Reference is made to a personal case in an account of the clinical and pathological criteria that provide the diagnostic hinges of angio-immunoblastic lymphadenopathy. Stress is laid on the systemic and progressive nature of the disease, and support is given to the view that is an affection of the immuno-competent system. The fact that the course is unpredictable is seen as a reason for adopting an extremely cautions therapeutic approach based on corticosteroid hormones. Resort to the cytotoxic chemotherapeutical drugs customarily employed in other lymphoproliferative forms should be reserved for a few selected cases.

Angioimmunoblastic lymphadenopathy with drysproteinaemia is reported (AILD) in four patients with different skin pictures. As the disease progresses two main forms predominate; papulonodular and erythroderma. In all cases the histological picture of the skin mirrors that of the lymph-node. Our results point to an increase in the peripheral blood, lymph-nodes and skin of T and subsequently of B lymphocytes suggesting that a proliferation of helper T cells and hence activation of the B-cell subpopulation may be involved in the pathogenesis of some cases of AILD.
Multiple Pacinian neurofibromas involving the three phalanges of the ring finger of the left hand in a 60-year-old woman are described. The abnormal proliferation of Vater-Pacini corpuscles in various stages of differentiation was associated with marked changes of the glomus type of arterio-venous anastomoses of the finger-tip. The close relationship between the new proliferated Vater-Pacini corpuscles, their altered baresthetic function and the pathological shunting of blood account for the painful Raynaud-like symptoms. The name 'Pacinian neurofibromatosis' is proposed.

The effect of a 75-hour vigil on the erythrocyte sedimentation rate (ESR), i.a., was studied in two experiments with 63 healthy male volunteers. The ESR was increased at the end of the vigil compared with pre-exposure values. The increases did not correlate significantly with concomitant changes in serum triglycerides, free fatty acids, cholesterol or gammaglobulins, except for a significant, negative correlation with cholesterol changes in one of the two studies. Although the mechanism for the increases in ESR in response to stressor exposure remains unclear, it is concluded that when using the ESR in clinical practice, allowance should be made for situational factors such as the patient having experienced some stressful days and sleepless nights.
Work should respect the worker's life and health, leave him free time for rest and leisure, and enable him to serve society and achieve self-fulfillment by developing his personal capacities. This is not what work looks like to hundreds of millions of workers all over the world. Occupational stress arises where discrepancies exist between occupational demands and opportunities on the one hand and the worker's capacities, needs and expectations on the other. Ill effects are mediated by three classes of mechanisms: (1) feelings of distress (e.g., anxiety, depression, alienation, etc); (2) behaviors (e.g., increase in alcohol and tobacco consumption, risk taking, self-destructive behavior, etc.); and (3) hyper-, hypo- and dysfunction in various organs and organ systems (e.g., physiological stress reactions as described by Selye; specific changes in endocrine and immunological function, etc.). These effects are common and they are a challenge to occupational medicine. Work environments are man-made and can be adapted by man for man. Local, national and international monitoring of occupational health should be aimed at making work man's servant and not his master.

Various approaches to the study of physiological and behavioural effects of energy deprivation are presented and discussed. An experimental, total energy deprivation in 20 healthy human volunteers during an 11-day period, preceded and followed by 4-day control periods, is described. Potentially pathogenic reactions which occurred in spite of precautions are analysed. It is concluded that the present controlled real-life model with acute total energy deprivation serves the purpose of studying hypotheses concerning potentially pathogenic mechanisms occurring in deprivational situations under conditions of natural and man-made catastrophes.
Ten days of total energy deprivation evoked the following endocrine changes in 12 healthy, normal-weight males: early and marked reductions and increments in the blood levels of T3 and reverse T3, respectively, with rapid returns to pre-starvation levels after refeeding; a slight and late decrease in the blood levels of T4; a minute reduction of the blood levels of TSH; a pronounced increase in the blood levels of growth hormone, but a return towards pre-exposure levels even before discontinuation of starving; a minor and gradual enhancement of the blood levels of cortisol, and an increase in nocturnal urinary adrenaline excretion. It is assumed that these changes reflect a complex regulatory mechanism, the purpose of which is to secure adequate energy supply to vital organs.

A controlled non-blind multicenter trial was conducted in 211 acne patients to test the activity of topical retinoic acid against sulfur-resorcinol--salicylic acid and placebo. Uniform evaluation criteria were used. After 8 weeks' treatment in comparable groups of patients, retinoic acid proved to be superior to the standard and to the placebo. The difference was statistically significant. Side effects were present in a number of patients treated with the active substances and with the placebo (mainly erythema), but rarely was the treatment discontinued.

Three families with hereditary angioneurotic oedema (HANE) are described. Serum CI-INH and C4 immunochemical determinations were performed on 23 members of the families: in 6 persons low CI-INH and C4 levels were found with typical symptoms of HANE, and in one asymptomatic subject. Normal ranges of both proteins for adults, children and newborns are given for comparative purposes. The importance of the early diagnosis and treatment is emphasized.
