Unifocal origin of multiform extrasystoles in concealed bigeminy.

A case of concealed bigeminy is presented in which two forms of extrasystolic complexes were evident on the ECG. One form (type B) appeared after a single conducted sinus beat, in a pattern of manifest bigeminy. The other form (type A) occurred after a sequence of more than one conducted sinus beat. The two configurations have previously been explained on the basis of different foci and mechanisms. Certain features of the ECG from our patient suggested that the two forms originated from a common site. An experimental model was developed in which the right ventricle of a dog was stimulated at sub- or suprathreshold voltages on alternate sinus beats to simulate concealed bigeminy. The ECG obtained during stimulation of a single site was remarkably similar to that recorded from the patient. We have proposed that the extrasystoles that occur after a compensatory pause may spread in a manner different from those that follow a basic cardiac cycle length. The different pattern of propagation is probably ascribable to the lengthening of the refractory period of the cardiac tissues by the compensatory pause.

Diabetic retinopathy in pregnancy: a 12-year prospective survey.

A prospective study extending over 12 years has surveyed 107 latent diabetics and 172 clinical diabetics during pregnancy. Among the clinical diabetics background retinopathy was evident or developed during pregnancy in 40 cases. Eleven cases showed proliferative changes, only 1 such case appearing de novo during gestation. There was a higher incidence of fetal loss in the diabetic than the latent diabetic group, and this loss was highest in those with proliferative retinal disease. Pregnancy was to associated with any increased risk to the mother of progression of retinal changes and visual loss. The only known direct relationship, namely, that between duration of diabetes and ocular complications, is confirmed.

Increased uptake of [3H]deoxyglucose and [14C]deoxyglucose in localized regions of the brain during stimulation of the motor cortex.

A part of the motor area which evokes short-latency activity in jaw-opening muscles was stimulated for 30-45 min in guinea pigs which had received an injection of [3H]deoxyglucose or [14C]deoxyglucose. In a number of specific regions of the upper brainstem and basal ganglia, bilateral increases of metabolic activity were demonstrated by autoradiography. These included the putamen, globus pallidus, substantia nigra, subthalamic nucleus, dorsomedial region of red nucleus, deep layers of the superior colliculus, and pretectal nuclei. A ventral region of the thalamus was labeled ipsilateral to the stimulation. It is suggested that these regions represent stations in the circuits involved in the production of the evoked muscular activity. Using [3H]deoxyglucose instead of [14C]deoxyglucose as a tracer improved the quality of the autoradiographic images.

Concealed quadrigeminy and quintageminy.

Electrocardiographic rhythm strips were analyzed from three patients who had frequent unifocal extrasystoles. Parasystole was excluded in all three cases. Analysis of the distributions of the numbers (S) of conducted sinus beats between extrasystoles revealed that the distributions conformed to two variants of concealed quadrigeminy (namely, the "S = 4n" and "S = 4n - 2" variants) and to one variant of concealed quintageminy (namely, the "S = 5n - 2" variant). The binomial theorem was used to test the likelihoods that these patterns of distribution of extrasystoles could occur on the basis of chance alone; the probabilities were all extremely small.

The effect of bacterially produced vitamin B12 analogues (cobamides) on the in vitro absorption of cyanocabalamin.

An incubation system containing rat jejunoileum was used to determine whether analogues of cyanocabalamin (CNCbl) inhibited the in vitro absorption of CNCbl. The naturally occurring analogues "pseudo B12," "factor A," and the nonnaturally occurring analogue desdemethyl B12, were produced by guided biosynthesis with Propionibacterium arabinosum. Factor B was produced by acid cleavage of cyanocabalamin. Inhibition of Co57-CNBbl uptake was demonstrated for cold CNCbl and for desdemethyl B12, although statistical significance was achieved only for cold CNCbl at 40X and 100X the molar quantity of the radiolabeled CNCbl. However, a trend towards progressive inhibition of specific CNCbl binding was shown as increasing concentrations of CNCbl or desdemethyl B12 were used. No inhibition of specific CNCbl binding could be demonstrated for any of the naturally occurring cobamides tested.

Effects of a sugar mixture on blood alcohol parameters and impairment in the intact rat.

The effects of a sugar mixture containing 43% glucose and 36% fructose on ethanol metabolism and on ethanol-induced impairment were studied in intact rats. When ethanol was given orally mixed with the sugars there was a significant reduction in the area of the blood alcohol time curve accompanied by a decrease in the peak ethanol concentration and a slowing in the rate of ethanol absorption. The whole curve followed a lower course and the time for complete removal of ethanol from the body was markedly reduced even though the slope of the elimination phase (beta) was not significantly increased. Ethanol-induced impairment as measured using a tilting plane device was significantly less when ethanol was given mixed with the sugars and paralleled the reduced area and duration of the blood alcohol curve. The magnitude of the effect observed on blood alcohol levels and impairment was dependent on the dose of sugars administered in relation to the alcohol dose. When the sugars were given orally after absorption and distribution of ethanol were complete the slope of the elimination phase (beta) was increase by 30%. These findings point to an increased rate of ethanol metabolism in rats in the presence of sugars. The action of the sugars prevails only for the first few hours after administration thereafter the rate of ethanol metabolism returns to normal.

Isolation of meningococci in meningococcal endophthalmitis.

We describe a case of bilateral hypopyon in a 7-year-old African male receiving systemic antibiotic therapy for meningococcal meningitis. Aqueous from paracentesis of the left eye contained intra- and extra-cellular Gram negative diplococci. We believe this is the first report of isolation of the organism since the advent of antibiotic treatment.

Remarks on transference-countertransference in psychotic states.

Clinical material of patients in psychotic states indicates that in those states the transference is directed into the therapist's body. The emotional experience thus created in the therapist, which is usually called countertransference in its broad meaning, should rather be seen as very early forms of object relationships. The complexities of the above easily influence the analyst in her role of interpreter. Therefore in the actual work with these patients the verbal interpretation, which represents an elaboration of the transference, in the author's view should be seen as countertransference.

Immune complexes in retinoblastoma.

Thirteen patients with retinoblastoma were tested for circulating immune complexes using the Raji cell radioimmunoassay. Eight of 13 patients had statistically significantly increased levels of immune complexes.

Dominant inheritance of multiple epiphyseal dysplasia, myopia and deafness.

An Afrikaner kindred in South Africa had a dominantly inherited skeletal dysplasia-blindness-deafness syndrome. The affected individuals had reduced stature and a round, flattened facies. The bone changes resembled multiple epiphyseal dysplasia with additional minor abnormalities in the phalanges, femoral heads and spine. Progressive myopia, retinal thinning and crenated cataracts led to visual disturbance, while conductive deafness represented the third component of the triad. The syndromic significance of associated asteroid hyalosis is uncertain.

Influence of pH on the efficacy of pilocarpine.

Pilocarpine 4% solutions at pH 4.1 and 5.8 were compared in a double-blind clinical trial on 24 eyes of patients with primary open-angle glaucoma. Each drug was used over a period of 1 week. No significant difference in the lowering of intraocular pressure was found, and the near-neutral solution of pilocarpine was found to be equally stable when compared to the acid solution over a 6-month period.

Readaptation time after photo stress. Alcohol-induced acute and post-alcohol 'hangover' changes in ocular readaptation time.

The effect of alcohol intake on readaptation time (RAT) after photo stress is studied. Ten healthy subjects were given 0.72 g alcohol per kg body weight to be consumed within 20 min. The tests were made during a total period of 11-12 h. The findings demonstrate that moderate blood alcohol concentrations induce definite changes in RAT. An average RAT-prolongation of 60-70% was seen during the acute phase of intoxication. RAT-prolongation ended before the blood alcohol concentration had reached zero, but did not have the same downward slope. Following a period of reduction, a new prolongation of RAT was observed. The lack of congruence between the RAT-curve and the blood alcohol curve in the acute phase of intoxication and the post-alcohol increase are discussed. It is concluded that the alcohol-induced RAT-changes probably are CNS-effects and that the equipment used constitutes a sensitive method of estimating these effects.

Computed tomographic scanning in the management of retinoblastoma.

We used the computed tomographic scanner in the assessment of ten cases of retinoblastoma in children. With each patient the stage of the disease before operation could be more accurately defined and therefore more definite treatment could be undertaken.
