[Hepatic safety of itraconazole intravenous solution in treatment of invasive fungal infection].

OBJECTIVE: To investigate the hepatic safety of itraconazole intravenous solution in the treatment of invasive fungal infection. METHODS: Forty-nine patients with invasive fungal infection, such as pneumonia, meningitis, endocarditis, and blood stream infection, caused by Aspergillus spp. Cryptococcus neoformans, Candida spp. Penicillium marneffei, and Prototheca wickerhamii, 50 of which had underlying diseases, including hepatic diseases, such as hepatitis, cirrhosis, fatty liver etc. were treated with intravenous itraconazole for 12.6 days (2 - 42 days). Liver function test was conducted before and after the treatment. The relationship between liver adverse reaction and itraconazole treatment was evaluated. RESULTS: Liver function test was abnormal in 20 patients. The patients with liver enzyme abnormalities before itraconazole treatment were easier to suffer from hepatic damage during itraconazole treatment (chi(2) = 11.54, P = 0.001). However, only in 2 patients with mild liver function test abnormality the liver adverse reaction could be associated with itraconazole treatment. CONCLUSION: Itraconazole treatment for invasive fungal infection is rather safe, even in the patients with underlying hepatic diseases. Liver function test is recommended for all patients suggestive of liver dysfunction and protective therapy for liver is needed.

[Prospective observational study on the effects of caesarean section on breastfeeding].

OBJECTIVE: To study the effects of caesarean section on breastfeeding. METHODS: Six hundred and two [301 cases was caesarean section (caesarean section group) and 301 cases was vaginal delivery (vaginal delivery group)] nulliparas were interviewed face-to-face at antepartum and postpartum in an indication-matched prospective study. RESULTS: There was a significantly lower postpartum prolactin (PRL) level in the caesarean section group (8.48 nmol/L, 95% CI: 7.80 - 9.21 nmol/L) compared with vaginal delivery group (9.61 nmol/L, 95% CI: 8.99 - 10.26 nmol/L) during 6 - 24 hours in the daytime after delivery. The median time of breastfeeding initiation was 12 hours and 2 hours after birth for caesarean section and vaginal delivery groups respectively. Caesarean section was an important hazard for a shorter duration of breastfeeding (RR = 1.21; 95% CI: 1.10 - 1.33) within one year after childbirth. CONCLUSIONS: Caesarean section is associated with significantly lower postpartum PRL, which is in line with the longer breastfeeding initiation and lower rate of successful breastfeeding. Necessary measures including promoting the secretion of postpartum PRL such as early contact, early sucking and analgesic method should be taken to improve the successful breastfeeding rate.

[8B7 spectrin--a new member of spectrin family].

OBJECTIVE: To analyze the nature of the protein encoded by 8B7cDNA (1 835 bp) and to examine the localization of the protein in cells. METHODS: The nature of the protein was analyzed using Blastn, Blastp, and TMpred. Expressions of 8B7 mRNA in tissues and cells were examined by Northern blotting. Recombinant expression vectors for localization test were constructed and transfected into COS-7 cells. Fluorescence emission in cells was examined upon a laser scan confocal microscope. RESULTS: The protein encoded by 8B7cDNA was 363 amino acids long and contained spectrin repeats. It was completely homologous to the C-terminal 363 amino acids of Enaptin, Nasprin-1, Mynel, and Syne-1 proteins. It belonged to Spectrin super-family and was called 8B7 spectrin. Northern blotting revealed a 1.8 kb mRNA expression in human spleen and small intestine tissues. EGFP-8B7 fusion protein was observed to express on the nuclear membrane and in the cytoplasm in a reticular form in a localization assay on COS-7 cells. The position of fluorescence in COS-7 cells transfected with pEGFP-delta SR8B7 was similar to that in the cells transfected with pEGFP-8B7cDNA. CONCLUSIONS: 8 B7 spectrin is a novel member of spectrin super-family. It localizes to the nuclear membrane and the cytoplasm in a reticular form in COS-7 cells. The localization is determined by the C-terminal KASH domain in 8B7 spectrin molecule.

[Differential gene expression in nasopharyngeal carcinoma cell with reduced and normal expression of 6A8 alpha-mannosidase].

OBJECTIVE: To detect the differential display of mRNA expression between human nasopharyngeal carcinoma cell CNE-2L2 with reduced malignancy caused by transduction of a DNA antisense to 6A8 alpha-mannosidase cDNA (AS cell) and the wild type cell (W cell). METHODS: Differential display of mRNA expression was analyzed using DNA microarray analysis. The datasets were confirmed by Northern blotting and RT-PCR. RESULTS: Out of the 1069 genes analyzed, 34 genes were up-regulated in AS cells relative to W cells. Conversely, 42 genes were down-regulated. The genes, up-regulation of which might have suppressive effect on tumor malignant behaviors, were P130 mRNA for 130K protein, TGF-betaIIR alpha, GABBR1, TGFBR1, TNFAIP1, STANIN, E-CADHERIN, CTNNA1 and 2, RFX2, TMPO, etc. The genes, down-regulation of which might have suppressive effect on tumor malignant behaviors, were CD44, NDRG1, TGFB1, RPS5, LEGUMAIIN, CBS, CD59, SNRPA1, etc. The microarray datasets were confirmed by Northern blot and RT-PCR analysis. CONCLUSIONS: In comparison to the W cell, AS cell has up-regulation of 34 genes and down-regulation of 42 genes. Changes of the gene expression may play a role in the malignancy reduction of AS cell.

[Intracytoplasmic sperm injection using cryopreserved-thawed testicular spermatozoa with testicular fine needle aspiration].

OBJECTIVE: To evaluate the effect of intracytoplasmic sperm injection (ICSI) using cryopreserved-thawed testicular spermatozoa with testicular fine needle aspiration (TEFNA) in patients with non-obstructive azoospermia. METHODS: Sixty-two patients underwent TEFNA. Mature testicular spermatozoa were found in 35 cases of patients and the testicular tissues were cryopreserved for later ICSI. Ovarian stimulation included a long protocol of GnRHa/FSH/hCG. Oocyte retrieval was performed under transvaginal ultrasound guidance, and ICSI conducted with cryopreserved-thawed testicular spermatozoa. RESULTS: A total of 35 couples underwent 35 ICSI cycles using cryopreserved-thawed testicular spermatozoa with testicular fine needle aspiration. The clinical pregnancy rate was 37.14% (13/35). CONCLUSION: ICSI using cryopreserved-thawed testicular spermatozoa with testicular fine needle aspiration is a main and effective method in the treatment of non-obstructive azoospermia, which can avoid further testicular fine needle aspiration.

Recurrence of chronic subdural hematoma after trepanation and drainage.

OBJECTIVE: To sum up the causes of recurrence of chronic subdural hematoma (CSDH) from failure of trepanation and drainage and explore its prevention and treatment. METHODS: From October 1988 to June 2002 a total of 358 patients with CSDH were treated with trepanation and drainage in our hospital. Among them 15 patients had recurrence of CSDH after operation. The data of the 15 patients were reviewed retrospectively. RESULTS: Of the 15 patients, 13 were cured by retrepanation and redrainage, one cured by removal of hematoma by craniotomy with bone flap, and one, a 1-year old child, gave up reoperation due to severe encephalatrophy. CONCLUSIONS: Most CSDHs which recur after trepanation and drainage can be cured by retrepanation and redrainage. For the patients with repeated recurrence of CSDH removal of hematoma capsule can be considered. The causes of recurrence of CSDH are related to disease course, the thickness of hematoma capsule, the severity of encephalatraphy and whether the hematoma cavity is drained or irrigated completely, and operation methods.

[Inhibition of ER alpha-mannosidase expression causes reduction and shortening of microvilli on rat liver epithelial cell WB-F344].

OBJECTIVE: To study the effect of N-glycosylation on the modification of microvilli on the surface of rat liver epithelial cell WB-F344 and the growth of the cells in culture. METHODS: Recombinant adeno-associated virus (rAAV) expression vector pAGX (+) containing an antisense or a sense fragment of 6A8 cDNA encoding a human alpha-mannosidase was constructed. The recombinant vectors or the mock were transfected into WB-F344 cells by means of lipofectAmine. The transfected cells were selected in G418 medium and cloned by means of limiting dilution. Integration of the transfected DNA into host DNA was detected by neo PCR. Rat liver ER alpha-mannosidase activity in cell supernatant was measured by using P-nitrophenyl-alpha-D-mannopyranoside as a substrate. Microvilli on cell surface were observed upon a scan electron microscope. The growth curves of the cells in culture were drawn. RESULTS: The cell clones transfected with antisense 6A8 showed reduction of ER alpha-mannosidase activity with various degrees. Clone AS1 and AS2 cell showed a pronounced reduction of the enzymatic activity. In the study on AS1 cells, Con A binding to the cells was found to be enhanced, cell growth in culture became slow from day 5. The microvilli on the cells were reduced and blunted. CONCLUSIONS: Transfection with antisense 6A8 resulted in reduction and blunting of microvilli on the surface of growing WB-F344 cells, which might be related to N-glycosylation modification.

[Inhibition of 6A8 alpha-manosidase expression induces decrease of adhesion to laminin and reduction of lamellipodia of human nasopharyngeal carcinoma cell CNE-2L2].

OBJECTIVE: To investigate the inhibitory effect of 6A8 alpha-manosidase expression on the adhesiveness of CNE-2L2 cells to laminin and the lamellipodia on cell surface. METHODS: 6A8 alpha-manosidase expression was detected by Western blotting. For assaying the adhesion of cells to laminin, cells were incubated in laminin-coated plate at 37 degrees C for 1 h, the adhered cells were stained with crystal purple dissolved in 0.1 mol/L Sodium Citrate/50% ethanol. Absorbance 540 nm was measured. Adhesion rate (R) was calculated according to formula R = AT/A100 x 100%. Here A100 represents 100% adhesion. lamellipodia on cell surface was observed upon a scanning electron microscopy. RESULTS: The adhesion rate of two clones (AS1 and AS2) with inhibition of 6A8 alpha-manosidase expression to laminin was 0.447 +/- 0.096 and 0.533 +/- 0.065 respectively. The adhesion rate of three controls with normal expression of 6A8 alpha-manosidase to laminin was 0.78 +/- 0.035, 0.7 +/- 0.05 and 0.80 +/- 0.04 respectively. The difference was significant (P < 0.01). CNE-2L2 cells with normal expression of 6A8 alpha-manosidase was rich in lamellipodia on their surface. Lamellipodia nearly disappeared on the cells with inhibition of 6A8 alpha-manosidase expression. CONCLUSIONS: Inhibition of 6A8 alpha-manosidase expression results in decrease of adhesion to laminin and reduction of lamellipodia of human nasopharyngeal carcinoma cell CNE-2L2.

[Study on intrapartum service model during normal labor].

OBJECTIVE: To study the availability of humanized service and healthy birth and it's effect on maternal and neonatal outcomes. METHODS: A multicenteral prospective randomized control study was carried out in 9 maternity hospitals from Nov. 2000 to June 2001. 6,758 laboring women were randomly divided into study group and control group. In study group (n = 3,437), midwives accompany laboring women in whole course of birth until two hours after delivery. According WHO "Monitoring rules for normal birth" medical staff support the mothers in physiological, psychological and physical aspects. In control group (n = 3,321) mothers receive traditional management. Midwives visit and check the mothers discoutinually. In two groups husbands can company laboring women according women's requirement. A questionnaire will be filled on the second day after birth. RESULTS: In study group the spontaneous delivery rate was significant higher than that of control group (85.5% vs 66.0%, P < 0.01). The cesarean section rate, postpartum hemorrhage rate, neonatal asphyxia rate and neonatal morbidity were marked lower in study group than those of control group (P < 0.01 and P < 0.05). CONCLUSIONS: Appropriate mode of intrapartum care can decrease the cesarean section rate and promote the vaginal delivery, also it can improve the maternal and neonatal outcomes.

[Molecular mechanism of metastasis inhibition of nasopharyngeal carcinoma cell CNE-2L2 induced by reduction of 6A8 alpha-mannosidase expression].

OBJECTIVE: To study the effect of inhibition of 6A8 alpha-mannosidase expression on adhesiveness among and E-cadherin expression on CNE-2L2 cells, and on metastasis of the tumors from the cells inoculated in nude mice. METHODS: Anchorage-independent adhesion among cells was examined in soft agar culture. E-cadherin expression was studied by immunofluorescence staining, immunohistological staining and RT-PCR. CNE-2L2 cells were subcutaneously inoculated into nude mice. Eight weeks later tumor metastasis was demonstrated by means of histological examination of lung sections. RESULTS: CNE-2L2 cells with suppression of 6A8 alpha-mannosidase expression (AS) became aggregated. E-cadherin expression on wild type cells was very weak. In contrast, it was greatly enhanced on AS cells. The enhancement was detected on both protein and mRNA levels. Lung metastasis of the tumor from inoculated AS cells were heavily inhibited in nude mice. CONCLUSION: Inhibition of 6A8 alpha-mannosidase expression results in enhancement of cell-cell adhesion and of E-cadherin expression on CNE-2L2 cells. Lung metastasis of the tumor grown from AS cell inoculate in nude mice is heavily suppressed.

[From clone selection to danger model].

How is the balance between immune response and immune tolerance dynamically kept in the complicated immune system of human body? To answer this question, many scientists have proposed various models since 1950's. A brief introduction to these models is given in this mini-review, which might reflect the historical process in the development of immunology in the past half century.

[Toll-like receptor and innate immunity in phylogenesis].

In protection against microbes, an organism recognizes the pathogen associated molecular pattern (PAMP) on microbes by pattern recognition receptor (PRR). Toll-like receptor is called innate immunity. A family of cell membrane receptor was found in recent years that can mediate innate immune responses through the activation of a series of immune-related genes. In phylogenesis, it is highly conservative. However, its functions are getting more diversified with the complication of the immune functions of organisms.

[B cell activated co-receptor].

B cell activated co-receptor plays important roles in linkage of innate and acquired humoral immune responses. CD21 molecule in the co-receptor complex is a receptor for C3dg and CD19 molecule enhances BCR signal transduction. CD21 also expresses on the surface of follicular dendritic cells, which mediates the long-term maintenance of antigens and is indispensable for maintaining the memory of B cells. B cell activated co-receptor also has an effect on the negative selection of B cells reactive to autoantigens.

[Cytotoxic T cell].

Cytotoxic T cell (CTL) covers several subtypes, which are CD8+, CD4 and CD4-CD8-. CTL derives from T cell repertoire in lymphoid hematopoietic stem cells. It matures in thymus and is activated in peripheral lymphoid tissues. Effector CTL kills the target cells by 2 ways. One is apoptotic effect mediated by FasL-Fas pathway and the other one is cytolytic effect mediated by granzymes. CTL has aroused great attention due to its significance in anti-tumor and anti-virus.

[Killing cell by granzyme].

Granzyme is an effector molecule of activated cytotoxic T cells and natural killer cells. It mainly mediates cell apoptosis. Its function could be explained by its molecular characteristics to some extent. Its cytotoxic effect is related to some other factors contributing to apoptosis induction. It deserves studying if perforin mediates entrance of granzyme into cells. As potential substrates of granzyme caspases and their substrates have been paid much attention to.

[NK cell receptors and related ligands].

NK cell has antigen-non-specific receptors on its surface. Based on the molecular structure, NK cell receptors can be divided into two types, including Ig superfamily and C-type lectin superfamily. Based on function, NK cell receptors can be divided into activation receptors (NKAR) and inhibition receptors (NKIR). NKAR includes CD16 mediating antibody dependent cytotoxicity, NKR-P1 mediating natural cytotoxicity, KAR conjugated with DAP12 and co-stimulatory receptor. They transduce signal through the ITAM motif in their cytoplasmic region. NKIR mainly include KIR and CD94/NKG2. Their ligands are MHC-I molecules. They are closely related to the recognition of auto cells by NK cells. Inhibition of NKAR activation and cytotoxic effect of NK cells by NKIR are carried out through the ITIM motif and require co-aggregation of NKAR and NKIR.

[CD94/NKG2A--a kind of inhibitory receptor belonging to C-type lectin superfamily].

CD94/NKG2A is a kind of inhibitory receptor belonging to C-type lectin superfamily. It specifically expresses on the surface of some lymphocytes such as NK cells and T cells, and mediates inhibitory signal. In this mini-review, the structure of CD94/NKG2A molecule is described and its biological significance is discussed.
