[Recent advances in hypoxic cell radiosensitizer].

The comparative studies on the radiosensitizing efficacy of misonidazole (MISO) and SR-2508, a new 2-nitroimidazole which is less neurotoxic than MISO, were performed using C3H/He mouse mammary tumors. The enhancement ratios of 1.42 for 0.5 mmole/kg of MISO and 1.82 for 1.5 mmole/kg of SR-2508, the equitoxic dose to MISO, were obtained when each drug was administered in combination with a single dose of radiation. When the radiation was delivered in three and five fractions, enhancement ratios decreased to 1.15 and 1.11 for MISO while they remained at 1.40 and 1.34 for SR-2508 respectively. Potentiating effect of diethyl maleate (DEM) on the radiosensitization of MISO was also studied using the same experimental tumor. Although DEM, in itself, has a radiosensitizing effect in the dose range of more than 600mg/kg combined use of DEM and MISO showed more efficient synergism, e.g., the enhancement ratio of 0.5 mmole/kg of MISO increased from 1.42 to 2.06 when it was given in combination with 760 mg/kg of DEM. However, synergistic effects of MISO and DEM observed were dependent upon given dose of each drug. i.e.

Adenoid cystic carcinoma of lacrimal gland.

Adenoid cystic carcinoma of the lacrimal gland is a relatively rare disease. Only two cases, a man of 61 and a woman of 54 were seen in our clinic from 1962-1981. They presented histopathology of the cribriform pattern characteristic of adenoid cystic carcinoma. The lesion in the male patient belonged to the solid type and was mainly composed of solid nests. It is emphasized that radiotherapy in addition to surgical excision of the tumor is recommended because of its infiltrative nature.

[Results of radiotherapy of primary lung cancer with reference to metastases in regional lymph nodes].

The results of radiotherapy in the treatment of primary lung cancer is presented with reference to metastases in regional lymph nodes. The study involved 382 patients with histologically proven lung cancer of all cell types who, between 1945 and 1955, initially underwent definitive radiotherapy with TDF's of more than 70. The evaluable patients comprised 66 for N0, 177 for N1 and 139 for N2. Cumulative five-year survival rates for N0, N1 and N2 were 23.9%, 6.1% and 5.6%, respectively. Although surgical treatment before radiotherapy did not significantly improve a cumulative five-year survival rate for each N category, it kept patients alive longer for the median survival time (month). In 89 patients with involved mediastinal lymph nodes that specific sites were identified at the time of the surgical treatment (pN2), prolonged survival was obtained by radiotherapy if lymphatic spread was absent in the sites of #1, #2, #3, #5 and #6. If present, however, none of patients were alive at the post-treatment time of three years. As to 155 patients with stage III disease comprising T3 and/or N2, both cumulative five-year survival rate and median survival time showed better results in the group of N2 disease than those in T3.

Possible mechanisms involved in the spontaneous rhythmicity in the guinea-pig tracheal smooth muscle as induced by glucose removal.

The rhythmic contraction induced by glucose removal was not eliminated by tetrodotoxin, atropine, propranolol or indomethacin, but was abolished by Ca2+ removal from the solution or administration of verapamil and was increased by Ca2+-ionophore (A-23187). The rhythmicity was eliminated by ouabain (1.7 X 10(-6) - 1.7 X 10(-5) M), and was also abolished by removal of potassium from the solution but re-appeared after re-admission of potassium. The addition of calcium or sodium to preparations incubated in calcium or sodium and glucose-free solution in the presence of ouabain did not induce rhythmicity. Our findings suggest that the rhythmicity produced by glucose removal in the guinea-pig tracheal smooth muscle is myogenic in origin and involves a depolarization-dependent calcium influx. The possibility that the rhythmicity may be related to alterations in Na-K ATPase activity was not ruled out.

[Intraoperative radiotherapy for locally advanced refractory cancer].

Clinical results of intraoperative radiotherapy (IOR) in carcinoma of the stomach and prostate, and malignant soft tissue tumors are reported. The 5-year survival rate was found to be increased by IOR in stages II-IV gastric cancer. From the analysis of the clinical results of prostatic cancer, a single dose of 3,500 rad was considered to be a potential curative dose for the tumor less than 3 cm in diameter. The local recurrence rate of patients with malignant soft tissue tumors who received a single dose ranging from 3,000 to 4,500 rad was 5.9 and the 5-year survival rate was 64.6%.

Gamma heavy chain disease. Ultrastructural and immunohistochemical study.

Electron microscopic and immunohistochemical studies were carried out on materials obtained from three patients of gamma heavy chain disease (gamma-HCD). Electron microscopically, proliferating cells showed various stages of maturation from immunoblast to plasma cell, and the majority of proliferating cells were proplasmacytes and plasma cells. From the intracytoplasmic immunoglobulin studies by immunoperoxidase method (PAP method) and electron microscopical enzyme-labeled antibody technique, proliferating cells, such as the immunoblast, plasmablast, proplasmacyte, and plasma cell, showed positive reaction to anti-gamma-heavy chain serum and anti-Fc fragment (IgG) serum, and also in a third case with Bence Jones protein, proliferating cells showed positive reaction to anti-kappa light chain serum. We would conclude that proliferating cells in gamma-HCD might be a single clone proliferation of B-cell synthesizing gamma-HCD protein, and the predominant proliferation cells are proplasmacytes and plasma cells situated near mature plasma cells in the B-cell line.

[Histological studies of the effect of intra-operative irradiation on osteosarcoma].

At the Department of Orthopaedic Surgery of Kyoto University Hospital, intra-operative radiotherapy by the high dose electron irradiation has so far been used in treating patients with osteosarcoma. The dose distribution was calculated by CT-number so that the focus was subject to at least 80% of the irradiation doses and the surgically opened lesion was irradiated with 5,000 rad of 12-26 MeV electron beams. In order to accurately assess its therapeutic effect, histological studies were conducted on 9 patients who underwent en bloc resection or amputation following irradiation treatment for osteosarcoma. Method The excised specimen was sagittally cut in half and was fixed in 10% neutral buffered formalin. The specimen was cut into 5 mm thick sections corresponding to CT cross sections. At the border region between irradiated and non-irradiated fields, specimens were prepared using sagittal sections to compare the histological findings of both fields. After all specimens were carefully macroscopically examined and then photographed with soft-X-ray film, some of them were prepared as non- delimed specimens in polyester resin and were observed by contact microradiography and fluorescent microscopy. The remaining specimens after deliming in Plank - Rychlo solution were sliced to 4-8 micron thick for study as large specimen stained with hematoxylin and eosin. Four patients had been administered tetracycline 3 days preoperatively. Results Histologically the changes of the specimens were evaluated using the Oohoshi and Shimosato 's criteria for the histological effects of irradiation. In two of the 9 patients, who had received unidirectional irradiation, the survival of Grade 2a-2b tumor tissue was observed in the superficial layers both inside and outside the periosteum. Six patients who were irradiated from both sides or three directions, showed complete necrosis in extremely small areas of Grade 2b-3 change of tumor cells. Only one of these 9 patients had a local recurrence after the irradiation. Marked fluorescence was observed in the recurrent tissue by tetracycline labeling, but no fluorescence was observed in any of the other cases in the tumorous tissues which had fallen within the range of irradiation. These findings strongly suggest that intra-operative irradiation is extremely effective in controlling local osteosarcoma lesion in the extremities.

Nodal B-cell lymphomas in Japan--particularly in Tohoku district.

There are different frequencies in the immunological phenotypes of malignant lymphomas in Tohoku and Kyushu districts of Japan. In the Tohoku district, the northern area of Honshu, B-cell lymphomas are more preponderant than T-cell lymphomas. This is just the reverse on the islands of Kyushu and Shikoku. Histologically diffuse lymphoma of large cell type, formerly termed reticulum cell sarcoma or histiocytic lymphoma, was the most frequent (48%) among B-cell lymphomas. It is characteristic of B-cell lymphoma that immunoglobulin is produced in either or both the cellular surface and cytoplasm. Cytoplasmic IgM in lymphoma cells was mainly detected by an electron microscopic enzyme-labeled method. Cytoplasmic-Ig was present both in the nuclear membrane and endoplasmic reticulum. This technique is particularly useful in medium-sized lymphoma cells because of the scanty cytoplasmic rim making light microscopic evaluation difficult. Histological transition from follicular to diffuse pattern is characterized by a change of cellular arrangement from labyrinth-like cellular connections in follicular lymphoma to more simple connections in diffuse lymphoma. The transition is also supported by the fact that a higher deoxyribonucleic acid content is observed in large cells than medium-sized cells in follicular lymphoma. The data also supports the hypothesis that a diffuse lymphoma evolved from follicular lymphoma mainly occurs in cases of large cell lymphomas.

[Determination of malignancy of cartilaginous tumors (2). Determination of malignancy by nuclear DNA contents and their distribution patterns in cartilaginous tumors].

In 40 different types of cartilaginous tumors, the number of binuclear cells was calculated and DNA content was examined using fluorescent Feulgen cytophotometric technique. In 7 enchondromas and 1 synovial chondromatosis, the DNA content was diploid in 7 and hypotetraploid in 1, and showed a unimodal normal distribution. The number of binuclear cells was 0.1-0.45%, and the mean DNA content was below 4. These cases showed no evidence of disease after curettage and bone graft. The follow-up period was from 4 years 6 months to 7 years 1 month, averaging 6 years 2 months. In 2 enchondromatoses and 2 benign chondroblastomas, the DNA content was diploid and showed a unimodal normal distribution. One chondromyxoid fibroma showed a broad-unimodal tetraploid distribution, and one benign chondroblastoma showed a broad-aneuploid distribution. The nuclear DNA content was sometimes histologically similar to that of chondrosarcoma in some benign cartilaginous tumors. In 5 secondary chondrosarcomas, the DNA content was hypo- or hypertetraploid, hexaploid, and showed broad-unimodal or broad-bimodal distribution. The number of binuclear cells was from 13% to 23%. Although the size of the nucleus was small, the number of binuclear cells was evidently numerous. The preferred treatment for secondary chondrosarcoma is principally wide resection, but amputation is sometimes indicated in cases of recurrence and large tumor size. The follow-up period was from 2 years 2 months to 18 years 11 months, averaging 9 years 11 months. All 5 patients were surviving at the time of follow-up. The DNA content in chondrosarcoma was mostly hypo- or hypertetraploid, hexaploid, or octaploid and showed broad-unimodal, broad-bimodal or broad-aneuploid distribution. The number of binuclear cells was 0.55-3.5%. The mean DNA content was proportional to the number of binuclear cells, the positive correlation between the mean DNA content and the number of binuclear cells was found (p less than 1%).
