[Primary lymphoma of the bladder].

The authors present the case of a 74-year-old man in whom bladder tumor was detected the reason for haematuria. The histological investigation of transurethral resected pattern showed lymphocytic lymphoma. In possession of the histological finding detailed haematological examination has been carried out, the patient was without evidence of lymphoma dissemination. He received multichemotherapy (6 courses) and locoregional cytostatic therapy and complete clinical remission was detected. The authors report our experience with extranodal bladder lymphoma included diagnostic and therapeutical notes as well as a review of the relevant literature.

[Clinical value of immunologic studies in patients with bladder tumors].

By means of mathematical calculations the authors tried to find correlations between the clinical and histological stages of vesical tumours and the results of immunological tests. Density calculation showed what parameters follow the degrees of malignity. Moreover the significance of the chain and basic proportional factors of the parameters calculated for diagnosis and follow-up is pointed out.

The investigation of cellular and humoral immune reactivity in patients with tumours of the bladder.

The authors investigated the cellular immune reactivity by measuring the lymphocyte blast transformation, immune globulins and some glycoproteins in the sera of patients with papilloma and carcinoma of the bladder. They found a change in the cellular immune reactivity, but none in immune globulin concentration. Among the glycoproteins the acute phase reactant proteins changed significantly.

[Transurethral resection].

In five years 938 transurethral resections were performed in the Department of Urology of Semmelweis University Medical School. The indications were in 483 cases tumours of the bladder, in 179 adenoma of the prostate and in 182 cancer of the prostate; the indications of the remaining cases were miscellaneous. The indications, most important surgical parameters, postoperative complications and treatment of transurethral resections are discussed from the authors' point of view. The results prove the advantages of transurethral resection and confirm its importance as a basic surgical intervention in urological practice.

Buschke-Loewenstein tumour of the penis.

Three cases of Buschke-Loewenstein tumour are reported. The clinical features of the process are described, its therapeutic aspects are outlined, the diagnostic problems involved by its microscopic structure are discussed. The features of the first case were typical of the process. The intermediate form between the Buschke-Loewenstein tumour and squamous cell carcinoma was exemplified by the second case. The small size of the tumour in the third case was compatible with an organ-saving surgical intervention.

[Transurethral resection of bladder neoplasms].

The technique of removal of bladder tumours is discussed on the basis of experience with transurethral resection performed on 200 patients suffering from bladder tumour (113 benign and 87 malignant tumours). The indications for the intervention are as follows. With benign tumours, resection has to be given preference over exposure. When malignant tumours are operable and situated on the mobile part of the bladder, radical surgery is recommended. When the tumour is in some other position, transurethral resection is the procedure of choice.

Treatment of prostatic cancer with Estracyt (estramustine phosphate).

The experience gained with Estracyt, kindly supplied by AB LEO, Sweden, is reported. On the basis of former data in the literature we only used the drug in estrogen resistant and advanced cases. Estracyt (estramustine phosphate) is a nitrogen mustard derivative of the urethan type, attached to oestradiol-17-phosphate. In histologically verified cases, it was administered in daily doses of 300 mg intravenously for three weeks, followed by maintenance doses of 300 mg a week in tablets for three months. During treatment, liver and bone marrow function was checked systematically. The changes in morphological picture were studied by means of biopsies during and at the end of treatment. In agreement with the data in the literature a favourable effect was observed in estrogen resistant patients, with no toxic effect whatever on the bone marrow. At the same time GOT and GPT and BSP retention examinations demonstrated a hepatotoxic side effect. The pathological values returned to normal after withdrawal of the drug. Histological examinations showed that the tumour cells had changed but failed to disappear after treatment.

Granulomatous orchitis and spermatic granuloma.

One case of granulomatous orchitis and three cases of spermatic granuloma are described and the clinical and pathologic features of the two syndromes are discussed. Since no certain diagnosis is possible on clinical grounds alone, it rests on the evidence of pathology. Both granulomatous orchitis and spermatic granuloma are of practical importance to the specialists, the first being apt to mimic tumours, and the second to tuberculous epididymitis. Histological study, possibly of biopsy specimens, permits to differentiate either process from malignancy and from tuberculosis with certainty. It is to be emphasized that in many cases of chronic epididymitis with clinical suspicion of tuberculosis careful histological search reveals spermatic granuloma. Since the processes under discussion are unresponsive to antibiotics, their therapy is surgical.

[Resection syndrome following transurethral resection of the prostate].

The essence and clinical aspects of the resection syndrome occurring in association with transuretheral prostate resection and the possiblity of determining the direction of the absorption of the washing solution are discussed. By means of radioisotope tests a direct correlation was detected between the extent of the haemorrhage and the intravasation of the washing solution. Great importance is ascribed to the vesico-renal reflux often occurring during resection, which might have a major role in the development of post-resectional pyelonephritis.
