Reliability of Katz's Activities of Daily Living Scale when used in telephone interviews.

The reliability of a five-item Katz's Activities of Daily Living (ADL) scale collected by self-report telephone interview is presented. A random sample of 6,472 South Carolina residents over 55 years of age selected from a statewide population is used. Factor structure, Guttman properties, internal consistency reliability, Mokken's index of test homogeneity, and Spearman's coefficient of rank-order correlation are used to show that ADL data gathered by telephone interview are reliable. Because telephone interviewing methods are faster, cheaper, and safer they are recommended as a viable way for researchers, policymakers, and practitioners to gather ADL information.

[Mitochondrial DNA polymorphism of cattle (Bos taurus) and mithun (Bos frontalis) in Yunnan Province].

An analysis of cleavage patterns of mtDNA by restriction endonuclease was performed for fifteen Yunnan native cattle, two Kunming black and white cow, and one mithun (Bos frontalis). Two types of mtDNA molecule were detected in Yunnan native cattle: five individuals showed the type of yellow cattle (Bos taurus), the other ten showed that of zebu cattle (Bos indicus), suggesting their European and zebu origins. The mtDNA in Kunming black and white cow also consists of two restriction types. The mithun mtDNA showed the type of zebu cattle, indicating the close relationship between the origins of mithun and zebu cattle.

[Gene flow in macaques].

Mitochondrial DNA was purified from liver tissue of macaques with the method modified in our laboratory Mitochondrial DNA polymorphism among Macaca mulatta, M. cyclopis, M. fuscata and M. fascicularis were investigated by using restriction enzyme analysis. Combining the earlier study of protein electrophoresis, divergence time of the four species was estimated on the basis of mtDNA and protein genetic distance, and the result ranged from 1.8-3.2 and 0.4-1.5 Myr, respectively. Integrate information on fossil record, behavior and zoogeography of macaques, our results indicate that there existed long time gene flow among the ancestral populations of M. mulatta, M. cyclopis, M. fuscata and M. fascicularis, which slowed down divergence of nucleus genome. This may be the reason why there were great differences between the estimated divergence times based on mtDNA and protein genetic distance. M. fascicularis may dispersed earliest among the four species. There also existed gene flow between species even after M. fascicularis, M. fuscata and M. cyclopis had been isolated on their habitat islands.

Health care in China: a rural-urban comparison after the socioeconomic reforms.

This article provides an overview of the current Chinese health care system with particular emphasis on rural-urban differences. China's post-1978 economic reforms, although they improved general living standards, created some unintended consequences, as evidenced by the disintegration of the rural cooperative medical system and the sharp reduction in the number of "barefoot doctors", both of which were essential elements in the improvement of health status in rural China. The increase in the elderly population and their lack of health insurance and pensions will also place enormous pressure on services for their care. These changes have disproportionately affected the rural health care system, leaving the urban system basically intact, and have contributed to the rural-urban disparity in health care. Based on recent data the article compares current rural-urban differences in health care policy, systems, resources, and outcomes, and proposes potential solutions to reduce them.

[Production of transgenic rabbits by micro injection].

The feasibility of using whole animal instead of bioreactor in genetic engineering has been investigated with transgenic domestic rabbits. The gene chosen is the surface gene (S gene) of hepatitis B virus. Two plasmids were specifically constructed for this purpose, pHBV3.0 contains the promoter pre S gene and a part of c gene of the virus; while MT-SA, the S gene and mouse MT promotor. These plasmids were made linear by suitable restriction endonuclease before they were transferred into maleprounclei by means of microinjection. From 757 microinjected and transplanted fertilized eggs 101 rabbits were obtained. 57% of these animals were found with integrated microinjected genes. 28 of the transgenic animals were tested for the presence of the surface antigen of the virus in the serum by ELISA method. 8 animals were found positive, approximately 30% of the tested transgenic animals. The second generation transgenics were obtained either by first generation ransgenics crossed with non-trans-genics or transgenics. Among them 73% contained the transgene and 15% had the surface antigen in the serum. Some experiments were also carried out with human growth hormone gene.

Purification of three cytotoxic lymphocyte granule serine proteases that induce apoptosis through distinct substrate and target cell interactions.

We recently reported the purification of a lymphocyte granule protein called "fragmentin," which was identified as a serine protease with the ability to induce oligonucleosomal DNA fragmentation and apoptosis (Shi, L., R. P. Kraut, R. Aebersold, and A. H. Greenberg. 1992. J. Exp. Med. 175:553). We have now purified two additional proteases with fragmentin activity from lymphocyte granules. The three proteases are of two types; one has the unusual ability to cleave a tripeptide thiobenzyl ester substrate after aspartic acid, similar to murine cytotoxic cell protease I/granzyme B, while two are tryptase-like, preferentially hydrolyzing after arginine, and bear some homology to human T cell granule tryptases, granzyme 3, and Hanukah factor/granzyme A. Using tripeptide chloromethyl ketones, the pattern of inhibition of DNA fragmentation corresponded to the inhibition of peptide hydrolysis. The Asp-ase fragmentin was blocked by aspartic acid-containing tripeptide chloromethyl ketones, while the tryptase fragmentins were inhibited by arginine-containing chloromethyl ketones. The two tryptase fragmentins were slow acting and were partly suppressed by blocking proteins synthesis with cycloheximide in the YAC-1 target cell. In contrast, the Asp-ase fragmentin was fast acting and produced DNA damage in the absence of protein synthesis. Using a panel of unrelated target cells of lymphoma, thymoma, and melanoma origin, distinct patterns of sensitivity to the three fragmentins were observed. Thus, these three granule proteases make up a family of fragmentins that activate DNA fragmentation and apoptosis by acting on unique substrates in different target cells.

A natural killer cell granule protein that induces DNA fragmentation and apoptosis.

We report the purification from a rat natural killer (RNK) large granular lymphocyte leukemia of a 32-kD granule protein that induces rapid DNA fragmentation and apoptosis. The protein, which we have called "fragmentin," was capable of causing DNA from intact YAC-1 cells to be cleaved into oligonucleosomal-sized fragments and producing severe chromatin condensation within 1 h. Amino acid sequence of tryptic peptides indicated that fragmentin was highly homologous to the NK and T cell granule serine proteases RNK protease 1 and mouse cytotoxic T cell protease I (CCPI)/granzyme B. Preincubation with the serine esterase inhibitor 3,4-dichloroisocoumarin blocked fragmentin-induced DNA damage, but had no effect on cytolysin. Fragmentin activity against four lymphoma target cells was completely dependent on the presence of cytolysin. Fragmentin produced rapid membrane damage as well as DNA fragmentation at nonlytic cytolysin doses, suggesting that fragmentin activity was not limited to its effects on the nucleus. Fragmentin and cytolysin activity were completely inhibited by EGTA, indicating the process was Ca2+ dependent. A role for cytolysin in endocytosis of fragmentin was suggested by the observation that treatment of YAC-1 with cytochalasin B or sodium azide and 2-deoxyglucose blocked DNA fragmentation but not cytolysin activity. A 30-kD N alpha-CBZ-L-lysine thiobenzyl esterase, which copurified with fragmentin, was inactive on its own but was able to synergistically amplify the DNA damage induced by fragmentin in the presence of cytolysin. Fragmentin activity was not dependent on protein synthesis, as cycloheximide treatment of YAC-1 cells did not prevent DNA damage. We postulate that fragmentin is the molecular mediator of NK cell-mediated DNA fragmentation and apoptosis.

The impact of increasing intensity of health promotion intervention on risk reduction.

The HealthWise Stepped Intervention Study (1988-1990) at Pacific Gas and Electric was conducted to evaluate how a health promotion program affects behavior change and whether increasing levels of preventive interventions improve health status. The basic intervention components consisted of health risk assessment and health newsletter (Levels 1, 2, 3, and 4). Additional interventions were health resource center and self-care books (Levels 2, 3, and 4), behavioral change workshops and Division Healthwise team (Levels 3 and 4), and case management and environmental policy (Level 4). The study employed a quasi-experimental design with nonequivalent control groups. The overall risk status has significantly improved at all four intervention levels. The comparison across the four intervention levels found that Level 4, combining environmental policy with "high risk" targeting, showed the most impressive performance. Participants in Level 4 consistently showed significantly greater improvement in life-style factors, and their overall risk status also showed the greatest improvement.

The relationship between primary care and life chances.

Many researchers criticize clinical medicine for its failure to improve mortality rates. But in their critiques, few distinguish primary care from expensive, high-technology specialized care. This research is concerned with the empirical relationship between the availability of health services resources (i.e., primary care, specialty care, hospital beds) and certain "life chances," as measured by overall and disease-specific mortality rates, and life expectancy. The model shows a significant direct association between primary care and favorable mortality outcomes, though the same does not hold true for variables such as hospital beds or physician specialists. There should be greater emphasis on prevention-oriented primary care as a mechanism for health improvement and cost control.

[Synaptonemal complexes of human spermatocytes and foetal oocytes observed by electron microscopy].

The electron microscopical studies on the synaptonemal complexes (SC) of Chinese spermatocytes and foetal oocytes have been made and used to construct SC karyotype with a modified surface micro-spreading technique. We also confirm the SC formation between the short arms of the X chromosome and Y chromosome as well as secondary association between the distal tips of Xq and Yq at pachytene. Moreover, attentions are paid to the morphology of the XY pairs and five pairing type of XY pairs. In our preparations, a rare triploid spermatocyte was observed and triple pairing was described. Some possible significances of the triple pairing of triploid spermatocyte in relation to the human chromosomal abnormalities have been supposed.

Characterization of a shoot-specific, GA3- and ABA-regulated gene from tomato.

A tomato (Lycopersicon esculentum) gene (GAST1) that encodes an RNA whose abundance increases > 20-fold in shoots of the GA-deficient gib1 mutant following spraying with GA3 has been characterized. An increase in GAST1 RNA levels is detectable 2 h after treatment and levels continue to increase for at least an additional 10 h. Between 12 and 24 h following treatment, the amount of GAST1 RNA begins to decline and at 48 h the level is nearly equivalent to that of water-treated control plants. Nuclear runoff analysis indicates that 8 h after treatment with GA3, transcription of the GAST1 gene has increased only threefold, suggesting that GA acts both transcriptionally and post-transcriptionally. ABA partially inhibits the GA-mediated increase in GAST1 RNA abundance while ethephon, kinetin, and 2,4-D have little effect. GAST1 RNA is detectable in untreated leaves, stems, petioles and flowers, but not in roots. The GAST1 gene encodes a 0.7 kb transcript. The sequence of the GAST1 cDNA and genomic clones indicates that the gene is interrupted by three introns and potentially encodes a 112 amino acid protein of unknown function.

[Electrophysiological effects of intravenous propafenone for treating atrial-ventricular nodal dual pathways reentrant tachycardia].

The acute electrophysiological effects of intravenous propafenone were studied through programmed electrical stimulation in 9 patients with atrio-ventricular nodal dual pathways reentrant tachycardia (AVNDP-RT). The results showed that propafenone prolonged significantly the effective refractory period of atrial ventricular nodal fast pathway (AVN-FP) in retrograde conduction and might further terminate the paroxysmal supraventricular tachycardia (PSVT). It is an effective drug for treating AVNDP-RT. Neither blood pressure dropping nor other side effects were observed.

[Role of the hypophysis in the renal responses upon stimulation of the brain osmoreceptor].

The experiments were performed in rats anaesthetized with alpha-chloralose and urethane. Intracerebroventricular administration of hypertonic saline (icv. HS) resulted in an increase in renal plasma flow rate, glomerular filtration rate, urine flow rate, urinary sodium excretion, urinary potassium excretion, and osmolar clearance, and a decrease in free water clearance. These responses were abolished in hypophysectomized rats, but were not significantly affected by intravenous injection of vasopressin (VP) receptor (V1 and V2) antagonist. The urinary dopamine (DA) excretion did not change significantly after icv. HS. Moreover, administration of benserazide, an inhibitor of the enzyme L-aromatic amino acid decarboxylase that converts L-dopa to DA, did not attenuate the diuresis and natriuresis induced by icv. HS. These results suggest that the renal responses upon stimulation of the brain osmoreceptor are dependent on the integrity of the hypophysis, while the VP and DA are not essential to these renal responses. The hypophysial factors responsible for the icv. HS-induced renal responses remain to be explored.

[The effects of stimulation of the brain osmoreceptor on the tubular reabsorption of sodium, chloride and potassium in rats].

The tubular reabsorption of sodium, chloride and potassium was studied with micropuncture technique and electron probe X-ray microanalysis before and following intracerebroventricular administration of hypertonic saline (icv. HS) in rats. At the late proximal convoluted tubules, the fractional delivery of sodium increased from 53.0 +/- 2.1% to 66.0 +/- 2.9% (P < 0.01), the fractional delivery of chloride increased from 65.4 +/- 3.4% to 78.2 +/- 3.9% (P < 0.05), but the fractional delivery of potassium and tubular fluid osmolarity were unaltered. At the early distal convoluted tubules, the fractional delivery of sodium increased from 8.2 +/- 0.9% to 13.6 +/- 1.8% (P < 0.05), the fractional delivery of chloride increased from 5.4 +/- 0.8% to 9.5 +/- 1.4% (P < 0.05), the tubular fluid osmolality increased from 139.8 +/- 6.9 mOsm/kg H2O to 181.3 +/- 15.6 mOsm/kg H2O2 whereas fractional delivery of potassium did not show significant change. Under the condition of diuresis provoked by the intravenous administration of furosemide the kaliuresis induced by icv. HS was abolished, while the icv. HS-elicited diuresis and natriuresis remained unaffected. These results indicate that stimulation of the brain osmoreceptor inhibits the proximal tubular reabsorption of sodium chloride which in turn enhances sodium-potassium exchange in the distal tubules and collecting ducts.

[Identification of sex in mouse preimplantation embryos by indirect immunofluorescent assay].

Indirect immunofluorescent assay was used for detecting male-specific H-Y antigen on preimplantation embryos. Our results showed that, embryos examined, 54.1% were H-Y positive (male) and 45.9% were H-Y negative (female). Significant difference was not found between natural sex ratio of kunming strain mouse (male: 52.1%; female: 47.9%) and our results (P greater than 0.05). Moreover, the results sequentially were confirmed by cytogenetic method. 75.50% H-Y positive embryos were male and 82.97% H-Y negative embryos were female respectively (P less than 0.01). Some factors influencing the accurency of sex selection with indirect immunofluorescent assay were also discussed.

2-Nitro-5-(6-bromohexanoylamino)benzoic acid test paper method for detecting microorganisms capable of producing cephalosporin acylases.

A novel method for detecting microorganisms capable of producing cephalosporin C (CPC) acylase and/or 7-(4-carboxybutanamido)cephalosporanic acid (GL-7-ACA) acylase has been developed. The method is based on the degradation of 2-nitro-5-(6-bromohexanoylamino)benzoic acid (NBHAB), a chromogenic substrate, into yellow 2-nitro-5-aminobenzoic acid by the action of the CPC acylase or the GL-7-ACA acylase. This method is very sensitive and quite specific, and has been successfully applied to screen the acylases from a variety of bacteria. A large number of colonies isolated on a plate surface from more than 67 samples and several known bacteria were tested by the NBHAB paper. Five NBHAB-positive strains and isolates were obtained. They were further examined by the reaction of their bacterial cells upon CPC and GL-7-ACA, respectively, and by thin-layer chromatography in order to distinguish the CPC acylase from the GL-7-ACA acylase.

The chromosomes of tufted deer (Elaphodus cephalophus).

Mitotic and meiotic chromosome preparations of the tufted deer (Elaphodus cephalophus) were studied to elucidate the sex-chromosomal polymorphism evidenced by this species. Females had 2n = 46 or 47 chromosomes, whereas males had 2n = 47 or 48 chromosomes. An X;autosome translocation was identified by synaptonemal complex analysis of spermatocytes at pachytene and confirmed by the presence of a trivalent at diakinesis/metaphase I. The present work, in combination with earlier observations by others, indicates that E. cephalophus possesses a varied X-chromosome morphology involving an X;autosome translocation and addition of varying amounts of heterochromatin. It is speculated that sex-chromosome polymorphism may be responsible for the observed differences in diploid chromosome number of tufted deer.

Effects of saponins of Panax notoginseng on sodium-potassium-adenosine triphosphatase and calcium-magnesium-adenosine triphosphatase.

Rat brain synaptosomal Na(+)-K(+)-ATPase was activated by Panax notoginseng (PNS, 0.1-1.0 mg.ml-1), fraction Rb1 (25-200 micrograms.ml-1), and fraction Rg1 (50-200 micrograms.ml-1). Activating rates were respectively 84-227%, 12-48%, and 12-22%. Results implied that Rb1 and Rg1 were not the major components of PNS, which were responsible for the activating effects. Ca(2+)-Mg(2+)-ATPase was inhibited by PNS (0.1-1.0 mg.ml-1) and Rb1 (100-200 micrograms.ml-1), but not by Rg1. It was proposed that PNS activated Na(+)-K(+)-ATPase, leading to a reduced Na+/Ca2+ exchange, a lowered intracellular Ca2+ level, and heart contractility.
