[A rare case of a kidney situated in the lesser pelvis. The diagnostic difficulties].

The minor aries to us an interesting case--woman, 30 years with two deliveries-at gynaecological examination in the polyclinic it is found a formation in the region of the left ovary. The laparotomy in this connection finds ectopic situated left kidney in the minor pelvis. The author calls attention to the differential diagnostic difficulties, which poses this casus to the practising gynaecologists and that it always must be thinking about this also on a large aspect.

[Shoulder dystocia--complications].

The author considers the causes for advent of shoulder dystocia and the complications from it for the mother and the foetus, making due conclusions. For the 6 years period (1985-1990) in the Clinic of Obstetrics & Gynaecology--St. Zagora, are occurred 18,466 deliveries commonly, at 45 of them it is founded shoulder dystocia. From the received results is clear, that, with the growth of the baby's weight increase the percentage of the shoulder dystocia and this difference become appreciable at weight of newborn babies over 4000-4500 g.

[The outcome of delivery in the breech presentation at the Stara Zagora Obstetrics and Gynecology Clinic during 1990-1992].

The outcome from the deliveries in breech presentation deserves close attention, because they are although 4% only from the total number of deliveries, but they exert important influence on the frequency of the operative deliveries by caesarean section and the level of the perinatal mortality. In the modern literature, the breech presentation is recorded as the one of the main causes, which leads to considerable increase of the caesarean sections frequency.

[The morphological characteristics of the cicatrix in repeat cesarean section].

It has been established that the cicatrix formation after a Caesarean operation is conditioned by various factors remaining to a large extent an individual reaction. Cicatrix structure, its vascularization, its relationship with surrounding muscular tissue and the presence or absence of inflammatory changes have their peculiarities. By a number of biopsies from the cicatrix area after a Caesarean operation we have tried to estimate the sufficiency of the cicatrix studying its morphological characteristics. For the investigation 2-3 biopsies on the average were taken from a total of 58 women. The histological and histochemical methods applied enabled us to determine quite precisely the quality of the cicatrix.

[A rare case of a retroperitoneally situated extrauterine pregnancy].

The variety of the extrauterine pregnancy clinical picture makes even the most experienced clinicians affirm that "the most characteristic symptom of extrauterine pregnancy is the fact that in most cases it has no characteristic symptoms and each case has its specific features." In the author's opinion it is obviously a case of ectopic pregnancy in the ampullar part of the left tube on its way out like a tubal abortion. The still fresh chorionic villi have arroded the peritoneum of the posterior abdominal wall and reached the retroperitoneal surface carrying away the ampule. This localization is an extremely rare phenomenon.

[Complications for the mother in vaginal delivery after a prior cesarean section and in parturients with a planned repeat cesarean section].

Over a period of 10 years (1980-1989) in the Clinic of Obstetrics and Gynaecology in Stara Zagora 740 women have delivered children after a previous caesarean operation. On the basis of this material the author has studied the vaginal delivery complications for the mother after a previous caesarean operation and in the cases of a second planned caesarean operation. The alternative and correlational analysis of the clinical data has shown that the complications for the mother in the latter group are 3 times more than those in the former group.

[How does the presence of normal vaginal delivery before and after a first cesarean section influence the outcome of a subsequent vaginal delivery].

The author has studied 740 cases of vaginal delivery after one or more caesarean operations over a period of 10 years (1980-1989) in the Clinic of Obstetrics and Gynaecology in Stara Zagora in his attempt to assess the effect of a normal delivery before or after the first caesarean operation on the outcome of the following vaginal delivery. The results obtained do not correlate with the data cited elsewhere. The author has established that the outcome of the next vaginal delivery is approximately 1% better in the latter group while elsewhere it is cited to be 6-7 times better.

[A breech presentation after a cesarean section--is cesarean section always needed?].

The author has made a survey of published materials supporting the conception of selective vaginal delivery in cases of breech presentation after a previous Caesarean section. A number of such cases have been studied with expected fetus weight between 2500 and 3500 g and there have been no complications for mother or child. Vaginal delivery in cases of breech presentation and a previous Caesarean section seems acceptably safe. The possibility for a vaginal delivery provided that all obstetric requirements as regards the mother and fetus are observed has been considered.

[A surgical method for conization of the cervix uteri].

Uterine neck conisation and the restoration methods applied after it are well-known. On the basis of 48 women admitted to the clinic for cervix uteri conisation on different indications the authors suggest a new method for the spontaneous epithelialization of the defect thus obtained. The operated patients have been under medical supervision and examined in the first and sixth month after the intervention. The results obtained show a complete epithelialization of the excised surface and no change in the anatomical characteristics of the uterine neck.

[Vaginal birth following a cesarean section in twin pregnancy].

On the basis of his own experience and investigation and a survey of published materials the author supports the view that the combination of a bigeminal pregnancy and a previous Caesarean operation should not in itself lead to an obligatory indication for a second Caesarean operation. The rigid criterion for a safe vaginal delivery applied in cases of unifetal pregnancy should be applied here, too. If there are no other contraindications the possibility of a vaginal delivery should not be excluded. Caesarean operation with all accompanying inconveniences, prolonged stay in the hospital and possible complications is not the best choice for a woman which is to look after two new-born babies.

[The status of the fetus delivered vaginally after a prior cesarean section and with a second cesarean section].

The author has set himself the task of studying the state of the fetus delivered vaginally after a previous caesarean operation and the state of the fetus delivered by a second caesarean operation. He has studied the state of 304 children vaginally delivered after a previous caesarean operation and 436 children delivered by a second caesarean operation as well as early neonatal death rate over a period of ten years (1980-1989). He has come to in the conclusion that the early neonatal death rate in the former group is not greater than that in the latter group.

[A case of vaginal delivery--11 months after a primary cesarean section].

The state of the uterine cicatrix and more specifically its sufficiency are extremely important for the obstetrician when he has to choose between a second caesarean operation and a vaginal delivery. This decision determines the post partum and postoperative techniques and behaviour. The author reports a case of a woman who has vaginally delivered a child 11 months after a caesarean operation. This case demonstrates that approaching personally the obstetrician can decide in favour of a vaginal delivery after a previous caesarean operation even in the cases of pregnancy which has begun before the 6 month period after the caesarean operation has elapsed.

[The age of women who deliver vaginally after a prior cesarean section].

The author has studied in detail the structure of age groups of women delivered vaginally after a previous caesarean section over a period of ten years. He supports the view that the combination of an "aged lying-in woman" (over 29) and a previous caesarean section should not be in itself an obligatory indication for a planned caesarean section as it is considered now. So, the conclusion is that the indication "aged primipara" (over 29) as an only indication for a planned caesarean section should be doubted since 70 women over the age of 29 and after a previous caesarean section delivered vaginally without any complications for the mother or fetus.

[Rupture of the uterine cicatrix in vaginal delivery after a prior cesarean section].

In the present investigation 740 deliveries were followed-up after undergone one or more cesarean sections (29.30%). 420 of deliveries were studied retrospectively and 320-prospectively. 436 women (58.91%) out of 740 women with cesarean section underwent repeated cesarean section, but 304 women (41.09%) delivered vaginally. 340 women delivered vaginally all together and the uterine cicatrix ruptured in 4 women without cases with maternal mortality, but a child died on the second day after delivery. The number of pure ruptures of the cicatrix was 3 as the percentage corresponded to 0.98%. In conclusion the author thinks that the spontaneous and traumatic rupture of the intact uterus is complete and catastrophic frequently. On the contrary the rupture of transverse uterine cicatrix is incomplete most frequently without special consequences for the mother and fetus.
