The correlation between cell survival curve and dose response curve of micronucleus (MN) frequency.

The correlation between cell survival curve and dose response curve of MN frequency following irradiations was studied using cytokinesis-block method. Both dose response curves were analyzed by linear quadratic model, i.e. SF = exp (-alpha D-beta D2) and MN frequency = aD + bD2 + C. A good correlation between alpha/beta and a/b ratios was observed in repeated paired experiments (= 0.97). When the cells were treated with BUdR, alpha-type radiosensitizer, a value in dose response curve of MN frequency increased but b value did not. These data present that MN frequency assay using cytokinesis-block method is available as a tool of rapid assay of radiosensitivity of cells.

[Crossed cerebellar diaschisis in hydrocephalus--a case report].

A case of hydrocephalus in which a crossed cerebellar diaschisis with single photon emission CT (SPECT) using 123I-iodoamphetamine (123I-IMP) disappeared by a ventriculo-peritoneal shunting operation, was reported. A 54-year-old female had a clipping surgery for a aneurysm of the left middle cerebral artery on April 9, 1987. Post-operatively, she had a transient mild aphasia. Cerebral blood flow (CBF) study with 123I-IMP-SPECT had showed low perfusion in the left frontal and temporal lobe. But after a month, clinical symptom and CBF findings had improved. She had been doing well after the discharge until October, 1987, when she developed disorientation. CT on re-admission showed the ventricular dilatation with periventricular low density (left greater than right). 123I-IMP-SPECT study showed the crossed cerebellar diaschisis which was a low perfusion in the left frontal, temporal, parietal, thalamus, basal ganglionic area and in the opposite cerebellum. A ventriculo-peritoneal shunting operation was performed which brought the disappearance of the crossed cerebellar diaschisis and the clinical improvement. The phenomenon of crossed cerebellar diaschisis has been reported in a lot of literatures. However, those diseases were located in unilateral supratentorium. It has been speculated that crossed cerebellar diaschisis is the transneural metabolic change through the cortico-ponto-cerebellar pathway by supratentorial lesion. Crossed cerebellar diaschisis in hydrocephalus has not been reported so far.(ABSTRACT TRUNCATED AT 250 WORDS)

[Evaluation of cerebral hemodynamics by 123I-IMP in cerebral AVMs before and after operation by using SPECT].

To evaluate cerebral hemodynamics by N-isopropyl-p-[123I]-iodoamphetamine (123I-IMP) preoperative and postoperative (after 1 day, and after 7-10 days) regional cerebral blood flow scan, 6 arteriovenous malformations (AVMs) which performed total removal operations were studied by using a ring type SPECT "HEADTOME". We performed the dynamic scan just after the intravenous injection of 222 MBq (6 mCi) of 123I-IMP, then the static scan 20 minutes after the intravenous injection. In preoperative dynamic scans of all cases, only the first frame image showed the increased activity on the nidus probably because of the blood pool. Preoperative static scans of all cases showed the remarkable decreased activity on the nidus, and decreased activity surrounding the nidus probably because of the peripheral steal phenomenon. In postoperative scan of the next day, 4 out of 6 cases showed the transient decreased peripheral steal, particularly 2 out of those 4 cases showed the transient hyperperfusion probably because of the normal perfusion pressure breakthrough (NPPB). And, 2 out of 6 cases showed transient peripheral low perfusion on much larger area than those of the preoperative scans probably because of the focal brain damages and edemas. We conclude that 123I-IMP SPECT on AVM is very useful to decide the indication of the removal operation, and to estimate the postoperative risk, and to evaluate the preoperative and postoperative cerebral hemodynamic changes in the peripheral area of AVM.

Metal hypersensitivity in dentists: a patch test study.

Patch test of dental alloys was conducted on dentists in the Department of Prosthetic Dentistry, Hiroshima University School of Dentistry. The positive rate of the first patch test in the test group was 47.1%, and that in the control group was 10.0%, showing a large difference between the two groups. Furthermore, a second patch test was conducted on those who showed a positive response together with pustular or follicular reaction. As neither positive response nor pustular or follicular reaction was observed in the second patch test, the results were regarded as negative. Only a very slight difference in the positive rate could be observed when the results of the two tests were combined. The results of the present study suggest that the possibility is small for dentists to develop metal hypersensitivity in normal clinical practice.

Oligosaccharide differences in the DF3 sialomucin antigen from normal human milk and the BT-20 human breast carcinoma cell line.

The murine monoclonal antibody DF3, prepared against a membrane-enriched fraction of a human breast carcinoma, recognized high molecular weight mucin-like glycoproteins from normal human milk and breast carcinoma cell lines. Although the epitope recognized appeared to be a peptide segment, recognition was altered by neuraminidase, suggesting carbohydrate contributions to antigen recognition. Examination of DF3 antigen isolated from normal human milk and the BT-20 human breast carcinoma cell line showed significant oligosaccharide differences. DF3 antigen from BT-20 cells contained three major oligosaccharides, the peanut agglutinin-binding disaccharide Gal beta 1,3GalNAc, which is the carbohydrate component of the Thomsen-Friedenreich antigen, and its mono-(NeuAc2,3Gal beta 1,3GalNAc and/or Gal beta 1,3(NeuAc alpha 2,6)GalNAc) and disialylated (NeuAc2,3Gal beta 1,3(NeuAc alpha 2,6)GalNAc) derivatives. In contrast, the DF3 antigen from normal human milk contained the tetrasaccharide Gal beta 1,4GlcNASc beta 1,6(Gal beta 1,3)GalNAc as its major neutral oligosaccharide and also sialylated derivatives of the tetrasaccharide, including a monosialo derivative (Gal beta 1,4GlcNac beta 1,6(NeuAc alpha 2,3Gal beta 2,3)GalNAc and/or NeuAc alpha 2,3Gal beta 1,4GlcNAc beta 1,6 (Gal beta 1,3)GalNAc). These results suggest the possibility of carcinoma-associated alterations in O-linked oligosacchardes of cell surface sialomucins and in the activity of the beta 1,6-glucosaminyl transferase involved in mucin biosynthesis.

[A case report of thyroid papillary cancer that manifested leukocytosis and hypercalcemia after radiotherapy in bone metastasis].

Bone metastasis from a thyroid papillary cancer of a 59-year-old woman had been successfully treated with radiotherapy (6,000 rad) and iodine-131 (120 mCi). One year later, the patient developed leukocytosis (maximum 143,000/mm3) and hypercalcemia (16.0 mg/dl). A colony stimulating factor (CSF) was detectable in her plasma, and nude mice that had been given metastatic tissues similarly developed leukocytosis and hypercalcemia. Leukocytosis and hypercalcemia seemed to have been caused by the CSF produced in the bone metastasized tissues of this thyroid cancer.

[Current status of hyperthermia for deep-seated tumors].

The current status of hyperthermia for deep-seated tumor is reviewed. Heating methods include external heating, intraluminal heating and interstitial heating, all of which have inherent advantages and disadvantages. Hyperthermia alone has significant effects on deep-seated tumors. The combination of hyperthermia and radiotherapy has been widely applied, resulting in 10-15% complete regression and 10-40% partial regression in the treatment of locally advanced tumors. The response rate of thermoradiotherapy was higher than that of radiotherapy for tumors in the brain, rectum and esophagus in the non-randomized study. The clinical application of thermochemotherapy is increasing and its effect is demonstrated in the prevention and treatment of peritoneal dissemination of gastric cancers. Reports of serious complications are rare. The technology for deep-heating and thermometry, and the new criteria for the assessment of thermotherapy for deep-seated tumors seems a promising new treatment modality.

Immunoepidemiological studies on subclinical infection among leprosy household contacts in Thailand.

Three-thousand-fourteen leprosy house-hold contacts in Thailand were surveyed by their personal history, physical examination, and immunological tests. The results were compared with those obtained from villagers in leprosy-endemic and nonendemic areas. The percentages of young people, students, children and grandchildren of the patient, the contacts of multibacillary leprosy cases, long duration of contact, BCG vaccination, FLA-ABS and Dharmendra's lepromin-positive responders were significantly higher in the household contacts than those in the villagers. The percentages of neural and dermal symptoms were not significantly different between the household contacts and the villagers in the endemic area, but the percentages were higher than those of the villagers in the nonendemic area. A PPD skin test was more frequently negative in the former two groups than in the latter. Both FLA-ABS and lepromin tests showed a significant correlation with the age of the contacts, their occupations, blood relation to the patient, the duration of contact, BCG vaccination, dermal signs such as an ill-defined plaque or macule with or without sensory loss, but did not correlate with sex, type of leprosy in the patient, or other skin diseases. The FLA-ABS test in the house-hold contacts and the villagers in an endemic area showed a significant correlation with the neural signs, such as enlargement of the peripheral nerve without sensory loss. These suspicious dermal and neural signs and symptoms were therefore considered signs of Mycobacterium leprae infection. The FLA-ABS test was sufficiently sensitive for detecting this infection and did not correlate with the lepromin or PPD skin tests. FLA-ABS-positive but lepromin-negative responders were found in 33.5% of the house-hold contacts. They were considered to be a high-risk group who may develop clinical leprosy. Nearly half of them were treated with dapsone or BCG according to the results of the PPD skin test. Follow up of these contacts, together with the remaining contacts without treatment, is in progress.

[Hypoxia and reoxygenation in experimental tumors].

Hypoxic fractions (HF) and reoxygenation of various murine tumors were studies using a paired survival assay. All irradiations were given to whole-body of mice without anesthesia or physical restraint, both of which can increase HF artificially. HF's of 4 different tumors of 1 cm in diameter were between 4.5% and 14%. Thus, variation of HF due to tumor type was quite small. HF was roughly correlated with tumor size and proportion of necrotic areas. Reoxygenation was examined in two tumor systems, and it occurred rapidly (within 1 hr after radiation) in one tumor but rather slowly in another.

[Overestimation of myocardial viability due to an increase in thallium uptake by the lung].

Thallium lung uptake (TL-uptake) was usually treated as background for myocardial image and increase of TL-uptake in exercise test was considered as marker of depressed cardiac function. It was reported that marked increase of TL-uptake in patients with acute myocardial infarction (AMI) corresponded to acute severe congestive heart failure. Here effect of TL-uptake on myocardial planar images was studied in 61 patients with AMI. In acute phase anterior, LAO 30 degrees and LAO 60 degrees myocardial images were collected. In 29 cases of 61 cases 3 to 6 hours delayed images could be collected. Each myocardial images was divided to 3 division and both images were compared. In 5 of 6 patients with marked increase of TL-uptake new defects were noted in anterior division of delayed images and in one case also in lateral division. In 7 patients of 12 patients with moderate increase of TL-uptake new defects were also noted in delayed images, i.e. 3 in anterior, 3 in inferior and one in apical division. It was concluded that over estimation of myocardial viability due to marked increase of TL-uptake was often noted in patients with AMI accompanying severe congestion. It became clear that delayed images were necessary to correctly estimate myocardial viability in such case.

[Long-term follow-up study of transcatheter arterial chemotherapy in patients with advanced breast cancer].

Intra-arterial infusion chemotherapy (I.A) was applied to 32 cases with advanced breast cancer. The subclavian artery was intubated and one shot of 30 mg of adriamycin was given every 3 days, with a total dose of over 150 mg. Then mastectomy was performed after this chemotherapy. Long-term follow-up study revealed that the response rates of the primary tumor to I.A chemotherapy provided the prospects for their prognosis. The 3-and 5-year survival of good responders (% tumor regression greater than or equal to 70%) was significantly better than those of poor responders (% tumor regression less than 70%). Then, four patients with advanced breast cancer were treated with combination of I.A infusion chemotherapy of Adriamycin and photoradiation by argon laser. The mean interval time for 50% tumor regression was shorter than in the cases treated with Adriamycin alone. In vitro, the effects of the photoradiation for MCF-7 breast cancer cell line were observed only at a high concentration of Adriamycin.
