A double-blind multicentre trial comparing the efficacy and side-effects of mianserin and chlorimipramine in depressed in- and outpatients.

Mianserin (60 mg daily) was compared with chlorimipramine (150 mg daily) in the treatment of 145 depressed in- and outpatients in four centres. The trial was double-blind and fully randomized. Both drugs were effective antidepressants. No significant differences in efficacy could be demonstrated by means of the Hamilton rating scale for depression, the Beck self-rating scale or the clinical global impression, for both in- and outpatients. Hypotension, dry mouth and tremor increased significantly more in inpatients with chlorimipramine than with mianserin. At the end of treatment weight gain was increased significantly more in outpatients after treatment with mianserin. No differences could be demonstrated between the drugs for other side-effects.

[Vitamin A and cancer (author's transl)].

During the past decade, an increasing number of publications have been dealing with the experimental and clinical activity of vitamin A on epithelial neoplasms. Many studies suggest that vitamin A and its analogs may be able to interfere with chemical carcinogenesis and to modify favorably the immune status of tumor bearers by mechanisms incompletely known. The use of vitamin A and of its analogs have been shown to enhance the effects of surgery, radiation therapy and chemotherapy in the treatment of epithelial cancer, possibly because they appear to destabilize lysosomal membranes and contribute to maintenance of the differenciated state in epithelial tissues. The potential role of such substances in preventing neoplastic transformation deserves further study.

[Malignant pheochromocytoma of Zuckerkandl's body; clinical data and treatment: one case (author's transl)].

A forty year old man with malignant pheochromocytoma of Zuckerkandl's body is reported. The case history is a severe hypertension with acute episodes, tachycardia and psychiatric disorders. The tumor extra adrenal site was got by abdominal scanning and its malignant type was demonstrated by pathological study and metastasis of lymph nodes occurring 4 months after surgery. The surgical treatment was associated to antimitotic and alpha blocking therapy. Clinical and biological status is still good after a twelve months follow-up.

[A clinical rating scale for personality. Its validation through factor analysis in geriatrics (author's transl)].

A rating scale has been established in our department. It is used to assess different psychological aspects. It can be applied in treatment trials as well as to provide a descriptive study of personality or behaviour. 26 two-pole items at five levels are covered by this scale. A factor analysis was used to validate the scale on a population of old people in institutional care. This showed the scale evaluates one general factor and nine group factors: affective maturity, social ease, inhibition, social adaptation, anxiety depression, psychic well-being, psychopathic tendencies, extraversion and receptivity, guilt-feeling. To appreciate the effects of psychotropic drugs on behaviour, these ten factors can be grouped into three: a factor we can call "mental health", a factor of social adaptation, an affective-emotional factor.

Phase I trial with peptichemio.

Thirty-two patients with tumor progression, even after conventional cytostatic drug treatment, were treated with peptichemio, with increasing doses for groups of 4 patients. The maximum tolerated dose (with minimum hematological toxicity and without any other evident toxicity) with repeated administrations, was 1.2 mg/kg twice weekly. The recommended doses for phase II trials are, as shown by the detailed analysis of the results, 0.9 mg/kg, twice weekly and administered alone, and 1.3 mg/kg, once weekly combined with other cytostatic drugs, in 500 ml of infusion fluid, with 25 mg of heparin and 25 mg of hydrocortisone to minimize the freqent risk of local phlebosclerosis.

[Evaluation of the effects of psychomotor reeducation in aged patients with dementia].

Psychomotor re-education has been attempted experimentally with a population of demented patients in a geriatric ward. It draws on the methods employed at the Bel Air Psychiatric Clinic, in particular the principle of transfer of training. To assess the effect, if any, of the therapy, two psychological rating scales devised specially for demented patients, were established by the team of psychologists at the Grenoble Teaching Hospital. These rating scales are based on the theoretical notions of J. de AJURIAGUERRA and colleagues, that is, the concepts of structural analysis, of involution and homogeneity or inhomogeneity. The psychological test rating scale proposed explores four aspects of mental activity : memory, operating mechanisms, visual defects, space perception and orientation. The rating scale is described in detail, with its references, instructions and scoring.

Adjuvant chemotherapy in the management of primary malignant melanoma.

In a prospective randomized study, the effect of chemotherapy (either systemic or combined intraarterial and systemic) was studied in 117 patients undergoing a curative resection of Clark's level III, IV or V malignant melanoma. Systemic chemotherapy was started one month after surgery one week courses with an I.V. injection of Vinblastin 6 mg/m2, Thiotepa 6 mg/m2, Rufocromomycine 60 microgram/m2, Methotrexate 15 mg/m2 on day one with procarbazine 30 mg/m2 orally daily were given every other week for three months and later every four weeks. Intraarterial chemotherapy of DTIC 80 mg/kg day for ten days was given 28 days prior to surgery. 65 patients with limb malignant melanoma were treated either by surgery only (27 patients), or by systemic chemotherapy (23 patients) or by preoperative intraarterial chemotherapy and systemic chemotherapy (15 patients): 52 patients with non limb malignant melanoma were treated either by surgery only (28 patients) or by systemic chemotherapy (24 patients). We drew curves of disease free survival following surgery and studied the levelling off of the curves, 24 months after surgery 65% of the patients treated by surgery alone were alive and free of disease whereas 81% of the patients treated by surgery and chemotherapy were alive and free of disease (p less than 0.05) suggesting a possible benefit of adjuvant chemotherapy. Intraarterial preoperative chemotherapy has not proved of additional benefit to date.

Treatment of nausea and vomiting related to anti-cancerous multiple combination chemotherapy: results of two controlled studies.

Two controlled double-blind studies were carried out successively on a cancerous out-patient population receiving multiple combination chemotherapy at successive treatment courses to show the anti-emetic efficiency of metopimazine compared to a placebo. The patient was taken as his own control and the treatment order was randomly selected. The first trial--consisting of sixty-seven patients, showed a weak statistically significant difference (0.05 less than p less than 0.10) between metopimazine at doses of 30 mg/day or 15 mg/day and a placebo. The second trial--comparing a higher dose of metopimazine (45 mg/day) among one hundred and sixteen patients, showed a highly statistically significant therapeutic advantage over a placebo (0.001 less than p less than 0.01).
