases].

The authors report four new cases of primary digestive carcinoma other than hepatoma with alpha 1 feto-protein in the serum (greater than 200 ng/ml). Two were carcinoma of the colon without liver metastases. The remaining two were also colonic carcinoma but with liver metastases. In the first cases, alpha 1 feto-protein disappear after surgical procedure. In spite of the rareness of primary digestive carcinoma with presence of alpha 1 feto-protein noted until now, these cases require reconsideration of the idea that AFP is specific for hepatoma.

Critical evaluation of prosthetic materials in repair of abdominal wall hernias: new criteria of tolerance and resistance.

Six different materials (3 mesh and 3 cloth) commonly used for the repair of abdominal wall hernias were evaluated in 180 rats after implantation through musculature and peritoneum. Serial macroscopic and bacteriologic investigations were done, and bursting strength of the wound was determined in all groups at intervals with an original device. Histologic criteria were used to characterize the resistance of the wound and the tolerance of the host to the foreign material. Statistical analysis of the results demonstrated: (1) after postoperative day 15, the resistance of the wound was similar for each material tested; (2) during this early period mesh materials exhibited more resistance to bursting pressures than cloth materials; (3) the incorporation of mesh material was constant, whereas encystment or extrusion was always observed after implantation of cloth material; (4) no infection occurred with mesh material, but significant bacteria were found in 18 per cent of cloth material implantations; (5) the extent of the cellular reaction, the enumeration of giant, inflammatory, and fibroblastic cells showed the superiority of mesh materials; and (6) the ratio of fibroblasts to inflammatory cells reflected closely the mechanical resistance and tolerance of the foreign material.

[Villous tumor of the doudenum (author's transl)].

Villous tumors of the duodenum are rare, but treatment may be problematic because of their association with invasive adenocarcinoma. One case of villous tumor of the duodenum is described and 44 others reported cases are reviewed. Diagnosis may be made by radiographic barium contrast evaluation of the duodenum, especially with the addition of air contrast hypotonic studies and by fibro-optic endoscopy with biopsies. 32 p. cent of villous tumors of the duodenum are associated with adenocarcinoma. Local excision is the treatment of choice for benign lesions. Duodenectomy or pancreatico-duodenectomy are recommended for tumors which include invasive carcinoma.

[Resistance and biological tolerance of six "inert" parietal protheses. Experimental and critical study (author's transl)].

Six different materials (3 mesh and 3 cloth) commonly used for the repair of abdominal wall hernias were evaluated in rats. 490 animals, distributed among four experimental groups, were implanted with one of the different prostheses through musculature and peritoneum. Serial macroscopic and bacteriological investigations were done. Using an original device, bursting strength of the wound was determined in all groups at intervals. Histological criteria were used to characterize the resistance of the wound and the tolerance of the host to foreign material. Statistical analysis of the results demonstrated that: 1) after the 15th post-operative day, the resistance of the wound was similar for each material tested; 2) during this early period mesh materials exhibited more resistance to bursting pressures than cloth materials; 3) the incorporation of mesh material was constant whereas encystment or extrusion was always observed after implantation of cloth material; 4) no infection occurred with mesh material but significant bacteria were found in 18 p. 100 of cloth material implantations; 5) the extent of the cellular reaction, the enumeration of giant, inflammatory cells and fibroblasts showed superiority of mesh material; 6) the ratio of fibroblasts/inflammatory cells reflected closely the mechanical resistance and tolerance of the foreign material.

The biliary excretion of cefazolin.

The biliary excretion of cefazolin was studied by perfusion of isolated rabbit livers. Under these conditions, 6.7 percent of the amount of cefazolin added to the circulating blood was excreted in bile, and concentrations of drug in bile were significantly higher than concentrations in serum. Biliary excretion of cefazolin was also studied in humans after the intravenous administration of 500 mg of the drug. In 10 normal subjects, maximal concentrations of cefazolin (17.10 plus or minus 8.5 mug/ml) in the fluid obtained by duodenal tubing were attained 1-2 hr after administration of cefazolin. Concentrations were similar in bile collected by external drainage from five cholecystectomized patients (maximal levels, 14.0 plus or minus 4.7 mug/ml) 1-2 hr after injection. Assays during cholecystectomy showed that 1 hr after injection, cefazolin levels were much higher in common duct bile (31 plus or minus 11 mug/ml) than in gallbladder bile (5 plus or minus 2 mug/ml).
