Novel fluorescent cationic phospholipid, O-4-napthylimido-1-butyl-DOPC, exhibits unusual foam morphology, forms hexagonal and cubic phases in mixtures, and transfects DNA.

The novel cationic triester of phosphatidylcholine, O-4-napthylimido-1-butyl-dioleoylphosphatidylcholine (NB-DOPC), has been synthesized: 1-amino-4-butanol was reacted with napthylic anhydride to form 4-hydroxybutyl-1-napthylamide, which was then reacted with triflic anhydride; the resultant triflate was reacted with dioleoylphosphatidylcholine so as to transfer the napthylimido-butyl group to the unsubstituted phosphate oxygen. The resultant compound is thus not only positively charged, but also has a bulky hydrophobic moiety attached to the headgroup. This novel cationic phospholipid exhibits a peculiar long-living foam-like morphology upon hydration, which could have applications in encapsulation and delivery. It is characterized by high adhesiveness to hydrophobic surfaces. X-ray diffraction showed that it forms a lamellar structure of rather short repeat period, indicative of an unusually small interlamellar separation and low hydration level. It readily incorporates DNA and organizes into lamellar lipoplexes. These DNA-lipid complexes effectively transfect DNA into cells. In an equimolar mixture of this lipid with the anionic dioleoylphosphatidylglycerol lamellar arrays coexist with disordered uncorrelated structures, however, these transform into a bicontinuous cubic phase, Pn3m, upon addition of DNA. When mixed with another anionic lipid, cardiolipin, at a NB-DOPC/ cardiolipin 2:1 molar ratio, it forms the inverted hexagonal phase which is of potential interest for nanotechnology applications.

Peripheral corneal relaxing incisions after excimer laser refractive surgery.

PURPOSE: To evaluate the effectiveness of peripheral corneal relaxing incisions (PCRIs) for correcting corneal astigmatism after excimer laser refractive surgery. SETTING: Cullen Eye Institute, Baylor College of Medicine, Houston, Texas, USA. METHODS: In this retrospective case series, PCRIs were performed in 33 eyes (30 patients) that had residual astigmatism after photorefractive keratectomy or laser in situ keratomileusis according to a nomogram based on age and preoperative refractive astigmatism. Uncorrected visual acuity (UCVA) and refractive and keratometric astigmatism were evaluated, and vector analysis using the Holladay-Cravy-Koch formula was performed. RESULTS: The percentage of eyes with a UCVA of 20/20 or better increased significantly from 6% (2/33) preoperatively to 61% (20/33) postoperatively (P<.001). Refractive astigmatism was reduced significantly, and the effect was stable up to 1 year after PCRIs. The percentage of eyes within +/-0.5 diopter (D) and +/-1.0 D of cylinder increased by 73% and 52%, respectively (both P<.001). No eye lost 1 or more lines of best spectacle-corrected visual acuity. CONCLUSION: Peripheral corneal relaxing incisions are an effective approach for correcting low amounts of corneal astigmatism in eyes that have had excimer laser refractive surgery.

Comparison of intraocular lens power calculation methods in eyes that have undergone LASIK.

OBJECTIVE: To compare methods of calculating intraocular lens (IOL) power for cataract surgery in eyes that have undergone myopic LASIK. DESIGN: Noncomparative case series. PARTICIPANTS: Eleven eyes of 8 patients who had previously undergone myopic LASIK (amount of LASIK correction [+/-standard deviation], -5.50+/-2.61 diopters [D]; range, -8.78 to -2.38 D) and subsequently phacoemulsification with implantation of the SA60AT IOLs (Alcon Surgical, Inc., Fort Worth, TX) were included (refractive error after cataract surgery, -0.61 +/- 0.79 D; range, -2.0 to 1.0 D). METHODS: We evaluated the accuracy of various combinations of: (1) single-K versus double-K (in which pre-LASIK keratometry is used to estimate effective lens position) versions of the IOL formulas; the Feiz-Mannis method was also evaluated; (2) 4 methods for calculating corneal refractive power (clinical history, contact lens overrefraction, adjusted effective refractive power [EffRP(adj)], and Maloney methods); and (3) 4 IOL formulas (SRK/T, Hoffer Q, Holladay 1, and Holladay 2). The IOL prediction error was obtained by subtracting the IOL power calculated using various methods from the power of the implanted IOL, and the F test for variances was performed to assess the consistency of the prediction performance by different methods. MAIN OUTCOME MEASURES: Mean arithmetic IOL prediction error, mean absolute IOL prediction error, and variance of the IOL prediction error. RESULTS: Compared with double-K formulas, single-K formulas predicted lower IOL powers than the power implanted and would have left patients hyperopic in most cases; the Feiz-Mannis method had the largest variance. For the Hoffer Q and Holladay 1 formulas, the variances for EffRP(adj) were significantly smaller than those for the clinical history method (0.43 D2 vs. 1.74 D2, P = 0.018 for Hoffer Q; 0.75 D2 vs. 2.35 D2, P = 0.043 for Holladay 1). The Maloney method consistently underestimated the IOL power but had significantly smaller variances (0.19-0.55 D2) than those for the clinical history method (1.09-2.35 D2; P<0.015). There were no significant differences among the variances for the 4 formulas when using each corneal power calculation method. CONCLUSIONS: The most accurate method was the combination of a double-K formula and corneal values derived from EffRP(adj). The variances in IOL prediction error were smaller with the Maloney and EffRP(adj) methods, and we propose a modified Maloney method and second method using Humphrey data for further evaluation.

Effects of microtubule-depolymerizing agents on the transfection of cultured vascular smooth muscle cells: enhanced expression with free drug and especially with drug-gene lipoplexes.

The microtubule-depolymerizing agents colchicine, vinblastine (VB), vincristine, nocodazole, and podophyllotoxin were found to increase dramatically the transfection of cationic phospholipid-DNA (CMV-beta-gal) complexes on cultured vascular smooth muscle cells (VSMCs). Pretreatment of cells with free colchicine before addition of lipoplexes increased transgene expression both in the presence and in the absence of serum. Free vinblastine had similar effects; however, vinblastine was more effective (approximately 30-fold maximal stimulation) when incorporated into the lipoplexes. Under optimal conditions, vincristine, nocodazole, and podophyllotoxin produced 25- and 39-, 31- and 14-, and 26- and 14-fold increases in the absence and presence of serum, respectively. Taxol, which stabilizes microtubules, had no effect on transfection, but it blocked the positive effect of colchicine. Cytochalasin B, which inhibits microfilament polymerization, had no effect on transgene expression. By fluorescence microscopy, normal lipoplexes colocalized with lysosomes. In contrast, there was little, if any, colocalization of VB lipoplexes with lysosomes. Because depolymerization of microtubules induces NF-kappaB-dependent gene expression, the effects of pyrrolidinedithiocarbamate and Nalpha-p-tosyl-L-lysine chloromethyl ketone, inhibitors of NF-kappaB activation, were tested; inhibition of vinblastine stimulation of transfection was 85 and 66%, respectively. Also, immunofluorescence microscopy showed that vinblastine induced the translocation of NF-kappaB from the cytoplasm to the nucleus. It is concluded that microtubule-depolymerizing agents, especially when incorporated into lipoplexes, dramatically increase transfection of VSMCs, probably by two mechanisms: (i) inhibition of transport of lipoplexes to lysosomes and (ii) activation of transcription (via NF-kappaB). There have been some reports on the use of pharmaceutical agents to enhance gene expression, but generally these have involved separate applications of drug and gene. The ability to deliver a drug and a gene in a single therapeutic formulation could have significant clinical implications.

Effects of alkali pretreatment of silk fibroin on microstructure and properties of hydroxyapatite-silk fibroin nanocomposite.

Nanocomposites comprising hydroxyapatite (HAp) and silk fibroin (SF) were synthesized from Ca(OH)2 suspension co-dispersed with SF fine particles and H3PO4 solution via a wet-mechanochemical route. The SF particles were modified with an alkali solution to increase contact points between HAp phase and SF matrix. HAp crystallites grow more preferentially along c-axis on alkali pretreated SF substrates. The composites exhibit porous microstructure with 70% of open porosity and about 70% of the interpores ranging from 40 to 115 microm in diameter. The peak shifts in amide II band of SF indicate that the chemical interactions between HAp crystals and SF matrix are intensified by the alkali pretreatment of SF. The stronger inorganic-organic interactions promote the formation of three-dimensional network extending throughout the composites, bringing about an increase of 63% in the Vickers hardness to the composite.

Increased expression of H11 kinase stimulates glycogen synthesis in the heart.

OBJECTIVE: H11 kinase is a serine/threonine kinase preferentially expressed in the heart, which participates in cardiac cell growth and also in cytoprotection during ischemia. A cardiac-specific transgenic mouse overexpressing H11 kinase (2- to 7-fold protein increase) has been generated, and is characterized by cardiac hypertrophy with preserved function and protection against irreversible damage during ischemia/reperfusion. In this study, we tested whether H11 kinase also participates in the metabolic adaptation to cardiac hypertrophy and ischemia. METHODS AND RESULTS: A yeast two-hybrid screen using H11 kinase as a bait in a human heart library revealed a potential interaction with phosphoglucomutase (PGM), the enzyme converting glucose 6-phosphate into glucose 1-phosphate. Interaction between H11 kinase and PGM was confirmed by co-immunoprecipitation. To test the biochemical relevance of this interaction, PGM activity was measured in the heart from wild type and transgenic mice, showing a 20% increase of Vmax in the transgenic group, without change in KM. Glycogen content was increased proportionately to the expression of the transgene, reaching a 40% increase in high-expression transgenic mice (7-fold increase in H11 kinase protein) versus wild type (p < 0.01). Increased incorporation of glucose into glycogen was coupled to a 3-fold increase in the protein expression of the glucose transporter GLUT1 in plasma membrane of transgenic mice (p < 0.01). CONCLUSION: H11 kinase promotes the synthesis of glycogen, an essential fuel for the stressed heart in both conditions of overload and ischemia. Therefore, H11 kinase represents an integrative sensor in the cardiac adaptation to stress by coordinating cell growth, survival and metabolism.

Copper adsorption with Pb and Cd in sand-bentonite liners under various pHs. Part II. Effect on adsorption sites.

The project examined the effect of Pb and Cd on Cu adsorption using sand liners containing 0, 5, and 10% sodium bentonite and exposed to metal solutions at three pH levels (3.7, 5.5, and 7.5). Aliquots of 2 g of liner material were exposed in duplicate, for 14 days, to solutions containing Cu alone or Cu with either Pb or Cd. Selective sequential extraction (SSE) was used to quantity the Cu adsorbed by each liner particle adsorption site (exchangeable, carbonate and hydroxide, oxides and residual). The results indicated that two main factors affected liner material behavior in adsorbing Cu, besides Cd and Pb competition: pH either above or below 6.5; liner cation exchange capacity (CEC) greater or equal and greater than the solution cation equivalence. In general, the liner carbonate and hydroxide fractions precipitated the greatest amount of Cu, under all environmental conditions while the exchangeable, oxide and residual adsorbed more or less the same amount. Lead, and to a lesser extent Cd, significantly increase the mobility of Cu, by competing especially for the exchangeable sites. While Cd also competed against Cu for oxide adsorption, Pb competes for calcium and hydroxide precipitation. Lead, and to a lesser extent Cd, competed especially for the exchangeable site where the adsorption is more dynamic, and less for the more permanent adsorption sites, involving precipitation, and electrostatic bonding.

Copper adsorption with Pb and Cd in sand-bentonite liners under various pHs. Part I. Effect on total adsorption.

Municipal solid wastes could be segregated based on their specific heavy metal content and disposing of them in separate landfill cells. Therefore, the objective of the project was to investigate the interaction between copper (Cu) and either lead (Pb) or cadmium (Cd) using equilibrium batch adsorption experiments. A first test consisted of soaking three types of sand-bentonite liner samples (0, 5, and 10% bentonite) with a respective cation exchange capacities (CEC) of 2, 6.4, and 10.8 cmol(+)/kg in one of nine solutions consisting of a combination of three pH levels (3.7, 5.5, and 7.5) and three heavy metal solutions (Cu alone, Cu with Cd, Cu with Pb) each offering a respective heavy metal equivalence of 1, 2, and 2cmol(+)/kg of liner. A second test set consisted in soaking 5% bentonite liner samples in three solutions at a pH of 3.7, with either Cu alone or with Pb or Cd, at 4.8 cmol(+)/kg of liner. For up to 14 days, duplicate samples were sacrificed to determine the supernatant Cu level and pH. The results indicated that under acidic conditions (pH< 6.5), the liner bentonite content, the solution pH and the presence of Pb or Cd significantly influenced Cu adsorption. Lead, and to a lesser extent Cd, competed with Cu for adsorption sites. Under alkaline conditions (pH> 6.5), carbonate and hydroxyl precipitation governed and masked the Pb and Cd competition. Thus, at low pH, limiting the presence of Pb in landfill leachate can improve Cu adsorption.

Phenotypic switching and beta-N-acetylhexosaminidase activity of the pathogenic yeast Trichosporon asahii.

The pathogenic yeast Trichosporon asahii is the major causative agent of deep-seated trichosporonosis in immunocompromised patients. Although infection by this microorganism is becoming increasingly frequent, information related to its pathogenicity and virulence factors is still limited. Therefore, we investigated phenotypic switching in colony morphology, and the production of extracellular enzymes as a virulence factor. Sixty-one clinical isolates of T. asahii produced four different morphological types on Sabouraud dextrose agar (SDA): 69% WF (white farinose), 18% WP (white pustular), 10% Y (yellowish white), and 3% WC (white cerebriform). Strains of the three major types (WF, WP, and Y) produced two to five colony types when cultured on SDA at 37 C. The frequency of switching between colony types was 10(-2) to 10(-4), as in Candida albicans and Cryptococcus neoformans. Notably, most of the colonies switched to the smooth (S) type irreversibly, at frequencies of 10(-2) to 10(-3). No secreted aspartic proteinase or phospholipase activity was detected in T. asahii, while beta-N-acetylhexosaminidase activity, which catalyzes the hydrolysis of terminal nonreducing N-acetyl-D-hexosamine residues in N-acetyl-beta-D-hexosaminides, was found. Furthermore, enzymatic activity of the S type was significantly greater than that of the parent type in all strains. No other clinically relevant Trichosporon species (T. mucoides, T. inkin, and T. ovoides ) produced this enzyme. These results provide basal information for understanding the pathogenic potential of T. asahii.

Expression and function of HD2-type histone deacetylases in Arabidopsis development.

HD2 (histone deacetylase) proteins are plant-specific histone deacetylases (HDACs). The Arabidopsis genome contains four HD2 genes, namely HD2A, HD2B, HD2C, and HD2D. We have previously demonstrated that HD2A, HD2B, and HD2C can repress transcription directly by targeting to promoters in planta. Here, we show that the N-terminal conserved motif (EFWG) and histidine 25 (H25), a potential catalytic residue, were important for the gene repression activity of HD2A. In situ hybridization indicated that HD2A, HD2B, and HD2C were expressed in ovules, embryos, shoot apical meristems, and primary leaves. Furthermore, all three genes were strongly induced during the process of somatic embryogenesis. HD2D mRNA was only detected in the stems and flowers with young siliques and may have adopted different functions. Using green fluorescent protein (GFP) fusions, we demonstrated that HD2A, HD2B, and HD2C accumulated in the nuclei of Arabidopsis cells. Overexpression of 35S::GFP-HD2A in transgenic Arabidopsis plants generated pleiotropic developmental abnormalities, including abnormal leaves, delayed flowering, and aborted seed development. The data showed that normal pattern of HD2 expression was essential for normal plant development and that HD2A, HD2B, and HD2C may be needed for embryogenesis and embryo development. Reverse transcriptase (RT)-PCR analysis revealed that a number of genes involved in seed development and maturation were repressed in the 35S::GFP-HD2A plants, supporting a role of HD2A in the regulation of gene expression during seed development.

In situ analysis of p16/INK4 promoter hypermethylation in esophageal carcinoma and gastric carcinoma.

OBJECTIVE: Inactivation of the tumor suppressor gene by CpG hypermethylation is a common event in a variety of tumors. The present study was designed to be a comprehensive analysis of p16/INK4 methylation in carcinomas of the upper digestive tract. METHODS: Series of esophageal carcinomas (34 cases) and gastric carcinomas (25 cases) were examined for CpG methylation in p16/INK4 using methylation-specific PCR (MSP). The tissue sections underwent MSP in situ and were then examined microscopically. Immunohistochemical detection of the expression of p16 in the tumor specimens was also performed. RESULTS: Immunohistochemistry detected positive p16 expression in 8 cases of esophageal squamous cell carcinoma and 15 cases of gastric carcinoma. In esophageal carcinoma, hypermethylation of the p16/INK4 promoter region was detected in 5 cases without statistical correlation with its loss of expression, whereas in the gastric carcinomas, p16 expression was positively correlated with the T-classification (r = 0.488, P = 0.01); p16/INK4 methylation was identified in 8 cases. In addition, p16 expression was lower in the methylated samples than in the non-methylated samples (25% vs 76.47%, P = 0.03). Analysis of the MSP-in-situ sections showed that the distribution of methylated cells in esophageal carcinoma differed from that in gastric carcinoma. CONCLUSION: The role of DNA methylation in the silence of p16/INK4 may different between these two types of upper digestive tract tumor.

Subjective memory deterioration and future dementia in people aged 65 and older.

OBJECTIVES: To study whether subjective memory deterioration is associated with future dementia in older people. DESIGN: A population-based prospective cohort study begun in 1994 with biennial follow-up interviews. SETTING: Community-based members of Group Health Cooperative, a large health maintenance organization in the Seattle area. PARTICIPANTS: A sample of 1,883 subjects, dementia free, aged 65 and older, who scored 91 or higher on the 100-point Cognitive Ability Screening Instrument (CASI) at study entry. MEASUREMENTS: Subjective memory was assessed by asking whether memory had changed on 5-point Likert scales (e.g., 1=definitely improved, 3=no change, 5=definitely deteriorated) with regard to five items: remembering names, faces, friends, and appointments and judging the time. The items were summed for a possible total score ranging from 5 to 25. Subjective memory deterioration was defined as present if the total score was 20 or above. Cognitive performance was measured using the CASI. Incident dementia cases were identified using Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition criteria. RESULTS: Of 1,883 subjects, 126 developed dementia during 5 years of follow-up. Subjective memory deterioration was associated with cognitive decline and incident dementia. Age modified the association between subjective memory deterioration and future dementia. For persons reporting subjective memory deterioration at the ages of 70, 75, and 80, the hazard ratios of developing dementia were 6.0 (95% confidence interval (CI)=2.1-18), 3.2 (95% CI=1.6-6.2) and 1.6 (95% CI=0.86-3.1), respectively. CONCLUSION: Subjective memory deterioration was found to precede the development of dementia in older people with normal cognitive screening results. These findings suggest that a high level of subjective memory deterioration in persons with normal objective cognitive function may identify a subset of individuals at greater risk for developing dementia.

Bile acids lower triglyceride levels via a pathway involving FXR, SHP, and SREBP-1c.

We explored the effects of bile acids on triglyceride (TG) homeostasis using a combination of molecular, cellular, and animal models. Cholic acid (CA) prevents hepatic TG accumulation, VLDL secretion, and elevated serum TG in mouse models of hypertriglyceridemia. At the molecular level, CA decreases hepatic expression of SREBP-1c and its lipogenic target genes. Through the use of mouse mutants for the short heterodimer partner (SHP) and liver X receptor (LXR) alpha and beta, we demonstrate the critical dependence of the reduction of SREBP-1c expression by either natural or synthetic farnesoid X receptor (FXR) agonists on both SHP and LXR alpha and LXR beta. These results suggest that strategies aimed at increasing FXR activity and the repressive effects of SHP should be explored to correct hypertriglyceridemia.

The complete mitochondrial genomes sequences of Asio flammeus and Asio otus and comparative analysis.

The complete mitochondrial genomes of Asio flammeus and Asio otus were sequenced and found to span 18858 bp and 18493 bp, respectively. It is surprising to find the former to be the largest among all avian mitochondrial genomes sequenced so far. The two genomes have very similar gene order with that of Gallus gallus, neither contains the pseudo control region, but both have a single extra base, namely Cytidine, at position 174 in ND3 gene. The control regions of Asio flammeus and Asio otus' mitochondrial genomes span 3288 bp and 2926 bp respectively, which are the longest among vertebrates except for Myxine glutinosa and contribute to the large size of two genomes. The 3' end of the control region of Asio flammeus and Asio otus contains many tandemly repeated sequences, which are highly similar to a putative control element, i.e. Mt5, and may form stable stem-loop secondary structures. Such repeated sequences probably play an important role in regulating transcription and replication of mitochondrial genome. Our results may provide important clues for uncovering the origin and evolution mechanisms of mitochondrion genome.

Capacity of Bacillus thuringiensis S-layer protein displaying polyhistidine peptides on the cell surface.

S-layer protein of Bacillus thuringiensis strain CTC was used as the carrier protein to display polyhistidine (poly[6His]) peptides on the cell surface. Poly(6His)n was fused with S-layer protein at two different sites, inserting just downstream of the S-layer protein homologous domain (slh) and replacing the non-slh region of S-layer protein, respectively. The two series chimeric proteins were both expressed by crystal negative B. thuringiensis strain 4Q7 and strain 171, respectively, as shown by sodium dodecyl sulfate polyacrylamide gel electrophoresis. The recombinant B. thuringiensis cells gained Ni(2+)- and Cd(2+)-binding ability and had a capacity to display up to nine copies of poly(6His). The Cd(2+) adsorption quantity of the recombinant strain with the strongest adsorption ability was twice that of the host strain.

[Effects of Ca2+ antagonist on cardiomyocytic apoptosis after experimental myocardial infarction].

OBJECTIVE: To explore the effects of Ca(2+) antagonist on apoptosis of cardiomyocytes after myocardial ischemia. METHODS: The model of myocardial infarction was made by ligating left coronary artery in SD rats. In experimental group, the rats were administrated with Adalat through oral cavity (1 mg/kg) before operation and through peritoneal cavity (0.4 mg/kg 3 hours after operation. In ischemic control group the rats were injected with placebo and in normal control group the rats were treated with sham operation (no ligating left coronary artery) and placebo. The rats were killed 6 hours after operation, with their left ventricular function had been measured. Apoptotic myocardial cells were detected by terminal deoxynucleotidyl transferase mediated dUTP nick end-labeling (TUNEL) method, Fas and Bcl-2 proteins by the ABC immunohistochemistry method. Apoptosis indexes were calculated under high magnification field of microscopy. RESULTS: The systolic pressure, end-diastolic pressure and dp/dt of left ventricle in experiment group were not significant different from those in the ischemic control group, they were 76.7+/-7.5/8.0+/-6.1 mm Hg vs. 74.9+/-11.1/11.6+/-8.3 mm Hg (P>0.05) and (777.3+/-128.6)mm Hg/s vs. (761.8+/-136.4)mm Hg/s (P>0.05) respectively; but those in both the experimental group and the ischemic control group were lower than those in the normal control group, they were 94.9+/-7.5/2.8+/-3.2 mm Hg (P<0.001) and (1131.5+/-112.8)mm Hg/s(P<0.001). There were a lot of positive myocytes with TUNEL stain in the ischemic region of left ventricle in both the experimental and the ischemic control group, apoptosis index in the experimental group was lower than that in the ischemic control group (0.201+/-0.053 vs. 0.261+/-0.045, P<0.05). The positive myocytes of Fas and Bcl-2 protein appeared in the region surrounding ischemic myocytes. There were no positive myocytes with the three types of stain in normal control group. CONCLUSION: Ischemia could induce apoptosis of myocytes and expression of Fas and Bcl-2 gene; Ca(2+) antagonist could protect myocytes from apoptosis.

[Influence of epidermal growth factor and gonadotrophin on the in vitro maturation of human oocytes].

OBJECTIVE: To study the effect of epidermal growth factor(EGF) and different concentrations of gonadotrophin (Gn) on the in vitro maturation of human oocytes. METHODS: EGF was added to the vitro culture medium in order to observe the effect of Gn combined with or without EGF on the result of in vitro maturation. The concentrations of hCG and FSH were changed respectively to observe the difference between the results. RESULTS: Adding EGF to the culture medium improved the maturation rate of oocyte significantly (P < 0.05). There was no difference between the results with different concentrations of hCG and FSH in the culture medium. CONCLUSION: EGF can improve the results of the in vitro maturation of human oocytes by increasing the maturation rate significantly. Increasing the concentration of Gn does not influence the results of in vitro maturation.

[Decreased peripheral nerve conduction velocity may be associated with lower-serum level of vitamin E in patients with infantile hepatitis syndrome].

OBJECTIVE: To explore the influence of vitamin E (VitE) concentration in serum on peripheral nerve conduction in patients with infantile hepatitis syndrome (IHS). METHODS: A retrospective study was carried out in 58 infants suffered from IHS without congenital biliary atresia and 31 of them were followed up. Thirty-two healthy infants were as control. The level of VitE in serum was detected with high performance liquid chromatography and nerve conduction was tested with surface electrodes along the nerves of limbs. The relationship between the level of VitE or total bilirubin (TB) or direct bilirubin (DB) and the nerve conduction velocity was analyzed comparatively. RESULTS: (1) The serum level of VitE was below the lower limit of 90% the normal value (13.78 micromol/L) in 71% (41/58) of patients, and was below the lower limit of 99% the normal level (9.17 micromol/L) in 48% (28/58) of patients. (2) The level of DB was more than 25.7 micromol/L in 86% (50/58) of the patients and was more than 102.6 micromol/L in 47% (27/58) of patients. Severe conjugated hyperbilirubinemia with cholestasis was demonstrated in most patients. (3) At least one abnormal parameter in nerve conduction test was found in 86% (50/58) patients. In 144 nerves tested, 60.4% (87/144) had at least one abnormal parameter. (4) Analysis for the association between bilirubin levels and VitE concentration in serum: in groups of DB > or = 25.7 micromol/L and DB < 25.7 micromol/L, the percentage of decreased VitE concentrations was 78% (39/50) and 25% (2/8), respectively, and the difference was significant (P < 0.01). Similar association between low VitE concentration and increased level of TB in serum could not be demonstrated. (5) Analysis for the association between abnormal nerve conduction and VitE concentration in serum: in the two groups with low and normal level of VitE, the percentage of abnormal nerve conduction was 93% (38/41) and 71% (12/17), respectively (chi(c)(2) = 4.93, P < 0.05). (6) Analysis for the association between abnormal nerve conduction and bilirubin in serum: There was no significant association between abnormal nerve conduction and serum level of either DB or TB. (7) Eight patients died and 9 patients had motor development delay in 31 patients during follow up. In these 17 patients with poor outcome, 88% (15/17) had very low VitE levels (< 9.17 micromol/L), which was markedly higher than the proportion of cases (43%, 6/14) with better prognosis (chi(c)(2) = 7.235, P < 0.01). CONCLUSIONS: (1) Low VitE serum levels were found in excess of the two thirds of patients with IHS and severely decreased levels in nearly a half of them. (2) A conjugated hyperbilirubinemia with cholestasis could be found in most patients (86%) suffered from IHS without congenital biliary atresia and about a half of them had serious cholestasis. (3) Conjugated hyperbilirubinemia with cholestasis could be the predominant cause of decreased serum VitE level in this study. (4) Abnormality of nerve conduction in patients with IHS might be related to VitE deficiency.
