[Vaginal delivery after 2 and more cesarean sections].

Eight women with deliveries after undergone two and more cesarean section are described. Delivery occurred vaginally in 6 of them, but cesarean section was performed in two of them, whose indications were diagnosed after full dilatation of the cervical canal because of nonadvancement of delivery. A review of literature on this question is presented. Taking into consideration the literary data and their own observations an opinion is expressed that there is basis for a change in the present management of pregnant women with two and more undergone cesarean sections and is proposed to accept the concept for "trial of labor"--evaluation of the cases for vaginal delivery and those for elective cesarean section, taking into consideration criteria for delivery after cesarean section.

[Labor induction and stimulation after a prior cesarean section].

The author thinks that oxytocin induction and stimulation should be used more freely under proper conditions: high pelvic score, lack of pelvic-fetal disproportion. Oxytocin induction and stimulation was used in 72 parturients out of 241 parturients, who delivered vaginally after cesarean section (CS). The risk of the parturient with preceding cesarean section, undergoing oxytocin infusion, ia not greater than that, which exists in each parturient after preceding CS and good clinical choice and dynamic observation. This correlates with literary data, indicated by the author, as well.

[Labor after having had a cesarean section].

The authors describe current literature, in which they defend a concept for wider vaginal delivery in women, undergone caesarian section (greater than or equal to 2/3 according to the literature). These figures are low--from 20 to 40%, unduly according to their experience. It is indicated that oxytocin induction and stimulation should be used more freely as well as stimulation under suitable conditions--high pelvic score, lack of pelviofetal disproportion. Absolute contraindication for vaginal delivery are not women, undergone more than one caesarian section. The authors report on successful case reports.

[Recurrent infarct in a young female diabetic].

A case with repeated myocardial infarction is reported in a 23-year old female diabetic, established at necropsy and not diagnosed while still living. The infarctions had been painless and their electrocardiographic manifestation had not been definite. Generalized atherosclerosis with coronary sclerosis was pathologoanatomically established, with no obturating thrombus in the magistral vessels. Diffuse microangiopathic changes, in myocardium including, were present. The pathogenetic possibilities, associated with the characteristics of the clinical picture were discussed.

[Collective medical care institutions in Uruguay. Impact of the regulation].

Collective medical care institutions have been in existence for more than a century and have provided medical care to more than half of Uruguayans through a modality that integrates insurance and provision of services. In the last decade, these institutions have presented a persistent deficit: 52% of the beneficiaries are members of institutions with a deficit of more than 4%. We analyze the impact of the regulation of certain aspects related to the behavior of supply (market concentration and quotas, risk selection, price fixing) and demand (adverse selection and the effect of the co-payments in services utilization). The regulation seems not to have promoted the necessary incentives to efficiency and quality by the IAMC, one of the key aspects to be approached in the context of sectorial reform.
