Enterokinase activity in human intestinalized gastric mucosa.

Enterokinase activity was demonstrated in biopsies obtained during gastroscopy specimens in 6 patients with extended intestinal metaplasia of gastric mucosa. These observations seem to support the hypothesis that intestinalized brush border cells of gastric mucosa may have the original absorptive and secretory capacity of true intestinal epithelium.

[The 2nd saccoradiculography: changes in the post-radiculographic and postoperative radiological morphology (340 cases apropos of 2,292 files)].

A study was made of the files 2292 patients examined by sacco-radiculography with watersoluble contrast medium, of whom 340 had been subjected to a second examination. 284 of these had undergone surgery with good results during the interval between the two examinations. In 56 cases, repeat sacco-radiculography was carried out but no operation performed. In both groups of patients a semiological study was made of the radiological appearances to determine which were the effects of the contrast medium and which could be attributed to the operation. The paper concludes with a discussion of the problem of genuine and false albuminoachies of sacco-radiculography using meglumine locarmate (dimer-X).

Ketoprofen suppositories in rheumatological practice.

Preliminary results are reported on 30 patients treated with suppositories of ketoprofen at a dosage of two daily (200 mg). Local side-effects, rectal burning and difficulty in retaining the suppository, were recorded in seven patients and caused treatment to be stopped from the first day, and in five others local (burning) or systemic (gastric pain) side-effects caused treatment to be abandoned at a later period. Interest in the suppository presentation may lie in its more prolonged action and this presentation, when given at night, may improve the over-all results obtained with ketoprofen capsules in inflammatory rheumatism.

Calcification of the basal ganglia as a cause of levodopa-resistant parkinsonism.

We present a patient with levodopa-resistant parkinsonism due to basal ganglia calcification related to surgically-induced hypoparathyroidism. The calcification was seen on computerized axial tomography but not on skull x-rays. Such calcification should be looked for in all cases of levodopa-resistant parkinsonism, since it is possible that treatment of hypoparathyroidism may halt progression of the disease. This case with similar ones from the literature suggest that striatal disease itself can cause parkingonism and that the parkinsonism in such patients tends to be less responsive or nonresponsive to levodopa.

[Cutaneous manifestations in disseminated intravascular coagulation syndrome].

The principal cutaneous manifestations indicating the need to seek intravascular coagulation are: purpura fulminans, extensive sheets of purpura, acralcyanosis, gangrene in a plaque distribution and perhaps, acute necrosis of the digits. The biological, anatomo-pathological and immunological signs of the conditions should be detected early and precisely by virtue of its nature as a medical emergency. The diversity of its clinical manifestations would seem to be related to the greater or lesser extent of the coagulopathy and resultant thrombosis in the microcirculation. Purpura, though of variable degree, is the constant cutaneous accompaniment of this condition, which is considered to be the equivalent of the experimental phenomenon of Sanarelli-Schwartzman. Heparin, which blocks the tendency to thrombosis in the small vessels in the treatment of choice, associated when possible with treatment of the underlying aetiology.
