[Effects of baoshen pill in treating chronic renal failure with long-term hemodialysis].

A clinical trial, to evaluate the effects of Baoshen Pill (Rheum palmatum extract, RPE) on hemodialytic patients was conducted. 42 cases with terminal stage of renal failure (Scr < 10 ml/min) on HD were divided at random into RPE treated group and control group. The results showed there were no statistically significant differences between two groups for Scr or BUN. Serum levels of TC, LDL-c, apo B and apo B/apo A1 dropped down and HDL-c, apo A1, albumin, pro-albumin and fibrinectin increased during the follow-up period (P < 0.05) in the treated patients. It is considered that the mechanisms of RPE in preventing chronic renal failure were not the action of whole body. Improving serum levels of albumin, lipoprotein, apolipoproteins might play important role in treatment.

[Clinical studies of recombinant human erythropoietin in patients on long-term dialysis].

Recombinant human erythropoietin (r-HuEPO) was administered in 68 dialyzed patients (32 on acetate hemodialysis, 24 bicarbonate hemodialysis and 12 on hemo-filtration). The mean initial Hb 52.7 +/- 8.0g/L, Ht 19.4 +/- 2.2%, serum ferritin > 100ng/L. Each patients received r-HuEPO intravenously, at the dose of 300U/Kg/w for 6.2 +/- 4.3 months. Target range: Hb 100-120g/L, Ht 30-35%. After r-HuEPO treatment, blood transfusion was not needed for any of the patients, anemia was ameliorated with increase of Hb and Ht levels. It was found that the minimum effective dose of the r-HrEPO was 150-300U/Kg/w. We conclude that r-HuEPO is effective as treatment for the anemia of dialyzed patients. However, hypertension, clotted dialyzers and dialysis access thromboses were been developed in some patients after correction of anemia. There is now a general consensus that these side effects may be minimized if r-HuEPO is initially given in small doses with increments to avoid a too rapid correction of the anemia.

Blood levels of sex hormone in lupus nephritis and their relationship to lupus activity.

One hundred and fifty-nine patients (140 females, 19 males) with lupus nephritis (LN) were studied. Renal biopsy was performed for each patient with light microscopy, immunofluorescence and electronic microscopic studies. 27 normal subjects were selected as controls. Blood levels of sex hormone including follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2) and testosterone (T) were measured by radioimmunoassay (RIA), and some clinical, laboratory and histological parameters were collected for analysis of lupus activity. It was found that serum FSH, LH and plasma E2 of all patients were much higher than normal (P < 0.01) while plasma T was much lower in female patients (P < 0.01) but normal in male patients. The lupus activity was proved to be closely related to plasma E2 and T (P < 0.05). There was no definite relationship between abnormal plasma E2 or T and serum IgG, C3 C4, ANA, A-dsDNA or A-Sm. The levels of sex hormones among every histological type of LN were not significantly different. It was concluded that: 1. The increment of plasma E2 is associated with lupus activity in both male and female patients. 2. The decrement of plasma T only in female patients is also related to lupus activity. 3. No relationship could be found among FSH, LH, E2 and T.

A Ca(2+)-ATPase from rat parotid gland plasma membranes has the characteristics of an ecto-ATPase.

A Ca(2+)-ATPase with an apparent Km for free Ca2+ = 0.23 microM and Vmax = 44 nmol Pi/mg/min was detected in a rat parotid plasma membrane-enriched fraction. This Ca(2+)-ATPase could be stimulated without added Mg2+. However, the enzyme may require submicromolar concentrations of Mg2+ for its activation in the presence of Ca2+. On the other hand, Mg2+ could substitute for Ca2+. The lack of a requirement for added Mg2+ distinguished this Ca(2+)-ATPase from the Ca(2+)-transporter ATPase in the plasma membranes and the mitochondrial Ca(2+)-ATPase. The enzyme was not inhibited by several ATPase inhibitors and was not stimulated by calmodulin. An antibody which was raised against the rat liver plasma membrane ecto-ATPase, was able to deplete this Ca(2+)-ATPase activity from detergent solubilized rat parotid plasma membranes, in an antibody concentration-dependent manner. Immunoblotting analysis of the pellet with the ecto-ATPase antibody revealed the presence of a 100,000 molecular weight protein band, in agreement with the reported ecto-ATPase relative molecular mass. These data demonstrate the presence of a Ca(2+)-ATPase, with high affinity for Ca2+, in the rat parotid gland plasma membranes. It is distinct from the Ca(2+)-transporter, and immunologically indistinguishable from the plasma membrane ecto-ATPase.

[Mechanisms and therapeutic effect of Cordyceps sinensis (CS) on aminoglycoside induced acute renal failure (ARF) in rats].

Nephrotoxic ARF model of rat was induced by IP injection of either Gentamycin or Kanamycin and treated with CS. The results of study showed that the simultaneous administration of CS with Gentamycin could protect the proximal tubular cells from Gentamycin toxicity and the use of CS after the establishment of Kanamycin nephrotoxic ARF could prompt an earlier recovery from ARF as compared with the control group. The possible mechanisms of CS on ARF include: (1) protecting tubular cell sodium pump activity; (2) attenuating tubular cell lysosome overfunction stimulated by phagocytosis of aminoglycoside; (3) decreasing tubular cell lipoperoxidation in response to toxic injury.

[The effect of lovastatin in the treatment of primary hypercholesterolemia].

The effect of Lovastatin, an HMG. CoA reductase inhibitor, on serum lipids and apolipoproteins was studied in 40 cases of primary hypercholesterolemia in a 4-month period of treatment. The level of serum lipids did not change significantly after a 35-day period of placebo treatment as compared with that of the baseline (P greater than 0.5). The patients then took Lovastatin with the evening meal in a daily dose from 20 to 80 mg for 3 months. The results were as follows: Lovastatin reduced significantly the mean serum level of total cholesterol (TC) by 31.5% (P less than 0.001), LDL-C by 39.8% (P less than 0.001), Apo-B by 27.3% (P less than 0.002), and the ratio TC/HDL-C by 35.9% (P less than 0.01). It also reduced the mean serum level of triglycerides (TG) by 22.1% (P greater than 0.05) and increased that of HDL-C by 6.3% (P greater than 0.2) and Apo-AI by 1.6% (P greater than 0.5), but without much significance. The drug was well tolerated by all the patients. Transient elevation of CPK was noticed in 2 patients and AKP in one patient. 7 patients complained of gastrointestinal discomfort. All these side effects did not necessitate stop of the medication. We are, therefore, of the opinion that Lovastatin is an effective agent for lowering the serum level of TC, LDL-C and Apo-B.

[Clinical and pathological features of type I and III membranoproliferative glomerulonephritis].

Analysis and comparisons were made between the clinical and pathological data obtained from 42 cases of type I and 17 cases of type III membranoproliferative glomerulonephritis (MPGN). It was shown that type I MPGN differed from type III remarkably in following respects: (1) Nephrosis was a salient feature of type III while hypertension, impairment of renal function and hematuria were more common in type I. (2) Pathological glomerular sclerosis, tubular interstitial changes and crescents formation occurred frequently in type I but not in type III. (3) Long term prognosis for type III was much better than type I. It is concluded the type III MPGN is a distinct clinical entity which differs from the classical type I MPGN in many aspects.

Koro epidemics in Guangdong, China. A questionnaire survey.

Koro epidemics, characterized by panic due to fear of genital retraction, involving more than 2000 victims, mainly young men, occurred in Hainan Island and Leizhou Peninsula of Guangdong Province, China, in 1984-85 and 1987. A questionnaire survey of 214 victims of these epidemics was conducted in 1988. The survey focused on symptom manifestation, personality profile, life problems, and folk belief. The results of the "koro" group were compared with those of a "clinic" group with minor psychiatric disorder and a "control" group from the non-clinical, non-koro episode population in the epidemic area. The results revealed that the victims of the koro epidemics were characterized by low intellectual endowment and strong acceptance of koro-related folk beliefs. The results support the hypotheses that: a) a koro attack is a psychologically induced anxiety-panic condition; b) the presence of an intensive belief in the culturally related concept of koro makes the subject vulnerable to the attack; and c) social crisis or tension serves as a trigger for the occurrence of the epidemic.

[Effects of Cordyceps sinensis, rhubarb and serum renotropin on tubular epithelial cell growth].

Primary cultured rat tubular epithelium was utilized to investigate the effect of Cordyceps sinensis (CS) on cellular proliferation and metabolism. Judging from incorporation rate of 3H-TdR, it was found that the addition of serum containing CS metabolites into the culture media could promote the DNA synthesis of tubular cells profoundly (P less than 0.001). In association with its beneficial effects on gentamycin nephrotoxity in vivo study, it is indicated that CS could enhance the regeneration of injured tubular cells. In addition, sera obtained from 5/6 nephrectomized rats (5/6 NT) and rhubarb treated rats were studied to see their effects on tubular cells growth. The results showed that the serum of 5/6 NT rats could promote the DNA synthesis of tubular epithelium, while the presence of experimental serum of rhubarb in culture median markedly inhibited the DNA synthesis of cells.

[Analysis of lymphocyte subpopulation in chronic dialyzed end stage renal failure by flow cytometry].

To investigate the change of immune function in patients with ESRF, peripheral blood lymphocytes of 15 maintenance hemodialysis patients were analyzed using FITC labelled monoclonal antibodies and laser flow cytometry (FACS 440). Lymphocytes of phenotype CD 3 (Pan T cell), CD 4 (helper/inducer), CD 8 (suppressor/cytotoxic), CD 16 (NK cell), CD 25 (IL-2 Receptor) and HLA-DR were enumerated. The results were summarized as follows: (1) CD 3, CD 8 and CD 16 were decreased markedly in ESRF patients. (2) CD 4 and CD 25 showed a tendency of decrement also yet statistically insignificant, (3) Value for HLA-DR was higher than normal, probably related with the hemodialysis. (4) CD 4, CD 8 and CD 4/CD 8 in a transfused group were not different from those in a non-transfused group. (5) CD 4/CD 8 was negatively correlated with the serum creatinine level (r = -0.524). In conclusion, there is a marked suppression of immune function in ESRF patients as evidenced from the change of T cell subpopulation.

[Clinical and experimental studies of rheum on preventing progression of chronic renal failure].

Rheum, a well known herb unique in its cathartic effect is now introduced to prevent progression of uremia. A clinical prospective trial was conducted to evaluate its effect in comparison with the Captopril. 30 cases with initial Scr level of 344.8 +/- 114.0 mumol/L were allocated randomly to 3 groups. Rheum E treated group, Captopril treated group and Rheum E + Captopril group. The long term (6-22 mos) follow-up results showed that the progression rate of renal failure, calculated by regression analysis of 1/Scr vs time, was found to be retarded after treatment, being more marked in both Rheum E and Rheum E + Captopril group. Uremic symptoms improved after the treatment with serum albumin level increased. To explore the mechanism of therapeutic effects of Rheum on CRF, a series of experimental studies were performed. Rheum can suppress the proliferation of glomerular mesangial cells in culture. In 5/6 nephrectomized rats, the O2 consumption of remnant kidney was obviously decreased by feeding the animal Rheum E and the hypermetabolic state was ameliorated as well. A group of 5/6 nephrectomized rats were fed with Rheum E for 28 wks. It was found that the level of azotemia of this treated group was lessened remarkably as compared with the controls. The serum albumin and transferrin contents of the treated animals were much higher than the controls while the blood cholesterol and triglycerides decreased unexpectedly. In conclusion, both the in vivo and in vitro studies have proved the effectiveness of Rheum in preventing the progression of CRF.

Hepatitis B virus propagated in a rat hepatoma cell line is infectious in a primate model.

The human hepatitis B virus (HBV) produced by a rat hepatoma cell line through transfection with HBV DNA is infectious in the human primate model--chimpanzee. Since hepadnaviruses are known to have an extremely narrow host range, our results support the idea that the host species barrier of HBV infection resides on the penetration/adsorption step rather than any postpenetration intracellular event during the virus life cycle.

Randomized trial of the effect of drugs on cervical dilatation for termination of first trimester pregnancy.

A randomized triple-blind and placebo-controlled clinical trial on the effect of lidocaine and Anodyne-lubricant jelly (ALJ) containing dicaine on cervical dilatation is reported. Three-hundred-seventy-two consecutive cases were randomly allocated to 4 groups. The four groups were given: ALJ and placebo (placebo 1); and lidocaine and placebo (placebo 2). ALJ and placebo 1 treatment was by topical application, and lidocaine and placebo 2 by injection. In parous women, a significant difference was found for satisfactory dilatation (SD) rate (p less than 0.01) among four groups. It was unexpected that there were no significant differences between drug and placebo groups, neither between ALJ treatment group and placebo 1 group (p greater than 0.5), nor between lidocaine and placebo 2 (p greater than 0.75). However, the combined SD rate was 60.9% for the topical groups compared with 39.0% for the injection groups (p less than 0.005). The findings suggested that the effect of ALJ on cervical dilatation was not mainly due to dicaine, but associated with the lubricant and the topical treatment.

Effect of serum renotropin on rat glomerular mesangial cell growth in vitro.

In vitro study of rat glomerular mesangial cell growth was carried out to investigate the pattern of serum renotropic stimulation in 5/6 nephrectomized (5/6NT) rats. Incorporation rates of 3H-TdR, 3H-UR and 3H-Leu into mesangial cells were used as parameters to measure serum renotropic stimulation. Renotropin activity increased immediately after 5/6NT, reaching peak at 48 h, and was succeeded by a rapid decline at 96 h postoperatively. A second rise occurred on the 7th day, followed by gradual decrement. It seemed evident that the serum obtained 48 h after 5/6NT surgery was capable of stimulating the uptake of 3H-TdR, 3H-UR and 3H-Leu by the mesangial cells (MCs). It may be concluded that renotropin promotes DNA, RNA and protein synthesis of MCs. The prolonged and constant effect of serum renotropin on MCs might be one of the important mechanisms in progressive sclerosis of the remaining kidney after subtotal nephrectomy.

[Is there a hepatitis B virus (HBV) associated glomerulonephritis? Evaluation of the significance of HBV antigen deposition in the kidney].

In order to clarify the relationship between HBV infection and glomerulonephritis (GN) and to explore the significance and mechanism of HBV deposition in the kidney, renal biopsies from 60 patients with various forms of GN and HBV antigenemia (Agb) were compared with those from 59 age, sex and renal histology matched controls. The biopsies were studied with 4 layer PAP immunoperoxidase and immunofluorescence techniques using monoclonal antibodies to HBV surface (HBsAg), core (HBcAg) and e (HBeAg) antigens. All the 3 HBV antigens were detected in the kidneys of greater than 67% of the patients with membranous nephropathy and in greater than 64% of the cases with lupus nephritis regardless of the presence of HBV Agb or not. In certain patterns of primary GN, including mesangioproliferative GN, IgA nephropathy and membrano-proliferative GN. HBsAg in the kidney was more common in patients with HBs Agb than those without (47.7% vs 20.5%, P less than 0.05). No difference in HBcAg or HBeAg deposition was observed in patients with or without HBV Agb Granular deposition of HBV antigens was shown in the same pattern as that of Ig(s) and complement deposition. Renal HBV deposition correlated closely with the extent of other deposits. We concluded that the deposition of HBV antigens in kidney is often non-specific, although HBsAg is more commonly seen in some HBsAg carriers with primary GN Massive immune complex deposits seem to be the prerequisite of renal HBV deposition. The role of HBV in the development of GN is therefore not confirmed and further evaluation is needed for "HBV-TC nephritis".

Clinical effects of rheum and captopril on preventing progression of chronic renal failure.

A clinical trial, to evaluate the effects of a Chinese herbal drug, Rheum E and angiotensin converting enzyme inhibitor, Captopril on chronic renal failure (CRF), was conducted. Thirty cases with initial serum creatinine (Scr) levels of 344.8 +/- 114.0 mumol/L were allocated randomly to 3 groups: Rheum E treated group, Captopril treated group and Rheum E + Captopril group. A control group of 12 cases were on dietary therapy alone. During the 6-22 months of treatment, all the patients were kept on low-protein (0.6g/kg/d), and low-phosphorus (10mg/kg/d) diet. The results showed that the progression rate of renal failure, calculated by regression analysis of 1/Scr vs time, was found to be retarded after treatment with the increased regression coefficient (b value). Scr levels and blood urea nitrogen were kept stable or fell slightly. Albumin rose during the follow-up period (P less than 0.05) in the treated patients, being more marked in both Rheum E and Rheum E + Captopril groups. Uremic symptoms of nausea, anorexia improved in most of the treated patients. It is concluded that long-term low-dose Rheum E taken orally is beneficial to CRF. Its effect is better than that of Captopril. The regime of Rheum E and Captopril is a preferable choice in the long-term treatment for preventing progression of CRF.

[Effect of nitroglycerin on coronary circulation of the ischemic region in dogs].

The purpose of this study was to define the effects of nitroglycerin on coronary circulation in ischemic region. In 18 open chest dogs, a critical stenosis of the left anterior descending coronary artery was produced by an external micrometer constrictor. Intracoronary infusion of nitroglycerin 0.5 micrograms/kg. min-1 increased coronary blood flow (CBF) and decreased distal coronary pressure (DCP), total coronary artery resistance (RT), small coronary artery resistance (RS); but did not affect mean aortic pressure, heart rate and large coronary artery resistance (RL). The whole blood viscosity in coronary vein was reduced. Intracoronary infusion of nitroglycerin 1 microgram/kg.min-1, CBF increased and RT, RL, RS, DCP decreased during the first 5min. Then the CBF decreased, RT and RL increased. These were associated with a decrease in DCP and an increase in whole blood viscosity. A passive narrowing of the stenotic area was observed. These results indicate that nitroglycerin has two effects on coronary circulation, it can increase CBF and decrease coronary artery resistance in relieving myocardial ischemia; it can also decrease CBF and cause passive narrowing of coronary artery in worsing myocardial ischemia. The beneficial or harmful effect of nitroglycerin may depend on its doses and DCP.
