We used wide-field color specular microscopy of the human corneal surface to study the morphologic changes that occur under extended-wear contact lenses. There was a shift in the frequency distribution of surface cells by size to larger cells, suggesting a prolonged residence time on the ocular surface. Other abnormalities included retained desiccated mucin, coarse mucus plaques, and palisading of cells.

Thirteen normal subjects and thirteen subjects with bona fide keratoconjunctivitis sicca were examined employing wide-field colour specular microscopy of the corneal epithelium. The prevalence of small, medium and large cells is different in these two groups with a shift to small cells in patients with keratoconjunctivitis. Increased rose bengal dye uptake filaments, coarse mucus plaques and pre-exfoliative sheets are also seen in keratoconjunctivitis sicca. These findings suggest an accelerated epithelial cell loss from the surface of the eye in keratoconjunctivitis sicca.
We used wide-field color specular microscopy to study the corneal epithelium of 13 normal subjects and 13 patients with keratoconjunctivitis sicca. The frequencies of small, medium, and large cells were different in the two groups, with a shift to small cells in patients with keratoconjunctivitis sicca. Increased uptake of rose bengal dye, filaments, coarse mucus plaques, and pre-exfoliative sheets were also found in the patients with keratoconjunctivitis sicca. These findings suggest accelerated epithelial cell loss from the surface of the eye in keratoconjunctivitis sicca.
Sixty consecutive patients with active ocular rosacea underwent Schirmer's test without anesthesia. A randomly selected group of 120 patients without rosacea, matched for age and sex, served as controls. The prevalence of dry eyes was significantly greater in the patients with rosacea. The coexistence of keratoconjunctivitis sicca and ocular rosacea probably causes a high level of symptoms that lead patients to examination and recognition.
Color specular microscopy, a noninvasive, in vivo microscopic technique, was utilized to study the corneal epithelium in 17 patients including eight with keratoconus, seven with bullous keratopathy, and two with Fuchs' corneal dystrophy. Color specular microscopy was also performed on rabbit corneas with experimental surgical trauma. Changes observed by specular microscopy in these diseased states correlated with alterations noted by light microscopy and scanning and transmission electron microscopy. Specular microscopy can provide detailed in vivo cellular morphology of the ocular surface, obviating the need for tissue biopsy. Thus, specular microscopy is a valuable diagnostic technique available for the clinician to monitor changes of the diseased ocular surface.
We report 13 cases of corneal ulcers caused by gram-negative bacteria. These cases occurred in aphakic eyes wearing hydrophilic contact lenses on an extended-wear basis. The risks for infection in the elderly aphakic eye with extended-wear hydrophilic lenses are increased by delay in removal of the lens. Caution in the use of these lenses is urged. The ability of either the patient or an attendant to remove the lens as soon as redness, diminished vision, or pain presents itself is essential in minimizing the risks for serious infection.
A patient with a corneal ulcer caused by Acanthamoeba polyphaga has been successfully treated with a penetrating keratoplasty. The pathologic specimen showed no inflammatory infiltrate surrounding the intact amoeba, whereas necrotic organisms were surrounded by an intense cellular reaction. Indirect immunochemical studies showed that the trophozoite stained profusely, but the cyst form exhibited the antigenic site on the cyst wall.

The mechanisms of tear flow, blinking, and tear drainage were studied employing high speed slit-lamp cinematography using a polystyrene microsphere suspension as a tracer of tear movement. It was demonstrated that: (1) Tears are drawn into the canaliculi during the relaxation phase of blinking. (2) Both superior and inferior puncta function as normal tear drainage routes. (3) The superior punctum alone is sufficient to handle tear drainage. (4) There is some regurgitation of tears from the canaliculi. (5) An intact blink mechanism is necessary for effective tear drainage. (6) There is an actual closure of the puncta during blinking. This is sufficient to effect tear exit without the lids meeting. The clinical impression that in order to effectively prevent tear drainage in the treatment of dry eyes, it is necessary to occlude both the superior and inferior puncta was confirmed.

The outer eye is furnished with a variety of defenses active in protecting against continual exposure to the hazards. These defenses include the bony orbit, the eyelids and cilia, the tear film, corneal and conjunctival epithelium, and a submucosal secretory immunoglobulin system. Compromise of one or more of these components in disease states can lead to ocular infection. Rapid engulfment of foreign material introduced into the tearfilm is demonstrated with the use of polystyrene microspheres.
