Endometrial epithelial cell plaques were enzymatically isolated from rabbit uteri 6 days after artificial insemination. These cells were judged viable by exclusion of [3H]mannitol, trypan blue and [57Co]EDTA, and contained receptors for the ligand, [3H]quinuclidinyl benzilate (QNB). The cell plaques were evaluated for their ability to accumulate prostaglandins (PGs) as a function of PG concentration and type (E2, F2 alpha), temperature, pH, incubation time and cell protein. The PG accumulation was unsaturable, thereby suggesting that the cells, as isolated, and under the conditions of these experiments, do not have identifiable PG receptors or detectable facilitated transport mechanisms. Cells preloaded with PG rapidly released PG after dilution into buffer containing either no or 50 microM unlabeled PG. We suggest that PGs can be accumulated, by endometrial epithelial cells, in the nonionized (nonelectrolyte) form which dissolves (partitions) into the lipid bilayer; the amount of PG accumulated could reflect environmental concentration (levels in the uterine secretions).
Abdominal abscesses located in the left upper quadrant may occur in two major anatomic locations, the subphrenic space or the lesser sac. The left coronary or triangular ligament extending from the dorsal aspect of the liver to the diaphragm separates the subphrenic space from the lesser sac. As the ligament usually extends posteriorly, the anterior subphrenic space extends under the dome of the diaphragm, occupying the most superior part of the left upper quadrant. Both the lesser sac and the left anterior subphrenic space extend to the right of midline. The left anterior subphrenic compartment is bounded on the right by the falciform ligament while the lesser sac extends to the right coronary ligament and foramen of Winslow. Therefore, abscesses either in the left anterior subphrenic compartment or lesser sac may extend across the midline into the right upper quadrant. Left anterior subphrenic abscesses will be immediately subdiaphragmatic while lesser sac abscesses extending to the right of midline will not usually extend up to the diaphragm. Six cases are presented demonstrating midline air-fluid levels and soft-tissue masses in the immediate subdiaphragmatic area, characteristic signs of anterior left subphrenic abscesses.
The comparative systemic toxicity of methotrexate (MTX) with citrovorum factor rescue (CF), MTX alone and actinomycin-D (Act-D) in the treatment of gestational trophoblastic neoplasms (GTN) was evaluated in the present study. Treatment with MTX-CF was associated with only a 4% incidence (1 of 25 patients) of hepatic and/or hematologic toxicity and total absence of either a generalized rash or marked alopecia. In contrast, both MTX alone and Act-D were associated with a 48% incidence (12 of 25 patients) of hepatic and/or hematologic toxicity. Actinomycin-D also induced a generalized rash and marked alopecia in 24% (6 of 25 patients) and 52% (13 of 25 patients) of the patients respectively. We found that MTX-CF is the least toxic single agent chemotherapeutic regimen in the management of GTN.
The fifth component of complement (C5) has been isolated from human serum in fully hemolytically active form by immunoadsorbent and anion exchange column chromatography. The immunoadsorbent column was prepared by the covalent coupling of the purified IgG fraction obtained from monospecific goat anti-human C5 antiserum to CNBr activated Sepharose 4B. Establishment of appropriate conditions for the dissociation and elution of functionally active C5 from the immunoadsorbent column was of central importance in the development of this purification procedure. The C5 preparations exhibited final yields of 20--50% with 570--710-fold purification factors based on recovery of specific hemolytic activity. These preparations were free of contaminating serum proteins as judged by SDS-polyacrylamide slab gel electrophoretic and immunochemical criteria. A C5-depleted reagent (C5D) was generated from the non-adsorbed protein containing fractions obtained subsequent to the passage of freshly drawn NHS plus 10 mM EDTA through the monospecific anti-C5 Sepharose 4B column. Upon reconstitution of C5D with Ca2+, Mg2+, and C1q, this reagent was utilized for the detection and quantitation of C5 hemolytic activity. The purified C5 preparations contained 1.5--2.5 x 10(12) effective molecules/mg protein and NHS expressed 0.5--2.0 x 10(11) effective molecules/ml.
Two questionnaire surveys were undertaken to investigate the problems experienced by patients and staff in coping with ileal conduits. A survey of 34 patients showed that the major problems were urinary leakage and skin excoriation. In 7 out of 13 patients, leakage was associated with contour problems due to abdominal hernias of recession of the stoma. Fifty-seven questionnaires were received from British urologists, who each constructed, on average, 7 ileal conduits per annum. Twenty of 57 surgeons had no stomatherapist. Leakage and skin reactions were once again the most commonly encountered problems. It was concluded that the most severe leakage problems could be prevented by careful surgical technique. However, most of the day-to-day problems could be handled by a competent stomatherapist.
Seventeen patients with hormone resistant prostatic cancer were treated with chemotherapeutic drugs. An overall response rate of 64% was obtained. In this study the toxicity rate was 29%. The problems of defining response in prostatic cancer are discussed and a protocol for the future is outlined.
Three cases are described in which conventional radiology had suggested that recurrent tumour was present at a ureteroileal anastomosis. Fibre-endoscopy of the ileal conduits was performed with the Olympus short colonoscope. In each case the anastomosis was seen to be normal and no complications ensued. It is suggested that fibre-endoscopy of the urinary conduit is a safe and simple adjunct to conventional radiology.
Fifty-one patients with pure squamous cell carcinoma of the bladder were treated between 1958 and 1978. The overall 5-year survival rate was 16%. Ninety-two per cent of cancer deaths were caused by failure to control local disease; treatment by radiotherapy and elective cystectomy resulted in no long-term survivors. It is concluded that earlier diagnosis would reduce the number of cases presenting with advanced local disease and that future research should be directed towards adjuvant studies with chemotherapeutic agents known to have activity against bladder cancer.
Two of 33 prostatic tumour xenografts were established and passaged. Testosterone supplementation did not improve tumour take rates. The histological grade of the parent tumour was the principal factor affecting xenograft survival. Preliminary studies on established tumour xenografts suggest that their value lies in the study of tumour biology, and that they have no direct therapeutic application at present.

Lung connective tissue proteins have been assayed in 12 normal juvenile baboons (Papio cynocephalus). The collagen concentration (26.8 +/- 5.1 microgram hydroxyproline per mg dry weight) is comparable to values reported for adult human lung, while the rate of collagen synthesis (0.242 +/- 0.082 nmoles hydroxyproline per mg DNA per hour) is approximately half that of human lung. Proteins other than collagen are synthesized at approximately equal rates in human and baboon lung. Elastin concentration was estimated to be 33 microgram insoluble elastin protein per mg dry weight, within the range reported for normal human lung. Values have also been calculated for the lobar contents of protein, DNA, and collagen, as well as the rates of synthesis of collagen and other proteins in the entire upper lobe of the right lung of these animals. The collagen concentrations in the lung show wide differences among various animal species. However, the baboon lung is similar to human lung. This similarity may be important in the interpretation of alterations in lung connective tissue metabolism associated with experimental diseases.
