The Nd:YAG laser and methylene blue staining in the diagnosis and treatment of premalignant vesical lesions and carcinoma in situ (CIS).

The aim of this report is to emphasize the use of methylene staining test and the Nd:YAG laser in the diagnosis and treatment of premalignant lesions and CIS of the bladder. The use of an appropriate solution of methylene blue (0.2% in 0.9 M NaCl for 15 min) permits the staining of premalignant areas and CIS, and their early diagnosis. Laser irradiation of the stained areas facilitates the treatment of lesions. The technique was utilized in 8 patients without noteworthy side effects. It allowed an adequate treatment of the lesions and provided good results.

Food allergy in preterm infants fed human milk.

In 80 preterms aged 9-24 months (mean age: 15.9) and in 80 sex- and age-matched full-terms the frequency of atopic diseases and of positive skin tests to 8 food and 6 inhalant allergens was determined. The two groups did not differ as to overall percentages of cutipositive subjects and patients with atopic diseases. In particular, frequencies of positive skin tests to foods and of atopic dermatitis (the peak prevalence of which occurs early in infancy) were similar in preterm (16.2 and 7.5%, respectively) and full-term (13.7 and 5.0%, respectively) infants. We suggest that preterm infants fed human milk are not at increased risk of developing food allergy and related diseases and that the absorption of antigens through the immature intestine does not seem to favor the development of an IgE sensitization to foods.

Nd: YAG laser treatment of vesical leiomyosarcoma.

The first in the literature of bladder leiomyosarcoma, treated by transurethral resection followed by laser irradiation, in a 75-year-old woman is reported. At 22 months follow-up the patient is well and has no evidence of metastasis.

Spontaneous impulse activity of rat retinal ganglion cells in prenatal life.

The existence of spontaneous neural activity in mammalian retinal ganglion cells during prenatal life has long been suspected. This activity could play a key role in the refinement of retinal projections during development. Recordings in vivo from the retinas of rat fetuses between embryonic day 17 and 21 found action potentials in spontaneously active ganglion cells at all the ages studied.

The organization of receptive fields in area 18 neurones of the cat varies with the spatio-temporal characteristics of the visual stimulus.

The spatial frequency tuning curves of neurones of area 18 depend upon the velocity of the visual stimulus. The higher the velocity the lower the spatial frequencies to which the cell is tuned. Since in area 17 the size of the cell receptive field is inversely related with the optimal spatial frequency to which the cell responds, we have investigated whether the shift of the optimal spatial frequency with the velocity corresponds to a "change" in the receptive field size. We recorded extracellularly from neurones in area 18; for each cell we selected two gratings, one of high spatial frequency drifting at low velocity and another of low spatial frequency drifting at high velocity to which the cell gave comparable responses. The results show that the masking of the cells receptive field which abolishes the response to the high frequency low velocity grating does not prevent the cell from responding to the low frequency high velocity grating. We conclude that the size of the receptive field of neurones in area 18 depends upon the characteristics (spatial frequency and velocity) of the visual stimulus.

Fecal alpha-1-antitrypsin excretion in acute diarrhea: relationship with causative pathogens.

Concentrations of alpha-1-antitrypsin (AT) in random fecal samples from 68 infants (92.7% outpatients) with acute diarrhea and 32 healthy controls were determined. The mean +/- s.d. for AT in infants with diarrhea was 2.07 +/- 1.88 mg/g dry stool (mg/g d.s.) compared with 1.29 +/- 0.72 mg/g d.s. (p less than 0.05) in controls. Fecal AT was significantly greater than that of the controls only for diarrhea caused by Rotavirus or Salmonella. Salmonella patients also had significantly higher fecal levels of AT than patients with diarrhea caused by Campylobacter. Fifty percent of the infants with Salmonella infections excreted more than 2.73 mg/g d.s. AT, which is above the control mean + 2 s.d. Fecal excretion of AT was highest in patients with macroscopic intestinal bleeding, but it still depended more on the causative agent than on bleeding per se. This finding of increased intestinal protein loss during acute diarrhea, even in the less severe diseases, emphasizes the necessity to rapidly restart adequate nutritional intake.

Binocular suppression in cortical neurons.

Dichoptic presentation of patterns similar in shape but of very different contrast results in the perception of only the high contrast pattern (binocular suppression). When recording from binocular neurons of the cat visual cortex, we have found an effect which is strikingly similar to this perceptual phenomenon. If a high and a low contrast grating are presented simultaneously, one to each eye, the cell's response to the low contrast stimulus is suppressed.

Adrenal myelolipoma: report of diagnosis by fine needle aspiration.

We report 2 cases of adrenal myelolipoma. The diagnosis was suspected preoperatively in the first case on the basis of radiographic findings, ultrasonography and computerized tomography, and was confirmed by an operation. In the second case the diagnosis was established by fine needle biopsy. Computerized tomography and ultrasonography help in the preoperative diagnosis of myelolipoma but only fine needle biopsy can differentiate this lesion from a malignant neoplasm and avoid an operation.

Spatial-frequency characteristics of neurones of area 18 in the cat: dependence on the velocity of the visual stimulus.

The spatial and temporal response properties of neurones of areas 17 and 18 were studied in single units (165) of anaesthetized and paralysed cats. The visual stimuli were drifting or alternating gratings. We confirmed and extended the observation by Tolhurst & Movshon (1975) showing that the spatial-frequency characteristics of neurones of area 17 are largely independent of the temporal parameters of drifting or alternating gratings. The spatial-frequency tuning curves of neurones of area 18 shift along the spatial-frequency axis when the velocity or the temporal frequency of the drifting grating are changed. The effect of an increase either of velocity or temporal frequency is to shift the cell spatial-frequency tuning curve down the spatial-frequency scale, keeping relatively constant the strength and band width of the response. The spatial-frequency tuning curves of neurones of area 18 do not show this temporal-frequency-dependent phenomenon when the stimuli are gratings alternated in phase. In this case neurones of areas 17 and 18 show a similar behaviour. The response properties of neurones of area 18 are compared with recent psychophysical results obtained in similar experimental conditions. The hypothesis is advanced that both areas 17 and 18 are devoted to the processing of spatial information. Area 17 would be responsible for the processing of patterns in stationary or quasi-stationary situations while area 18 would be responsible for that of patterns moving at high velocities.

Single nightly administration of ketoprofen (Orudis) and indomethacin suppositories in the management of osteoarthritis: a double-blind trial.

In a double-blind trial carried out on 60 patients with osteoarthritis at various localizations, ketoprofen (Orudis), administered as a single nightly 100 mg suppository for ten days, was significantly more active and better tolerated than indomethacin, administered in the same way. Patient's and doctor's assessment was also in favour of Orudis. Laboratory examinations were not affected by either drug.
