[Preventive effect of endothelin-1 mRNA antisense oligonucleotide on acute myocardial ischemic arrhythmia in rats].

Acute myocardial ischemia was induced by occlusion of the anterior descending of left coronary artery (LAD) in rats; the resultant arrhythmia in 1 h after LAD occlusion was evaluated. In order to prevent expression of endothelin-1 mRNA, human endothelin-1 mRNA antisense oligonucleotide (ET-1 AS-ODN) was intravenously injected 2 h before LAD occlusion. The effect of AS-ODN on plasma ET-1 concentration and the acute ischemic arrhythmia were observed. The results showed that plasma ET-1 was significantly decreased in rats pretreated with AS-ODN, and both the incidence and severity of the acute ischemic arrhythmia were decreased dose-dependently as compared with normal saline control and sense oligonucleotide control, indicating that ET-1 AS-ODN could prevent acute myocardial ischemic arrhythmia and that endogenous endothelin-1 may play an important role in the development of acute ischemic arrhythmia in rats.

[The effects of zinc on liver metallothionein in scalded rats].

OBJECTIVE: To investigate the changes in liver metallothionein (MT) in scalded rats. METHODS: The scalded rats in deficient zinc condition were fed with diets containing zinc 4 micrograms/g, 10 micrograms/g, and 80 micrograms/g respectively. The changes in serum zinc, liver zinc and liver MT were observed. RESULTS: The serum zinc decreased, liver zinc and MT increased. The higher diet zinc levels were, The higher serum zinc, liver zinc and MT became. The correlation analysis showed that the changes between liver zinc level and liver MT was positive relationship(r = 0.85, P < 0.01). CONCLUSION: Serum zinc decrease, liver zinc increase after burn. This is connect with increment of MT synthesis in liver, zinc is inducer to MT. It can improve synthesis of liver MT.

An analysis of factors influencing electrocardiogram stress test for detecting coronary heart disease.

OBJECTIVE: To evaluate factors influencing electrocardiogram (ECG) stress test. METHODS: The treadmill stress test and coronary angiogram (CAG) records were reviewed retrospectively in 435 consecutive patients with chest pain between 1991 and 1997. There were 44 cases with normal CAG but positive exercise stress test results (false-positive, Group A), and 33 cases with both negative CAG and negative exercise test (true-negative, Group B). The baseline ECG, echocardiogram, abnormal systolic blood pressure response and history of diabetes mellitus and hypertension in the two groups were compared. RESULTS: There was no significant difference between the two groups with respect to age, gender, and antianginal medication. More patients in Group A had hypertension (52.3% vs 27.3%, P < 0.05) and diabetes mellitus (15.9% vs 0%, P < 0.05). Left ventricular (LV) hypertrophy in baseline ECG and by echocardiogram was more often present in Group A (25.0% vs 3.0%, P < 0.05; 36.4% vs 12.1%, P < 0.05). There were no significant differences between the two groups with respect to maximal heart rate, maximal systolic blood pressure (BP), metabolic equivalents, postexercise systolic BP, termination for exercise test, baseline ECG with bundle branch block and ST depression. CONCLUSION: Our results suggest that LV hypertrophy and a history of hypertension or diabetes mellitus are the main factors influencing ECG stress test for detecting coronary heart disease.

Study on the date of compilation of Wen bing tiao bian (Systemic Discourse on Warm Diseases).

It has long been recognized that Wu Jutong compiled Wen bing tiao bian in the year 1798. Through study on the printing time of the book, prefaces in the book written by Zhu Bin and Zheng Bao, and the contents of the book, it is claimed that the book was written in 1798, and by then his work was not yet finished. The first manuscript was completed in 1804, finalized and printed in 1813. The chapter of "Supplemented discourse on predominant qi of autumn dryness" was written in 1835, while Pili (thunderbolt) powder and its description were supplemented in 1836. In other words, the compilation of the whole work, from writing to finalized first manuscript, lasted for 15 years, and that for the completion of the whole book, 38 years.

[Studies on the properties of dendritic cells from cord blood CD34+ cells and peripheral blood monocytes].

OBJECTIVE: Ex vivo expansion and purification of dendritic cells(DC) with typical phenotype, morphology and function are critical to the further studies on DC and their clinical application. METHODS: CD34+ cells were isolated from umbilical cord blood by using a high-gradient magnetic cell sorting system (MACS), and peripheral blood mononuclear cells (PBMC) were depleted of T and B cells by mixing them with T/CD2 and B/CD19 magnetic beads. DC were induced and expanded with different combinations of cytokines. They were identified for their properties by FACS, electronic microscopy, microscopy, and mixed lymphocyte reaction (MLR). RESULTS: Cultures of cord blood CD34+ cells with the combination of GM-CSF + TNF-alpha + SCF + FL yielded (27.18 +/- 1.56)% CD1a+ cells, much higher than that (0.65 +/- 0.38)% with GM-CSF alone. The combination of GM-CSF and high dose IL-4(1,000 U/ml) was the most potential for expanding CD1a+ cells [(21.8 +/- 0.32)%] for PBMC. Both of the DC from different sources were similar in phenotype and morphology, and had the capacity to stimulate proliferation of allogeneic T lymphocytes. CONCLUSION: Both of cord blood and peripheral blood are the source for generation of a large number of typical DC, and can be adopted for the application according to different purposes. These results lay foundation for the immunotherapy with DC.

Effect of rhein on glucose transporter-1 expression and its function in glomerular mesangial cells.

OBJECTIVE: To explore the effect of rhein on the regulation of glucose transporter-1 (GLUT1) overexpression and the possible molecular mechanism that rhein antagonize the effect of transforming growth factor beta 1 (TGF-beta 1) in glomerular mesangial cells. METHODS: Cultured mouse mesangial cells were used. The expression of GLUT1 mRNA was detected by Northern blotting; the ability of glucose uptake was determined by 2-deoxy-[3H]-D-glucose uptake assay. RESULTS: Rhein had no effect on glucose uptake in mesangial cells cultured in normal glucose concentration. TGF-beta 1 could upregulate the expression of GLUT1 mRNA and glucose uptake in mesangial cells. This effect was markedly attenuated by the addition of rhein in a dose dependent manner. CONCLUSIONS: TGF-beta 1 could upregulate the expression of GLUT1 mRNA and glucose uptake in mesangial cells, resulting in excessive glucose consumption and extracellular matrix production in diabetic nephropathy. Rhein antagonized the effect of TGF-beta 1 in mesangial cells, so it might be a hopeful remedy for the treatment of patients with diabetic nephropathy.

Significance of anti-neutrophil cytoplasmic antibodies in patients with pauci-immune crescentic glomerulonephritis.

OBJECTIVE: To evaluate the significance of serum antineutrophil cytoplasmic antibodies (ANCAs) and the effects of immune suppressive treatments on its activity in patients with pauci-immune crescentic glomerulonephritis (PICGN). METHODS: Serum ANCAs and myeloperoxidase (MPO)-ANCA were detected by indirect immunofluorescence and enzyme-linked immunosorbent assay (ELISA) methods respectively, and the renal tissues infiltrating cells including CD4+, CD8+, CD68+ and PCNA+ cells were determined by four-layer peroxidase-antiperoxidase (PAP) method. The clinical manifestations, pathologic features and immune pathologic changes in patients with positive ANCAs were compared with that in patients with negative ANCAs. The effects of immune suppressive therapy on clinic and pathologic changes as well as ANCAs activity were also investigated in ANCAs positive and ANCAs negative patients. RESULTS: Both of clinic active manifestations such as the degree of hematuria, rapidly progressive renal failure and pathologic active features including segmental capillary necrosis and vasculitis were much common in patients with positive ANCAs as compared with that in patients with negative ANCAs. The number of infiltrating cells in renal tissue, especially CD4+ cells, was markedly higher in ANCAs positive patients than that in ANCAs negative patients. The effects of immune suppressive therapy were also much better in patients with positive ANCAs than that in patients with negative ANCAs. CONCLUSION: Serum ANCAs is not only a marker for diagnosis of systemic vasculitis, but also a sensitive predictor for evaluation of diseases' activity and treatment in patients with PICGN. The good effect of immune suppressive treatments on patients with PICGN is partially associated with the degree of ANCAs activity.

[Postoperative infection outbreak of M. chelonae subsp. abscessus].

OBJECTIVE: To study the pathogen of postoperative infection outbreak in a hospital in Shenzhen. METHODS: According to the Laboratory Science Procedure of Diagnostic Bacteriology in Tuberculosis published by Chinese Antituberculosis Association, Bergey's Manual of Determinative Bacteriology (ninth edition), and the Clinical Bacteriology Examination & Development, 97 wound exudates were cultivated by the traditional bacteriologic method. The strains were identified with three standard strains of M. chelonae subsp. chelonae, M. chelonae subsp. abscessus and M. fortuitum as controls. RESULTS: 26 fast-acid positive and fast growth nontuberculous mycobacterium were isolated from 97 exudates, then 41 strains including other 15 strains which were collected from the hospital were identified as M. chelonae subsp. abscessus. CONCLUSION: The outbreak of the postoperative infection happened in the hospital was caused by M. chelonae subsp. abscessus.

[Identification of postoperative M. chelonae infection outbreak by polymerase chain reaction of 16s-23s ribosomal DNA spacer sequence].

OBJECTIVE: To determine the pathogen of the infection outbreak in a hospital in Shenzhen at gene level with molecular biological technique, and to establish a rapid identification method of M. chelonae subsp. abscessus by polymerase chain reaction technique. METHODS: A single pair of primers and oligonucleotides probe of M. chelonae subsp. abscessus were designed, according to the sequence of mycobacterium of 16s-23s ribosomal DNA spacer sequences. 53 clinical strains were amplified by polymerase chain reaction, then dot-blot hybridization of PCR product was made. On the basis of this study, 259 lesion, tissue and normal tissue were amplified by 16s-23s rDNA PCR. RESULTS: 380 bp PCR product for 53 clinical strains was yielded by 16s-23s rDNA PCR, in the meantime, the specific hybridization dot appeared. The PCR positive rate of 259 specimens was 60.6%. CONCLUSIONS: The PCR products and dot-blot hybridization showed that the pathogen of the infection outbreak was M. chelonae subsp. abscessus at gene level. The 16s-23s rDNA PCR investigative system is sensitive and specific, which can identify M. chelonae subsp. abscessus.

[Study of 6 cases of AIDS with pulmonary complication].

OBJECTIVE: To improve and to increase the awareness of pulmonary complications of AIDS. METHODS: Six cases of confirmed AIDS from may 1992 to August 1997 were analyzed. RESULTS: Of the six cases, there were five complicated with pneumocysitis carinii pneumonia (PCP) (in which one had PCP as the first occurred manifestation). The clinical presentations of PCP were fever(5/5), dyspnea on exertion or at rest(4/5), and hypoxemi with mean PaO2 of 58.1 mm Hg. Chest X-ray showed bilateral diffuse interstitial or alveolar infiltrates. Pulmonary tuberculosis, tuberculous lymphadenities (fast-acid staining positive but PPD negative) and bronchial fungal infection were found in three cases. CONCLUSIONS: AIDS patients are at high risk of suffering from pulmonary complications, of which PCP is more common. If young pateints who were healthy in the past suddenly suffered from pneumonia and respiratory failure, PCP should be considered. When opportunistic pulmonary infections are diagnosed under special circumstances, one should be alert to the possibility of AIDS. In these cases serum HIV antibodies should be checked immediately.

[In situ observation of apoptosis and proliferation in breast cancer and its precancerous lesions].

OBJECTIVE: To investigate the effects of apoptosis and proliferation on malignant transformation of breast precancerous lesions by in situ observation. METHODS: The density and distribution of apoptotic cells were examined in 31 cases with breast cancer, 20 with dysplasia and 20 with hyperplasia, using TUNEL (TdT-mediated dUTP nick end labeling) technique and immunohistochemical method respectively. Eight normal breast tissues were used as the control. RESULTS: In normal breast tissue, apoptotic cells were located at the lacteals and gland follicles, facing the cavity. The proliferating cells were located at the basal layer of mucosa. These characteristics of distribution were lost in breast cancer and precancerous lesions. The density of apoptotic cells and proliferating cells of dysplasia was significantly higher than that in breast cancer and normal mucosa, The density of apoptotic cells and proliferating cells in breast cancer was significantly higher than that in normal mucosa. The ratio of apoptotic to proliferating cells in hyperplasia and dysplasia was 1:1.3-1.4, 1:1.1-1.2 respectively, and 1:2.3-2.4 in breast cancer. CONCLUSION: Abnormality in cell apoptosis and proliferation may play important role in the genesis of breast cancer.

[The value of spiral CT in CT arterial portography and CT hepatic arteriography].

OBJECTIVE: To evaluate the usefulness of spiral CT in CT arterial portography (CTAP) and CT hepatic arteriography (CTHA). METHODS: The CTAP and CTHA manifestations in 50 patients with small hepatocellular carcinoma (< 3 cm in diameter) were analyzed and compared with those in bi-phase enhanced CT. RESULTS: The detection rate of CTAP and CTHA was 92.2% and 90.1%, respectively. They could reveal tumors of 0.2 cm-0.5 cm in diameter. Both CTAP and CTHA gave false-positive findings, such as perfusion defects in 13.0% of CTAP and non-pathologic enhancement in 19.1% of CTHA. CONCLUSION: With the use of spiral CT technique, the quantity of contrast material administrated in CTAP and CTHA can be considerably reduced, and the quality of CT images significantly improved. The detection rate of CTAP and CTHA is higher than that of biphase enhanced CT. Simultaneous use of both procedures may help decrease the false-positive rate, and increase the sensitivity of diagnosis for small hepatocellular carcinoma.

[Studies of microsatellite instability and loss of heterozygosity at chromosome 17 in non-small cell lung carcinoma].

OBJECTIVE: To identify the presence of microsatellite instability (MI) and loss of heterozygosity (LOH) in non-small cell lung carcinoma (NSCLC). METHODS: Four microsatellite markers TP53(17p13.1), THRA1 (17q11.2-12), D17S579(17q12-21) and D17S855(17q21) were used to examine 35 cases of NSCLC tumor-normal paired tissues for MI and LOH at chromosome 17 using PCR based analysis. RESULTS: 22 of 35 tumors(63%) displayed MI or LOH. 14 tumors(40%) exhibited MI, 11 tumors(31%) exhibited LOH, while 3 tumors (9%) exhibited MI and LOH concurrently. The frequency of MI or LOH was obviously higher in the early-stage(stages I and II, 79%) than in the advanced-stage (stage III, 44%), P < 0.05. However, the frequency of MI or LOH had no significant difference between high-grade differentiated NSCLC tumors and low-grade ones, P > 0.05. No relationship was observed between the presence of MI or LOH and the histologic subtype of NSCLC, P > 0.05. CONCLUSIONS: The results suggest that MI and LOH at chromosome 17 may play a significant role in the development of NSCLC. The high frequency of MI or LOH in the early-stage tumors indicates that these genetic alterations could occur early during NSCLC development.

[Mortality of primary liver cancer in China from 1990 through 1992].

OBJECTIVE: To study the geographical distribution and mortality of primary liver cancer (PLC) in China from 1990 through 1992. METHODS: A death survey among one tenth of Chinese population was conducted in 1990-1992 and the data on PLC mortality were analyzed. RESULTS: The crude mortality for PLC was 20.4 per 100,000 population (29.0 per 100,000 for males and 11.2 per 100,000 for females), accounted for 18.8% of the total cancer deaths in 1990-1992. In China, PLC mortality ranked second after stomach cancer. The age-standardized mortality rate (adjusted by the world population) of male Chinese was 33.7 per 100,000 which was 2. 2 times as high as that of male Japanese and 4.6 times as high as that of male Italian. The age-standardized rate of female Chinese was 12.3 per 100,000 which was 3.1 and 5.1 times as high compared to female Japanese and female Italian, respectively. PLC occurred in every age group but the mortality rate was highest in the age group of 30-44 years. The mortality was slightly higher in the rural than in the urban population. The cumulative death rate in the urban population was 3.5% for males and 1.2% for females, while that in the rural population was 4.1% for males and 1.6% for females. PLC showed geographical clustering along the south-east coast of China. CONCLUSION: To prevent primary liver cancer remains to be the major task of cancer control in China.

[Correlations between invasion of gastric cancer to serosal layer and other relevant factors].

OBJECTIVE: To study correlations between invasion of stomach cancer to serosal layer and such factors as size and type of tumor, depth of ulcer, gastric wall contraction, etc. METHODS: Gastric cancer specimens from 150 patients admitted to the hospital in March 1993 through March 1997 were examined. RESULTS: The frequency of tumor invasion to the serosa in gastric cancer of Borrmann type I, II, III and IV was 8.3%, 23.4%, 32.9% and 76.2%, respectively. The frequency of serosal involvement was 13.6% in tumors with a diameter < or = 3.0 cm which was significantly less frequent than in tumors with a diameter > 3.0 cm. The frequency of serosal involvement increased with the increase in depth of ulcer and with spastic contraction of gastric wall as observed in pre-operative barium meal examination. That of serosal involvement varied with degree of differentiation of the tumor, being much lower in well- and moderately well-differentiated adenocarcinomas (10.3%) than in poorly differentiated ones (62.8%). The serosa was frequently invaded by mucinous adenocarcinoma (42.9) and signet cell carcinoma (83.3%). CONCLUSION: Invasion of gastric cancer to serosal layer correlates, to various extents, with the tumor size and type, status of cell differentiation, and depth of ulcer.

[Observation on left ventricular remodeling in acute myocardial infarction].

OBJECTIVE: After an acute myocardial infarction (AMI), there will be regional dilation and global remodeling of the infarcted left ventricle. This study was aimed to observe the features of the ventricular remodeling after AMI. METHODS: Based on the time of onset of the symptoms, 37 patients (pts) with AMI were divided into 4 groups. The time of onset of AMI for group 1 (16 pts), group 2 (7 pts), group 3 (11 pts) and group 4 (3 pts) was < or = 3 hours (hrs), > 3 hrs - < or = 6 hrs, > 6 hrs - < or = 12 hrs and > 12 hrs respectively. The size of the left ventricle in all the patients were analysed with resting myocardial gated SPECT (GSPECT). Tc-99m MIBI was infused at admission and data of GSPECT were collected 2 hours later following the infusion. End-diastolic volume (EDV), end-systolic volume (ESV) and LVEF were shown by the system. Stroke volume (SV) was then calculated. RESULTS: EDV for group 1 to 4 was (98.5 +/- 31.9) ml, (99.4 +/- 55.6) ml, (128.1 +/- 55.1) ml and (140.0 +/- 25.3) ml, respectively. ESV for group 1 to 4 was (57.0 +/- 30.9) ml, (55.1 +/- 40.3) ml, (77.4 +/- 39.8) ml and (81.3 +/- 26.0) ml, respectively. SV for group 1 to 4 was (57.0 +/- 30.9) ml, (55.1 +/- 40.3) ml, (77.4 +/- 39.8) ml and (81.3 +/- 26.0) ml, respectively. LVEF for group 1 to 4 was (44.6 +/- 13.1) ml, (49.7 +/- 13.1) ml, (42.8 +/- 13.5) ml, and (42.7 +/- 8.6) ml, respectively. In pts with anterior infarction, EDV, ESV and SV after 6 hrs of the onset of AMI were greater than those in pts with in 6 hrs of the onset. It was the same in pts with inferior infarction. CONCLUSION: It is suggested that the left ventricular size increased in 3 hrs after AMI. Following the initial dilation of left ventricle, EDV, ESV and SV did not change significantly with in 6 hrs, but then increased gradually after 6 hrs from the onset of the symptoms. The early and significant dilation of left ventricle after AMI may be one of the risk factors of severe cardiac events.

[The effects of nitric oxide and endothelia levels on plasma in patients with chronic heart failure].

OBJECTIVE: To study the effects of prostaglandin E1 on plasma nitric oxide (NO) and endothelin (ET) levels in patients with chronic heart failure. METHODS: Pulmonary artery pressure (PAP), pulmonary arterial and venous plasma ET and angiotensin converting enzyme (ACE) levels were determined in patients with chronic heart failure (CHF) and the effects of prostaglandin E1 (PGE1) were evaluated. RESULTS: (1) Pulmonary hypertension was present in patients with CHF. Plasma NO and ET levels in the pulmonary artery were higher than those in the pulmonary vein (P < 0.05), the levels in the latter was in turn higher than those in normal subjects (P < 0.05). (2) Plasma ACE levels showed a similar change, (3) The increase of plasma ET was more than the increase of NO in patients with CHF, thus, NO/ET ratio was decreased in CHF (P < 0.05). (4) PGE1 caused significant decrease in PAP but increase in cardial index. It decreased plasma NO, ET, ACE, and increased NO/ET ratio (P < 0.05). These effects became more apparent with the increase of PGE1 level. CONCLUSION: NO and NO/ET ratio might be involved in CHF by causing pulmonary hypertension and local vascular endothelium perturbation. PGE1 could inhibit these effects and play an important role in the prevention and treatment of CHF.
