The Mafucci-Kast Syndrome. Dyschondroplasia with hemangiomas and frontal lobe astrocytoma.

The authors describe radiologic and pathoanatomic findings in the very rare Mafucci-Kast syndrome. In a 39-year-old Libyan patient multiple enchondromas of the fifth ray of the left hand were found with signs of malignant transformation. Furthermore, there were angiomas of the skin, of the soft meninges, and the bone. In addition, there was a low-grade malignant astrocytoma of the frontal lobe of the brain. Chromosome analysis revealed a normal male chromosome set. The tendency to develop malignant tumors which is repeatedly emphasized in the literature was also shown in the present case. The reason for this, especially for the high spontaneous rate of malignant transformation of multiple enchondromas, is unknown. The occurrence of angiomas and multiple enchondromas in cartilaginous performed bone suggests the presence of mesodermodysplasia.

[Experience in 77 changes of hip-joint prostheses (author's transl)].

The article reports on 77 changes of hip-joint prostheses. The main indications are deep infections and loosening. The surgical procedure and complications are described. Results are encouraging. Even in cases of changes of prostheses on account of fistulating, deep infection, the infections can be controlled in 75% of the cases. Long-term results must be awaited.

Classification criteria for systemic lupus erythematosus. Frequency in normal patients.

One hundred healthy women were examined prospectively during their annual physical examination for any of the preliminary criteria of systemic lupus erythematosus (SLE). No patient had more than three of the criteria; 50 (50%) had none. The preliminary criteria were effective in excluding normal patients. Three of 19 patients taking oral contraceptives had serum antinuclear antibody (ANA), whereas only one of 81 non-pill users had serum ANA (P less than .05). None of these patients with positive ANA had clinical evidence of SLE.

[Report on a two-year experience with an ultrasterile operatory (author's transl)].

2,078 orthopaedic operations were carried out. No infections were observed at 640 hip-joint total endoprosthesis, 26 knee-joint endoprosthesis and 185 osteosynthesis. One primary infection happened in one case only, namely at an elbow-joint endoprosthesis. The compressed air installation, which drives the airturbines, was found to be the reason for the air not being germ-free what caused the infection. It is important that the turbines operate with germ-free air, and that the air recycling equipment is air-tight. The germ-free air of the horizontal displacement flow in an ultra-sterile operatory gives optimum ambient conditions for the operations, particularly when alloplastic interventions with big extraneous objects entail the risk of infections. After also many hours of operation, the air germination was extremely low and practically insignificant and cannot be obtained at such a low degree for conventionally air-conditioned operatories.
