This study presents the main reasons for currently not using contraceptives given by currently in-union, nonpregnant women 15-44 years of age in three Asian countries (Bangladesh, Korea, and Thailand) and three Latin American countries (Barbados, Colombia, and Costa Rica). Desire for next child, side effects or health reasons, and temporary infecundity were the most commonly given reasons for current nonuse. In Korea, Thailand, and Barbados, there is little or no disapproval of contraceptives, but in the other countries there is an appreciable amount. Variations in the reasons given by selected socioeconomic and demographic characteristics of the respondents are examined. Among these characteristics, age of the women, number of living children, and ever use of family planning methods are generally the most powerful discriminators of the main reason for nonuse.

Thirty-five patients diagnosed with obstructive sleep apnea syndrome (OSAS) underwent palatopharyngoplasty (PPP) after an objective evaluation. They were all monitored polygraphically after surgery. The group's postsurgical results were positive, with significant improvement in the apnea-hypopnea index (A + H index) and oxygen desaturation. However, patients with hypopharyngeal or mandibular problems or massive obesity had poor postsurgical results. As no subject has been followed for longer than two years, the long-term prognosis for this surgical approach is unknown.
A surgical procedure performed to advance the mandible in patients with retrognathia and obstructive sleep apnea is described. The case history of an obese patient with severe sleep apnea problems who underwent a number of treatments, responding only to mandibular advancement, is presented.

The history, pharmacological properties, interactions and clinical efficacy of clozapine are discussed, citing experimental evidence and case studies. The indications for using clozapine and the implications of treatment, including monitoring strategies, cost-effectiveness and therapeutic interventions, are addressed. The limitations of clozapine and scope for further research are also discussed.
A case study of a computer-based experiential learning program involving the use of criterion-referenced testing as a quantitative evaluation tool is described. The simulation was designed to introduce nurses to basic computer operations. One hundred and thirty-eight nurses participated in the evaluation using a specially prepared version of the program that recorded the time required to perform actions, the number of unsuccessful attempts to complete instructions, and the number of correct responses to specific questions. The ability of the nurses to "drag and drop" increased significantly from less than 20% to more than 90% (p < .05, Student's t-test). Their knowledge of the definitions of four computer-related terms (i.e., trackball, desktop, window, and icon) increased significantly (from 65% to 80%, p < .1). In subjective evaluations, 82% of respondents reported that they enjoyed the lesson and 89% wanted more computer-based instructional units to be developed. Use of criterion-referenced testing helped to shed new light on the utility of such computer aided instructional systems.
This article gives insight into the challenges facing the home health nurse of today. To maintain autonomy and meet the health needs of the patient in his or her home environment, the nurse must be creative, flexible, and often unconventional in his or her approach.
