Angiotropic large cell lymphoma (intravascular malignant lymphomatosis) of the kidney: presentation as minimal change disease.

We report the first case of angiotropic large-cell lymphoma (intravascular malignant lymphomatosis) presenting as minimal change disease (MCD) and diagnosed by renal biopsy. Neoplastic lymphoid cells were disseminated throughout the glomerular capillary bed and were associated with diffuse foot process effacement. The tumor had the immunophenotype of a B cell lymphoma (reactive with LCA and L-26 and unreactive with FVIII-R-Ag, Leu-M-1, alpha-1-antichymotrypsin, lysozyme, UCHL-1, Leu-22, kappa, and lambda). The temporal association between the onset of lymphoma and MCD, and the failure of the nephrotic syndrome to respond to immunosuppressive therapy support a role for lymphoma in the pathogenesis of MCD in this patient.

Influence of anaesthetics on the movement of the mobile charges in the algal cell membrane of Valonia utricularis.

Voltage relaxation studies in the presence of anaesthetics were performed on cells of the giant marine alga Valonia utricularis using intracellular microelectrodes. From the decay of the initial membrane voltage which can be described by two relaxation processes the conclusion can be drawn that protein-linked, mobile charges are present which are probably involved in turgor-pressure-dependent potassium transport (Büchner, K.-H., Rosenheck, K. and Zimmermann, U. (1985) J. Membrane Biol. 88, 131-137). The anaesthetics halothane and chloroform were found to affect reversibly, procaine and tetracaine irreversibly the translocation rate k of the mobile charges at concentrations which were equal to (for halothane and chloroform) or significantly below (for procaine and tetracaine) clinical and nerve blocking levels. The concentration of the mobile charges Nt as well as the specific membrane resistance Rm and the specific membrane capacitance Cm remained unchanged in these concentration ranges. The data suggest a specific interaction of anaesthetics with specialized target sites of a transport protein to which the mobile charges are coupled.

[Electroencephalographic value of acoustic stimulation within the scope of the diagnosis of forms of cerebral seizure in childhood and adolescence].

In this report we represent the phone-stimulation as an important method of provocative to activate the bioelectric potentials of brain, if epileptic seizures will be appeared in age of childhood and youth. The method ist able to demonstrate irritations of the reticulothalamic system. The information on the activity of the substantia reticularis will be demonstrated by the bioelectric reaction under phone-stimulation.

The effect of continuous normalization of serum hemolytic complement on the course of lupus nephritis: a five year prospective study.

We have completed a five year prospective study of the effect of continuous normalization of serum hemolytic complement (CH50) in 25 patients with lupus nephritis. At the end of five years 22 patients were being actively followed; 13 in a CH50 controlled group and nine in a CH50 uncontrolled group. Serial renal biopsy specimens were obtained from 19 patients. The results demonstrate a trend toward stabilization of renal histology, creatinine clearance and serum creatinine at a lower final mean dose of prednisone in the complement controlled group.

Hypoglycemia in chronic hemodialysis patients: association with propranolol use.

We have observed six episodes of severe symptomatic hypoglycemia in 5 nondiabetic patients on chronic maintenance hemodialysis. The common factor in all 5 patients was propranolol administration for the treatment of hypertension. This observation suggests that beta-adrenergic blockade may cause profound hypoglycemia in dialyzed patients with other predisposing factors such as poor nutrition, liver dysfunction or stress. On the basis of our experience, we recommend blood sugar determinations for propranolol-treated patients during dialysis against glucose-free dialysate.

Subnormal plasma adrenal androgen levels in men with uremia.

The 24-h mean plasma concentrations of 8 hormones were measured in 11 men with chronic uremia and 32 normal men. Our findings confirm previous reports of subnormal levels of testosterone, T3, and T4 and elevated levels of LH, PRL, and cortisol. In addition, we observed a new finding: markedly subnormal levels of the adrenal androgens dehydroisoandrosterone (DHA) and DHA sulfate. The mean DHA level in the patients was 164 +/- 46 (SD) ng/dl, compared with 320 +/- 124 in age-matched controls (P < 0.0001); the geometric mean DHA sulfate level was 40 micrograms/dl (95% confidence limits, 11-113) in the patients and 76 micrograms/dl (95% confidence limits, 26-214) in age-matched controls (P = 0.005). The depression of adrenal androgen levels in the face of elevated cortisol levels suggests a biosynthetic block in the adrenal cortex at the step where the C-19 and C-21 pathways diverge, namely the removal of the 2-carbon side chain by C-17, 20-lyase. If a similar defect were present in the testes, it could account for the diminished synthesis of testosterone, which is a further metabolite of DHA in the testes.

[Experiences in sleep induction with sinophenin in children (a contribution to the differential diagnosis of cerebral seizure disorders)].

In view of the necessity of the search for time-economically justifiable bioelectrical investigation procedures yielding the required information in practice and meeting the demands for a supplementation of the electro-encephalographic diagnosis, an attempt was made to assess the importance of the Sinophenin sleep provocation method under consideration of frequent neuropsychiatric problems in children. The correlation between age-bound specificity of the kind of fits, the sleeping-waking rhythm and its different pharmacological modifiability were established as an expression of a functional approach.
