[Effect of nicorandil on coronary circulation in ischemic region].

To clarify the role of nicorandil (Nic) in relieving myocardial ischemia, the effects of Nic in canine models of coronary arterial stenosis were studied. During stenosis of the anterior descending coronary artery by an external micrometer constrictor, intracoronary infusion of Nic 1, 5 micrograms/(kg.min) increased coronary blood flow and decreased distal coronary pressure, total coronary arterial resistance and small coronary arterial resistance; but did not affect mean aortic pressure, heart rate and large coronary arterial resistance. Whole blood viscosity, plasma viscosity and hematocrit in coronary vein after intracoronary infusion of Nic were reduced. The results intravenous infusion of Nic were similar to those of intracoronary infusion. These results indicate that Nic is capable of increasing coronary blood flow, alleviating myocardial ischemia due to direct dilatation of coronary artery and reduction of blood viscosity in ischemic region.

[Effect of Salvia miltiorrhiza on the left ventricular diastolic function in coronary artery stenosis].

In 24 open-chest mongrel dogs, coronary artery critical stenosis were produced by a micrometer constriction on left circumflex coronary artery. It was reduction about 87% in lumen area. CBF, -Vce, -dp/dt max were decreased, T was increased (P less than 0.01). 15 minutes after stenosis, the Salvia miltiorrhiza injection was injected into left atrium. CBF, -dp/dt max, -Vce were increased and T was decreased after the injection of Salvia miltiorrhiza. Moreover, the change of CBF was the earliest. The results proved that the Salvia miltiorrhiza could improve the left ventricular diastolic function in coronary artery stenosis.

In vitro propagation of human hepatitis B virus in a rat hepatoma cell line.

A rat hepatoma cell line (Q7) of Morris hepatoma origin was transfected with a construct containing the tandem dimer genome of human hepatitis B virus (HBV) and the neomycin-resistant selection marker. The culture medium of several neomycin-resistant single-cell clones was found to accumulate high levels of secreted HBV surface antigen and core-related e antigen. HBV-specific replication intermediates, including relaxed circular and single-stranded DNA with a minus-strand polarity, could be found in both the intracellular fraction and the extracellular culture medium by the Southern blot procedure. One of these clones, designated Q7 HBV-21, was characterized in further detail. DNA polymerase activity was present in the virus particles produced by Q7 HBV-21 cells. Characteristic transcripts of HBV, including the 3.5-, 2.5-, and 2.1-kilobase mRNA as well as a core-gene-related transcript of 2.2 kilobases could be detected. Electron microscopic examination of the conditioned medium from Q7 HBV-21 cells identified 42-nm Dane-like particles as well as 22-nm subviral particles with a spherical or filamentous shape. This Q7 HBV-21 cell line has been maintained in the absence of neomycin for 1 year without losing the properties of HBV DNA replication and Dane-like particle production. Our results strongly suggest that the species barrier of HBV infection is at an early step of viral absorption onto or penetration into the target hepatocytes. This nonhuman system for HBV production in culture could be used to complement the human HepG2 system.

Primary glomerulonephritis in China. Analysis of 1001 cases.

The morphologic classification and clinical spectrum of 1,001 cases of primary glomerulonephritis (GN) in Jinling Hospital during 1980-1988 were analysed. It was found that the incidence of mesangial type lesion including mesangial proliferative GN, IgA nephropathy and IgM nephropathy ranks first (21.5%, 32.1% and 10.1% respectively), which is not only the commonest cause of patients clinically manifesting nephrotic syndrome and gross hematuria but also the predominant type of nephritis causing uremia and hypertension.

[Experimental study on lung injury following severe burn. Early changes in function and hemodynamics and morphologic changes in the lung of dogs with burns].

This study investigated the pulmonary and hemodynamic dysfunction and morphologic changes after major burns. Full-thickness burns, 40% TBSA were produced in 9 dogs. Respiratory Rate (RR), airway resistance (Raw), effective compliance (Ec), lung impedance Zo (LIZo), pulmonary arterial pressure (PAP), pulmonary arterial wedge pressure (PAWP), blood gas monitoring, light and electron microscopic studies were performed. Following burns, RR, Raw, PAP and PAWP increased and Ec, LIZo, RaCO2 and PaO2 decreased significantly while morphologic evidence of endothelial and interstitial edema was associated with significant cellular infiltration of lung tissue, especially by leukocytes, showing evidence of degranulation. This results shown that a 40% TBSA burn without inhalation injury can produce lung injury similarly to adult respiratory distress syndrome. This lung injury is characterized by increasing pulmonary capillary permeability. Ec and LIZo monitoring may help to early diagnose of lung injury after major burns.

Is there a hepatitis B virus-associated glomerulonephritis? Identification of HBsAG, HBcAG and HBeAG in kidney with monoclonal antibodies.

In an attempt to clarify the relationship between hepatitis B virus (HBV) infection and glomerulonephritis (GN), and to explore the significance and possible mechanisms of HBV deposition in kidney, renal biopsy specimens obtained from 69 HBV carriers with various forms of GN and 69 age-, sex-, and renal histology-matched controls were studied with 4-layer PAP immunoperoxidase and indirect immunofluorescence techniques using monoclonal antibodies to HBV surface (HBsAg), core (HBcAg) and e antigen (HBeAg). The 3 HBV associated antigens were detected in the kidney in 18/18 patients with membranous nephropathy and in 21/26 (80.8%) patients with lupus nephritis regardless of whether HBV antigenemia was present or not. In certain types of primary GN, including IgA nephropathy, mesangial proliferative GN and membrano-proliferative GN, HBsAg in the kidney was more common in patients with HBs antigenemia than in those without it (49.1% vs 26.4%, P less than 0.05). No significant difference was observed between patients with and without HBV antigenemia in terms of HBcAg or HBeAg deposition in kidney. Immunopathological studies showed granular deposition of HBV antigens in exactly the same pattern as that of Ig(s) and complement components, and the characteristics of HBV deposition in the kidney were closely correlated with the extent of immune deposits. We conclude that the deposition of HBV-associated antigens in the kidney is often non-specific, although HBsAg is more commonly seen in some HBsAg carriers with GN.(ABSTRACT TRUNCATED AT 250 WORDS)

A sociocultural study of koro epidemics in Guangdong, China.

Koro, a culture-related psychiatric disorder characterized by panic due to fear of genital retraction, occurred as the rare phenomenon of koro epidemics in a remote region of Guangdong, China, in 1984-1985 and 1987. The sociocultural and historical backgrounds of the area are described. The life pattern and attitudes toward supernatural beings and the commonly shared folk belief of evil-induced genital retraction were considered grounds for the panic, while the community's anxious reaction and hysterical atmosphere facilitated the intensification and recurrence of the episodes. Geographic seclusion associated with localism in folk beliefs and practices may have kept the epidemics confined to the region.
